Abstract

Background: Root canal treatment is a treatment that aims to clean pulp tissue or microorganisms so that the root canal can be filled properly. Prevention of microleakage in the coronal part from contamination of oral fluids and microorganisms into the root canal can be achieved with a good coronal seal. Case: A 37-year-old female patient came with complaints of a broken right front upper jaw tooth caused by being hit by a door one month ago. Examination showed that tooth 11 had pulpitis irreversible asymptomatic with periodontitis apikalis symptomatic. The treatment plan is root canal treatment. Case Management: Root canal treatment begins with access opening, crown-down preparation, obturation using a single cone technique using F3 protaper guttapercha, and the coronal seal of the root canal being sealed with Universal Type Glass Ionomer cement. Conclusion: The choice of glass ionomer cement material as a root canal coronal seal against external factor contamination has quite good quality. Abstract

Orbital cellulitis is a rare but serious complication of dental infection and if left untreated can progress to blindness, cavernous sinus thrombosis, meningitis, subdural empyema, brain abscess and death. Awareness of the possible spread of odontogenic infection to the eye socket is important, so that appropriate treatment can be initiated as early as possible. The case of a three-year-old five-month-old girl presented with a three-day history of progressive right periorbital swelling associated with low-grade fever. The swelling first appeared in the right upper perioral region, extending upward to the right cheek and right lower eyelid. However, no red eye was observed, nor were there any watery, eye, ear, or nasal discharge. There is no history of trauma. Intraoral examination of the upper right area revealed 52 residual root elements, 53 irreversible pulpitis and 54 pulp necrosis with the upper pulp uncovered. Oral hygiene was poor in the area of tooth 54 where the mucobuccal folds were found to be raised and reddish, making tooth 54 the suspect cause of odontogenic infection. The therapy given was removal of the pulp roof on tooth 54 and medication in the form of D5 fluid infusion, intravenous injection of ceftriaxone, santalgesic, ranitidine and dexamethasone. There was significant improvement after 3 days of treatment. Periorbital cellulitis in this case is an odontogenic case that can be fatal if not treated properly. this case shows how important the treatment of pediatric primary teeth is.

Abstract

Background: The Periodontopathogen bacteria is the main etiology of periodontal disease such an fibromatous epulis. Epulis is such a gingival hyperplasia (tumor-like) that came from connective tissue of periodontal. It can caused by Periodontopathogen bacteria and aggravated by a local irritation that occur in long period. The inflammation process made the epulis getting bigger and painfull sometimes. Patient feels unconfidence with this lump mean while the epulis getting bigger. On the other hand, there is some halitosis and uncomfort feeling during chewing and speaking regarding to the epulis. Epulis that grows on the anterior maxilla area can be inconvenient because it became an aesthetics problem and also disrupts the function of the mastication system. Therefore, it has to be completely removed by surgery and some host immunomodulation to eliminate the inflammation and collagenases activity. Case Report: A 44-year-old male patient presented with a chief complaint of a lump on the gum of the anterior teeth on his upper jaw that made him unconfident to smile, difficulties in chewing and speaking. The lump consistency is dense and chewy, bleeds easily, had been there for 3 years. Panoramic reveal a slight bone resorption at the right maxillary second incisor. Case Management : Gingivectomy and gingivoplasty procedures has been done to correct the condition of epulis which is excision and gingival contouring for aesthetic need and Followed with host immunomodulation therapy by using 20 mg low dose doxycicline twice a day for 14 days. Discussion : The condition of epulis can be corrected by gingivectomy and gingivoplasty procedure also by removing the main cause of the epulis (sub-gingival calculus). The aesthetic result can be obtained by gingival contouring after the excision. Host modulation theraphy been given to the patient by using low dose doxycycline for eliminating collagenases activity and breaking the inflammation process in gingiva in order to stop the epulis relapsed. This 20 mg dose of Doxy prevented the elevation of the pro-inflammatory enzyme COX-2 and its downstream product PGE2, which is a potent inflammatory mediator. Conclusion : periodontal surgery treatment need to be followed by certain therapy such as host modulation therapy for gaining successful treatment of periodontal diseases. Keywords : Epulis Fibromatous, inflammation, low dose doxycycline

Abstract

Background: Murine double minute-2 (Mdm-2) gene expressed overly to the various type of human cancers. Murine double minute-2 associates with p53 affecting negatively with its gene products for activation of its transcriptional function. These genes mainly expressed in oral squamous cell carcinomas with p53 mutation condition. MicroRNAs nowadays becoming a promising diagnostic and prognostic biomarkers for malignancy through the expression and regulating the transcriptional function. It is reported that mir-218 expression may indicate the occurrence of OSCC. Purpose: The literature describes the putative pathway and function of mdm2 gene as transcriptional antagonist function of p53 in OSCC of mir-218 expression. Review(s): The relationship of mdm2 with mir-218 expression in OSCC may indicate the alternative mechanism of p53 dysfunction condition. Downregulated mir-218 will tend less binding to promoter of p53 causing the mutant state. The overexpression of mdm-2 may constitute inactivation of p53. Conclusion: The study findings may suggest mdm2 protein causing in OSCC due alternative mechanism of p53 as downregulated expression of mir-218.
