Folia Medica Indonesiana, as indicated by its name, focuses on publishing good quality articles about research and education on health science and medicine in Indonesia. However, due to the fast growth of science and knowledge in these fields, we also welcome submitted articles from around the world, especially the ones that contain related matters from lower-middle income countries. Folia Medica Indonesiana is an open-access, peer-reviewed journal that is published online at least four times a year. The scope covers various aspects of basic medical sciences includes anatomy, physiology, pathology, microbiology, pharmacology, and molecular medicine) and clinical medicine (covers specialties like internal medicine, surgery, pediatrics, oncology, psychiatry, etc). We highlight the pathology and potential treatment of metabolic syndromes and infectious diseases. Folia Medica Indonesiana also encourages the publication of articles about health education. The scope includes, but is not limited to, articles that emphasize on preventive education on certain diseases in a community, also research report of various materials and/or methods to develop medical education. We recognize the importance of this type of articles to be published alongside the assigned topic in each of our yearly issues, to provide our readers with updated information in medical sciences’ research and education simultaneously.

Abstract

Storing EBM at room temperature in several hours before consuming, frequently found in Indonesia. Based on Academy of Breastfeeding Medicine guidelines EBM can last for 6 to 8 hours in room temperature (25oC or 77oF). However, currently there hasn't been study in tropical country especially Indonesia for the guidelines. This study aimed to assess microbiological quality of EBM on room temperature, including bacterial growth and major bacterial found on EBM for health care and society recommendations. An observational study of 30 expressed breast milk samples provided by 30 healthy women with term baby below 6 month old. The samples were kept sterile and laid at plates for 0 hours, 2 hours, 4 hours and 6 hours in room temperature (26°-32° C) and used drop plate technique on several culture media. Data was analyzed by Chi-square and paired sample T-test. Thirty of unheated fresh EBM from 30 lactating mothers were stored at room temperature, examined for the degree of bacterial contamination at 0 hour, 2 hours, 4 hours, and 6 hours. All the EBM samples were contaminated at 2 hour. Bacterial species identified was Coagulase-negative Staphylococcus (CNS), Escherichia coli, Klebsiella pneumoniae and Streptococcus faecalis, range of growth 109 cfu/ml-63 x 109 cfu/mm3 after 6 hour of storage. The EBM exposed at room temperature (30-36 0C) for more than two hour reduce the quality and do not recommended to be given to the infants.

Abstract

Highlights: • Rare occurrence of a large, well-differentiated primary renal neuroendocrine tumor in a young man. • Identifying primary renal NET from the workup and histological examination of renal tumors needs to be done to understand more of this infrequent type of tumor. Abstract: Neuroendocrine Tumors (NETs) are a diverse range of neoplasms with various biological and histologic features and therapeutic responses. The prevalence of primary renal carcinoids is scarce worldwide. At the moment, complete surgical resection is the primary treatment against primary neuroendocrine tumors of the kidney. Nephrectomy followed by the lymph node dissection is a standard procedure for localized primary renal NETs. Since renal carcinoid tumor is extremely rare, we decided to present a unique case of a 25-years-old male with neuroendocrine renal carcinoid tumor following radical nephrectomy. The results indicated a solid, solitary tumor verified on the frozen section because a postoperative CT–scan showed a recurring mass in the renal fossa following radical nephrectomy. The case emphasized the need to investigate primary renal NET in the workup and histological examination of renal tumors and also contributed to our understanding of this infrequent clinical entity.

Abstract

It is expected that probiotics may act synergistically to improve the clinical efficacy of sublingual immunotherapy (SLIT). The aim of this study was to investigate the role of probiotics in enhancing efficacy of sublingual immunotherapy in childhood allergic asthma. This was a randomized single blind clinical trial conducted on 6-17 year-old asthmatic children sensitive to house dust mite allergens. Subjects were allocated to Group A receiving SLIT, Group B receiving probiotics and SLIT, Group C receiving probiotics only. Clinical parameters (FEV1 reversibility, medication score, and symptom score) and immunological parameters (total IgE, eosinophil count, IFN-γ, IL-2, IL-4, IL5, IL10, and TGF-b were evaluated in week 0 until 14. Statistical analysis revealed that the difference of clinical parameters (FEV1 reversibility, medication score, and symptom score) between groups were not significant. The difference of the immunological parameters of total IgE, IFN-γ, IL-2, IL-4, IL5, IL10, and TGF-íŸ were also not significant. Eosinophil count decreased in subjects who received combination SLIT with probiotics. In conclusion, probiotics enhanced the efficacy of sublingual immunotherapy in childhood allergic asthma by decreasing the eosinophil count.

Abstract

Right ventricular dysfunction and failure receive much less attention than the left ventricular failure. Right ventricular dysfunction or failure is associated with increased mortality rates in cardiac surgery, surgical cases other than cardiac surgery and also in patients treated in the ICU. The purpose of this article review was to describe the anatomy, physiology and pathophysiology of right ventricular failure, its detection and diagnosis, and management considerations from anesthetic point of view, including preoperative, intraoperative and postoperative stages. Cardiac surgery may result in right ventricular failure. For example, 0.1% post cardiotomy patients experience severe right heart failure and require long-term inotropic support, and so do 2-3% of post-transplant patients, and 20-30% of patients installed with instrument in his left heart. Therefore, anesthesiologists play a major role in perioperative and postoperative intensive care and are obliged to comprehend the nature of right ventricular dysfunction and failure so as to carry out early detection, prevent and manage patients with right ventricular dysfunction.

Abstract

This study aimed to define the disease free survival (DFS) and factors associated with recurrence in stage I endometrial cancer after surgery with and without adjuvant treatment. The demographic data, pathological results, adjuvant treatment (AT) and the outcome of patients with endometrial cancer stage I after surgery in Songklanagarind Hospital between January 2002 and July 2014 were collected. The DFS was analyzed by survival analysis and represented by Kaplan–Meier curves. The difference of DFS between AT and non-adjuvant treatment (NAT) groups was tested by the log-rank test. Distributions of risk factors by AT and recurrent status were analyzed using chi-square or Fisher exact tests for discrete factors, and unpaired t or Wilcoxon rank-sum tests for continuous factors. The 5-year DFS was; 91.6%, from a total of 268 patients. DFS in the NAT group was significantly better than that in the AT group (95.2 versus 86.5%, p-value = 0.01). Factors associated with recurrence in the NAT group were age, tumor grading, tumor size, and presence of lymphovascular involvement. Among the AT group, age and ratio of myometrial invasion were associated with recurrence. DFS in NAT was better than in AT and the potential factors associated with recurrence, after surgery with or without AT, were not the same.

Abstract

The volume of internal organs, including pancreas, show potential health problems. Several medical conditions are associated with the volume of the pancreas. The study aimed to determine the pancreatic volumes using a computed tomography (CT) scan in normal Indonesian adults and to determine whether the age, sex, and body mass index (BMI) influence the measurement of pancreatic volumes. The study was conducted by 119 people aged between 20-77 years old. Pancreatic volume measurement was performed using the summation of area technique. Statistical analyzes used independent t-test and Pearson correlation test. The study showed a significant difference of pancreatic volumes between normoweight and overweight individuals with p=0.041 (p<0.05). The results showed a significant difference of pancreatic volumes between male and female with p=0.020 (p<0.05). The results showed a significant correlation between pancreatic volumes and age p=0.004 (p<0.05). These findings suggest that the change in pancreas volume with age, sex, and BMI is comparable among different ethnicities, which is also in line with other studies.

Abstract

Epispadias as an isolated embryologic defect is a rare anomaly, with an incidence of 1 in 117.000 males. Isolated epispadias with continence is a very rare condition comprising less than 10% of all epispadias cases. Epispadias is a closure defect of the dorsal wall of the urethra. The biggest challenges for epispadias treatment are creating adequate penile length, straightening the severely curved penis, and creating a penopubic angle for allowing comfortable penetration of the penis. An 11-year-old boy underwent surgery for his continent penile epispadias in Dr. Soetomo General Academic Hospital. The surgical technique chosen for this patient was Modified Cantwell-Ransley repair. The dressing was opened on the 3rd day after the operation. The urethral catheter was removed on the 10th day. The patients underwent regular follow-up examinations for 6 months and were evaluated for subjective and objective outcomes. There was no post-operative fistula or urethral narrowing requiring urethral dilatation. The Uroflowmetry and post voiding residual ultrasound result was good. The cosmetic appearance of the penis was satisfying. Outcomes related to sexual activity were not evaluated because the patient had no sexual activity yet.

Highlight: • Standing position duration and shoe characteristics to MSDs's complaints on sales promotion girls at cosmetic stores in Surabaya were analyzed. • There was no correlation between height of heels with complaints of MSDs and shoe type, while, the duration of standing position and complaints of MSDs were correlated. Abstract: One of the occupations that have a risk of Musculoskeletal Disorders (MSDs) is sales promotion girl (SPG), especially the SPG at a cosmetic store, because their job position is mostly done by standing and wearing high heels. This study aimed to analyze the correlation between duration of standing position and shoe characteristics to complaints of MSDs on sales promotion girl at cosmetic stores in Surabaya. This study was quantitative research with observational analytic research, where the data collection was carried out by cross-sectional. This research was conducted at 21 cosmetic stores from three shopping centers in Central Surabaya. The population in this study was 64 SPG. The sample was taken using simple random sampling with exclusion criteria, so that 47 people were obtained. The independent variables consisted of duration of standing position and shoe characteristics included shoe type and the height of heels. The dependent variable was complaint of MSDs. The data analysis was carried out using the Spearman correlation test. The result showed that there was a correlation between the duration of standing position and complaints of MSDs (Pvalue=0.011; r=0.369; positive direction), no correlation between shoe type and complaints of MSDs (Pvalue=0.735; r=0.051; positive direction), and no correlation between the height of heels and complaints of MSDs (Pvalue=0.256; r=0.169; positive direction). It was concluded that there was a correlation between the duration of standing position and complaints of MSDs, and there was no correlation between shoe type and the height of heels with complaints of MSDs.

Highlights: 1. This study compared the serological markers of pristane-induced mice to humanized mouse models of lupus achieved by transplanting stem cells from lupus patients, which is a novel method in Indonesia. 2. This study will allow for more accurate research into the pathophysiology of the disease and the development of new lupus treatment strategies. Abstract More studies related to systemic lupus erythematosus (SLE) therapy are urgently needed because of the current insufficiency in treatment effectiveness. However, due to ethical limitations, researchers use experimental animals as a substitute for conducting studies on humans. Models commonly used to study lupus include the pristane-induced mouse model and the recently developed humanized mouse model. The second model involves implanting human immune cells into immunodeficient mice. This study compared the serologic profiles of lupus antibodies, the antinuclear antibodies (ANA) and anti-double stranded DNA (anti-dsDNA), in both mouse models. The aim was to determine which one is more promising for use as a lupus animal model. Thirty BALB/c mice (Mus musculus) were used as subjects and divided into three groups: K1, K2, and K3. K1 served as the control group, consisting of healthy mice that received a placebo. The K2 mice were intraperitoneally injected with 0.5 cc of pristane. The K3 mice were transplanted with stem cell cultures from SLE patients, resulting in humanized mice with immune deficiencies. The mice were observed for 16 weeks, during which the ANA and anti-dsDNA levels in their serum were obtained for analysis using the Kruskal-Wallis test (p<0.05). The comparison revealed differences in the average ANA and anti-dsDNA levels among the three groups. K3 had the highest ANA and anti-dsDNA levels, followed by K1 and K2. The Kruskal-Wallis test indicated that the differences were not significant in the mean levels of ANA (p=0.156) and anti-dsDNA (p=0.061). In conclusion, the humanized mouse model has higher ANA and anti-dsDNA antibody levels compared to the pristane-induced mouse model, albeit without a significant difference. This suggests a positive picture of the humanized mouse model of lupus, making it an invaluable tool for studying the disease and testing potential therapeutic interventions.

Highlights: • Neutrophil to lymphocyte ratio (NLR) has been reported by several studies for its role as a biomarker in various diseases, however, the role of NLR in testicular GCT is still unclear. • The characteristics and responses of testicular GCT patients among Indonesian men show similarity to other reports worldwide. • As a parameter, NLR shows promise to be used as a potential biomarker for prognosis in testicular GCT. Abstract: The global incidence of testicular cancer is 1-2% from all cancers. The attempts to maintain high therapeutic rates while decreasing the treatment-related side effects and toxicity have become the current concern. However, the reports regarding testicular germ cell tumors (GCT) in Indonesia are limited. Thus, we aimed to evaluate the clinical characteristics of testicular GCT patients undergoing bleomycin, etoposide, and cisplatin (BEP) chemotherapy, as well as their chemotherapy response and side effects. We reported the data of patients with Testicular Germ Cell Tumor from January 2015 to December 2019. Several data were retrieved, including patient demographics, tumor characteristics, treatment, and prognosis outcome. A total of 67 patients with testicular germ cell tumors were included in this study. The mean age was 28.9 years old. The chemotherapy regimens used were four cycles of (BEP) in 36 patients (53.7%), followed by three cycles of BEP in 22 patients (32.8%). Patients with seminoma GCT mostly had a complete response (54.1%), whereas most patients with non-seminoma GCT had progressive disease (47.8%). The multiple logistic regression analysis showed that NLR and S staging were independently associated with the patient's response to chemotherapy (OR 2.14, 95% CI 1.22, 3.78, p <0.01,OR 9.43, 95% CI 1.81, 49.14, p < 0.01). The clinical characteristics and response of testicular GCT patients among Indonesian men showed similarity with the current worldwide data. The NLR could be used as a potential biomarker for prognosis.
