Abstract

Risk of violent behavior is one of the psychiatric nursing problems in patients with severe mental disorders caused by the inability to control emotions and aggressive impulses, which may potentially harm themselves, others, or the environment. Based on medical record data at RSJ Prof. HB. Saanin Padang, cases of risk for violent behavior remain relatively high, with more than 100 patients treated each year. This condition indicates that the risk of violent behavior continues to be a serious issue in psychiatric nursing services and requires optimal and continuous management. The purpose of this case study was to obtain an overview of the implementation of psychiatric nursing care for patients with the risk of violent behavior. The method used was a case study approach through the nursing process, including assessment, diagnosis, intervention, implementation, and evaluation. Nursing interventions were provided through deep breathing exercises, hitting pillows, education on medication adherence, effective verbal communication, and spiritual activities. The results showed that the patient became calmer, more cooperative, able to control anger, did not use harsh words, and adhered to medication. It can be concluded that psychiatric nursing care is effective in reducing the signs and symptoms of the risk of violent behavior. Nurses and families are expected to play an active role in supporting treatment and preventing patient relapse.

Abstract

Hallucinations are false perceptual distortions that occur in maladaptive neurobiological responses, patients actually experience sensory distortions, but respond to them as real. WHO data in 2024 shows that mental disorders have increased to exceed 1.1 billion people. Based on preliminary data obtained from Prof. HB Saanin Mental Hospital, Padang in 2026, the most common nursing problem in the last three years was hallucinations with a total of 17,446 patients. This study aims to apply psychiatric nursing care to patients with hallucinations at Prof. HB Saanin Mental Hospital, Padang. The method used in this scientific paper is a case study with a nursing process approach that includes assessment, diagnosis, planning, implementation, and evaluation. The implementation of nursing care to patients was carried out from February 14 to 18, 2026, with 5 visits. The results of the assessment showed that the patient experienced signs and symptoms of hallucinations in the form of hearing whispers and seeing shadows. The main nursing diagnosis was hallucinations with a companion diagnosis of Self-Care Deficit (DPD). The results of the implementation of Mental Health Nursing Care showed that the patient was able to control his hallucinations independently through implementation strategies (SP) 1-4. After the hallucination management nursing intervention was carried out, these results showed that the implementation of implementation strategies was effective in helping patients control hallucinations and improving self-care skills. Through this study, it is hoped that everyone can understand the importance of mental health nursing care, thereby increasing awareness in maintaining and preserving mental health, and implementing behaviors that support the achievement of mental well-being in everyday life.

Abstract

Data from the World Health Organization (WHO) in 2024 showed that mental disorders have increased, affecting more than 1.1 billion people worldwide. Based on medical record data at Prof. HB. Saanin Mental Hospital Padang, cases of violent behavior were reported as many as 213 cases in 2023, 138 cases in 2024, and 159 cases in 2025.This study aimed to provide mental health nursing care for patients with a risk of violent behavior. The method used in this scientific paper was a case study approach with the nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. The implementation of nursing care was carried out from February 9 to 14, 2026, with five meetings. Nursing interventions were provided through deep breathing exercises, hitting a pillow as a safe outlet for anger, medication adherence education, effective verbal communication, and spiritual activities. The results showed that the patient became calmer, more cooperative, able to control anger, did not use harsh language, and adhered to medication. In conclusion, mental health nursing care can reduce the signs and symptoms of the risk of violent behavior. It is expected that nurses and families play an active role in preventing relapse.

Abstract

Diabetes Mellitus is a non-communicable disease with a continuously increasing incidence and is a public health problem (WHO 2021). Data in Padang Pariaman Regency shows an increase in cases from 5,238 in 2024 to 8,932 in 2025. The purpose of this paper is to describe family nursing care for families with Diabetes Mellitus in the Lubuk Alung Community Health Center (Puskesmas) working area. Diabetes Mellitus can cause complaints such as increased thirst, frequent hunger, fatigue, and tingling, and carries a risk of complications. The method used is a case study with a nursing process approach including assessment, diagnosis, intervention, implementation, and evaluation. Case analysis revealed the main problems were acute pain, unstable blood glucose levels, and ineffective health maintenance. Interventions focused on health education, monitoring the patient's condition, and increasing the family's role in care. The results indicate an increase in family knowledge and independence in caring for patients and efforts to prevent complications.  The recommendations given are that families are expected to be able to support patients in controlling blood sugar regularly every month and taking medication regularly, and health workers can improve education and family nursing services to support optimal management of Diabetes Mellitus.

Abstract

According to WHO data in the journal (Andari, 2023), the number of gastritis cases in the world every year is estimated to reach around 1,800,000 to 2,100,000 people. Meanwhile, in the Southeast Asian region, the incidence of gastritis is recorded as many as 583,635 cases per year from the total existing population. The purpose of this study is to provide nursing care for the elderly with gastritis at the Tresna Werdha Sabai Nan Aluih Sicincin Social Home. Gastritis is an inflammation of the gastric mucosa that can cause complaints such as heartburn, nausea, vomiting, and bloating, and can cause complications if not treated properly. The research method used was a case study by taking one patient as the research subject. The nursing care process is carried out through the stages of review, enforcement of nursing diagnosis, planning, implementation, and evaluation. The assessment will be carried out from February 11, 2026 to February 16, 2026. The results of the nursing diagnosis that emerged, namely chronic pain, fatigue, and health management were ineffective. Interventions include pain management, energy management, as well as health education and monitoring of patient conditions. After undergoing the application for 5 days, results were obtained that showed a reduction in pain, energy after rest, and an improvement in the patient's ability to manage their health. It is hoped that patients can maintain a healthy lifestyle, regulate their diet regularly, avoid foods that trigger gastritis, and regularly carry out health checkups so that the condition does not recur.

Abstract

Acute Respiratory Tract Infection (ARI) is a common disease in children and is a public health problem because it can disrupt respiratory function and daily activities (WHO 2023). Data from the Pauh Community Health Center in Padang City shows a high incidence of ARI in children, reaching 39.3% in 2025. The purpose of this paper is to describe Family Nursing Care for Families with ARI in the Pauh Community Health Center in Padang City. Acute respiratory tract infections (ARI) are usually transmitted through the upper or lower respiratory tract and can cause various illnesses, from mild to serious. The method used is a case study with a nursing process approach including assessment, diagnosis, intervention, implementation, and evaluation. Case analysis revealed the main problems were ineffective airway clearance, disturbed sleep patterns, and readiness to improve health management. Interventions focused on monitoring respiratory status, providing education on ARI care, improving rest quality, and empowering families in managing their children's health. Results showed improvements in respiratory conditions, sleep quality, and increased family knowledge and involvement in patient care. The recommendation given is that families are expected to be able to carry out independent care, maintain a healthy environment, and utilize health service facilities optimally to prevent complications and recurrence of ARI.

Abstract

Acute Decompensated Heart Failure (ADHF) is a cardiovascular emergency characterized by an acute deterioration in heart function that requires immediate treatment. ADHF is a global health issue, with approximately 3 million cases reported annually. In Indonesia, the prevalence of heart disease is 0.85%, while in West Sumatra Province it is 0.87%. Medical record data at Padang Pariaman Regional General Hospital show an increase in ADHF cases, with 100 cases in the past year in the HCU ward. This case study aims to provide an overview of the implementation of nursing care for patients with ADHF. The method used was a case study employing a nursing process approach through observation and interviews, conducted over 5 days from February 25 to March 1, 2026, in the HCU Ward of Padang Pariaman Regional General Hospital on one patient diagnosed with ADHF. The results of the case study revealed five primary nursing problems: decreased cardiac output, ineffective airway clearance, acute pain, activity intolerance, and sleep pattern disturbance. Interventions were implemented over 5 days, focusing on cardiac care, airway management, pain management, energy management, and sleep support. Evaluation showed a gradual improvement in the patient’s condition, with sleep disturbances resolved by day 2, acute pain resolved by day 3 with a pain score of 2, airway clearance resolved by day 4, The decreased cardiac output resolved by day 5, with an oxygen saturation of 98%, and activity intolerance was partially resolved. It is hoped that the patient and family will follow the healthcare providers’ recommendations, maintain a healthy lifestyle, and attend regular follow-up visits at a healthcare facility to prevent recurrence.

Abstract

According to the World Health Organization, the prevalence of gout arthritis worldwide continues to increase each year, with an incidence ranging from 1%–4% in the general population. It is more common in men than women, especially in Western countries, with a prevalence of around 3%–6%, and in individuals over 80 years old it can reach 10% in men and 6% in women. Gout arthritis is a joint inflammation caused by the accumulation of uric acid crystals due to purine metabolism disorders. If not managed promptly and appropriately, it can lead to joint damage, tophi formation, heart disease, kidney stones, and reduced quality of life. This case study aims to apply family nursing care to a family with gout arthritis. The method used is the nursing process, including assessment, diagnosis, planning, implementation, and evaluation with a descriptive design. Data were collected through interviews, observations, and physical examinations. The results showed that the family experienced joint pain and a lack of knowledge. Three nursing diagnoses were identified: chronic pain, ineffective health management, and deficient knowledge. After 9 days of intervention including health education, pain management, and family involvement, there was a reduction in pain and an improvement in the family’s knowledge and ability to provide care. Family nursing care for patients with gout arthritis was effective in improving knowledge, reducing pain, and enhancing family independence in care through education and the application of a healthy lifestyle.

Abstract

Acute Gastroenteritis (GAE) is a major health problem in children that contributes to high morbidity and mortality rates. Globally, there are approximately 1.7 billion cases per year, with approximately 443,832 deaths in children aged <5 years. In Indonesia, the prevalence of GEA reaches 60–80% in children, while in the Marwa Room of Aisyiyah Pariaman Hospital, cases increased from 42 cases (2023), 60 cases (2024), to 61 cases (2025) and ranks second most common disease. This study aims to describe nursing care for children with GEA through the stages of assessment, diagnosis, intervention, implementation, and evaluation. The method used is a case study on An. E on February 14-18, 2026 in the Marwa Room of Aisyiyah Pariaman Hospital. The results of the study showed nursing problems in the form of diarrhea, hypovolemia, hyperthermia, and nutritional deficits. Interventions include monitoring vital signs, administering fluids, monitoring intake-output, and family education. Evaluation showed a decrease in diarrhea frequency, normal body temperature, and improved hydration status. Parents are advised to administer oral rehydration salts promptly, maintain hand hygiene, ensure food hygiene, and recognize signs of dehydration such as dry mouth and lethargy. Healthcare workers need to increase family education to prevent complications of GEA.

Abstract

Dengue Hemorrhagic Fever (DHF) is an infectious disease caused by the dengue virus and transmitted through the bite of the Aedes aegypti mosquito. This disease remains a public health concern; globally, there have been 14.6 million reported cases of Dengue Hemorrhagic Fever (DHF) with more than 11,000 deaths. In Indonesia, the incidence rate is quite high, reaching 131,393 cases with 544 deaths. West Sumatra recorded 88,593 cases, including in the city of Pariaman, which has geographical conditions conducive to vector development, recording 85 cases. Based on the data, DHF cases rank among the top 15 diseases at Prof. H. Muhammad Yamin Pariaman Regional General Hospital, accounting for 6.32% of cases. The objective of this study is to provide comprehensive nursing care for patients with DHF. This study employed a case study method with one patient in the internal medicine ward at Prof. H. Muhammad Yamin General Hospital in Pariaman from February 9–13, 2026, using the nursing process approach, which includes assessment, diagnosis, intervention, implementation, and evaluation. Data collection techniques included interviews, observations, physical examinations, and document review. The results of the study showed that the nursing diagnoses identified were hypovolemia, risk of bleeding, hyperthermia, and risk of nutritional deficit. The evaluation showed that hypovolemia improved on the 5th day, the risk of bleeding improved, and hyperthermia was resolved The patient’s fever peaked on the fourth day, and their nutritional status improved, as evidenced by an increased appetite. The patient is advised to maintain adequate fluid intake, get plenty of rest, and seek immediate medical attention if symptoms recur. Family members are encouraged to actively support the patient, practice good hygiene and healthy living habits, and prevent mosquito breeding in the home environment.
