Abstract

Diabetes Mellitus in the elderly is a global concern. According to the World Health Organization (WHO), the number of elderly people with diabetes mellitus in the world reached approximately 158.3 million in 2024. In Indonesia, the number of elderly sufferers reached approximately 20.4 million people. Diabetes mellitus is a chronic metabolic disease characterized by elevated blood glucose levels due to impaired insulin secretion, impaired insulin function, or a combination of both. This case study aims to provide comprehensive nursing care for elderly people with Diabetes Mellitus. The method used is the nursing process which includes assessment, nursing diagnosis, intervention, implementation, evaluation, and documentation with a descriptive design. The results of the assessment showed that the client experienced tingling in both hands and feet, itching, dry skin, fatigue, frequent thirst, frequent urination especially at night, and blurred vision. The examination results showed a random blood sugar of 257 mg/dL. The client took Metformin 500 mg twice a day, but not regularly. The established nursing diagnoses were unstable blood glucose levels, ineffective health maintenance, and anxiety. Implementation was carried out over five days. Evaluation results indicated that unstable blood glucose levels were not resolved, as indicated by a drop in blood sugar levels to 192 mg/dL. Meanwhile, health maintenance was ineffective, and anxiety was deemed resolved. Nursing care yielded positive results, but continued monitoring is still necessary. It is hoped that the client will maintain medication adherence, adhere to a diabetic diet, perform regular blood sugar checks, and maintain a healthy lifestyle. Recommendations for caregivers and healthcare professionals include providing support, education, and regular monitoring to maintain controlled blood glucose levels and prevent complications.

Abstract

Lung cancer is a malignancy of the lung tissue or bronchi caused by uncontrolled cell growth and is one of the cancers with the highest mortality rates. According to Globocan 2022 data, there were approximately 2.3 million new cases and 1.8 million deaths from lung cancer. In Indonesia, 408,661 cases were recorded, resulting in 242,099 deaths. This indicates that lung cancer remains a serious health issue requiring attention in prevention and management efforts. An increase in cases has also been observed in West Sumatra, with 1,142 cases at Dr. M. Djamil General Hospital in Padang during the 2021–2025 period. Therefore, this study aims to describe nursing care for lung cancer patients admitted to the pulmonary inpatient ward in 2026. The method used was a case study conducted over 5 days, from January 30 to February 3, 2026, in the Pulmonary Ward of Dr. M. Djamil General Hospital in Padang, involving one patient. The research findings, obtained through interviews, observations, and physical examinations, identified six nursing diagnoses: ineffective airway clearance, ineffective breathing pattern, acute pain, nutritional deficit, sleep disturbance, and risk of infection. The evaluation showed that several issues had been partially resolved, such as airway clearance, ventilatory support, pain, and improved intravenous medication administration, while nutritional deficit and sleep pattern disturbance had been resolved. Following the provision of nursing care, it can be concluded that the patient and family are advised to understand the condition of lung cancer, adhere to treatment and undergo regular follow-ups, adopt a healthy lifestyle, and provide optimal support to improve the patient’s quality of life.

Abstract

Based on data from the World Health Organization (WHO), the prevalence of gout arthritis at the global level increased in 2023, where the number of sufferers was around 44,000,000 people with a prevalence of 0.56%. Gout arthritis is a disease caused by a purine metabolism disorder that causes the accumulation of uric acid crystals in the joints. This condition is influenced by age, purine consumption, with symptoms such as pain, swelling, stiffness. If left untreated, it can cause joint damage and kidney disorders. The purpose of this study is for the author to be able to provide nursing care to elderly people with gout arthritis. The research method used was a case study for 6 days through interviews, observations, and checking uric acid from February 26 to March 3, 2026. The results of the study obtained 3 nursing diagnoses, namely: chronic pain, impaired physical mobility, ineffective health management. Where the interventions described are pain management, mobilization support, decision-making support and implementation carried out according to the prepared plan. After 5 days of implementation, the results obtained were reduced pain, decreased uric acid levels, increased mobility, and improved health management. The evaluation on the fourth day showed that the diagnosis of ineffective health management was resolved, and the diagnosis of chronic pain and mobility impairment was not resolved, so the caregiver continued. Educating the client to take medication regularly according to the prescribed dosage, maintaining a healthy diet, for example, reducing consumption of coconut milk and green vegetables, and educating the caregiver by reminding the client to take medication regularly, inviting the client to participate in activities held by the nursing home, such as exercise, health education, and arts.

Abstract

Appendicitis is an inflammatory condition of the appendix caused by irritation and infection of the appendiceal wall as a result of obstruction in the lumen. The Indonesian Health Survey (SKI) reported that in 2023 there were 621,435 cases of appendicitis in Indonesia. Medical record data at Prof. H. M Yamin, S.H Pariaman Regional Hospital showed that the number of patients with appendicitis in 2025 was 88 cases. The purpose of this case study was to obtain an overview of the implementation of nursing care for patients with appendicitis and to improve students’ ability to provide nursing care for patients with appendicitis in the Surgical Inpatient Ward of Prof. H. M Yamin, S.H Pariaman Regional Hospital in 2026. The method used was a case study including interviews and observations conducted in the Surgical Inpatient Ward of Prof. H. M Yamin, S.H Pariaman Regional Hospital for 5 days, from February 8 to February 12, 2026, with the sample being Ms. C diagnosed with appendicitis. The results of the case study identified five nursing problems: acute pain, hyperthermia, anxiety, impaired physical mobility, and risk of infection. Nursing interventions carried out included pain management, hyperthermia management, anxiety reduction, mobilization support, and infection prevention. Evaluation obtained during the 5 days showed that all nursing problems, namely acute pain, hyperthermia, anxiety, impaired physical mobility, and risk of infection, were resolved so that the patient was allowed to go home. Patients are advised to maintain a balanced, high-fiber nutritious diet and comply with treatment recommendations to accelerate recovery and prevent complications.

Abstract

Hypertension is a chronic disease characterized by an increase in blood pressure ≥140/90 mmHg and commonly occurs in the elderly. Hypertension can cause serious complications such as stroke, heart failure, and kidney failure if not properly managed. This study aimed to provide gerontic nursing care for hypertensive patients at the Sabai Nan Aluih Sicincin Social Nursing Home, Padang Pariaman Regency. The method used was a case study with a nursing process approach conducted on one hypertensive patient from February 26  to March 3, 2026. The assessment results showed that the patient experienced chronic pain, ineffective health management, and risk of falls. Nursing interventions included pain management using deep breathing relaxation techniques, progressive muscle exercises, blood pressure monitoring, health education, a low-salt diet, and fall risk prevention. Nursing evaluation showed an increase in the patient’s knowledge regarding hypertension and the importance of adherence to treatment. After five days of nursing interventions, there was a decrease in pain intensity, improved therapy adherence, and improvement in the patient’s general condition. The recommendations given were to maintain a healthy lifestyle, improve medication adherence, and routinely monitor blood pressure. The conclusion of this study showed that the implementation of comprehensive and systematic gerontic nursing care was highly effective in overcoming nursing problems in hypertensive patients.

Abstract

Typhoid fever is an intestinal disease that occurs suddenly due to infection with the Salmonella typhi bacterium. According to the WHO, approximately 81% of people contract typhoid fever. Based on data obtained from Aisyiyah Pariaman Hospital, typhoid fever ranked 4th out of the 10 most common diseases at Aisyiyah Pariaman Hospital in 2024, with 262 cases of typhoid fever in the Muzdalifah ward. The objective of this study is to provide nursing care for patients with typhoid fever. The case study method was used, based on data obtained through observation, interviews, physical examinations, and documentation. The study was conducted in the Muzdalifah Ward of Aisyiyah Hospital in Pariaman from September 19 to 23, 2025, involving one patient, Mr. J. The study identified the following nursing issues: hyperthermia, nausea, sleep disturbances, risk of nutritional deficit. The evaluation results and implemented interventions showed that nausea and the risk of nutritional deficiency were resolved by the 4th day, while hyperthermia and sleep disturbances were resolved by the 5th day. In this case study, it is hoped that the patient and family will increase their knowledge of personal hygiene and maintain their health to avoid diseases, particularly typhoid fever.

Abstract

Rheumatoid Arthritis is a chronic disease that often occurs in the elderly and can cause pain, joint stiffness, and limitations in daily activities. This condition can reduce the quality of life of the elderly if not treated properly. This study aims to provide geriatric nursing care to rheumatoid arthritis patients at the Sabai Nan Aluih Sicincin Social Home for the Elderly in Padang Pariaman Regency. The method used is a case study with one client managed through the process of assessment, diagnosis determination, intervention, implementation, and evalu ation of nursing conducted from February 26 to March 3, 2026. The results of the study showed nursing diagnoses in the form of chronic pain, impaired physical mobility, and risk of falls. Interventions provided included pain management, mobilization support, and environmental safety management. After nursing actions for 5 days, the client's pain level decreased, physical mobility abilities increased, and the risk of falls could be minimized through the nursing interventions provided. It is hoped that the elderly can apply relaxation techniques, simple mobilization exercises, and maintain environmental safety to help reduce pain and prevent the risk of falls.

Abstract

Based on data from the World Health Organization, cardiovascular disease is the leading cause of death, worldwide, as many as 19.8 million people died from cardiovascular disease and 38% of these deaths were caused by Coronary Heart Disease (CHD). Based on data from the Indonesian Health Survey, Coronary Heart Disease caused death, as much as 0.85% or as many as 765,000 cases. From the survey results, West Sumatra is ranked 10th in the provinces with Coronary Heart Disease with a prevalence of 0.87%. Based on data obtained at Padang Pariaman Regional Hospital, Coronary Heart Disease cases were ranked 15th in 2025 with 56 cases. This case study aims to obtain an overview of the application of nursing care to patients with Coronary Heart Disease, including use, establishing nursing diagnoses, planning nursing, carrying out, implementing, evaluating, acting and documenting. The methods, data collection used in this case study are observation interviews, physical examinations and medical records. This case study was conducted in the HCU room of Padang Pariaman Regional Hospital from February 25 to March 1, 2026. The results of the study obtained nursing problems in the form of ineffective breathing patterns, decreased cardiac output, fatigue, acute pain and the risk of ineffective peripheral perfusion. Nursing problems were resolved on the 5th day with nursing actions provided in the form of airway management, cardiac care, pain management, susigi management and shock prevention. It can be concluded that the treatment of Coronary Heart Disease patients requires medical action and patient awareness in managing lifestyles to improve the quality of nursing care in hospitals. It is hoped that patients and their families can understand the importance of awareness in maintaining a healthy lifestyle to prevent complications due to Coronary Heart Disease.

Abstract

Breast cancer (Carcinoma Mammae) is a condition in which cancer cells have lost their normal control and mechanisms, resulting in abnormal, rapid, and uncontrolled growth occurring in breast tissue. According to the International Agency for Research on Cancer of the World Health Organization (WHO), in 2022 Indonesia ranked 8th out of 10 countries. Breast cancer accounts for approximately 16.2% of all cancer cases (66,271 breast cancer cases). West Sumatra ranks third among provinces with 0.9% of cases (2,285 cases) and an average mortality rate of 17 per 100,000 population. Data from the medical records of Dr. M. Djamil General Hospital in Padang show 474 cases (2025), indicating an annual increase in breast cancer cases. This study aims to provide nursing care for patients with post-mastectomy breast cancer in the women’s surgical inpatient ward at Dr. M. Djamil General Hospital in Padang on February 12, 2026. The assessment of Mrs. A revealed that the client complained of pain at the postoperative wound site on the left side of her chest, with the wound covered by a bandage. Four diagnoses were identified for Mrs. A: acute pain related to a physiological injury, impaired skin integrity, body image disturbance due to changes in body structure and shape, and activity intolerance. Interventions included pain management, skin integrity care, relaxation therapy, and body image promotion over a 5-day period. The evaluation showed that the problems were resolved within 5 days, from February 12–17, 2026, in SOAP format. It is hoped that this final project can serve as a source of information and knowledge for patients and their families regarding breast cancer.

Abstract

Based on data from the World Health Organization (2023), an estimated 1.28 billion adults aged 30-79 years worldwide suffer from hypertension, most of whom live in low- and middle-income countries. Hypertension is a major factor leading to cardiovascular diseases such as heart attacks, kidney failure, and stroke. The purpose of this study is for researchers to be able to provide nursing care to elderly people with hypertension. The research method used is a case study, where researchers took 1 patient as a sample. This study was conducted at Wisma Sago PSTW Sabai Nan Aluih Sicincin Padang Pariaman on February 11-16, 2026. After the assessment, the nursing diagnoses were found to be: chronic pain, disturbed sleep patterns and risk of falling. The interventions carried out included the use of pharmacological techniques (administration of amlodipine), non-pharmacological (application of relaxation techniques with deep breathing, autogenic relaxation techniques), and prepared education. After 5 days of implementation, headaches decreased, blood pressure decreased, pain scale decreased, sleep patterns improved, falls while walking decreased. Patients are expected to control their blood pressure by taking antihypertensive medication, managing pain, adopting a healthy lifestyle with regular exercise, and avoiding anything that could worsen their condition. Caregivers are expected to monitor and accompany patients in carrying out the care they have been taught, and to remind them to have their blood pressure checked regularly at the home's clinic.
