Abstract

The current implementation of JKN brings major changes in the health care and financing system, especially in hospitals. The payment system in hospitals has changed, which previously used a fee-for-service system, was changed to a prospective payment system using the INA-CBGs rate and BPJS Kesehatan was appointed as the implementing institution for the program. The INA-CBG's payment system was chosen because it is possible to control health costs, encourage quality services according to standards, limit unnecessary or excessive health services and encourage providers to carry out cost containment. This study aims to determine the impact of the implementation of the JKN program on health costs in FKTL. This research is a literature review (narrative review) by conductinga literature review of 12 journals. Journals were obtained from Google Scholar and PubMed searches with the keywords Impact of JKN on Healthcare Costs in FKTL OR Hospitals which were selected with the criteria of national and international journals with good reputation and indexed by SINTA and SCOPUS which were published no later than 2016. the purpose of this study and passed the selection. The results of the analysis show that the impact of JKN, especially on health costs, has an effect on the pattern ofJKN utilization by the community, patient satisfaction, hospital efficiency, Gross Profit Margin and Net Profit Margin of hospitals, and has an effect on hospital income. It can be concluded that there has been an influence on FKTL since the JKN era entered Indonesia.

Abstract

Sugarcane workers have a higher risk of being exposed to pesticides where pesticides have high toxicity. One of the most common diseases is kidney disorders, including acute kidney injury and chronic kidney disease of unknown disease or known as mesoamerican nephropathy. This study is a literature review to determine the comprehensive management of kidney disorders in sugar industry workers. A comprehensive review was conducted by searching for national and international journals in the last 6 years which were identified according to the topic. Kidney disease in sugarcane workers can take the form of acute kidney injury in the acute phase and become chronic if the risk factors cannot be overcome. Risk factors for kidney disease in sugarcane workers are dehydration, pesticides, heat stress, strenuous physical activity, exposure to sugarcane burning smoke. Preventive steps that can be taken include providing good fluid intake, providing good rest areas, setting working hours, modifying special backpacks for water bottles, procuring appropriate personal protective equipment and using bagasse to reduce cadmium ions in pesticides. If kidney disease has occurred, monitoring and treatment is carried out according to the level of the disease so that it does not get worse. Sugar industry workers have various risks for the occurrence of kidney disease. Management that includes promotive, preventive, curative and rehabilitative can be done to provide comprehensive services for kidney disorders in sugarcane workers.

Abstract

Work in steel mills is close to exposure to dust, fumes, and gases. Pollutants from activities in factory workers such as iron dust and other particles can increase the permeability of the respiratory tract mucosa. Iron dust can cause damage to lung surfactant and lung function and quartz can pose a risk of silicosis, lung cancer, cardiovascular disease, and mortality. This study uses several literature reviews through several international journals from 2015 to 2020. Search articles related to the risk of respiratory disorders in steel factories. Exposures to factory workers include metal fumes, dust, silica, acid gases. Respiratory disorders appear in the form of clinical symptoms which are divided into chronic and acute symptoms as well as changes in lungfunction as seen from changes in FVC and FEV1 from spirometry examination. The influencing factors are metallic agents that can be inhaled, the length of time workers are exposed to, working in confined spaces, lack of adequate ventilation, lack of occupational safety and health training, and worker compliance in the use of personal protective equipment. The risk of respiratory disorders in steel industry workers is a decrease in lung function accompanied by acute and chronic respiratory disorders. There are several factors that affect respiratory disorders in workers so that steel production companies are advised to take a comprehensive holistic approach in dealing with the risk of respiratory problems in steel factories.

Abstract

Vaccination programs are one of the most cost-effective and efficient medical therapies in history. This study aims to elicit an immune response after the COVID-19 vaccination and the occurrence of herd immunity. This research is a literature review research by conducting journal searches carried out with the keys: immune response to Covid 19, Covid 19 Vaccination and Herd Immunity. Articles are collected using search engines such as google pubmed, schooler, EBSCO, Sciencedirect, and Proquest. The criteria for the articles used are those published in 2020 – 20232. The Covid 19 virus is a member of the Sarbecovirus subgenus and is distinguished by a stick-shaped spike protein that protrudes from the virion's surface. Global immunization initiatives boost immune systems. Immune cells and proteins that circulate in the body have the ability to  provide protection from illness. Prior to immunization, antibodies from patients who had already been exposed to the virus resembled those produced following the first vaccination in healthy individuals. In people who were previously infected after the first vaccination, the level is the same as in people who were not infected after the second vaccination, booster administration has been shown to increase immunity with a 1.5 to 2-fold increase in titers and herd immunity is the only way to end the Covid 19 pandemic.

Abstract

One of the most common occupational diseases in the world is musculoskeletal disorders. Tobacco farmers have ahigher risk of musculoskeletal disorders in the form of lower back pain than other farmers because transdermalexposure to nicotine to tobacco farmers has an impact on intervertebral disc nutrition and increased levels of proinflammatory cytokines. The purpose of this study was to analyze the determinants of musculoskeletal disorders intobacco farmers in Antirogo Village, Jember Regency, which is known as the largest tobacco producing district inEast Java. This research is an analytic observational study with a cross-sectional study design. The number of samplesis 53 people based on the Slovin formula with the consecutive sampling method. Musculoskeletal disorders weremeasured using the Nordic Body Map. Data analysis used was descriptive univariate analysis and bivariate analysisusing the Spearman test, Chi-Square 2x3, and Kolmogorov-Smirnov. Significant variables were re-analyzed bymultivariate through ordinal logistic regression analysis. The results showed that most of the tobacco farmers inAntirogo Village, Jember Regency experienced musculoskeletal disorders with moderate complaints (45.3%) mostlyin the waist (33.9%), upper neck (32%), and back (30.1%). Bivariate analysis showed that the significant variableswere age (p= 0.000), years of work (p= 0.000), and work position (p= 0.008). When followed by multivariate analysisit appears that the only significant variable was years of work (p = 0.003). It can be concluded from this study thatthe determinant of musculoskeletal disorders in tobacco farmers in Antirogo Village, Jember Regency is years ofwork.
