Abstract

Dermatitis stasis merupakan penyakit kulit inflamasi yang umum terjadi di ekstremitas bawah, ditandai dengan eritema, skuama, erosi, krusta, dan terkadang juga dapat ditemukan vesikel, serta ulserasi kecil di kulit yang dikenal sebagai ulkus stasis atau ulkus venosum. Faktor risiko dermatitis statis antara lain usia di atas 50 tahun, obesitas, dan jenis kelamin perempuan. Dilaporkan tiga kasus dermatitis stasis pada pasien geriatri laki-laki yang datang ke Rumah Sakit dr.Cipto Mangunkusumo dan ketiganya disertai komorbiditas obesitas. Tata laksana pada ketiga pasien ini melibatan departemen lain (multidisiplin) dan membuahkan hasil yang baik.

Abstract

Platinum-based chemotherapy regimens with two types of drugs, such as paclitaxel-carboplatin and pemetrexedcarboplatin, are first-line therapy for pulmonary adenocarcinoma patients with negative epidermal growth factor receptor (EGFR) mutation. This study aimed to determine the efficacy, toxicity, and cost profile of paclitaxel-carboplatin compared to pemetrexed-carboplatin. This is a cross-sectional study. Pulmonary adenocarcinoma negative EGFR mutation naïve patients treated with paclitaxel-carboplatin or pemetrexed-carboplatin were included. Effectiveness was assessed based on the overall response rate (ORR) according to the response evaluation criteria in solid tumours (RECIST). A pharmacoeconomic analysis is performed based on clinical outcomes consisting of effectiveness and direct medical costs. Medical records from 21 patients with paclitaxel-carboplatin and 21 patients with pemetrexedcarboplatin were successfully evaluated. The effectiveness of the two chemotherapy regimens was not significantly different (OR, 1.25; 95% confidence interval, 0.34 to 4.64; p = 0.739). Frequent haematological toxicities experienced in the two groups were grade 1-2 anaemia, neutropenia, leukopenia. Grade 3 anaemia, leukopenia, and neutropenia were more common in the paclitaxel-carboplatin group. The two groups’ nonhematological toxicities were nausea vomitus and hair loss, with peripheral neuropathy more experienced by the paclitaxel-carboplatin group. Costminimization analysis reveals that the average cost per patient with pulmonary adenocarcinoma negative EGFR mutation with paclitaxel-carboplatin regimen was cheaper IDR 10,986,257.55 or 50.25%, compared to pemetrexedcarboplatin. In conclusion, there was no significant difference in the effectiveness of the two regimens. The most common adverse effects in both regimens were haematological toxicities. The average cost per patient with a paclitaxel-carboplatin regimen was cheaper compared to pemetrexed-carboplatin.

Abstract

Autoimmune hemolytic anemia (AIHA) is often presented with severe anemia. Currently, several centers in Indonesia use washed red blood cells (RBCs) to transfuse AIHA patients. However, washed RBCs are not widely available, causing a delay in therapy. We herein report two cases of pediatric AIHA who presented with severe anemia with hemoglobin level of 4.1 g/dL and 5.3 g/dL. Both patients were delayed for two and five days to be given blood transfusion because of the unavailability of washed RBC in Sorong General Hospital and Fatmawati Hospital and must wait to be referred to RSCM to receive washed RBCs. We conducted literature searches to review several articles that focus on AIHA transfusion management. AIHA is characterized by the production of autoantibodies directed against antigens on the surface of RBCs. RBC washing is not currently required in AIHA patients. It is also not widely available, takes a long time to process, and can cause a delay in emergency cases. Although RBC transfusion is not contraindicated in AIHA, its use should be limited to life-threatening cases of anemia with signs of cardiac and neurological compromises. In emergency cases, ABOmatched and Rhesus-matched RBCs can be safely administered if alloantibodies are reasonably excluded based on the previous transfusion and pregnancy history. ABO-matched and Rh-matched RBC transfusion is safe as AIHA’s transfusion management for patients with severe anemia with clinical symptoms.

Abstract

Upper airway cough syndrome (UACS), previously known as post-nasal drip syndrome, is a persistent cough that lasts more than four weeks in children or more than eight weeks in adults, which is associated with nasal and sinus diseases, such as allergic rhinitis (AR), chronic rhinosinusitis (CRS), and adenoid hypertrophy (AH). UACS is one of the major causes of chronic cough. Several theories support the pathophysiology of chronic cough in UACS, namely post-nasal drip, airway inflammation, and sensory neural hypersensitivity theories. Persistent and excessive cough generates a deterioration in patient’s quality of life and social interaction. This review summarizes the knowledge on pathophysiology and underlying disease of UACS, aims to help clinicians identify and treat this syndrome.

Abstract

Chronic kidney disease (CKD) patients have higher risk to develop cardiovascular disease (CVD) than non-CKD patients. Neutrophil lymphocyte ratio (NLR) in CKD patients reflects inflammation status and role as complementary prognostic marker to evaluate cardiovascular risk in stage 3 to 5 CKD. High prevalence of vitamin D deficiency commonly found in CKD patients, leads to endothelial dysfunction and increase inflammation. NGAL is used as renal injury biomarker but nowadays NGAL has been known plays important role in CVD pathophysiology. CVD identification in CKD patients is necessary to obtain CVD risk and to stratify mortality earlier in CKD patients. This study aimed to obtain the differences and continued to obtain cut off of NLR, vitamin D and NGAL in CKD patients undergoing hemodialysis (HD) with and without CVD if the differences was significantly. A cross-sectional study was conducted in dr. Cipto Mangunkusumo National Hospital Jakarta, in December- February 2020. Total subjects were 83, consists of two groups with and without the CVD. There were no significant differences of NLR and vitamin D in CKD patients undergoing hemodialysis (HD) with and without CVD. The significant difference was found only in NGAL with cut off 5.64 ng/ml. NLR was lower, meanwhile vitamin D and NGAL were higher in CKD patients undergoing HD with and without CVD.

Abstract

Radiotherapy can potentially cause damage to the salivary gland, muscles, and nerve that is important to oropharyngeal swallow, leading to xerostomia and dysphagia. Reporting a case of radiotheraphy-induced xerostomia and dysphagia in HNC patients. A 34 years old man with NPC stage IV B (T4N3M0) came with difficulty swallowing, dryness in the throat and mouth, coughing while eating and drinking, choking, hoarseness, and pain when swallowing. The patient had done 14 times of radiation administration with a total dose of 60 Gy with conformal 3D radiation technique. The amount of saliva measured 0.02 ml/minute. A standing secretion was found in the vallecula, right and left piriformis and postkrioid sinuses in preswallowing assessment. The presence of penetration or aspiration of secretions into the airway were detected. The patient was diagnosed with neurogenic dysphagia and advised to use NGT for diet and consult medical rehabilitation. Literature searching was conducted on March 31, 2021 in the 3 journal database, including PubMed, Cochrane, and EBSCOhost, using particular keywords based on PICO. The inclusion criteria were full text article, observational studies, case-control, cohort, randomized controlled trial, systematic review, written in English, and studies investigating the correlation between HNC patient with radiotherapy and dysphagia or xerostomia. There were 3 systematic reviews, 1 cohort, and 2 cross-sectional studies investigating the correlation between chemoradiotherapy in head and neck cancer and xerostomia/dysphagia. Chemoradiotherapy correlated with dysphagia and xerostomia in head and neck cancer. Xerostomia and disfagia were prevalent in HNC patients after radiotherapy. Besides, there was association between the dose of radiotherapy and incidence or severity of xerostomia and dysphagia.

Abstract

Mitochondria are highly dynamic organelles that can adapt to different environmental stimuli. This study aims to analyze differences in the function and biogenesis of mitochondria and anaerobic glycolysis activity between obese adult rats that were undernourished in childhood (LCD28) and those which were normal (SD28) or already fat (HFD28). Sprague-dawley rats were assigned groups and given diet treatments from December 2017 until August 2018 in the Laboratory Animal Management Unit, Faculty of Veterinary Medicine, Bogor Agricultural Institute. To observe mitochondrial function and biogenesis, MnSOD activity and PGC1α levels were analyzed using ELISA. To investigate the existence of anaerobic glycolysis, LDH activity was analyzed using a spectrophotometry method. The results showed that MnSOD activity and PGC1α levels of LCD28 increased compared to the other groups, signifying higher mitochondrial function and biogenesis. Meanwhile, no significant difference in LDH activity was found, signifying similar levels of anaerobic glycolysis.

Abstract

Di Indonesia infeksi cacing usus masih menjadi masalah kesehatan yang penting. Hal ini dikaitkan dengan higienitas dan sanitasi yang buruk, kekurangan air bersih, kekurangan nutrisi, serta kontak dengan tanah yang berisi telur cacing. Penelitian ini bertujuan untuk mengetahui perbandingan prevalensi infeksi cacing dan dihubungkan dengan kawasan pedesaan dan perkotaan. Penelitian menggunakan desain cross-sectional berbasis sekolah yang dilakukan pada tahun 2018. Sebanyak minimal 7 siswa dipilih secara acak dari 30 sekolah setingkat taman kanak-kanak atau Sekolah Dasar di Kabupaten Bogor (sebagai kawasan pedesaan) dan Kota Administrasi Jakarta Barat (sebagai kawasan perkotaan). Total sample feses yang terkumpul sebanyak 610 dan diperiksa di Laboratorium Parasitologi Klinik, Fakultas Kedokteran Universitas Indonesia. Sampel feses dibuat sediaan dan diwarnai dengan lugol kemudian diperiksa secara langsung dengan mikroskop. Data diolah dengan SPSS versi 20.00 Prevalensi infeksi cacing usus di kawasan rural 3,2% sedangkan di kawasan urban 1% (chi square, p = 0,09; OR = 4,33; IK 95% = 0,93 – 11,99). Infeksi cacing usus didominasi spesies Ascaris lumbricoides.  Infeksi cacing memiliki prevalensi relatif rendah. Pada penelitian ini tidak terdapat perbedaan prevalensi infeksi cacing usus pada anak di kawasan pedesaan dan perkotaan.

Abstract

Ovarian cancer is the fourth most cancer in Indonesia. Treatment of ovarian cancer is surgery. Malnutrition may complicate healing, so studying the association between malnutrition and surgical outcome in ovarian cancer patients is necessary. A cross-sectional study was conducted in Dr. Cipto Mangunkusumo General Hospital from July 2018 - March 2019. fromo 63 patients underwent surgery, four were excluded due to non-malignant ovarian pathology results. Parameters assessed were characteristic subjects and risk factors of malnutrition in ovarian cancer patients. Malnutrition was assessed using the Patient-Generated Subjective Global Assessment (PGSGA) and surgery factors assessed were the length of surgery, size of the tumor, and blood loss during surgery. The nutritional status was classified as well-nourished and malnourished. The receiver operating characteristic (ROC) curve is used to assess malnutrition, length of stay, size of the tumor, and blood loss during surgery. The result shows that the prevalence of malnutrition in ovarian cancer was 78% (moderate was 42.4% and severe was 35.6%), and the median length of stay was eight days. There is an association (chi-square test, p<0.05) between malnutrition and surgical outcome in ovarian cancer patients. Malnutrition is associated (chi-square test, p<0.05) with a length of stay and increased blood loss during surgery.

Abstract

Model 4C/ID adalah desain instruksional yang menciptakan lingkungan pembelajaran berbasis keseluruhan tugas. Kuesioner Satisfaction Scale for e-Learning Process merupakan kuesioner untuk mengukur kepuasan peserta didik terhadap proses pembelajaran daring. Penelitian ini bertujuan untuk melakukan adaptasi kuesioner Satisfaction Scale for e-Learning Process versi bahasa Indonesia. Penelitian terdiri atas empat tahap yaitu forward translation, backward translation, expert review, dan pilot study. Forward translation adalah penerjemahan kuesioner ke bahasa Indonesia oleh penerjemah bersertifikat. Backward translation merupakan penerjemahan kembali kuesioner ke bahasa Inggris oleh penerjemah yang berbeda. Expert review adalah proses validasi konten oleh 8 orang ahli menggunakan skala CVI dan S-CVI. Pilot study dilakukan terhadap 30 mahasiswa yang dipilih secara acak. Forward dan backward translation menunjukkan kesesuaian maksud dengan kuesioner aslinya. Nilai I-CVI menunjukkan bahwa 28 butir kuesioner valid (skor >0,75), dan satu butir tidak valid (skor <0,75). Nilai S-CVI (skor ≥0,90) menunjukkan bahwa keseluruhan kuesioner adalah valid. Kuesioner Satisfaction Scale for e-Learning Process versi bahasa Indonesia adalah valid, sehingga dapat digunakan sebagai instrumen untuk mengevaluasi penerapan model 4C/ID pada pembelajaran keterampilan klinis secara daring.
