Abstract

Background: Chronic kidney disease (CKD) imposes a significant global burden, with diabetes and hypertension being major contributors. In the severe stage, CKD patients require long-term hemodialysis (HD) treatment and rely on family caregivers for support. Caregivers often experience mental health challenges, including anxiety and depression, which can impact their quality of life and patient outcomes.Objective: This study aimed to identify the anxiety and depression incidence in HD family caregivers at Soehadi Prijonegoro Regional Public Hospital (SPRPH), identify the risk factors associated with these conditions, and investigate the relationship between anxiety and depressionMethods: A cross-sectional analysis was carried out at the Hemodialysis Department from November 2022 to February 2023. Caregivers accompanying CKD patients undergoing HD participated voluntarily. Anxiety and depression levels were evaluated using the Hamilton Anxiety Rating Scale (HAM-A) and Hamilton Depression Rating Scale (HAM-D). Results: Out of 106 participants, 66% were female. Most caregivers (99%) experienced anxiety, with 91% classified as mild and 8% as mild to moderate. Depression was reported by 42%, with 2% experiencing mild to moderate symptoms. Spearman’s analysis revealed a significant correlation between anxiety and depression (p=0.000). No specific caregiver characteristics were found to be associated with anxiety or depression.Conclusion: Family caregivers of HD patients demonstrated a high incidence of anxiety and a lower prevalence of depression. Anxiety levels were correlated with depression levels. We did not identify specific risk factors for these mental health conditions among caregivers. These findings emphasize the need for mental health screening and appropriate management to improve the quality of life for HD family caregivers.

Abstract

Mesenchymal prostate tumours are relatively rare compared to epithelial tumours. To diagnose mesenchymal tumours on transurethral resection of the prostate (TURP) specimen, doctors have to consider a few differential diagnoses, including gastrointestinal stromal tumour (GISTs). We reported a 63 years-old male patient, presented with urinary retention in one week. Based on an initial scan of the abdomen, a large lobulated mass in the pelvic cavity from the prostate pushed up the urinary bladder. Histopathologic examination demonstrated cellular spindle cell neoplastic proliferation, a moderate degree of atypia, and a mitotic count of >5 per 50 high-power fields (HPFs) with a fascicular growth pattern. Immunohistochemically, the tumour had positive expression for CD117 (c-KIT), CD34, and discovered on Gist-1 (DOG-1), while smooth muscle actin (SMA) and S-100 were negative. The pathological report was consistent with a high-risk group of GISTs in the prostate. Subsequent imaging revealed that the tumour mass was centrally located between the rectum and prostate, infiltrating bladder and liver metastases. Comprehensive differential diagnoses of mesenchymal tumours involving the prostate are necessary because of the inadequacy of specific clinical signs, symptoms, and unexpected location. Diagnoses of GISTs was performed based on spindle cell pattern and the positive immunohistochemistry expression for CD117, CD34, and DOG-1. The tumour mass involved in the prostate with spindled morphology should be considered as a GIST, assisted with appropriate immunohistochemistry marker panel. It is a challenge to diagnose the GIST particularly involving the prostate.

Abstract

The development of smartphones worldwide has rapidly increased the number of smartphone users. Although smartphones have many benefits, prolonged use causes various visual disorders, of which transient smartphone blindness (TSB) is one of the most important concerns. This study aims to analyse the phenomenon of TSB and explore its etiology, pathophysiology, symptoms, and examination to determine the diagnosis and management. Transient smartphone blindness is a new physiologic phenomenon caused by exposure to a certain level of smartphone bright light and the body posture of the smartphone user. It leads to the use of monocular vision to stare at the smartphone. By exploring recent literature, this study found that patients with TSB experienced retinal photopigment bleaching, which decreased light sensitivity. Typically, the sensitivity returned physiologically after several minutes. Patients with this blindness showed several symptoms. For example, they perceived shapes, outlines, or figures during the blackout. The diagnosis of TSB should be primarily based on a detailed history of smartphone use, with the differential diagnosis based on circumstance or occurrence. This diagnosis is useful to distinguish it from transient ischemic attack (TIA). Transient smartphone blindness is a basic monocular transient loss that does not require therapy or drugs. However, it was found that the reassurance of patients’ condition and suggestions on smartphone use were sufficient for the intervention. With a limited scientific study of TSB, this review is the first to raise the discussion topic. It is expected to help physicians to diagnose TSB significantly. In addition, this study will enrich the literature, particularly on the scientific and clinical mechanisms of recognising TSB for clinical diagnosis and management.

Abstract

Background: Diabetic Nephropathy (DN) is the most common complication of diabetes mellitus. Interleukin 6 (IL-6) known as pro-inflammatory cytokine increases extracellular matrix expression and proliferation in DN progression. One single nucleotide polymorphism (SNP) of the Interleukin (IL)-6 gene, rs1800796 play a role in the progression of DN among Asian. Objective: This research seeks to investigate the correlation between IL-6 gene polymorphism (rs1800796) and diabetic nephropathy in individuals diagnosed with type 2 diabetes mellitus (T2DM) among the Balinese population.Methods: This research employed an observational approach through a case-control design involving a total of 60 subjects for each group selected by a simple random sampling method. The occurrence of the rs1800796 polymorphism was identified through polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). The genotype and allele distribution acquired in this study were assessed using the chi-square (χ2) test and Hardy-Weinberg equilibrium law.Results: There were no significant differences identified in the occurrence of (DN) based on the genotype or allele distribution of the IL-6 gene rs1800796. Genotype CG-GG had a higher risk of becoming DN with Odd ratio 1.230, but not statistically significant (p=0.577). Allele G with OR 1.198 (p=0.502) was not significant in causing DN. Conclusion: No significant association was identified between IL-6 polymorphism (rs1800796) and diabetic nephropathy in Balinese individuals with type 2 diabetes mellitus. The lack of association could be due to the presence of other haplotypic connections or might require a larger sample size for conclusive results.

Abstract

Background: Tuberculosis (TB) is one of the deadliest infectious diseases worldwide, causing approximately two million deaths annually. Multidrug resistance presents a significant global challenge in TB treatment, necessitating prolonged medication use and extended treatment durations. There is an urgent need to discover alternative novel antimicrobial compounds. Herbal medicine offers promising prospects for drug discovery and development, warranting the screening of plant-derived compounds with favorable safety margins. Objectives: This study aimed to investigate the in vitro antimicrobial activity of extracts from five medicinal plants against Mycobacterium tuberculosis in vitro.Methods: Extracts from five herbs —Morinda citrifolia L, Nicotiana spp.L, Centella asiatica L, Allium sativum L, and Annona muricata L, —were investigated for their in vitro antimicrobial activity against M. tuberculosis using Lowenstein-Jensen medium. Antimicrobial activity was assessed by comparing bacterial colony growth on extract-containing media with that on extract-free control media. The agar dilution method was used to determine antimicrobial activity.Results: Among the tested extracts, Nicotiana spp L and Annona muricata L exhibited the strongest antimicrobial activity. These herbs contain various bioactive compounds, including alkaloids, flavonoids, and steroids, which are presumed to contribute to bacterial susceptibility. Conclusions: The findings suggest that Nicotiana spp. L. and Annona muricata L. may have therapeutic potential in TB treatment. These herbs could serve as alternative agents for combating TB and may help mitigate the adverse effects associated with standard anti-tuberculosis drugs.

Abstract

Background: The sexual orientation of students is influenced by various factors. Islamic boarding school is one of the places where risky sexual orientation can occur among students. This behaviour must be addressed so that it does not cause more severe health impacts.Objective: This study aims to explore the risk and protective factors of sexual orientation from the experiences of students at Islamic Boarding School in Jember Indonesia. Methods: This study was a qualitative study with a phenomenological approach. There were 16 informants consisting of key informants, main informants, and additional informants. The research informants were selected using the snowball sampling techniques. In-depth interviews were conducted to study the risk and protective factors of sexual orientation among Islamic Boarding School students in Jember, Indonesia. Data were analysed using thematic content analysis. Results: The study showed that, in terms of risk factors, all informants had been victims of bullying because of a negative social, which made the informants feel depressed. All informants experienced mental trauma due to stigma and discrimination which caused the informants to feel depressed. The presence of protective factors was considered beneficial for individuals because this condition transformed negative emotional perceptions into positive perceptions, thereby strengthening personal development and resilience. The influence of religion and internal factors have a strong influence on making individuals have heterosexual desires and helping them recover from the bad effects experienced during periods of homosexual behaviour. Conclusion: Factors that influence students’ sexual orientation towards homosexuality include discrimination, weak individual resilience, and oppression. Based on this, assistance is needed to help students with homosexual orientation maintain a stable condition.

Abstract

Dhikr is a form of Islamic prayer that involves repeating certain phrases or words to focus on God's presence. This article proposed the management of coronary heart disease (CHD) through changes in individual character through reminiscence. This review aims to provide an overview of current knowledge on how dhikr practice can affect cardiovascular health. We conducted a systematic search across various databases such as PubMed, Google Scholar, and Scopus, utilizing keywords like "dhikr," "coronary heart disease," "cardiovascular health," "stress reduction," and "prayer." Only peer-reviewed articles published between 2014-2023 were eligible for inclusion in this review. Articles were chosen based on their pertinence to the subject matter and their methodological robustness. Several studies have demonstrated that regular dhikr practice can lead to reduced stress levels and improved mental health outcomes. Additionally, there is growing evidence suggesting that dhikr may also play a role in promoting physical health by lowering blood pressure and cholesterol levels. The total number of papers extracted from the initial search with the specific keyword was more than 50. The most relevant papers were selected based on their alignment with the basic theory of CHD, risk factors, mechanisms, and therapeutic approaches. These findings suggest that dhikr could potentially serve as a valuable tool for preventing CHD. However, more research is needed to fully understand the underlying mechanisms behind these effects. Based on the available data, it appears that dhikr has the potential to reduce stress and improve overall health, making it a promising intervention for preventing CHD. Further research is necessary to better elucidate the exact mechanisms through which dhikr exerts its beneficial effects on cardiovascular health.

Abstract

Arteriovenous malformations (AVMs) are congenital vascular disorders. In their early phase, they often go unrecognized due to their asymptomatic nature until they develop clinical symptoms such as swelling, skin discoloration, and signs of arteriovenous shunting. The high flow from the arteries directly into the veins may potentially increase the risk of vessel rupture. We reported a case of a 54-year-old male who complained of back bleeding, accompanied by pain and sores that had persisted for the previous three days. The patient had been aware of a mass since birth, progressively worsening with age. A physical examination revealed bleeding from the pulsating mass and measured approximately 17.5 cm x 15 cm x 3 cm on the back. The patient was diagnosed with ruptured AVM by Computed Tomography (CT) angiography. He was managed with endovascular coil embolization, polyvinyl alcohol particles, and skin grafts. AVMs are often asymptomatic; an ultrasound examination can be the initial diagnostic tool, while CT Angiography is considered a gold standard. AVMs are managed through embolization and surgical intervention. Regular follow-up is necessary due to the potential for recurrence.
