Hypertension is a major risk factor for chronic kidney disease (CKD) with a complex bidirectional relationship. Appropriate selection of antihypertensive drugs in patients with hypertension and end-stage CKD is crucial to prevent disease progression and improve clinical outcomes. This study aimed to compare the effectiveness of Calcium Channel Blockers (CCB) and Angiotensin Receptor Blockers (ARB) on changes in blood pressure and kidney function in patients with hypertension and stage 5 CKD at RSI Ibnu Sina Bukittinggi. A descriptive-analytic design with a retrospective approach was employed, involving 30 patients who met the inclusion criteria from a total of 78 patients during the period February 2023–December 2024. Parameters observed included systolic/diastolic blood pressure, serum creatinine, glomerular filtration rate (GFR), and urea levels before and after therapy. The results showed that both therapy groups experienced reductions in blood pressure and improvements in kidney function. The decrease in systolic/diastolic blood pressure for CCB was 14.7/11.4 mmHg, while for ARB it was 14.2/11.2 mmHg. Increases in GFR, reductions in creatinine, and decreases in urea occurred in both groups with no statistically significant differences (p>0.05). In conclusion, CCB and ARB have comparable clinical effectiveness in controlling blood pressure and improving kidney function parameters in patients with hypertension and end-stage CKD.

The use of corticosteroids in the community is still quite high, even though these drugs are classified as strong medicines that should only be used with a doctor's prescription. Insufficient knowledge regarding indications, methods of use, and side effects can pose serious health risks. This study aims to determine the level of patient knowledge regarding the use of corticosteroid drugs and the types of corticosteroid drugs purchased at the New Bintang Ilahi Pharmacy in Bukittinggi. The research method used is descriptive prospective with data collection through questionnaires administered to 85 respondents during June 2025. The research results show that the respondents' level of knowledge regarding corticosteroid use is low, with an average correct score of 56.91%. The distribution of knowledge levels is: poor category 55.3%, sufficient 34.1%, and good 10.6%. The most frequently purchased type of corticosteroid was Dexamethasone 0.5 mg (57.64%), followed by Methylprednisone 4 mg and prednisolone 4 mg (35.29%). The conclusion of this study is that the majority of patients at the New Bintang Ilahi Pharmacy in Bukittinggi have a low level of knowledge regarding the use of corticosteroids, therefore more optimal education and counseling from pharmacy personnel is needed.

The use of antibiotics without prescription remains a community health issue that increases the risk of bacterial resistance and reduces treatment effectiveness. This study aimed to determine the relationship between the level of knowledge and attitudes of the community regarding the use of antibiotics without prescription in two pharmacies located in Pangkalan Koto Baru District. The research employed an analytic design with a cross-sectional approach. The study involved 100 respondents selected using purposive sampling who met the inclusion criteria. Data were collected using a structured questionnaire covering sociodemographic characteristics, knowledge, and attitudes related to antibiotic use. Data were analyzed using univariate, bivariate chi-square tests, and multivariate analysis to determine the strength of associations between variables. The results showed that most respondents had a moderate level of knowledge and a positive attitude toward the use of antibiotics without prescription. Chi-square analysis indicated a significant relationship between knowledge level and community attitudes (p < 0.05). The study concluded that better knowledge is associated with more positive attitudes toward rational antibiotic use. Continuous education through pharmacies and healthcare providers is necessary to improve community awareness of appropriate antibiotic use and prevent the emergence of resistance.

Health literacy is an important aspect in making health-related decisions, including behaviors in managing leftover medicines. Improper storage and disposal of unused or expired medicines can lead to serious issues such as drug misuse and environmental pollution. This study aimed to determine the relationship between the level of health literacy and leftover medicine management behavior among the community of Bukittinggi City. This research employed an observational analytic method with a cross-sectional design and cluster sampling technique involving 100 respondents. Data were collected using the HLS-EU-SQ10-IDN questionnaire to measure health literacy, along with an additional questionnaire to assess knowledge and behavior regarding leftover medicine management. The results showed that most respondents had a moderate level of health literacy (33.12%), good knowledge (96%), and good management behavior (76%). However, there was no significant relationship between health literacy level and knowledge (p = 0.613) or between health literacy and management behavior (p = 0.253). In conclusion, improving health literacy alone is insufficient to change community behavior in managing leftover medicines. Therefore, specific educational approaches, adequate disposal facilities, and clear household-level pharmaceutical waste regulations are needed to prevent potential health and environmental risks.

Bronchopneumonia is an inflammatory lung condition affecting one or more lobes, characterized by infiltrative patches caused by bacteria, viruses, fungi, and foreign substances. The purpose of this study was to obtain an overview and analysis of the appropriateness of antibiotic use. This was a descriptive observational study with a retrospective approach using secondary medical record data. The results for the description of antibiotic use were Seftriakson (53.4%), Sefotaksim (27.6%), and meropenem (19.0%). The results of the quality assessment using the Gyssens Method revealed that 1.7% of antibiotic prescriptions were in category III B, 19% in category IV A, and 79.3% in category 0. These data indicate that there is still irrational use of antibiotics. Assessed using the ATC/DDD methodology, resulting in a total of 25.57 DDD per 100 days. The calculation findings show the highest Defined Daily Dose (DDD) value for the antibiotic Seftriakson at 18.06 DDD per 100 days of hospitalization, while the lowest value was seen for Meropenem at 2.09 DDD per 100 days and Sefotaksim at 5.42 DDD per 100 days of hospitalization. Clinical parameters showed a P value > 0.05, with p = 0.88, indicating no statistically significant difference between antibiotic administration and leukocyte levels in pediatric bronchopneumonia patients in the Melati ward at Mukomuko Hospital in 2024.

Tuberculosis (TB) is a global health problem requiring patient adherence to treatment. This study aimed to evaluate the effect of pharmacist-led medication counseling on tuberculosis medication adherence in patients at Puskesmas Rasimah Ahmad and Puskesmas Mandiangin, Bukittinggi City. The research employed a quasi-experimental design with a pretest-posttest one-group approach. The sample comprised 24 TB patients selected through total sampling. Data were collected using the Morisky Medication Adherence Scale-8 (MMAS-8) questionnaire before and after the counseling intervention. Data analysis included univariate and bivariate analyses using Kendall's tau-b, Wilcoxon signed-rank test, and Mann-Whitney U test. The findings indicated no association between age and TB treatment adherence before and after counseling. However, there was an association between gender, education, and occupation with TB treatment adherence after counseling. Treatment duration also showed a significant influence on patient adherence. Patient adherence levels increased significantly after counseling in both health centers (Puskesmas Rasimah Ahmad: p=0.024; Puskesmas Mandiangin: p=0.034). No significant difference in adherence levels was found between the two health centers post-counseling (p=0.320). This study concludes that pharmacist-led medication counseling effectively improves TB patient adherence. Healthcare professionals, particularly pharmacists, are encouraged to provide interactive and repetitive counseling to TB patients to support treatment success.

The use of herbal medicines as an alternative therapy remains prevalent among rural communities for managing digestive disorders, despite limited scientific understanding of their safety and effectiveness. This study aimed to evaluate the level of knowledge, attitudes, practices, perceived effectiveness, and safety of herbal medicine use among residents of Tanjung Agung Village. A descriptive semi-quantitative design with a case study approach was employed, involving 50 respondents selected based on inclusion criteria. Data were collected through structured questionnaires and open-ended interviews, and analyzed descriptively. The findings revealed that most respondents were of productive age with basic educational backgrounds and commonly used herbal plants such as guava leaves, turmeric, young sapodilla, and papaya, predominantly prepared by boiling. Attitudes toward herbal remedies were generally positive, as they were perceived to be safer than chemical drugs, and many participants reported symptomatic relief. However, understanding of safety aspects remained limited, particularly regarding potential side effects and interactions with modern medications. This study concludes that the use of herbal medicines is primarily driven by cultural traditions rather than scientific literacy. Community-based health education is recommended to promote safer and more rational use of herbal remedies.

Pollution of liquid waste containing synthetic dyes is an environmental problem that is also relevant in the industrial and pharmaceutical laboratories. One of these dyes is Congo Red, which is stable, difficult to degrade, and has the potential to cause negative impacts on the aquatic environment. This study aims to produce the ability of chitosan which is generally synthesized from shrimp shells, this time synthesized from the pupa sheath of the Black Soldier Fly (Hermetia illucens). The adsorption test was carried out in batches with variations in pH, contact time, and initial concentration of the dye. Chitosan-BSF before and after absorbing Congo Red was characterized using FTIR and SEM-EDS. The results showed that the adsorption process was greatly influenced by the environmental pH and the pHPZC value of chitosan. The adsorption percentage increased at pH below pHPZC due to the electrostatic interaction between the chitosan surface which produces a positive signal and the Congo Red molecule which produces a negative signal. Contact time affected the rate of achieving adsorption equilibrium, while increasing the initial concentration tended to decrease the adsorption percentage. These results indicate that chitosan from Black Soldier Fly pupae casings has the potential to be an alternative adsorbent that has added value and is environmentally friendly and supports the principle of the desire for dye waste processing.
