Abstract

Covid-19 patients with comorbidities, such as hypertension, diabetes, and respiratory diseases, have a higher risk for severe disease and mortality compared to patients without comorbidities. Some data shows that the administration of high dose vitamin C (1–2 g/day) can reduce the amount of the inflammatory mediators and shorten the length of hospital stay. This study aimed to determine the relationship between vitamin C administration and of length of hospital stay for Covid-19 patients with comorbidities. This study was a retrospective observational study on  consecutively sampled medical record data of UNS Hospital patients from July 2020 to January 2021. A total of 78 patient medical records were included of this study. The inclusion criteria were all medical records of patients who suffered from Covid-19 with comorbidities admitted to the hospital while the exclusion criteria were medical records of patients who did not get oral or intravenous vitamin C. The chi-square bivariate test was used to determine the relationship between the administration of vitamin C and the length of hospital stay for Covid-19 patients with comorbidities. Of the 78 subjects only 52 subjects were included in the statistical analysis. Medical records were divided into two groups: group 1, that consisted of medical records of patient who received more than 1,000 mg Vitamin C, and group 2, who received less than 1,000 mg Vitamin C. Results demonstrated that the administration of high dose vitamin C of more than 1,000 mg was not associated with the length of hospital stay (p=0.677). Thus, high dose of vitamin C is not associated with the length of hospital stay for Covid-19 patients with comorbidities, such as hypertension, diabetes, and respiratory disease.

Abstract

It is estimated that 6% of the US population suffer from diabetes with 15% of them experience diabetic foot. Several studies have explored the various benefits of sea cucumber for human being, including its antioxidant nature. This study aimed to investigate the effect of surf redfish, one species of the sea cucumbers, in diabetic wound healing. This was an animal experimental study performed at the Pharmacology and Pharmaceutical laboratory of the Faculty of Pharmacy, Universitas Tjut Nyak Dhien Medan, Indonesia, during the period of February- July 2019. Twenty five mice in diabetic condition were divided into five groups: control, standard, ethanol extract of surf redfish-1, ethanol extract of surf redfish-2, and ethanol extract of surf redfish-3 3. The surf redfish was obtained from Lamreh Village in Aceh and the ethanol was extracted using the maceration method. The parameters used for evaluating the efficacy of surf redfish ethanol extract were blood glucose level, length of wound incision, and histopathological features. Surf redfish extract presented a significant reduction of blood glucose level which that followed the increase in the dose of the ethanol extract (p-value <0.05) where the most significant reduction was seen in the group with the highest dose on the last day. This trend was also seen for the length of the wound, where the reduction was significantly higher in treatment groups when compared to control (p-value <0.05). The histological study also confirmed the improvement in the clinical appearance of wound by showing significantly increased fibroblast cell number and collagen density (p-value <0.05). Overall, the surf redfish can help better incision's wound healing in diabetic condition.

Abstract

Spinocerebellar ataxia (SCA) 3 is a neurodegenerative disease which involves cerebellum and extra cerebellum. Neuropathy in SCA3 manifests in various ways, including axonal and demyelination lesions in sensory and motor nerves. There has not been any study that describes the peripheral neuropathy characteristics of SCA3 patients in Indonesia at the time of this publication. This paper reports a case of a 43-year-old male with known spinocerebellar ataxia 3 presented with hereditary ataxia and mild numbness in both palms since two years before. No abnormalities were found during the sensory examination. The NCS showed severe axonal demyelinating sensorimotor peripheral neuropathy. In magnetic resonance imaging (MRI), an atrophy in the cerebellum with cerebral multiple lacunar infarction was identified. Electrophysiological results revealed profound axonal lesion in peripheral nerves. To conclude, peripheral neuropathy in SCA3 represents the dominance of axonal lesions in motor nerves.

Abstract

Mortality and health disturbances in children often correlate with maternal health and fertility. Avocado, mung bean sprouts, and holy basil have been traditionally used to improve maternal health, before and during pregnancy. This study was aimed to assess the efficacy of herbal combination of avocado, mung bean sprouts, and holy basil (Jamu ATOKE) in optimizing reproductive health. Eighteen female Sprague Dawley rats (9–10 weeks old, BW: 180–250 g) were randomly divided into 3 treatment groups (n=6 rats for each group) of control, group I and II. ATOKE were added into rats drinking water and consumed for 30 days before pregnancy. After the rats were pregnant and gave birth, pup per parent ratio, pup mortality, and pup health performance (body weight gain, feed and drink consumption, motor activity, pup speed in finding light and feed, red blood cell (RBC) count, white blood cell (WBC) count and differential, SGPT, SGOT, blood urea nitrogen (BUN), creatinine, and proinflammatory mediators (TNF-α and IL-6) were analyzed. Pups born to in group I had a generally better health performance compared to the control group. There was a 100% mortality in the litter of group II due to the excessive aggressive behavior of the dams as a sign of toxicity. Administration of 2.5% Jamu ATOKE showed the best effects on the health and intelligence of the rat pup.

Abstract

In facilities where a track required is not available for gait speed test, a simpler method using a shorter track and test duration is needed. One of such methods is the 4-meter gait speed. This study aimed to determine the cut off values for 4MGS and its correlation with the 6-minute walk test (6MWT), a more common functional performance test with shorter track and time. Data for this study were collected at the Department of Rehabilitation of Dr. Cipto Mangunkusumo General Hospital during the period of August 2016 to March 2017. The consecutive sampling approach was used to recruit healthy Indonesian adults (n=61) aged 18–50 years into this cross sectional study. The anthropometric and respiratory variables were measured as descriptive data and the cut-off values were determined by calculating the mean 4-meter gait speed (4MGS)-2SD for each gender. The 6MWT was performed afterwards to assess the correlation between 4MGS and 6-minute walk distance (6MWD). Results presented a mean 4MGS value of 2.114±0.309 m/s (male) and 1.908±0.227m/s (female) with the cut-off values for normal gait speed of 1.496 m/s and 1.454m/s for male and female, respectively. Factors affecting 4MGS were body height, body weight, forced vital capacity (FVC), and forced expiratory volume in 1 second (FEV1). The 4MGS is only shown to be weakly correlated with 6MWD (r=0.314, p=0.014). The cut-off values for 4MGS can be used as a reference to assess the normality of gait speed as a parameter of functional mobility in healthy Indonesian adults and should be used in adjunct to 6MWT to represent a comprehensive functional capacity status.

Abstract

Pelvic organ prolapse (POP) is a condition with a high incidence rate and often creates difficulties in surgical coloproctology and gynecology. The aim of this study was to evaluate the effectiveness of laparoscopic ventral reticular sacrocolporectopexy and sacrorectopexi in women and men with POP, respectively. This study was conducted at the Educational-Surgical Clinic of Azerbaijan Medical University and Department of Surgery of the Faculty of Medicine of Ankara University (2016-2019) on 21 patients with POP (15 women and 6 men). Results of diagnostics and surgical treatment of POP were studied with preferences towards endoscopic, radiation, and functional methods. The surgical methods used in these patients included laparoscopic fixation methods (sacrocolpopexy, sacrocolporectopexy) of protruding organs (uterus, vaginal vault/vaginal cuff, rectum) and simultaneous vaginal (colporrhaphy, colpolevatoroplasty, vaginal plastic surgery) and proctological surgeries (circulatory resection, hemorrhoidectomy, sphincteroplasty). The findings demonstrated that the most progressive POP mostly occurred in women of premenopausal age and during menopause. Based on the results of the long-term evaluation of the surgical treatment (6-12 months), the rates of recurrence of prolapse and complications were low (up to 4.8% and 9.5%, respectively) with favorable long-term functional results,  such as a decrease in the degree of fecal incontinence and constipation, observed in the evaluation. Due to the concomitant weakness of the ligamentous apparatus of the pelvic floor in these patients, there is the need for intra-abdominal apical support of organs. In conclusion, that laparoscopic sacrocolpopexy in women and sacrorectopexy in men are reliable surgical method to treat POP. However, specific skills need to be acquired by both gynecologist and coloproctologist to be able to do these laparoscopic surgery techniques.

Abstract

Diabetic retinopathy (DR) is one of the eye disorders caused by complications of diabetes mellitus (DM). Indonesia has a high prevalence of DR (43.1%) compared to other Asia-Pacific countries. Damage and blockage of blood vessels in the retina causes impaired vision, which may lead to a total vision loss. DR requires special treatment, coupled with regular treatment, that is expensive and may result in depression and poor quality of life. This study aimed to determine the association between the degree of diabetic retinopathy with the level of depression and quality of life in people with DR in the eye clinic of Zainoel Abidin General Hospital Banda Aceh. This cross-sectional study was conducted from April 2018 to January 2019. Data were obtained through funduscopy reading, Hamilton Depression Rating Scale (HDRS) questionnaire, and Short-Form Health Survey (SF-36). The subjects of this study were all diabetic retinopathy patients who were sampled using the accidental sampling method. Based on the Spearman correlation analysis, there was a correlation between degree of DR and depression level (p value of 0.004) and quality of life (p value of 0,037). In conclusion, there is a moderate correlation between the degree of DR with depression level and quality of life among DM patients.

Abstract

Forensic medicine is a branch of medicine that examines victims for the benefit of justice. Doctors play an important role in documenting injuries or abnormalities on the victim's body in the patient's medical record. This study aimed to determine the characteristics of cases and injuries of patients at M Djamil General Hospital during the period of 2010–2020. This was a descriptive retrospective study on secondary data from the medical records of patients who were examined and consulted to the Forensic Department of Dr. M. Djamil General Hospital, Padang during 2010–2020. During this period, 11.729 patients (victims) were examined by and consulted to the Department. Most of the victims were male (70.6%) aged 10–19 years old (29.2%). Most of the victims were involved in a traffic accident, followed by persecution. Victims of traffic accident dominated the forensic pathology cases after sudden death.  Abrasions and open wound injury were the most frequent types of injury, with head and neck as the body parts mostly affected. In terms of the type of wound, most victims experienced injuries caused by blunt force (87%). This study concluded that the forensic cases managed at the Dr. M. Djamil General Hospital vary with blunt forces by a traffic accident as the most frequent cause of injuries while the head and neck become the body parts that are  most susceptible to injury, both for survivors and those who died.

Abstract

Diabetes mellitus (DM) is a metabolic condition where the body's ability to produce and utilize insulin is impaired; thus, wound healing process is slower than usual in patients with DM. Lemon pepper has both antioxidant and anti-inflammatory properties. This study aimed to investigate the effect of topical application of lemon pepper fruit ethanol extracts on the healing process of oral traumatic ulcers in diabetic rats. This study was performed at the Pharmacology Laboratory, Faculty of Medicine, Universitas Prima Indonesia from September 2021–October 2021. Thirty males diabetic Wistar rats were thermally injured on the lower labial mucosa and divided into five groups: control, standard, and lemon pepper gel 5%, 7.5%, and 10% groups. The ulcer size was measured after injury and on the 7th and 14th day. On the last day, all rats underwent both routine blood count and histology study for oral ulcer tissue. On the 14th day, all treatment groups showed some significant difference wound contraction, with the highest tendency of wound contraction seen in both lemon pepper gel 10% and standard groups (62.50%. p-value =0.001). The improvement of wound contraction was also supported by reduced white blood cell count (p-value<0.05) in routine blood count and regeneration of epidermis and dermis tissues in the histology study. Hence, it can be concluded that the lemon pepper extract gel in all concentrations has good physical stability and the lemon pepper gel with the highest concentration group presents the best ulcer healing activity compared to other concentrations and has similar ulcer healing activity as the standard group that receives Kenalog® in Orabase.
