Abstract

Background: Data on malnutrition in the developing countries showed that 1–3 elderly in every 5 elderly were malnourished. However, the data in Indonesia were limited. Malnutrition in elderly is often underdiagnosed and thus it usually goes undetected at earlier stage.Objective: The aim of this study was to examine nutritional profile of elderly by using several indicators: anthropometry, Mini Nutritional Assessment (MNA) Questionnaire, and dietary intake.Methods: A cross-sectional study was conducted among community dwelling elderly (aged =60 years) and elderly in the institutional care. The outcome variables were prevalency of malnutrition (undernutrition and overnutrition) and macro and micronutrient intake. Nutrition profiles were assessed using anthropometry, MNA and 24-hour food recalls. Comparative analysis between continuous variables was conducted using Mann-Whitney or ANOVA. Differences between categorical variables were assessed using Chi-square tests or Fisher’s exact test.Results: Based on BMI, in total, 1 in 2 elderly was either undernutrition or overnutrition. Based on MNA, 1 in 2 elderly were malnourished or at risk of malnutrition. Place of residency and gender were associated with the distribution of malnutrition in elderly (p<0.001). Approximately 1 in 2 elderly from the community dwelling group were overweight and obese whereas 1 in 3 elderly from the institutional care group were severely thin and thin. The intakes of 4 in 5 elderly were deficient in vitamin D, calcium, vitamin E and folate.Conclusion: Elderly in our population group were experiencing the double burden of malnutrition and this coexisted with micronutrient deficiency.

Abstract

Background: Methicillin-Resistant Staphylococcus aureus (MRSA) has become a major pathogene and its incidence continues to increase in various countries. The resistance to methicillin is caused by the mecA gene that encodes the expression of Penicillin Binding Protein 2a (PBP2a) so it has a low affinity for beta-lactam antibiotics. One of the factors that can increase the incidence of MRSA is inappropriate antibiotics use. Objective: This study is to identify presence of the mecA gene in MRSA and to compare histories of inappropriate antibiotic uses of healthcare workers and communities in Banyumas.Methods: This was a cross-sectional study conducting 120 samples from nasal swabs. Isolates of MRSA tested by bacteriological examinations and PCR of the mecA genes. Histories of antibiotic uses were assessed by using questionnaires and then analysed descriptively by using Fisher Exact test (SPPS, version 20).Result: One sample of S. aureus from the microbiology examination (0.83%) was an MRSA although there was no a mecA gene identified by using the PCR method. 66.1% of the healthcare workers and 88,3% of the communities used antibiotics inappropriately. The results showed that there was a statistically significant difference in the histories of irrational uses of antibiotics (p?0.05).Conclusion: A number of the communities who used inappropriate antibiotics was higher than the health workers. Furthermore, one MRSA strain with a negative mecA gene was found in the health worker group.

Abstract

Fahr’s disease or Fahr’s syndrome is a condition characterized by intracranial calcifications especially in basal ganglia and dentate nuclei. They are a rare condition with a broad range of manifestations from asymptomatic condition to severe neurological and psychiatry signs and symptoms. The aim of this report to present Fahr’s syndrome as an alternative diagnosis of neuropsychiatry symptoms in systemic lupus erythematosus (SLE) patients. We present a 17-year-old teenage girl who have had a preexisting diagnosis of SLE and have been on remission. The patient recently had experienced headache, auditory hallucination and memory loss despite having a low activity score of SLE and no laboratory relapse. Head CT scan showed bilateral basal ganglia calcifications suggesting a Fahr’s disease. Clinical outcome in this condition might vary but might worsen neurocognitive functions.

Abstract

Adjuvant chemotherapy has been widely applied in patients with breast malignancy after surgery to prevent its recurrence and to improve survival comprehensively. Febrile neutropenia is one of the most life-threatening chemotherapy-associated toxicities. A 66 years old woman came to an emergency department of a secondary healthcare facility one week after her final chemotherapy with dose regimen of docetaxel and epirubicin, followed by having nausea, vomitus, and general fatigue with a fever of 38.2°C. Based on her medical record, she had experienced breast malignancy for almost two years and had already conducted mastectomy three months after her diagnosis. Her leukocyte number was only 300 cells per mm3, with 23% neutrophil, hypoalbuminemia, and hypokalaemia. She was then treated in an internal ward and was given intravenous ceftazidime, fluid, granulocyte colony-stimulating factor, albumin transfusion, intravenous potassium replacement, and symptomatic medication. After two days of inpatient care, she was referred to an oncology team in a tertiary healthcare facility. Febrile neutropenia is considered as a fever with a low neutrophil number as a result of bone marrow suppression. The neutrophil plays an essential role as the body’s first line of defence. This condition could make the patient more vulnerable to infections. The Patient was given broad-spectrum antibiotics and typically hospitalized to allow close monitoring until the neutropenia was  resolved. Febrile neutropenia possesses a serious threat to patients with breast malignancy who are on cytotoxic chemotherapy. Urgent full blood count and immediate broad-spectrum antibiotic administration are significant to reduce morbidity and mortality.

Abstract

Irritant contact dermatitis (ICD) is an inflammation of skin caused by direct cytotoxic effects of chemical, physical or biological agents on epidermal cells without a presence of specific antibody production. It was reported that a 40-year-old man, complaining of itching and a burned sensation on his skins accompanied by pain after contaminated by sap of Rengas. Diagnosis of ICD was established by history taking and physical examination: there are erythematous, multiple, discrete papules, partly necrotic-erythematous skin base on his forearm and right lower abdomen. The patient was treated with mometasone furoate smeared and antihistamines once a night. A characteristic of the sap is black on its woods. Compounds of the sap contain ursiol, rengol, glutarengol, laccol and thitsiol that are irritants causing the ICD.

Abstract

Background: Iron Deficiency Anaemia (IDA) is one of major factors for human health, especially young children. Iron supplementation can indicate a good response in haemoglobin concentration. Objective: This study is to observe prevalence of anaemia and effectiveness of iron supplementation for children in Purbalingga district, Central Java.Methods: This study was a quasi-experimental study by measuring haemoglobin concentration before (pre) and after (post) treatment. Its participants were determined by a purposive sampling method. 64 children (less than 5 years old) were included in criteria of anaemia. Their blood samples were collected before the treatment to measure SI (Serum Iron) and TIBC (Total Iron Binding Capacity). Administering 4 mg of iron per kg of body weight per day was conducted for a month. Data of participants such as age, nutritional status, socio-demographic, and medical history were also collected. A paired t-test was used to analyse mean differences of haemoglobin levels before and after treatment.Results: Among the 64 children with anaemia, 98.4% indicated Iron Deficiency (ID) with 59.4% of them indicating IDA based on level of SI and TIBC. Anaemia has more prevalence in group of 6 - <12 months of age. There was a statistically significant increase of haemoglobin level with a mean difference of 1.52 g/dL between the pre and post treatment (p=0.000).Conclusion: The prevalence of IDA in children (less than 5 years old) in Purbalingga District Hospital was 59.4%. Adequately oral iron supplementation for a month indicated a statistically significant increase of haemoglobin levels

Abstract

Atopic dermatitis is hypersensitivity respond that occurs by exposure to allergens environment, which is inheritance in familial pattern. The role of lgE is importance in pathogenesis of atopic dermatitis. Exposure of House dust mite could be JgE specific laboratory examinations were increased and causing relapse of Atopic dermatitis, the patch tests used of house dust mite result are positive in atopic dermatitis.

Abstract

Three years after great earthquake on May 2006, Bantul has succeeded to be recovered in many sectors, including health. However, Bantul still face many health problems that should be overcome in their limited resources. Therefore,this research identify, analyze, and set the priorities of health problems in order to give recommendations-for eficient and effective health policies and program planning in Bantul. This analysis is a descriptive observational research whichanalyzes secondary data from the Health Department of Bantul District. Health problem priority was set using Hanlon method. In a series, the top ten priority health problem are Maternal mortality, HIV/AIDS, Infant mortality, Tuberculosis,Diabetes Mellitus, Dengue Haemorhagic Fever, Hypertension, Malnutrition, mortality of children under five, and Diarrhea. Thereby, Bantul district still has double burden. In one side, infectious diseases are still in a high rate. On theother hand, the degenerative diseases also proceed to increase and reach a great number of sufferer.

Abstract

Autism had become a problem which is experienced by the family around the world, and autism happened on 5 among every 10.000 birth. Autism can be classified as light autism, medium autism and heavy autism. With having autism child, will give the anxiety to their parent especially the mother. The objectives of the research is to determine the relation between the autism children classifications based on CARS with the level of mother anxiety in autism SLB of Bina Anggita. This research is in <J nalytically non-experimental research with using cross-sectional method. Numbers of thesubjects used are 23 children with their mother from autism SLB of Bina Anggita. The data colleted with two ways, first direct observation to the autism children and the second filling the form of questionnaire by the mother. The analysis of the data used is fisher test. The classification of autism in Bina Anggita autism is generally as light autism to heavy autism (52,2%). The level af anxiety happened to the mostly mother are the light anxiety to medium anxiety {65,2%). There is no significant relation between the classification of autism child based on CARS with level of mother's anxiety (p=0,67).

Abstract

Drug abuse (Narcotics, psychotropics, and addictives) as social illness is harmful to various aspects of human life such as health, education, security, and social economy, and this is a serious threat to Indonesia as a big nation. Drug abuse has strong relationship with str ss triggered by stressor. Some researches report that to prevent stress, social support is one of important sources applied beside constitution, intelligence, financial resources, faith and religion, hobbies and aspirations. The objective of this study is to determine the relationship between social support and stress among drug abuse inmates in Yogyakarta Narcotics Penitentiary. We used descriptive analysis with cross sectional method. This correlational study was conducted in Yogyakarta Narcotics Penitentiary with 52 subjects using stress scale and Social Support Questionnaire. Analysis on correlation using Kendall Tau formula. 51,9% of total respondent had medium stress level and 57,7% had very high social support level. Statistical test found significant relationship between stress with social support ( P-value = 0.002, t = -0 .328) and duration of drug abuse (P-value = 0.015, t = 0.312). There is significant relationship between social support and stress among drug abuse inmates in Yogyakarta Narcotics Penitentiary.
