Abstract

Background: The postpartum period is a critical phase for maternal health, as mothers are vulnerable to various complications that may lead to serious morbidity or mortality if not detected early. Self-warning knowledge regarding danger signs during the postpartum period plays an essential role in enabling mothers to recognize abnormal conditions and seek timely health care. However, limited knowledge and inadequate counseling remain common challenges in postpartum services. Structured postpartum counseling is expected to improve mothers’ understanding of self-warning signs and enhance early detection of complications. Objective: This study aimed to determine the effectiveness of structured postpartum counseling on self-warning knowledge among postpartum mothers at the Ome Inpatient Health Center. Methods: This study employed an analytical observational design with a cross-sectional approach. The sample consisted of 16 postpartum mothers selected using a total sampling technique. Data were collected using structured questionnaires to assess mothers’ knowledge of postpartum self-warning signs before and after receiving structured counseling. The counseling intervention was delivered using standardized educational materials. Data were analyzed using the Chi-square test to determine the effectiveness of structured postpartum counseling on self-warning knowledge. Results: The results showed an increase in self-warning knowledge among postpartum mothers after receiving structured counseling. Statistical analysis using the Chi-square test indicated a p-value of 0.000 (p < 0.05), demonstrating a statistically significant effect of structured postpartum counseling on improving postpartum mothers’ self-warning knowledge. Conclusion: Structured postpartum counseling is effective in enhancing self-warning knowledge among postpartum mothers. The implementation of structured and continuous counseling during the postpartum period is recommended to support early detection of complications and improve maternal health outcomes.

Abstract

Malnutrition among children under five remains a major public health concern, particularly in areas with limited access to nutritious food and health services. This study aimed to determine the prevalence of stunting, underweight, and wasting among children under five in Kuatnana District, South Central Timor Regency in 2025. This research used a descriptive cross-sectional design with secondary data obtained from nutrition program reports at Tetaf Public Health Center. The sample included all children aged 0–59 months, totaling 1,179 children. Data were analyzed using univariate analysis to calculate frequency distribution and percentages based on WHO anthropometric indicators (z-score). The results showed that the prevalence of stunting was 44.78%, underweight 40.46%, and wasting 14.76%. Most children had normal nutritional status based on weight-for-height index (88.38%), although a considerable proportion still experienced undernutrition. In conclusion, malnutrition among children under five in Kuatnana District remains high, particularly stunting and underweight. Integrated interventions are needed, including improving caregiving practices, increasing access to nutritious food, and strengthening health services to reduce malnutrition sustainably.

Abstract

Telepharmacy is a digital-based pharmaceutical service innovation aimed at improving access to healthcare services and enhancing the quality of pharmaceutical care, particularly in areas with limited healthcare resources. This study aimed to evaluate the implementation of telepharmacy services at Sehat Plus Pharmacy based on the six healthcare quality dimensions established by the World Health Organization (WHO), namely safety, effectiveness, timeliness, efficiency, equity, and patient-centeredness, and to analyze their association with respondent characteristics. A quantitative descriptive-analytic study with a cross-sectional design was conducted. The study involved 35 respondents selected using a total sampling technique. Data were collected through a structured Likert-scale questionnaire and analyzed using univariate analysis and Spearman correlation tests with a significance level of α = 0.05. The results indicated that the overall implementation of telepharmacy services was categorized as very good (88.8%), with the safety dimension achieving the highest evaluation score among all quality dimensions. Most respondents were female and aged between 21 and 25 years. Statistical analysis revealed a significant association between age and perceptions of telepharmacy implementation (p = 0.004; r = -0.471), indicating a moderate negative correlation. In contrast, gender, educational level, and occupation showed no significant association with service implementation (p > 0.05). It can be concluded that telepharmacy services at Sehat Plus Pharmacy have met WHO healthcare quality standards and provide positive user experiences, while age remains an important factor influencing service perception and acceptance.

Abstract

Down syndrome, or Trisomy 21, is the most common chromosomal disorder in humans and is primarily caused by nondisjunction events during meiosis. Although advanced maternal age has long been recognized as the major risk factor, recent scientific evidence indicates that genetic mutations, gene polymorphisms, and meiotic recombination abnormalities also play important roles in the occurrence of Trisomy 21. This literature review aims to analyze the role of genetic mutations in the pathogenesis of Trisomy 21, with emphasis on the mechanisms of nondisjunction, oocyte aging, meiotic recombination errors, and folate metabolism disorders. The method used was a narrative literature review based on seven major scientific sources discussing the genetic and molecular factors underlying Trisomy 21. The findings indicate that genetic mutations and variations affecting meiotic regulation, recombination positioning, chromosome cohesion, and DNA methylation significantly increase the risk of chromosome 21 nondisjunction. In addition, age-related decline in oocyte quality and polymorphisms in folate metabolism genes further exacerbate chromosomal instability. Understanding these mechanisms is essential for the development of genetic counseling, early detection, and more effective prevention and management strategies in the future.

Abstract

Diabetes Mellitus Type II is a chronic metabolic disease characterized by elevated blood glucose levels due to insulin resistance and impaired insulin secretion. Globally, the number of people with diabetes in 2024 is estimated to reach 800 million, with more than 95% being Type II Diabetes Mellitus. At RSUD Prof. M. Yamin, SH Pariaman in 2025, there were 189 recorded cases of Type II Diabetes Mellitus without complications. The purpose of this scientific paper is to provide nursing care to a patient with Type II Diabetes Mellitus in the Internal Medicine Ward of RSUD Prof. M. Yamin, SH Pariaman. The method used is a case study with a nursing process approach, including observation, interviews, physical examination, and data collection from medical records. This study was conducted in the internal ward for 5 days, from February 6, 2026 to February 10, 2026, with the subject Mrs. N. The assessment results identified nine nursing problems: unstable blood glucose levels, acute pain, impaired skin/tissue integrity, ineffective peripheral perfusion, impaired physical mobility, sleep pattern disturbance, fatigue, risk of infection, and risk of nutritional deficit. The evaluation after 5 days of nursing care showed that five diagnoses were resolved, namely unstable blood glucose levels, ineffective peripheral perfusion, fatigue, risk of infection, and risk of nutritional deficit. Meanwhile, the other four diagnoses were not fully resolved but showed improvement. It is expected that the patient and family will maintain adherence to a healthy lifestyle, therapy, diet, and regular follow-up in order to prevent complications.
