Abstract

This study examines the impact of family support on the effectiveness of Crohn's disease therapy through a meta-analysis of 18 studies involving 5,876 patients. The analysis reveals a significant positive effect (Hedges' g = 0.58, 95% CI [0.42, 0.74], p < .001) of family support on various therapeutic outcomes. The strongest effect was observed in the dimensions of quality of life (g = 0.67, 95% CI [0.48, 0.86]) and clinical remission (OR = 2.14, 95% CI [1.68, 2.73]). Furthermore, emotional support was found to have the largest effect (g = 0.63, 95% CI [0.45, 0.81]) compared to other types of family support. Mediation analysis indicated that medication adherence (indirect effect = 0.15, 95% CI [0.08, 0.22]), stress reduction (0.12, 95% CI [0.05, 0.19]), and increased self-efficacy (0.09, 95% CI [0.03, 0.15]) were significant mediators in the relationship between family support and Crohn's disease treatment efficacy. Additionally, a stronger effect of family support was observed in patients with more severe disease (Q = 4.82, p = .028) and samples with a higher proportion of females (? = 0.012, p = .03). These findings extend the work of Sewitch et al. (2001) and Moradkhani et al. (2013) by providing a more precise quantification of the effects of family support and identifying several key moderators. Unlike Trindade et al. (2017), this study consistently found a positive effect of family support on Crohn's disease therapy. The novelty of this research lies in identifying specific mechanisms and moderating factors affecting the relationship between family support and therapy effectiveness, offering a deeper understanding of the complex role of family support in Crohn's disease management.

Abstract

Diabetes mellitus is a chronic metabolic disease characterized by hyperglycemia. Prolonged hyperglycemia leads to oxidative stress, which plays a crucial role in causing further complications, including liver damage characterized by increased levels of Serum Glutamate Pyruvate Transaminase (SGPT) and Serum Glutamate Oxaloacetate Transaminase (SGOT). This study aimed to examine the effects of Cinnamomum burmanii bark extract on SGPT and SGOT levels in a mouse model of diabetes mellitus. The research employed an experimental design using a completely randomized design. It consisted of five treatment groups: K- (normal mice), K+ (diabetic mice), P1, P2, and P3 (diabetic mice treated with Cinnamomum burmanii bark extract at doses of 125, 250, and 500 mg/kg BW). Streptozotocin was injected into groups K+, P1, P2, and P3 at a dose of 0.1 ml per mouse for 14 days. If blood glucose levels increased, extracts were administered to groups P1, P2, and P3 according to the predetermined doses for 14 days. On day 28, surgery was performed to collect blood serum for SGPT and SGOT analysis.. The results of the study indicated that Cinnamomum burmanii bark extract could reduce SGPT and SGOT levels in the diabetic mouse model. This demonstrates the potential of Cinnamomum burmanii as a therapeutic agent that may help reduce liver damage often observed in diabetes mellitus

Abstract

Telemedicine has emerged as an innovative solution for providing healthcare services, particularly in coastal areas that often face limitations in access and medical facilities. In coastal regions, the main challenges include the long distance from healthcare centers, limited infrastructure, and difficulties in accessing adequate medical care. Telemedicine, by utilizing communication technology to deliver remote medical services, offers an effective way to address these barriers. The purpose of this study is to evaluate the implementation of telemedicine as a health consultation medium for communities in coastal regions. The method used is book reference (reference from books and research journals). The data collection techniques employed in this study include gathering literature, reading, sorting data, taking notes, identifying, and validating the accuracy of the data for subsequent use in analysis. The results of the study show that telemedicine has a very strong aspect related to communication. In terms of utility, telemedicine offers a significant solution to the problem of disparities in healthcare services by utilizing communication channels that transform how people interact in the context of healthcare. Through telemedicine, healthcare providers can reach patients located in distant or remote areas, thereby expanding access and improving the quality of care.

Abstract

Intubation failure occurs in 1 case out of 200-2000 cases under selective general anesthesia. A difficult airway is a clinical situation in which the anesthesiologist has difficulty in ventilation using a mask, tracheal intubation or both. A difficult airway is an interaction between patient factors, clinical conditions, and the practitioner's skills and preferences. In carrying out intubation, there is a need for an initial assessment of the airway, where there are several factors that influence the level of difficulty. Apart from that, there are also techniques to ensure that the patient is patient. His breathing was going well during intubation or ventilation. However, when carrying out techniques for intubation or ventilation, you need to pay attention to contraindications and future complications. After the surgical process, extubation strategies for difficult airways are carried out, this depends on the patient's condition and the anesthesiologist's abilities.

Abstract

Pregnancy can trigger Chronic Energy Deficiency (CED). The main factors influencing CED in pregnant women include food intake, family welfare, and nutritional status. Efforts to reduce the prevalence of CED in Indonesia encompass early screening, nutrition education, and the provision of supplementary food. The novelty of this study lies in identifying the relationship between family welfare levels and the incidence of CED in pregnant women. The study aims to describe family welfare levels, examine the incidence of CED, and analyze the relationship between family welfare and CED occurrence. This research utilized an analytical observational approach with a cross-sectional design. The study population consisted of pregnant women in the UPTD Puskesmas Mayong I, Jepara area, in 2022, with inclusion criteria being pregnant women in their second and third trimesters who attended antenatal care at the maternal and child health clinic. The sample size was 87 respondents, calculated using the Slovin formula (error margin 0.05%), and sampling was conducted using non-probability consecutive sampling. Data analysis employed the chi-square test. Most families were categorized as "prosperous," with 79 respondents (90.8%). The majority of pregnant women did not experience CED, accounting for 61 respondents (70.1%). Additionally, most of the prosperous families had pregnant members without CED, comprising 59 respondents (67.8%), with a p-value of 0.012 (p-value < 0.05). There is a significant relationship between family welfare and the incidence of Chronic Energy Deficiency (CED) among pregnant women in UPTD Puskesmas Mayong I, Jepara Regency.

Abstract

Violence reflects social conflicts that can harm both the physical and psychological well-being of victims while violating legal norms. Although physical crime trends in Indonesia showed a decline from 2017 to 2019, North Sumatra remained the epicenter of violence incidents, particularly assault cases. The handling of such cases is inseparable from the role of Visum et Repertum, a legal document that serves as key evidence in the criminal justice process, as stipulated in Article 184 of the Indonesian Penal Code (KUHP). Through a case study of a 69-year-old man who became a victim of minor assault, this study highlights the process of medical investigation and legal analysis. A bruise on the head caused by a bare-handed blow did not hinder daily activities, classifying it as a minor assault punishable by up to three months in prison under Article 352 (1) of the Penal Code. This study emphasizes the need for synergy between controlling socio-economic factors and enforcing legal procedures to address violence, while strengthening the role of Visum et Repertum as a vital element of justice
