Abstract

Short stature of toddlers is a height discrepancy according to age in toddlers which can cause interference with the development of fine movements, gross movements, speech and language as well as social independence. Short stature in toddlers can result in long-term deficits in physical and mental development so that they are unable to learn optimally at school compared to children of normal height. Short stature can be a normal or abnormal variant. Children with normal variations of short stature do not require treatment, while abnormal short stature in children needs to be recognized early. The aim of this study was to explore the impact of height discrepancy according to age on the risk of developmental delays in toddlers at the Children's Polyclinic at Panti Nirmala Hospital in Malang. This research design uses a cross-sectional method with a sampling technique carried out in this way, namely purposive sampling of 60 respondents, data collection is carried out by measuring the child's height and measuring development using the Denver test. Based on test Chi Square it can be seen that the value p = 0.000, this value is smaller than p <0.05, so the conclusion from this study is that there is a significant influence between the two variables where this result indicates short stature can increase the risk of delays in achieving the developmental stages of toddlers through validation of the DDST measuring instrument. This research can be input for nurses to do more aware, more agile and more improved knowledge regarding the development of management models for weight-age, height-age and nutritional status of children towards the achievement of toddler development stages.

Abstract

During the postpartum period, mothers experience various physiological changes, including a decrease in progesterone levels and an increase in prolactin hormone after childbirth, which initiate the lactation process. Physical changes related to lactation include breast enlargement as a sign of breast milk production. Breast milk (ASI) is the best nutrition for newborns up to six months of age because infants’ digestive systems are not yet able to process foods other than breast milk. Inadequate breast milk production may lead to failure of exclusive breastfeeding, which can result in digestive disorders and decreased immunity in infants. One non-pharmacological intervention that can help stimulate breast milk production is oxytocin massage. This study aimed to determine the effect of oxytocin massage on breast milk production in postpartum mothers. This study employed a case study method involving two postpartum mothers as respondents. The intervention was conducted for three consecutive days with a frequency of two sessions per day and a duration of 10–15 minutes per session. Data were collected through interviews, observation, and documentation to obtain subjective and objective data on breast milk production before and after the oxytocin massage intervention. The collected data were analyzed descriptively by comparing breast milk production before and after the intervention and presented in narrative form. On the first day of oxytocin massage, both respondents showed no significant changes in breast milk production. However, after the intervention on the second and third days, an increase in breast milk production was observed, as indicated by fuller breasts, smoother milk flow, and infants appearing satisfied after breastfeeding. Breast milk production increased after the oxytocin massage intervention. Therefore, oxytocin massage can be considered an effective nursing intervention to support breastfeeding, particularly during the postpartum period.

Abstract

Male participation in vasectomy remains low globally and in Indonesia, despite evidence suggesting that psychological and social factors contribute to intention formation. The persistent imbalance in contraceptive responsibility suggests that intention, not just access, plays a central role. Objective to synthesize empirical evidence regarding the determinants influencing married men's intention to undergo vasectomy, focusing on attitudes, partner support, community norms, and perceived incentives within the framework of the Theory of Planned Behavior. The research design used was a literature review. The review was conducted by searching, reading, and analyzing relevant scientific articles to understand the factors influencing married men's intention to undergo vasectomy. This study used the SPIDER approach consisting of sample, phenomenon of interest, design, evaluation, and type of study. Articles were searched through Google Scholar, PubMed, and Scopus databases using access filters between 2021 and 2025. Keywords used in the search were "vasectomy", "intention", "partner support", "community norms", "incentives", "personality", and "awareness". The initial search yielded 12 articles. A screening process yielded 11 articles that met the criteria. Married men's intention to undergo a vasectomy is shaped by attitudes, partner support, community norms, and incentives. Attitude emerged as the most consistent predictor. Partner support, particularly the wife's influence, was shown to influence vasectomy decision-making. Community norms and cultural narratives further reinforced hesitation, while incentives served as secondary facilitators. Vasectomy acceptance should prioritize addressing misconceptions, strengthening partner-based counseling, and reducing stigma through culturally appropriate advocacy.

Abstract

Service quality is considered an important factor in building patient loyalty. The aim of this study is to assess hospital service quality from the patients’ perspective and to determine the importance of each quality dimension in predicting patient loyalty. To evaluate the structural construct of hospital service quality from the patients’ perspective and to determine the importance of quality dimensions in predicting patient loyalty. This was an analytical study with a cross-sectional design conducted in 2025. The study population consisted of outpatient individuals aged ≥18 years who had visited at least twice, with a total sample of 164 respondents. The research variables included demographics, service quality, and patient loyalty. The study instrument used a validated and reliable questionnaire; the service quality variable comprised 5 dimensions with 27 statements, while the loyalty variable consisted of 9 indicators with 27 statements. Multivariate analysis was performed using multiple logistic regression with a significance level of α = 5%. Both loyal and non-loyal patients were predominantly female, of productive age, lived ≥5 km from the hospital, had an education level of elementary to senior high school, were non-workers, and used insurance as their payment source. There was a significant relationship between all five dimensions of service quality (tangibles, reliability, responsiveness, assurance, and empathy) and patient loyalty (p < 0.001). Tangible was the most dominant factor influencing patient loyalty (adjOR = 5.68; 95% CI: 1.96–16.41), followed by responsiveness (adjOR = 4.63; 95% CI: 1.51–14.15) and assurance (adjOR = 5.54; 95% CI: 1.90–16.19), which also had a significant effect on patient loyalty. Patients’ experiences with hospital services have a significant impact on outcome variables such as their willingness to return to the same hospital, reuse its services, or recommend it to others. The relationship between service quality and patient loyalty demonstrates its strategic importance in improving service quality to attract and retain patients as well as to expand market share.

Abstract

Nurse performance is a key determinant of patient safety and quality of care, yet remains highly influenced by multidimensional internal and external factors. This literature review aims to examine and synthesize the theoretical and empirical evidence regarding the determinants of nurse performance in the implementation of nursing care. This review draws upon theoretical perspectives of organizational behavior, motivational theory, and competency-based performance, supported by empirical findings from research published between 2020–2025. Using the PRISMA 2020 method, 1,310 articles were initially identified, from which 11 studies met the inclusion criteria. The literature was systematically reviewed through a structured process involving database searching (PubMed, DOAJ, Google Scholar, Garuda, Sinta, and Scopus), screening based on predefined inclusion and exclusion criteria, independent data extraction by two reviewers using a standardized form, and methodological quality assessment applying the JBI Critical Appraisal tools. Given the heterogeneity of study designs, a narrative synthesis approach was used to categorize determinants of nurse performance. Evidence synthesis demonstrates that nurse performance is shaped by individual factors (competence, tenure), psychological factors (stress, anxiety, burnout), motivational variables (self-efficacy, intrinsic and extrinsic motivation), and organizational determinants (leadership, managerial support, work culture), alongside environmental conditions (workload, shift patterns, equipment availability). Competence, work motivation, effective leadership, and supportive work environments consistently enhanced nurse performance, whereas workload burden and pandemic-related stress were associated with performance decline. Methodologically, most studies relied on cross-sectional designs and subjective self-report instruments, limiting causal interpretation and standardization. Nurse performance is driven by an interplay of personal, psychological, organizational, and systemic factors, necessitating a holistic improvement strategy incorporating competency development, motivational support, leadership strengthening, and work environment optimization.

Abstract

The research, conducted from August 2020 to January 2021, was motivated by an indication of the quality of life problems of children with meningitis with prolonged treatment at dr. M. Djamil Padang related to the identification of complex neuropsychological sequelae in children. On the other hand, there has been an increasing trend in the number of pediatric meningitis patients hospitalized in these hospitals in recent years, especially the 2017–2019 period. The main objective of the study was to identify quality of life of children with meningitis post-treatment at dr. M. Djamil Padang in 2017–2019. The research method used was quantitative descriptive with a cross-sectional approach. Sampling in this study used purposive sampling, which found 34 respondents. The results showed that more than half (52.9%) of the sample of children with post-treatment meningitis had a poor quality of life, 29.4% had a poor quality of life with physical dimensions, 58.8% had a poor quality of life, respectively for the emotional and social dimensions, and 41.7% had a poor quality of life in the environmental/school dimension. In terms of age groups, poor quality of life was identified in 57.1% of children aged 2–4 years, 66.7% of children aged 5–7 years, and 40% of children aged 8–12 years.

Abstract

Clinical capability development is a crucial component of nursing education, enabling the production of competent and ethical nurses. Clinical education emphasizes not only technical skills but also the formation of professionalism, soft skills, and moral values. This study aims to understand the experience of nursing students in developing clinical abilities through the clinical education process. The study employed a qualitative phenomenological approach with 12 final-semester nursing students who were purposively selected. Data was collected through in-depth interviews and was analyzed thematically. The analysis involves identifying units of meaning, structuring themes and subthemes, and verifying these interpretations with participants to ensure their accuracy. The findings show four main themes: (1) clinical readiness and application of theory, students experience initial challenges in applying theory to real practice; (2) mentoring and support, which improves technical skills and confidence; (3) self-reflection and self-learning, which strengthens the understanding of self-improvement practices and strategies; (4) development of soft skills and professionalism, including effective communication, teamwork, and the application of professional ethics. The interaction between practical experience, guidance, reflection, and professional development forms a holistic and continuous clinical learning process. Clinical education provides a well-rounded experience to shape students' technical abilities, soft skills, and professionalism so that they are prepared to become competent and responsible nurses.

Abstract

Hypertension in pregnancy is one of the reason main maternal morbidity and mortality. Suboptimal treatment can increase risk postpartum complications. Initiation Early Breastfeeding (IMD) is suspected own effect physiological in lower pressure blood through release oxytocin triggers relaxation and vasodilation. However, evidence empirical about the influence of IMD on change pressure blood in the mother giving birth with hypertension Still limited. Analyze influence action Initiation Early Breastfeeding (IMD) against change pressure blood in the mother giving birth with hypertension. Study quasi-experiment with design pretest–posttest control group design conducted on 60 mothers giving birth with hypertension, consisting of from the right IMD group time (n=30) and slow IMD (n=30). The sampling technique used was purposive sampling. Pressure blood systole and diastole are measured before and after IMD action. Normality test use Kolmogorov–Smirnov, and the differences before after analyzed use paired t-test. The right IMD group time show decline significant pressure blood systolic (mean 2.5 mmHg; p=0.000) and diastolic (mean 2.63 mmHg; p=0.001). In the slow IMD group, the decrease pressure blood systolic (0.3 mmHg; p=0.510) and diastolic (1.1 mmHg; p=0.068) were not significant. Correlation test obtained p=0.000 both at pressure blood systole and pressure diastolic blood pressure in the IMD group was correct time. IMD is right time effective lower pressure blood systole and diastole significant in mothers giving birth with hypertension compared to slow IMD. IMD has the potential become safe and beneficial non - pharmacological interventions for stabilization postpartum hemodynamics.

Abstract

Cognitive decline known as one of long-term effect of chronic disease. Individuals with cognitive dysfunction tend to show low quality of life. Yet limited studies has been conducted to determine the prevalence of this issue among people with chronic disease in Indonesia. Therefore, this study was aimed to identify the prevalence and relationship between chronic disease and cognitive function. This study also evaluated the factors influencing this relationship. This research was a cross-sectional analysis that utilized data from 2,396 participants with chronic conditions, drawn from the 2014 Indonesian Family Life Survey (IFLS) through purposive sampling. This study focus on people with hipertention, stroke, and diabetes mellitus. Cognitive impairment was measured using Telephone Interview for Cognitive Status (TICS). The data was analysed using analysed using bivariate logistic regression and multivariate Hierarchical Logistic Regression. The relationship between variables indicated based on Odds Ratio (OR), exponential value values ꞵ (eꞵ), and significancy level 5%. The overall prevalence of impaired cognitive function was 2171(61.7%) with the highest frequency found in people with diabetes mellitus (64.5%). Logistic regression test found a significant relationship between chronic disease and impaired function (p<0.001). Hierarchical Logistic Regression test on all models found-that age, gender, disease duration, and education had a greater influence, with R2 0.467 (46.7%). This study showed people with chronic diseases with older age, female, low education, longer disease duration, smoker, and obese have higher risk of developing impaired cognitive function.

Abstract

Metabolic syndrome is a cluster of interrelated metabolic risk factors that have the potential to cause chronic complications such as type 2 diabetes mellitus and cardiovascular disease. This condition affects not only physical aspects but also the psychological and social dimensions of patients. This study aimed to explore the experiences of patients with metabolic syndrome in their daily lives after being diagnosed. A qualitative approach with a phenomenological method was employed to explore and understand the lived experiences of patients with metabolic syndrome. Data were collected through Focus Group Discussion (FGD) and observations involving eight patients aged 56-60 years diagnosed with metabolic syndrome (seven women and one man), each presenting at least three diagnostic components. Data were analyzed using the Miles and Huberman Model, which includes data reduction, data display, and conclusion drawing/verification. Five main themes were identified: (1) initial post-diagnosis experiences, including denial and anxiety; (2) the impact of metabolic syndrome on physical and psychological conditions; (3) family support in emotional, functional, and acceptance forms; (4) adaptation and coping, consisting of self-initiated efforts, treatment adherence, and following healthcare professionals’ recommendations; and (5) life changes after diagnosis, including lifestyle modification, spiritual strengthening, challenges, and the formation of new habits. Patients with metabolic syndrome undergo complex emotional and adaptive processes. Family and healthcare professional support play important roles in helping patients adjust to this chronic condition. These findings contribute to the development of nursing practice based on a holistic bio-psycho-socio-spiritual approach.
