Abstract

Background: Coal mining operations involve high-risk activities that require effective oversight and comprehensive risk management to prevent occupational accidents, operational disruptions, and financial losses. Despite increasing digital transformation initiatives within the mining sector, studies examining the effectiveness of digital oversight systems in supporting risk management remain limited, particularly among Indonesian coal mining contractors. Purpose: This study aimed to evaluate the implementation of digital systems in mining oversight at PT X, a coal mining contractor in Indonesia. Method: A qualitative case-study approach was employed using in-depth interviews with key informants, field observations, and document reviews. Data were analyzed through thematic analysis and triangulation to evaluate the implementation of digital technologies integrated into operational supervision and risk-management processes. Results: The findings revealed that PT X implemented several digital oversight technologies, including QHSE Automation, digital mining-sign monitoring, Slope Stability Radar (SSR), dashboard-based analytics, and Mining Eyes (CCTV) systems. These technologies improved real-time monitoring, hazard reporting, corrective-action tracking, and operational visibility. The implementation of digital systems contributed to a reduction of more than 20% in the Incident Frequency Rate and a 77% reduction in incident cost. Furthermore, digitalization improved hazard identification, accelerated risk-control processes, strengthened supervisory effectiveness, and supported data-driven decision making. Conclusion: Digitalization plays an important role in strengthening risk management and improving oversight effectiveness within mining operations. The integration of digital technologies supports proactive hazard management, improves safety performance, enhances operational efficiency, and contributes to safer and more sustainable mining practices.

Abstract

Background: Spiritual care is an essential component of holistic healthcare, with equal importance to physical, psychological, and social care. Nurses have the primary responsibility for assessing and addressing patients' spiritual needs, while chaplains or spiritual care providers serve as professional partners who complement spiritual care services, particularly for patients in critical, palliative, and end-of-life settings. However, the implementation of collaborative spiritual care between nurses and chaplains continues to face various challenges. Purpose: This scoping review aimed to map the existing evidence on the roles of nurses and chaplains in providing spiritual care, identify key concepts related to spiritual care competencies, interprofessional collaboration, and barriers to implementation, and explore evidence gaps in the literature, particularly within Indonesia and other Muslim-majority countries. Method: A scoping review was conducted following the Arksey and O'Malley framework, as refined by the Joanna Briggs Institute (JBI). Literature searches were performed in PubMed, CINAHL, Scopus, Google Scholar, and Garuda Portal for studies published between 2021 and 2025. The article selection process followed the PRISMA Extension for Scoping Reviews (PRISMA-ScR) guidelines, resulting in 40 studies that met the inclusion criteria. Results: Three major themes emerged from the synthesis: (1) the roles of nurses, including spiritual assessment, spiritual interventions, patient advocacy, and documentation; (2) the roles of chaplains or spiritual care providers, including spiritual counseling, end-of-life support, mediation among patients, families, and healthcare teams, and education for healthcare professionals; and (3) interprofessional collaboration, demonstrating that integrated spiritual care models contribute to improved patient satisfaction, enhanced quality of care, and reduced spiritual distress. Conclusions: Nurses and chaplains play complementary and indispensable roles in addressing patients' spiritual needs. Strengthening nurses' spiritual care competencies, standardizing chaplain competencies within hospital settings, and developing integrated interprofessional collaboration models are essential strategies for improving the quality of patient-centered spiritual care.

Abstract

Background: HIV/AIDS cases in Aceh reached 1,641 in 2024, with 176 cases in Bireuen and 11 patients at Cot Ie Jue Community Health Center. Stigma and discriminatory attitudes among healthcare workers (HCWs) pose a major challenges for people living with HIV/AIDS, affecting service quality. Purpose: To explore the causes, manifestations, and perceived consequences of HIV-related stigma among healthcare workers at Cot Ie Jue Community Health Center, Bireuen, Indonesia. Method: Qualitative research was employed. Data were collected through in-depth interviews and focus group discussions (FGDs) with 25 informants (doctors, nurses, midwives, etc.) in July 2025, analyzed through reduction, display, and conclusion drawing. Results: HCW stigma is driven by a lack of knowledge and misconceptions about HIV transmission and its impact. This stigma causes anxiety and discomfort in providing care, leading to discriminatory attitudes, avoidance, and low empathy. Patients’ understanding of the importance of ARV therapy and side effects also remains a challenge. Conclusion: Continuous education and training on HIV/AIDS (transmission, prevention, treatment, care), along with effective communication training, are crucial to enhance HCWs' knowledge and ensure empathetic, stigma-free, and supportive interactions with patients.

Abstract

Background: Cultural diversity in health care, particularly within HIV/AIDS programs in Papuan communities, necessitates that nursing students develop leadership skills sensitive to variations in values, beliefs, and community practices. Cultivating multicultural leadership is therefore a critical component in strengthening the professional competence of future nurses. Purpose: To examine the experiences of nursing students in developing multicultural leadership during the implementation of HIV/AIDS programs in Papuan communities. Method: A descriptive phenomenological study was conducted among 12 nursing students using purposive sampling. Data were collected through semi-structured interviews and analyzed using Colaizzi's seven-step phenomenological method. Results: Five major themes emerged: (1) multicultural awareness as the foundation of leadership; (2) adaptation processes in culturally diverse settings; (3) the role of mentors in leadership development; (4) communication challenges in community practice; and (5) the formation of culturally sensitive leadership. Field experiences were found to play a pivotal role in shaping students’ multicultural leadership competencies. Conclusion: Multicultural leadership among nursing students develops through experiential learning, reflection, and community engagement. Integrating culture-based learning into nursing curricula is essential to prepare students for the complexities of delivering health services in diverse populations.

Abstract

Background: Fatigue contributes to approximately 20% of occupational incidents and is identified as an underlying factor in 25% of fatal and serious accidents. This condition is often exacerbated by high job demands, which can induce emotional strain in the form of occupational stress. Globally, occupational stress affects an estimated 264 million workers. Purpose: To analyze the relationship between occupational stress and work fatigue among small-vessel crew members at PT X. Method: This quantitative study used a cross-sectional design involving 54 small-vessel crew members. Data were collected using the Multidimensional Fatigue Inventory (MFI-20) and Depression Anxiety Stress Scale (DASS-21) questionnaires. The data were analyzed using the chi-square test. Results: A significant relationship was found between occupational stress and work fatigue among small-vessel crew members, with a significance value of 0.013 (p < 0.05) and an odds ratio (OR) of 5.714. Among small-vessel crew members experiencing occupational stress, 23 individuals (95.8%) also experienced work fatigue. Conclusion: The findings indicate that occupational stress is significantly associated with work fatigue and represents an important risk factor for fatigue development among small-vessel crew members. Small-vessel crew members experiencing occupational stress had 5.714 times higher odds of experiencing occupational fatigue compared with those without occupational stress.

Abstract

Background: Outpatient waiting time is a key quality indicator that significantly influences patient satisfaction. Waiting times exceeding 60 minutes, as stipulated by the Minimum Service Standards (SPM), have been associated with reduced patient satisfaction and a decline in perceived healthcare service quality. Purpose: To map and synthesize scientific evidence regarding the relationship between outpatient waiting time and patient satisfaction across various healthcare facility settings. Method: This study employed a literature review design. Relevant articles were identified through systematic searches of electronic databases, including PubMed, Scopus, Garuda, CINAHL, Medline, and Google Scholar. Studies published between 2020 and 2025 that examined the association between outpatient waiting time and patient satisfaction were included. The selected studies were critically reviewed and narratively synthesized to identify patterns of findings and factors influencing the relationship between waiting time and satisfaction. The article selection process followed the PRISMA framework, resulting in 22 articles included for thematic analysis. Results: The findings indicate that outpatient satisfaction generally decreases as waiting time increases. This relationship is influenced by patient-related factors, including individual perceptions, expectations, and the availability of information regarding waiting processes. In addition, organizational and service-related factors, such as limited healthcare personnel, high patient volume, and inefficient service delivery systems, contribute to prolonged waiting times and reduced satisfaction. Conclusion: Waiting times that comply with Minimum Service Standards (≤60 minutes) are consistently associated with higher outpatient satisfaction. Effective waiting time management should not only focus on reducing actual waiting duration but also on managing patient expectations through clear communication and adequate information provision.

Abstract

Background: Anemia remains a major health problem among adolescent girls. Although the distribution of iron tablets continues to expand, adherence remains low due to forgetfulness, lack of motivation, and insufficient monitoring. The FeUP (Fe uptake package) Box is an integrated adherence support intervention that combines a pill box, educational and instruction cards, a self monitoring card, and parental involvement to simultaneously address these barriers and improve adherence to iron tablet intake. Purpose: To analyze the effectiveness of the FeUP box in improving adherence to iron tablet intake among adolescent girls with anemia. Method: A quasi-experimental study was conducted using a nonequivalent control group with a pretest–posttest design. Participants were selected through purposive sampling from SMPN 29 Bandung, within the Ledeng Community Health Center service area, and SMPN 2 Parongpong, within the Ciwaruga Community Health Center service area. The sample consisted of 52 adolescent girls diagnosed with anemia, divided equally into an intervention group (n = 26) and a control group (n = 26). Adherence to iron tablet consumption was assessed using the Morisky Medication Adherence Scale-8 (MMAS-8). The intervention group received FeUP Box educational materials for four weeks, while the control group followed the school’s standard iron supplementation program without additional education. Data analysis employed the Wilcoxon Signed-Rank Test to examine within-group changes in adherence and the Mann-Whitney U Test to compare adherence outcomes between the intervention and control groups. Results: Adherence scores in the intervention group increased from 3.57 ± 1.37 (95% CI: 3.01–4.12) to 5.25 ± 1.23 (95% CI: 4.75–5.75) after the intervention, with a significant improvement (p < 0.001; r = 0.692). In contrast, no significant change was observed in the control group, with adherence scores changing from 3.04 ± 1.46 (95% CI: 2.45–3.63) to 2.86 ± 1.33 (95% CI: 2.32–3.39) (p = 0.317; r = 0.196). The increase in adherence scores was significantly greater in the intervention group than in the control group (p < 0.001; r = 0.585). As a supporting objective measure, hemoglobin levels in the intervention group showed an upward trend after the intervention; however, the increase was not statistically significant. Conclusion: The FeUP box effectively improved adherence to iron tablet intake among adolescent girls with anemia. Integrating the FeUP box into school-based iron supplementation programs may provide a practical strategy to strengthen adherence and support anemia prevention among adolescent girls.

Abstract

Background: Smoking among primary healthcare workers remains an important public health concern because healthcare workers are expected to serve as role models and play a central role in tobacco prevention and smoking-cessation interventions. However, tobacco use persists among healthcare workers and may affect their credibility and engagement in providing smoking-cessation support to patients. Purpose: To identify the prevalence and determinants of smoking behavior among primary healthcare workers. Method: A systematic review of studies published between 2010 and 2026 was conducted using PubMed, Scopus, and ScienceDirect, following the PRISMA 2020 guidelines. A total of 193 articles were identified, of which nine cross-sectional studies involving primary healthcare workers met the inclusion criteria and were included in the review. Results: The included studies were conducted in Syria, Saudi Arabia, India, Jordan, Qatar, Italy, South Africa, and two regions of Iraq. The prevalence of smoking and tobacco use varied considerably across countries, professional groups, sex distributions, and operational definitions of tobacco use. Smoking prevalence was consistently higher among male than female healthcare workers. Smoking behavior was associated with multiple interconnected factors, including gender, professional and occupational characteristics, knowledge and attitudes, occupational stress, family and peer influences, social and workplace norms, enforcement of smoke-free policies, and the availability of smoking-cessation services. Although healthcare workers generally showed awareness of the harmful effects of tobacco and favorable attitudes toward tobacco control, a persistent knowledge, attitude, and practice gap was identified. Healthcare workers who smoked were also less likely to provide cessation counseling or support tobacco-control policies. High interest in quitting was reported among current tobacco users; however, limited cessation support, nicotine dependence, occupational stress, and environmental influences remained important barriers. Conclusion: Smoking among primary healthcare workers is influenced by individual, occupational, social, and organizational factors. Comprehensive interventions involving cessation support, stress management, professional training, and consistent smoke-free policies are needed.

Abstract

Background: Craft workers are particularly vulnerable to developing Musculoskeletal Disorders (MSDs), with reported prevalence ranging from 38.5% to 100%, including among bag artisans. MSDs are influenced by individual factors, work-related factors, and environmental conditions. The Tanggulangin area is well known as a center for the bag and suitcase industry and was identified as a population at risk based on preliminary observations. Purpose: To describe individual factors, work-related factors, vibration exposure, and complaints of Musculoskeletal Disorders (MSDs) among bag artisans. Method: This study employed a descriptive analytical design with a cross-sectional study. A total of 38 bag artisans participated in the study. Data were collected using the Rapid Entire Body Assessment (REBA) questionnaire to assess work posture, the Nordic Body Map (NBM) questionnaire to evaluate MSD complaints, and a vibration meter to measure vibration exposure. Results: The findings indicated a tendency for smoking habits (OR = 1.123) and awkward working posture (OR = 1.500) to be associated with MSD complaints. Meanwhile, environmental exposure in the form of vibration intensity did not exceed the established Threshold Limit Value (TLV). Conclusion: No statistically significant associations were identified between the examined individual or occupational factors and MSD complaints. All measured vibration exposures were below the applicable threshold limit, preventing comparison between different exposure levels.

Abstract

Background: Delayed onset of lactogenesis II often triggers early formula feeding, which hinders the success of exclusive breastfeeding. Nutritional fulfillment and mechanical stimulation are needed simultaneously to optimize prolactin and oxytocin hormones. Purpose: To analyze the synergy between consuming fresh Moringa oleifera (moringa) juice and oxytocin massage on accelerating the onset of lactogenesis II in postpartum mothers. Method: This study used a cross-sectional descriptive comparative-correlational design involving 70 postpartum mothers. This quantitative study used a quasi-experimental design with a posttest-only control group design, conducted in Bangun Rejo Village, North Sumatra, from February to April 2026. A sample of 36 normal postpartum mothers was selected using purposive sampling and divided equally into two groups (n=18 each). The intervention group was given juice from 20 grams of fresh Moringa leaves twice daily and oxytocin massage for 10–15 minutes daily, while the control group received only standard care. Data normality was tested using the Shapiro-Wilk test, and bivariate analysis was performed using the Independent T-Test. Results: The baseline demographic characteristics (age, parity, education, and Body Mass Index) of both groups were homogeneous (p > 0.05). The mean onset time of lactogenesis II in the combination intervention group was significantly faster (34.50 ± 4.25 hours postpartum) than in the control group (56.60 ± 5.80 hours postpartum), with a difference of 22.10 hours. Independent T-Test results showed a highly significant statistical difference (p < 0.001). Conclusion: The synergy between fresh Moringa oleifera juice consumption and oxytocin massage significantly accelerated the onset of lactogenesis II. This dual-action approach efficiently combines the natural galactagogue properties of Moringa oleifera to stimulate prolactin with the neuroendocrine reflex of massage to facilitate milk ejection.
