Abstract

The implementation of the Hospital Management Information System (HMIS) in patient registration at RSUD Drs. H. Abu Hanifah, Central Bangka Regency, encountered several issues. These included a 22.30% delay in the transmission of inpatient data in November 2022 due to unstable internet connections, a lack of user guidance, slow patient data retrieval, and frequent system errors. These problems decreased the registration staff's willingness to use HMIS. This study aimed to evaluate the use of HMIS in patient registration using the Technology Acceptance Model (TAM). The research employed a qualitative design, collecting data through interviews, observation, documentation, focus group discussions, and brainstorming sessions. The subjects of the study included 1 Head of the Medical Records Installation, 1 IT staff, and 6 registration staff. The results indicated that not all registration staff had received training on HMIS, there was no HMIS user manual available, the registration staff were unaware of the policies and SOPs for using HMIS, and the internet quality was often unstable. The suggested solutions included training for all registration staff, the creation of a HMIS user manual, distribution of copies of HMIS implementation policies and SOPs, and upgrading the equipment used to operate HMIS.

Abstract

The management activities in medical records unit involve several occupational risks, one of which is ergonomic risk. It has been found that the space between shelves for staff movement is too narrow, and the condition of shelves and work chairs for staff is not ergonomic. This could increase risk of accidents and work-related illnesses. Objective of this study is to analyze ergonomic risks in medical records unit of Bhayangkara Bondowoso Hospital. This research is descriptive in nature and employs data collection techniques including interviews, observations, documentation, and measurements. The study utilizes HIRARC method (Hazard Identification, Risk Assessment, and Risk Control). Subjects of the study include four medical records staff members, head of medical records, and hospital's K3 committee. The study identified high ergonomic risks, such as non-ergonomic chairs and handling a large volume of retention files. Medium risks involved non- ergonomic shelves, prolonged computer use, working room, and narrow shelf spacing. Control measures included replacing facilities with ergonomic ones, conducting retention, destruction, and implementing EMR systems.

Abstract

The provided information outlines a situation at the Kotakulon Health Center where inpatient medical record files are incomplete, failing to meet the standards for medical record filing completeness. The highest percentage of incompleteness was identified in several specific areas, including Pharmaceutical Care Forms, Drug Record Cards, Informed Consent, Entry and Exit Summary, and Discharge Planning, with an 87% incompleteness rate. To address this issue, a study was conducted to analyze the factors contributing to incomplete medical records for inpatients at the health center. The study results identified several factors contributing to incomplete medical records, including a lack of training, low self-awareness, poor discipline, insufficient communication, and the absence of a reward system. To address these issues, the proposed solution involved conducting outreach, training, and seminars, particularly within the inpatient unit, and encouraging officers to adhere to their standard operating procedures (SOPs) more diligently. This approach aims to improve medical record completeness and enhance the overall performance of the officers.

Abstract

Jember Klinik Hospital has been using SIMRS since 2012 to help the process of service to patients, but when implemented, there are still obstacles encountered, namely data derived from SIMRS (Hospital Management Information System) needs to be processed manually again, the monitor screen turns off, there is no educational SPO and socialization efforts have not reached an optimal level. The purpose of this study was to evaluate the Hospital Management Information System (SIMRS) of the inpatient unit using the Human Organization Technology (HOT-FIT) method and apply a cross-sectional research design and quantitative analysis at Jember Klinik Hospital. The sample size was 84 respondents utilizing a Lemeshow formula, the sampling method was by applying proportionate stratified random sampling. The results showed that there was a significant and strong relationship between information quality with system use and user satisfaction, a strong relationship between service quality and user satisfaction, a strong relationship between organizational structure and user satisfaction, a strong relationship between leadership support and system use.

Abstract

The inaccurate coding of primary injury and external causes diagnoses is a classification issue in the ICD-10, posing risks of financial loss for hospitals and compromising the validity of statistical data. Between 2020 and 2022, the coding accuracy for inpatient injury diagnoses at RSD Dr. Soebandi Jember was 70%, while external100% inaccuracy because were not coded at all. This research aimed to analyze the factors causing inaccuracies in coding these diagnoses based on the Robbin performance theory and propose improvement strategies through brainstorming. In this qualitative study, data was collected through in-depth interviews, observations, and document analysis. Findings revealed ablity factor cause by the lack of Emergency Room physicians knowledge’s in completing injury case histories and discrepancies in medical terminology selection by attending physicians. Motivation factors were identified suboptimal reward and punishment systems, and the absence of feedback in evaluating medical record quality. The opportunity factor encompassed unclear task boundaries between coders and internal verifiers, and the coding process deviated from standard operating procedures (SOP). Suggestion to address these issues, re-socialization of SOPs related to coding procedures, advocating for appropriate reward and punishment, and organizing internal training on injury and external causes diagnosis coding.

Abstract

The long waiting time is due to the large number of prescriptions that must be served by the staff, the writing of the prescription is unclear and incomplete (for example: dose, amount, and patient name) so that the pharmacy staff must confirm it first with the doctor who wrote the prescription, this is because Doctors' knowledge of the availability of medicines is not well confirmed and doctors write prescriptions for medicines that are difficult for staff and patients to understand if they want to redeem medicines again. To overcome this problem, a drug redemption information system is needed. Research and development with data collection through interviews, observation, and documentation was used as the research method. The development method used is waterfall by pressman which consists of the stages of system requirements analysis, system design, coding and testing. This research produces a website-based drug redemption information system at the RSIA Muhammadiyah pharmacy installation in Probolinggo City which has advantages including redeeming medication electronically using a prescription given by a doctor, medication redemption can be done anywhere and then you can choose to pick it up at the hospital or have it delivered. Home, chat with pharmacists in real time which aims to make it easier for patients to ask questions about medicines, and electronic prescriptions to reduce prescriptions that are difficult to read.

Abstract

Between 2020 and 2022, there was an increase in morbidity and pneumonia-related deaths at Citra Husada Jember Hospital. This study aimed to analyze the possible association between asthma, chronic obstructive pulmonary disease (COPD), and pneumonia. The research was quantitative with a case-control design. Data were collected from medical records using a checklist sheet. The study population was divided into two groups: the case population included 115 medical records of pneumonia hospitalizations from January to March 2023, while the control population comprised 512 medical records of internal medicine hospitalizations in the same period. A sample size of 89 medical records was selected using simple random sampling. Data were analyzed using univariate techniques for frequency distribution and bivariate methods, such as the Fisher exact test. The results showed a significant association between asthma, COPD, and pneumonia incidence. It was concluded that asthma and COPD are associated with an increased risk of pneumonia. It is recommended that the government and hospitals collaborate to implement the PCV (Pneumococcal Conjugate Vaccine) vaccination program. Individuals with asthma and COPD should undergo regular health assessments and comprehensive treatments, including allergen immunotherapy, to prevent pneumonia-related complications.

Abstract

Chronic Kidney Disease (CKD) is one of the leading causes of death in indonesia with 2,221 deaths recorded in 2019. Kaliwates General Hospital located is one of the hospital in jember city that recorded an increase in chronic kidney disease cases from 2020 to 2022. The aims of this study is to identify the causative factors influencing the occurrence of chronic kidney disease based on inpatient medical records at Kaliwates General Hospital. The method used is a quantitative analytical approach with a case-control design. The research sample includes 79 cases of chronic kidney disease and 79 cases of urological disorders selected through purposive sampling. Data were analyzed using uniariat and bivariat approaches with chi-square test. The results of the bivariate analysis found a significant relationship between chronic kidney disease and several factors, including age (p value 0,005), hypertension (p value 0,001), diabetes mellitus (p value 0,002), and cardiovascular disease (p value 0,003). The hospital is expected to provide health education related to the causes of chronic kidney disease to patients, so that they can better understand the risk and take preventive actions to manage these risks.

Abstract

Survei kepuasan dan keluhan masyarakat yang dilaksanakan di Puskesmas Wiradesa Kabupaten Pekalongan masih dilakukan secara manual dengam memberikan lembar kuesioner kepada masyarakat yang telah mendapatkan pelayanan. Tahun 2020 dan 2021 Puskesmas Wiradesa tidak melaksanakan Survei kepuasan dan keluhan dikarenakan covid-19. Penelitian ini bertujuan untuk merancang dan membuat sistem informasi kepuasan dan keluhan masyarakat di Puskesmas Wiradesa yang mendapatkan output pengolahan data survei kepuasan dan keluhan secara elektronik dan menghasilkan laporan yang cepat dan aktual. Penelitian ini menggunakan penelitian kualitatif dengan metode pengembangan sistem Waterfall. Hasil penelitian Requirement analysis and definition adalah tampilan yang simpel dan mudah dioperasikan, serta laporan survei kepuasan dan keluhan yang sesuai dengan format yang ditetapkan oleh Permenpan. System and software design sistem ini memiliki dua jenis pengguna yaitu admin dan pasien. Implementation and unit testing peneliti menggunakan bahasa pemrograman PHP dan Framework Laravel. Integration and system testing sistem dilakukan menggunakan teknik blackbox menunjukkan bahwa sistem ini berfungsi dengan baik dan dapat membantu petugas rekam medis dan mempermudah pekerjaan mereka.

Abstract

Widodo Ngawi Hospital is currently handling the high volume of medical record documents, with 11,986 outpatient documents and 13,065 inpatient documents in 2023. Retention activities have not been carried out since 2015 due to the unavailability of a system that supports transferring medical record documents and retention alerts. The absence of retention activities has reduced the storage capacity of medical record documents, resulting in medical record documents being easily damaged, not neatly arranged, and not in accordance with their preparation, causing services to patients to be less optimal and efficient. The purpose of this study is to design and create a web-based medical record document transfer and retention information system to support media transfer and retention of medical record documents. The method used is the Waterfall system development method, which consists of needs analysis, design, code generation, testing, and maintenance, though the maintenance stage is not carried out in this study. The result of this research is an information system that helps media transfer, retention, and destruction. Based on the test results, the system functions properly, and no errors were found in all modules on both admin and user access rights.
