Abstract

Introduction: Lipodystrophy in systemic lupus erythematosus (SLE) causes deformity and psychological problem for patients. The recommended treatment is autologous fat transfer (AFT) because of its effectiveness. Case: A 19-year-old woman with facial lipodystrophy due to SLE was treated with AFT. Fat tissue was aspirated from the lateral side of the left thigh and then processed into microfat and nanofat using the Luer-Lok connectors. The total volume of fat injected was 28 mL with a significant improvement of skin contour. She was satisfied with the results. There were no postoperative complications. She is still under monitoring for the next six months to assess AFT retention. Discussion: There are various modifications of the AFT technique to improve outcomes and avoid complications. Microfat aims to fill the volume loss, whereas nanofat is beneficial to improve skin contours. The main drawback of AFT is the unpredictable retention of fat volume. Patient factors, fat processing methods, and AFT technique play a role in post-AFT retention. Although extremely rare, fat embolization can occur during AFT which can be fatal. Injection with a blunt cannula, use of vasoconstrictors, and mapping of facial vascularization may reduce the risk of fat embolization. Conclusion: Autologous fat transfer is recommended for lipodystrophy because it is easy to perform, inexpensive, and non-immunogenic. Dermato-venereologists have an important role in the management of lipodystrophy due to SLE.

Abstract

Background: Leprosy is one of the oldest and most neglected diseases in the world. During the COVID-19 pandemic, leprosy reaction triggered by COVID-19 has not been widely reported. Case: A 38-year-old woman, complained of erythema nodules in the eyelids for 1 week, increasing in size and pain accompanied by fever and nasal congestion. There is no previous history of disease or treatment. Physical examination revealed increased body temperature and anemia with bilateral madarosis. Dermatological status showed multiple erythematous nodules with regional distribution on bilateral superior palpebrae that were palpable with pain. There were multiple lesions of erosion and excoriation in the bilateral plantar pedis region. Sensory function is impaired with stocking hypoesthesia of the digits and lateral plantar pedis bilaterally. The results of the laboratory examination were SARS-CoV-2 PCR positive, ELISA anti PGL-1 IgM 8,159 u/ml, IgG 1,421 u/ml and the Bacterial Index (IB) 3+. Histopathology HE demonstrating lobular panniculitis with vasculitis and FF negative AFB. Discussion: Based on the medical history, physical examination and laboratory, the patient was diagnosed with an Erythema Nodosum Leprosum (ENL) reaction in a new case of Morbus Hansen (MH), suspected to be triggered by a COVID-19 infection. Neutrophil activity and cytokine storms due to COVID-19 are thought to trigger the ENL reaction. Conclusion: In the pandemic era, COVID-19 infection needs to be watched out for as a possible trigger for leprosy reaction.

Abstract

Pendahuluan: Disfungsi ereksi menjadi perhatian di golongan homoseksual dan sering dikaitkan dengan depresi sebagai salah satu pemicunya. Tujuan: Mengetahui hubungan antara tingkat depresi dengan kejadian disfungsi ereksi di komunitas Lelaki Seks Lelaki (LSL) di Surakarta. Metode: Penelitian analitik observasional dilakukan di Surakarta periode Juni-Juli 2021. Subjek adalah 41 laki-laki berusia 20-50 tahun, orientasi seksual dengan sesama jenis dan menyetujui untuk mengikuti penelitian. Penilaian tingkat depresi menggunakan kuisioner Patient Health Questionnaire-9 (PHQ-9). Penilaian disfungsi ereksi menggunakan kuisioner International Index of Erectile Function (IIEF) dalam domain fungsi ereksi. Analisis data menggunakan uji korelasi spearman rank SPSS versi 21 dengan nilai signifikasi p<0,05. Hasil: Tingkat depresi dan faktor usia tidak memiliki hubungan yang signifikan dengan disfungsi ereksi (r=0,158 dan p=0,323; r=0,020 dan p=0,901). Variabel yang berhubungan signifikan dengan kejadian disfungsi ereksi adalah frekuensi hubungan seksual (r= -0,389 dan p=0,012) dimana semakin jarang subjek penelitian melakukan hubungan seksual maka semakin tinggi kemungkinan terjadinya disfungsi ereksi. Tingkat koefisien korelasi antara frekuensi hubungan seksual terhadap kejadian disfungsi ereksi adalah kategori lemah (r=0,200-0,399). Kesimpulan: Tingkat depresi tidak berkorelasi terhadap kejadian disfungsi ereksi, namun kejadian disfungsi ereksi berkorelasi negatif dengan frekuensi hubungan seksual pada komunitas LSL di Surakarta.

Abstract

Introduction: Squamous cell carcinoma (SCC) of the penis is a rare malignancy originating from the epithelial cells of the inner prepuce or glans penis. The most common feature is the presence of a visible and palpable lesion on the penis, which is accompanied by pain, discharge, bleeding, or foul odor. Case report: A 66-year-old man came to the skin and genital clinic of Adam Malik Hospital with complaint of a lump in the penis which had been getting bigger since 3 months ago. Since 5 months ago the patient complained of sticky and painful foreskin on the penis. There was no history of trauma or warts. The patient is not circumcised and a heavy smoker. Discussion: Not circumcised are one of the risk factors for an increased incidence of malignancy of the penis. SCC of the penis has a predictable pattern of metastases, with regional lymph nodes appearing first, followed by deep lymph nodes and then distant metastases. The results of the pathology examination showed the presence of cancer cells originating from the squamous epithelium that were still well differentiated. This examination can determine the type of cell, the degree of pathology, involvement of the corpora cavernosa, corpora spongiosum, inguinal lymph nodes to cancer-free determination at the proximal border of the penectomy incision. Conclusion: Penile cancer is a cancer that has many risk factors. Patients who have been diagnosed with penile cancer, the treatment chosen is a total or partial penectomy depending on the area of ​​the affected tissue

Abstract

Introduction: Primary cutaneous aggressive epidermotropism CD8+ cytotoxic t-cell lymphoma (PCAE-CD8+ CTCL) is an aggressive variant of cutaneous T-cell lymphoma which has poor prognosis. Because of its rarity, its relatively variable clinical dan histopathological presentation, and no diagnostic criteria for PCAE-CD8+ CTCL, establishing diagnosis can be challenging. Case: A case of PCAE-CD8+ CTCL in a 67-year-old man whose early histopathological features resembling parapsoriasis was reported. Physical examination showed multiple lesions in form of erythematous plaques with ulcers and crusts in almost part of the body. Histopathological features showed epidermotropism and infiltrates of atypical also pleomorphic lymphocytes. Immunohistochemistry result was positive for CD8, CD4, Granzyme, CD3, and Ki-67 supported PCAE-CD8+ CTCL. Discussion: Clinical manifestation of PCAE-CD8+ CTCL varies among patients. Histopathological and immunohistochemistry examination are needed to establish diagnosis. Conclusion: The understanding of clinical manifestation of PCAE-CD8+ CTCL is needed. Because of its poor prognosis and high proliferative activity, early diagnosis may be helpful in improving the likelihood of patient survival.

Abstract

The histopathological examination helps in prompt diagnosis and type of disease in some cases of leprosy. Determination of the biopsy site on the skin lesion is important. We report a case of borderline tuberculoid (BT) leprosy in a 54-year-old female who complained of numb red patches on the right forearm. From physical examination, three confluent hypoesthetic round shapes and annular erythematous macules, accompanied with one satellite lesion on the right forearm were found. The bacteriological examination revealed no acid-fast bacteria. Multiple skin biopsies were performed on round, annular, and satellite lesions. Histopathological features of the round erythematous macule demonstrated atrophic epidermis, lymphocyte infiltration, with epithelioid granulomas in the dermis, which supported BT leprosy. Histopathological features of the annular and satellite lesions showed lymphocyte infiltration in the dermis, perivascular, around the eccrine glands, and hair follicles, which corresponded to indeterminate leprosy. In leprosy, the concordance between histopathological and clinical features shows varying results. Multiple skin biopsies should be performed for multiple different skin lesions. The diagnosis of BT leprosy was established in this case after a clinicopathological correlation was done. In a leprosy patient, multiple skin biopsies obtained from different skin lesions could reveal a varied histopathological picture. Therefore, clinical and pathological correlation is needed in order to establish the diagnosis.

Abstract

Anogenital warts (AGWs) are benign epithelial proliferation of the skin/mucosa, caused by human papillomavirus (HPV), with clinical manifestation of warts in the genital/perianal areas. Cellular immunity plays a role in the elimination of HPV, which characterized by the expression of cytokines with Thelper (Th)-1 cell pattern, namely interleukin (IL)-12 which will increases Tcell proliferation and differentiation into Th-1, pro-inflammatory cytokines secretion, also induce natural killer cells to increase proliferation and pro-inflammatory cytokines. In in vitro studies, PPD increased IL-12 expression in serum verruca vulgaris patients (p<0,05) compared to control. There are no study of serum IL-12 levels in AGWs petients who received intralesional PPD injection in Indonesia, so this is the aim of this study. This research isone group pre- and post-test design based on an experimental analytic study. The samples were 14 serum of AGWs patients before and after receiving PPD therapy. Serum IL-12 levels meassured by enzyme linked immunosorbent assay. Results showedserum IL-12 levels before receiving therapy was 45.73±53.186pg/ml. After receiving PPD therapy, serum IL-12 levels increased by an average of 45.9pg/ml, became 91.68±79.321pg/ml (p=0,158). As conclusion,level of IL-12 in the serum of AGWs patients does not increase after receiving PPD therapy.

Abstract

Abstract Introduction: Vitamin D is known to have many functions in the body, one of them is acts as an immunomodulator. This vitamin works through gene transcription of certain cells that express vitamin D receptors (VDR). Objective: To more understand the role of vitamin D in vitiligo based on RVD. Methods: This study using observational analytic study with cross-sectional method. The case group consisted of vitiligo patients whose skin was taken from the lesion area and perilesion through a plong biopsy procedure and stored as stored biologic material, while the control group were burn patients and chronic wounds from Plastic Surgery Outpatient, that underwent a split-thickness skin graft from the healthy skin. RVD expressions were examined by immunohistochemical using monoclonal antibodies and assessed by two reviewers. Results: The results showed that the mean VDR histoscore in the skin lesions of vitiligo (4.96 ± 1,852) was higher than controls (2.47 ± 0.405). The mean skin perilesion of vitiligo (5.07 ± 1,321) was higher than controls (2.47 ± 0.405). The mean histoscore of lesion was higher (4.96 ± 1,852) but was not statistically significant compared to perilesion vitiligo (5.07 ± 1,321). Conclusion: The expressions of RVD in perilesion vitiligo patients are lower than lesion. However, RVD expression of vitiligo patient was not lower than control group. This result shows that RVD plays little role in pathogenesis of vitiligo.

Abstract

Exogenous ochronosis (EO) is a skin pigmentation disorder caused by the long-term use of topical skin lightening agents, especially hydroquinone. Cases of EO are rare. The etiopathogenesis of EO is still unknown. Clinical manifestations of EO are asymptomatic gray-brown or blue-black macules with a predilection for sun-exposed areas and no systemic involvement. The clinical manifestations of EO often resemble other dermal hyperpigmentation, so an adequate history and physical examination is necessary to avoid misdiagnosis. Histopathological examination is the gold standard for the diagnosis of EO. Management of EO is still a challenge in dermatology because EO is difficult to treat with variable and often unsatisfactory therapeutic results. Exogenous ochronosis management can be divided into non-pharmacological, pharmacological, laser and non-laser procedural, and combinations. Keywords: Exogenous ochronosis, EO, diagnosis, treatment

Abstract

Urtikaria Kronis Spontan (UKS) berdampak signifikan pada kualitas hidup pasien. Autologous Plasma Skin Test (APST) dan C-reactive protein (CRP) merupakan skrining praktis pada UKS. Hasil positif disebab- kan oleh sitokin inflamatorik saat degranulasi sel mast dan dapat berkorelasi dengan keparahan UKS. Studi analitik observasional dengan desain potong-lintang dilakukan selama bulan Juli-September 2021 melibatkan lima puluh pasien UKS berusia 17-51 tahun. Keparahan penyakit dinilai menggunakan Urticaria Activity Score (UAS7). Hasil APST dinilai berdasarkan kriteria Sabroe. Nilai serum CRP diukur menggunakan high sensitiv- ity CRP (hsCRP). Hasil APST positif ditemukan pada hampir semua pasien (95%) dan hanya empat pasien dengan CRP positif (8%). Keparahan berkorelasi bermakna terhadap diameter urtika (P = 0,038) dan nilai CRP (P = 0,041). Analisis odds ratio (OR) menunjukkan bahwa pasien dengan hasil APST positif memiliki risiko 32 kali mengalami UKS sedang-berat. Tidak ditemukan korelasi bermakna antara nilai APST dan serum CRP pada UKS.
