Abstract

High turnover is a problem in the management of human resources at hospitals. The nurse turnover rate at Islamic Hospital "Ibn Sina" Yarsi Bukittinggi, West Sumatra, has tended to rise each year, with data from 2009 and 2010 showing rates of 21.3% and 24.3%, respectively. The purpose of this research is to identify factors associated with nurse turnover trends at Islamic Hospital "Ibn Sina" Yarsi Bukittinggi, West Sumatra. This research uses a descriptive analytic cross-sectional study approach. The sample consisted of 75 people, and data was collected using questionnaires. Data analysis was conducted using the Chi-Square test. The results showed that 54.7% of nurses had a tendency to leave the hospital. There was a relationship between external factors, namely the availability of jobs in other institutions, and the tendency to leave (p-value 0.039). However, no relationship was found between internal organizational factors (such as the size of the organization, leadership, compensation, job satisfaction, career development, job weights, centralization) and individual employee factors with the tendency to leave. To reduce nurse turnover, it is important for hospital administrators to review programs aimed at addressing the issue. Such programs should focus on improvements in nursing leadership, career development, and efforts to enhance job satisfaction for nurses.

Abstract

Pandan Wangi (Pandanus amaryllifolius Roxb) is commonly used by Indonesian people. The chemical contents of pandan wangi are: tannin, saponin, alkaloid, flavonoid, and color essence (DepKes RI, 1997). According to some research on other vegetation such as star fruit (Rezano.A, 2008), which found that the active ingredients have an anti-fertility effect on mice. High-rate population problems in Indonesia nowadays that increased 1.3 percent per year (BPS, 2007) creating a demand for safe, effective, high-reversibility contraception methods which are not influenced by libido and sexual activity. So, herbal medicine is one of the alternatives to provide family planning methods that meet the requirements. The objective of this research was to find out the influence of prescribing pandan wangi leaves extracts in the depreciation of testicle mass weight and diameter of tubulus. The nature of this research was experimental research using a Posttest Only Control Group Design. Research conducted in the laboratory of the Medical Faculty of Andalas University, starting from July 25, 2008, to August 30, 2008. The population was 24 Japanese's strain mice originated from the Mathematics and Sciences Faculty of was used in analyzing with one control group. ANOVA test was used in analyzing with a degree of significance was 95%, and if the results are significant, then it will be extended with a Bonferroni type Multiple Comparisons test (Post Hoc Test). General results of this research initiate that prescribing the extracts of pandan wangi leaves have influences in the depreciation of testicle mass weight p=0.000, tubulus diameter p=0.001. The null hypothesis is rejected (p < 0.05). Meanwhile, at the Bonferroni Multiple Comparisons test found that statistically, there was no significant influence between groups of treatment. As a conclusion of this research, it was that active ingredients containing in pandan wangi leaves could distract the spermatogenesis process on mice in the depreciation of testicle mass weight and diameter of tubulus. Extension research suggested using different and increased prescribing dosages.

Abstract

The low adherence of pregnant women in consuming Fe tablets is regarded to be the main cause of the ineffectiveness of the Fe supplementation program held by the government to overcome anemia issues on pregnancy. One of the factors that influence the adherence of pregnant women is family support. This research was aimed at finding out the relationship between family support and the adherence in consuming Fe tablets among pregnant women in Puskesmas Nanggalo Kecamatan Nanggalo, Padang, West Sumatra in 2010. This research was designed in a correlation study with a cross-sectional approach and 32 respondents. Respondents were those who had got Fe tablets in their last visit from Puskesmas and also had a health history with anemia in pregnancy. The data was collected by using a questionnaire. Univariate and bivariate analyses were done to variable family support as well as the adherence of pregnant women. Bivariate analysis was done by cross-tabulation with a Chi-Square test. The result of the research showed that 54.9% of respondents got low family support and 71.9% of respondents had low adherence in consuming Fe tablets. From the statistics experiment, it can be concluded that there is a relationship between family support and women's adherence in consuming Fe tablets (p < 0.05). Therefore, it is suggested that nurses or other health workers in order to start asking and taking the family members, especially for husbands, to join treatment toward pregnant women with anemia.

Abstract

Diabetes mellitus is a metabolism disease associated with a number of microvascular (retinopathy, neuropathy and nephropathy) and macrovascular (ischemic heart disease, cerebrovascular disease and peripheral vascular diseases) complications. Soybeans have been consumed for a long time as an important protein source to complement grain protein soybean contain various nutritious and functional components such as isoflavonoids, which are beneficial against stress oxidative. This study aimed to explain soybean effect to nephropathy diabetic. Soybean have some advantages like isoflavonoids, protein, fiber, and fitoestrogen. All of advantages are used to eliminate diabetic complication.

Abstract

One of the non-pharmacological therapies that can be used to reduce the frequency of hallucinations is general individual therapy by the implementation of communication strategies. The purpose of this study was to determine the effect of general individual therapy in reducing the frequency of hallucinations of patients at RSJ. H.B Saanin Padang. This study used a quasi-experimental design in one group (one group pre-test - post-test design). Sampling was done by purposive sampling. The samples of this study consisted of 13 people. Samples were provided with individual therapy using the implementation of communication strategy approach for 14 days. After that, samples were measured with regular interviews. This data was then analyzed using the Wilcoxon test with a 95% degree of confidence. Wilcoxon test results obtained p-value = 0.001. This result showed that the individual therapy approach to the implementation of communication strategies was effective in reducing the frequency of hallucinations of patients with hallucinations in the psychiatric hospital H.B Saanin Padang.

Abstract

Stroke exercise physical therapy is routinely performed in Dr. M. Djamil Hospital with the purpose of increasing the functional abilities of stroke patients. This study aimed to determine the difference in effectiveness of exercise therapy twice a week and three times a week in improving the functional abilities of stroke patients. The study was conducted for four weeks using a quasi-experimental design with a non-randomized approach without a pretest-posttest control group design. The sample consisted of 22 respondents who experienced ischemic stroke. The instrument used for data collection was a sheet measuring functional ability based on the Barthel index scale. The analyses were performed using the Shapiro-Wilk normality test, paired t-test, and Mann-Whitney test. The results showed that exercise therapy can improve stroke patients' functional ability after stroke, with the mean of therapy exercise stroke twice and three times a week being 9.636 (p=0.000) and 10.909 (p=0.000), respectively. However, the difference in the increase in functional ability between the two groups of participants was not statistically significant (p-value = 0.198). Based on the findings of this study, it is suggested that the management of the department of Dr. M. Djamil Padang establish guidelines for this therapy in the form of SOPs, and that the frequency of stroke therapy be performed as calisthenics three times a week for optimum improvement of functional ability to be felt by patients who follow this therapy.

Abstract

Clinical learning environment is the vehicle of clinical learning. However, a less supportive clinical environment may trigger anxiety in students while learning. The purpose of this study was to determine the correlation between clinical learning environment relationships and levels of anxiety in student nurses in the Nursing Science Study Program, Faculty of Medicine, Andalas University, 2011. This research used a cross-sectional approach with 69 students as respondents. The data was collected by applying two questionnaires, SECEE and DASS. The study showed that 49% of student nurses have a bad clinical learning environment, and 56.5% of student nurses have high levels of anxiety. There was a significant association between the clinical learning environment and levels of anxiety, with Spearman rank analysis p-value=0.000 and r=-0.769. Application of a mentorship method is needed in any room while having clinical learning in order to increase trust, self-dignity, confidence, and competence achievement of students.

Abstract

Self-care deficit is an impaired ability to perform self-care activities. Lack of self-care results from changes in thought processes, leading to a decline in the ability to perform self-care activities. The token economy method is a behavior modification technique that involves giving a token or gesture as soon as each target behavior appears. This study used a pre-experimental design with a pretest-posttest one group approach to patients with self-care deficits in the Prof. HB. Sa’anin Mental Hospital, Padang, involving a total of 15 people from 19 November to 9 December 2011. The aim was to determine the effect of the token economy method on self-care activities. Sampling was conducted using a total sampling technique. The research variables were the token economy method and self-care activities. Data collection was done by measuring the respondents' self-care activities before and after treatment using an observation sheet. Data analysis was performed using SPSS with the Wilcoxon test. Results showed a significant effect of the token economy method on increasing self-care activities, with statistical test results showing p < 0.05. It is expected that Prof. HB Sa’anin Mental Hospital, Padang, will use the token economy method as a means to address self-care deficits.

Abstract

Most of the impact of the onset of stroke is weakness in the limbs, leading to decreased productivity and economic ability. Early rehabilitation is needed to reduce these disadvantages, using early mobilization after acute stroke. The research objective was to see the effectiveness of early mobilization on muscular strength, muscle tone, and functional motor skills in hemiparesis after ischemic stroke. This type of research is a quasi-experimental pretest-posttest group design. The subjects were hemiparesis patients after ischemic stroke, distributed into two groups: the first group, consisting of 10 people, was given mobilization training 2 times/day; the second group, consisting of 10 people, was given mobilization training 3 times/day. Training started on day 2 after stroke until day 5. Analysis was conducted using paired and unpaired t-tests, with a significance level of p < 0.05. The results showed an increase in muscle tone, muscle strength, and functional motor ability in both groups, with p = 0.000 for each group. Better improvement was obtained in muscle tone and muscle strength (shoulder, elbow, wrist, thigh, knee, ankle) and functional motor skills in the therapy group that received 3 times/day training compared to the 2 times/day group, with significance values: shoulder muscle strength p = 0.016; elbow muscles p = 0.037; wrist muscles p = 0.042; hamstring muscle p = 0.004; and ankle p = 0.050. Increased ability to tilt to the healthy side p = 0.000; increased ability to sit supine p = 0.000; maintain sitting balance p = 0.007; and the ability to sit up p = 0.007. It is suggested that early mobilization exercise therapy be used as an independent nursing intervention, performed 3 times/day or 2 times/day to help speed recovery and prevent further weakness complications.

Abstract

Chronic pain in children and adolescents is a problem that can affect every aspect of individual functions. The big change in the quality of life of a child dealing with chronic pain. Impaired function generally occurs i.e., sleeping, employment, social functioning, and daily activities. Monitoring of chronic pain should be a concern to the clinician in monitoring the way the pain so as to provide effective care and treatment. Pain is a subjective thing and needs expressed by individuals who experience such pain. One method of assessment of chronic pain in children and adolescents that can be applied in clinical practice is the use of electronic pain diary that combines handheld hardware technology advances such as smartphones and PDAs with pain assessment software program innovations that have proven their quality index. In order to use this application to apply the necessary attention to the ethical aspects, financial aspects of the tools and training, aspects of change in perspective on the importance of understanding the chronic pain patient in order to change behavior.
