Abstract

Leprosy relapse is defined as the reappearance of leprosy’s signs and symptoms following completion of adequate Multidrug Therapy (MDT) regimen. Indonesia is the third biggest contributor for leprosy relapse with 284 cases in 2018. Several factors contribute to the occurrence of relapse, which include presence of persisters, history of inadequate therapy, drug resistance, history of irregular treatment, history of dapsone monotherapy, high initial bacteriological index, high number of skin and nerve lesions, negative lepromin test, history of antireaction therapy, human immunodeficiency virus (HIV) infection, and pregnancy. Diagnosis of relapse can be established based on clinical, bacteriological, and therapeutic criteria, also histopathological and serological criteria, if possible. With those criteria, it should be easier to differentiate relapse from reaction, resistance, reactivation, and reinfection. The principal management for leprosy relapse is re-administration of MDT according to the type of relapse; however, therapy should be modified if there are any evidences of drug resistance. Things which are important to be taken into account during relapse treatment are leprosy type, past medication history, and the possibility of drug resistance. With proper knowledge and diagnostic algorithm, it is expected that the treatment can be administered immediately in order to prevent disability and transmission of infection.

Abstract

E-cigarettes also known as electronic nicotine delivery systems (ENDS) are device for delivering nicotine and other liquids by using heat energy from battery power using heat energy from battery power by converting liquids into aerosol. E-cigarettes were create as a safer option than conventional tobacco products, meanwhile e-cigarettes still contain toxic and carcinogenic ingredients than conventional cigarettes. E-cigarettes were created as a safer option than alternative tobacco products, although aerosols from e-cigarettes still contain more toxic and carcinogenic ingredients than conventional cigarettes. Harmful effects of using electricity have been reported on the respiratory, cardiovascular and skin systems. Abstract

ABSTRACT Introduction: Coronavirus disease (COVID-19) is a global pandemic that caused unprecedented morbidity and mortality. Aside from respiratory symptoms, dermatological manifestations have been widely reported, with the predominant manifestation was the hair loss. Case report: A 32-year-old male resident doctor experienced a solitary patchy alopecia areata (AA) on vertex, started 1 month after being reinfected by SARS-CoV-2. Personal history was negative for autoimmune and trichological disorders, no family history with AA. Trichoscopy examination revealed yellow dots, black dots, broken hairs, and exclamation-mark hairs. Treated with topical minoxidil and tretinoin for three months with excellent response. Discussion: The SARS-CoV-2 infection and related anxiety or emotional stress due to two times COVID-19 infections could have triggered AA. SARS-CoV-2 infection induces an activation of cytokine cascade involving interferon to disrupt hair follicle immune privilege (HFIP). Meanwhile, psychological stress enhances neurogenic inflammations and induces a shift to the immune response to type 1 T-helper cells and increases apoptosis of epithelial cells that affects hair growth. There were three prior reports about new onset AA in post COVID-19. Our report is the fourth case of new onset AA in post COVID-19, but the only case with COVID-19 reinfection. Conclusion: Our report on new onset AA in post COVID-19 reinfection provides an insight into the possible association between SARS-CoV-2 infection and AA. The onset of AA in COVID-19 patient represents a sequelae of post COVID-19 caused by immune responses or psychological distress, and there appears to be a close association in timing between them.

Abstract

The Effect of Probiotics Lactobacillus plantarum in Management of Atopic Dermatitis ABSTRACT Lactobacillus plantarum is one of probiotics from Indonesian traditional food which is isolated from cow milk. One of the function from probiotics is reduce allergic reaction. Atopic dermatitis is skin inflammation which are chronic and recurrent with accompanied by allergic reaction. This finding to search the effect of probiotics Lactobacillus plantarum in management of Atopic Dermatitis with rated by SCORAD index, the effectivity giving by single or mixture of probiotics, and the side effects of probiotics. This finding using literature review as method. Populations in this finding are 10 articles about probiotics and atopic dermatitis in international journal which had SCIMAGO index at 2017 until 2022. The results of this finding are there is significancy decline of SCORAD index, the mixture of probiotics are better than single probiotic, and there is no side effects reported. The conclusion this finding is there is effect of giving probiotics Lactobacillus plantarum in atopic dermatitis management. Key words: atopic dermatitis, Lactobacillus plantarum, SCORAD

Abstract

Chronic spontaneous urticaria (CSU) is a mast cell driven skin disorder that is characterized by the development of recurrent itchy wheals for 6 weeks or more due to unknown causes. Diagnosis is purely clinical while assessment of disease activity, its impact on quality of life, as well as treatment efficacy are based on history, physical examination, and the use of questionnaires. However, questionnaires have the disadvantage of being subjective. In the last decade, studies have been focusing on understanding of chronic spontaneous urticarial pathomechanism and identification of potential biomarkers to determine disease activity, clinical course, and treatment efficacy. Some of the biomarkers that have been studied are D-dimer, C reactive protein, interleukin-6, FcεRI, and vitamin D. Biomarkers will then be classified into clinical and molecular biomarkers. With identification of biomarkers that considered to be more objective and reliable, personal approach in evaluating the course of the disease and the most suitable treatment for patients can be achieved.

Abstract

Background: Herpes zoster (HZ) is an acute infection caused by varicella zoster (VZV) reactivation that manifests clinically as groupings of vesicles and eruptions on a reddish skin base, along with unilateral radicular pain that is often localized to one dermatome. Case Ilustration: A 50-year-old woman arrived complaining of uncomfortable wet bumps on her left back. The patient has received consistent care at the Internal Medicine Polyclinic for the past 7 years despite having a prior history of rheumatoid arthritis. In the left posterior thoracic area, there was evidence of a dermatological condition. Groups of vesicles filled with clear fluid were observed there, along with skin that was unilaterally distributed and erythematous, zoosteriform, and plaque-based. Diagnosed with herpes zoster in the patient. Acyclovir, gabapentin, and paracetamol were administered orally. Mupirocin 2% cream and a wet compress with NaCl solution were used as topical treatments. Rheumatoid arthritis (methotrexate and sulfasalazine) treatment was stopped during HZ treatment. Skin blemishes had improved after receiving treatment for two weeks. Conclusion: After a prior varicella assault, the latent endogenous varicella zoster virus (VZV) in ganglionic neurons reactivates to cause herpes zoster. Herpes zoster risk will grow in immune-compromised states.

Abstract

Cellulite is characterized by skin topography alterations that have been described as peau d'orange, cottage cheese or mattress-like appearance. All age groups and both sexes could have cellulite, especially post-puberty females. Cellulite is more common seen in Caucasian than Asian woman. Based on the study of woman skin anatomy, fat cells and connective tissue are aligned vertically so that fat cells could protrude into the skin layer and cause cellulite. The pathogenesis of cellulite is controversial and the etiology are multifactorial. If the etiopathogenesis are already known, more effective treatments could be developed. Three main cellulite theories are adipocyte accumulation, water retention and septa fibrosis. Several combination theories associated with cellulit include genetic, hormonal, vascular insufficiency, chronic inflammation, dietary and neurological effects. Clinical evaluation and associated conditions are important for assessing the severity of cellulite and are useful for determining various treatment options. To date, cellulite treatment still a challenge, require long-term stability of influencing factors.

Abstract

Introduction: porphyria is a group of disorders caused by enzyme deficiency in heme synthesis pathway. Congenital erythropoietic porphyria (CEP) is an extremely rare cutaneous porphyria caused by mutations in the gene encoding uroporphyrinogen III synthase leading to accumulation of porphyrins in erythrocytes and tissues such as skin, teeth and bone resulting in hemolytic anemia, photosensitivity and erythrodontia. Case: we present two cases of CEP with typical manifestation of blistering neonatal-onset photosensitivity, erythrodontia and red-colored urine, as well as one case with hemolytic anemia. Both had no prior history of acute neurological episodes or abdominal pain that establish the cutaneous porphyria diagnosis. Pink fluorescence was visible on the teeth and urine under the Wood's lamp. Further investigations were not conducted. Discussion: the diagnosis of CEP can be established on its typical clinical manifestations and simple investigations with Wood's lamp, confirmed by examination of urine and feces porphyrins, identification of UROS or X-linked GATA 1 gene mutations. CEP management is in the form of comprehensive photoprotection and treatment with various disciplines because the clinical manifestations and severity of CEP vary in each patient. Early age of onset and severity of haematological symptoms are factors that determine poor prognosis. Abstract

Introduction: Hematological-related malignancy-induced eosinophilic dermatosis or He Remained is a rare paraneoplastic cutaneous reaction particularly associated with various hematological malignancies. It has a vast clinical presentation ranging from papules, plaque, to blisters and bullae and resembles other dermatoses. We report a case of He Remained on patient with chronic lymphocytic leukimia on remission. Case: A 49-year-old women who had been diagnosed with chronic lymphocytic leukimia presented with a pruritic rash which had been present for a month. Histopathology examination revealed eosinophilic infiltrate and the diagnosis of He Remained was established. She was treated with systemic steroid and achieve relieve of symptoms. Discussion: The pathogenesis of the dermatosis has not been fully interpreted. The cutaneous symptoms have been found to be unrelated to the hematologic malignancy course. Histopathology studies have an important role as one of the diagnostic criteria. Various treatments have obtained some response in short term; however treatments to control the chronic and relapsing course of the disease has been lacking. Conclusion: He Remained has a negative impact for patients and remains a diagnostic and therapeutic challenge for Dermatovenereologist. This case reports hopes to raise awareness given the rarity of the dermatoses.

Abstract

Introduction: Recessive dystrophic epidermolysis bullosa (RDEB) is a rare bullous disease caused by mutations in the COL7A1 gene encoding anchoring fibril collagen that maintains skin integrity. Case: Male, 67-year-old, presented with blisters and wound on various parts of the body, worsening in the recent year. They had been known to easily appear after trauma since. Vesicles-bullae, erosions-excoriations, milia, and atrophic scars were found on the axillae, groins, and limbs, as well as hyperkeratosis and anonychia of the hands and. Histopathological examination showed subepidermal cleft, milia and fibrosis, without immunoglobulin deposits on immunofluorescence. Discussion: Contrary to classical RDEB, blisters in mitis variant do not always cause impairment. The pronounced symptoms in advanced age might be mistaken for epidermolysis bullosa acquisita. Negative immunoglobulin deposition is the diagnostic key, confirming that blister does not result from an autoimmune process, but rather, from trauma to the abnormal skin, more so by flattening of the rete ridges in the elderly. In the absence of definitive therapy, treatment is symptomatic. Conclusion: The diagnosis RDEB-mitis might be missed in geriatric. Correct diagnosis has implication on management because not only does RDEB-mitis require no immunosuppressant, measures to minimize blister formation is more important to save patient from complication.
