Abstract

Stunting remains a priority challenge in Indonesia's health development due to its long-term impact on physical growth, cognitive development, and human resource quality. Accelerating stunting reduction requires a contextual, culturally based approach to improve the acceptance and sustainability of interventions. This study aims to explore the transformation of the Petompo'a tradition a seven-month ceremony for pregnant women in North Buton Regency as a medium for nutrition and health education in preventing stunting from the pregnancy period. The study used a qualitative approach with a phenomenological design, involving pregnant women, traditional leaders, and health workers selected purposively. Data were collected through in-depth interviews, participatory observation, and Focus Group Discussions, then analyzed using thematic analysis. The results show that Petompo'a holds great potential as a culturally based educational medium because it is widely accepted, carries legitimacy through traditional leaders, and is rich in symbolism supporting health values. Integrating nutrition and health messages increased pregnant women's awareness of nutritional intake, antenatal care visits, and the prevention of anemia and stunting. Positive behavioral changes were also found, such as improved dietary patterns, adherence to health advice, and stronger social support. The transformation of Petompo'a proves that a culturally based approach can serve as an effective entry point while strengthening intervention sustainability. The study recommends integrating this tradition into maternal and child health policies through collaboration among traditional leaders, health workers, and local government.

Abstract

Priapism is a urological emergency characterized by a penile erection lasting more than four hours without sexual stimulation, which may result in serious complications such as permanent erectile dysfunction if not promptly managed. While it is commonly associated with medication use, trauma, or hematological disorders, its occurrence as an initial manifestation of Chronic Myeloid Leukemia (CML) is exceedingly rare. We report the case of a 33-year-old man who presented to the Emergency Department of RSUD Wonogiri with a persistent painful erection lasting more than 48 hours, without any history of stimulant drug use, trauma, or prior sexual activity. Physical examination revealed a fully rigid and tender penis, pallor, tachycardia, and hepatosplenomegaly. Laboratory evaluation showed marked leukocytosis (320,000/µL), anemia (Hb 7.5 g/dL), and thrombocytopenia. Cavernosal aspiration yielded dark, hypoxic blood, and peripheral smear demonstrated a predominance of granulocytes with immature forms, consistent with chronic-phase CML. The patient underwent cavernosal aspiration and irrigation, and was subsequently referred to the hematology department for further management. This case emphasizes the need to consider priapism as a potential early sign of systemic diseases such as CML, and highlights the importance of rapid multidisciplinary intervention to prevent long-term complications.

Abstract

Slow learners in inclusive elementary schools often face obstacles in building social relationships, which impacts the fulfillment of their psychological need for acceptance; meanwhile, research on the external factors shaping their social acceptance remains limited. This study aims to explore the external factors that support the social acceptance of slow learners in schools. Employing an exploratory qualitative approach, the study involved five slow-learning students from inclusive elementary schools in Surabaya—selected via purposive sampling—alongside insights from their teachers and parents. Data were collected through semi-structured interviews, non-participant observation, and documentation, then analyzed using reflective thematic analysis; data validity was ensured through source and technique triangulation. The results identified two main themes—social support and social interaction—encompassing four sub-themes: parental support, teacher support, social contact, and conflict resolution. The findings indicate that support from parents and teachers, as well as acceptance during peer interactions, play a crucial role in shaping social acceptance for slow learners; although some still experience rejection and communication limitations, these issues do not become insurmountable obstacles if the environment responds proactively. This study underscores that the success of inclusive education requires a school environment that supports children's psychological needs through supportive social relationships.
