Abstract

Diabetes Mellitus (DM) is a metabolic disorder characterized by elevated blood sugar levels due to impaired insulin production or function. One of its chronic complications is diabetic foot ulcers, which can lead to infection, gangrene, and even amputation, especially in patients with poor glycemic control, as assessed by HbA1c. This condition increases morbidity and mortality, and reduces quality of life. This study aims to determine the relationship between uncontrolled HbA1c levels and the incidence of minor amputations in patients with diabetic foot ulcers at Banten Regional Hospital. This study was an observational analytical study with a cross-sectional design using a retrospective approach through medical records of diabetic foot ulcer patients at Banten Regional Hospital. The sampling technique used was consecutive sampling with a sample size of 100 subjects who met the inclusion and exclusion criteria. Data analysis was performed using univariate and bivariate analysis using SPSS. The chi-square test was used to determine the relationship between HbA1c and the incidence of minor amputations. The results of the study from a total of 100 subjects showed that from univariate analysis, in the uncontrolled HbA1c group (>7%) the majority experienced minor amputations (53.8%), while in the controlled HbA1c group (≤7%) the majority did not experience amputations (57.1%). From bivariate analysis, there was no significant relationship between HbA1c levels and the incidence of minor amputations (p=0.402). The results of this study also have a 95% Confidence Interval (CI) with a lower value of 0.679 and an upper value of 3.561. There was no significant relationship between uncontrolled HbA1c levels and the incidence of minor amputations.

Abstract

Gastric perforation is caused by damage to the gastric mucosa, leading to peritonitis and requiring emergency surgery. This study aims to determine the relationship between the duration of sudden abdominal pain complaints and the timing of surgical intervention with mortality in patients with gastric perforation at RSUD Banten. The research method used analytical observational study with a cross-sectional design involving 110 patients selected through consecutive sampling. Data were obtained from patient medical records from January to December 2023. The result showed the mortality rate of patients with gastric perforation was 16.4%. Bivariate analysis indicated no significant association between the duration of sudden abdominal pain complaints until surgery (p=0.463) and intraoperative systolic blood pressure (p=0.187) with mortality. However, there was a significant relationship between patient age (p=0.004) and comorbidities (p=0.030) with mortality, where patients aged ≥60 years and those with comorbidities such as heart disease had a higher risk of death. This study highlights the importance of comorbidity management and special attention to elderly patients in order to reduce mortality in cases of gastric perforation.

Abstract

Exclusive breastfeeding during the first six months of an infant’s life is the primary recommendation from WHO and UNICEF, as it has been proven to offer significant health benefits for both infants and mothers. However, the coverage of this practice remains suboptimal in many countries, including Indonesia, due to various multidimensional factors. This article aims to systematically review the scientific literature from the past ten years (2015–2025) on the factors influencing the success of exclusive breastfeeding, with a focus on maternal knowledge, socioeconomic factors, social support, and policies. The review findings indicate that education level, maternal understanding of the benefits and techniques of breastfeeding, employment status, household income, and access to healthcare services play crucial roles in the success of exclusive breastfeeding practices. Several studies also highlight the importance of social support from family, friends, and community, as well as government policies such as maternity leave and educational programs that promote exclusive breastfeeding. Community-based interventions and supportive policy environments have also been shown to effectively increase exclusive breastfeeding rates. Therefore, a holistic approach is needed—not only to improve maternal knowledge, but also to address the social and policy-related factors that influence breastfeeding behavior.

Abstract

Diabetic retinopathy is one of the most common microvascular complications in patients with type 2 diabetes mellitus (T2DM) and a significant cause of blindness. This study aimed to analyze the association between hypertension and the incidence of diabetic retinopathy in T2DM patients at RSUD Banten. This study used a cross-sectional design with a retrospective approach based on medical records. A total of 62 T2DM patients at RSUD Banten during the period 2023–2025 were selected using non-probability consecutive sampling. Hypertension status was determined based on blood pressure ≥140/90 mmHg, a physician diagnosis of hypertension, or documented use of antihypertensive medication, while diabetic retinopathy was assessed through funduscopy or fundus photography findings. Of 62 subjects, 31 (50%) had hypertension and 23 (37.1%) had diabetic retinopathy. The proportion of diabetic retinopathy in the hypertension group was 41.9% and 32.3% in the non-hypertension group. Chi-Square analysis showed no significant association between hypertension and diabetic retinopathy (p = 0.430; PR = 1.30; 95% CI 0.674–2.508). The mean HbA1c was 5.48% ± 0.58%, indicating well-controlled glycemia across all subjects. There is no significant association between hypertension and diabetic retinopathy in T2DM patients at RSUD Banten. These findings suggest that in patients with controlled glycemia, hypertension alone may not be sufficient to demonstrate a significant contribution to diabetic retinopathy, highlighting the multifactorial nature of this complication.

Abstract

Purpose: Evaluating the results of cataract surgery with high astigmatism and determining the type of IOL and surgical technique. Methods: Evaluating data from a patient with cataracts and high astigmatism who underwent phacoemulsification with IOL implantation. The patient's preoperative and postoperative visual acuity, refraction, aurorefractometer (AR), and keratometer results are shown.Result: The results of the examination of a patient with OD grade 2 buratto cataract with high astigmatism (AR: S+2.50 C-4.75X70). While OS grade 2 buratto cataract with AR: S+2.25 C-0.75 x140. The patient also found cylindrical anisometropia. OD UCVA 6/24 and BCVA 6/21 preoperatively after phacoemulsification + IOL / LA became 6/12 with BCVA 6 / 7.5 at the third week. The results of the preoperative corneal astigmatism in the patient were 0.25 diopters with astigmatism originating from the lens 4.50 diopters. In this operation, phacoemulsification was performed with the installation of foldable EDOF Elon IOL and did not choose the installation of foldable toric IOL. The AR results at the third week showed S-0.25 C-0.25 x40. This case report also demonstrates a reduction in the patient's lens  astigmatism after cataract removal, which also eliminated the patient's astigmatism. Phacoemulsification surgery also eliminated the patient's cylindrical anisometropia. Conclusion: Good surgical results were obtained in cataract patients with high astigmatism who underwent phacoemulsification surgery with the choice of nontoric IOL when most of the astigmatism came from the lens and only a small amount of corneal astigmatism was left.

Abstract

Cancer is defined as the uncontrolled growth of cells that can spread to other parts of the body, leading to significant health issues and affecting the quality of life patients. One hallmark of cancer is its ability to reprogram metabolism to support rapid cell proliferation, even under adverse conditions such as low oxygen levels. This article aims to explore how cancer cells predominantly utilize aerobic glycolysis, converting glucose into lactate, a process known as the Warburg Effect. This metabolic shift allows cancer cells to quickly generate energy and produce necessary biomass, including NADH, which is vital for maintaining redox balance and supporting continued proliferation. Pyruvate Kinase M2 (PKM2) is highlighted as a primary regulator of aerobic glycolysis in cancer cells. It facilitates the conversion of phosphoenolpyruvate (PEP) to pyruvate, which is crucial for lactate production. The low affinity of PKM2 for this conversion contributes to the metabolic advantages that cancer cells exploit. The review emphasizes the potential of targeting PKM2 as a therapeutic strategy for cancer treatment. Inhibiting PKM2 could disrupt the metabolic pathways that cancer cells rely on, with studies indicating that high doses of docosahexaenoic acid (DHA) can reduce PKM2 expression, presenting a promising direction for future research.

Abstract

Medical students experience substantial academic demands that increase their risk of developing academic-related stress (ARS). Caffeine is commonly consumed to improve alertness and academic performance, yet excessive intake may adversely affect sleep quality and psychological well-being. This study aimed to examine the associations between caffeine consumption, sleep quality, and academic-related stress among medical students. A cross-sectional analytic study was conducted among 139 medical students. Caffeine intake was assessed using a modified Caffeine Consumption Questionnaire (CCQ), sleep quality was evaluated using the Pittsburgh Sleep Quality Index (PSQI), and academic-related stress was measured using the Academic Related Stressor (ARS) domain of the Medical Student Stressor Questionnaire (MSSQ). Associations between variables were analyzed using the Chi-square test or Fisher's exact test, with statistical significance set at p < 0.05. Most participants were classified as low caffeine consumers (92.8%), with a mean daily caffeine intake of 36.65 ± 38.61 mg. Poor sleep quality was reported by 75.5% of participants, while 70.5% experienced severe to very severe academic-related stress. No significant association was found between caffeine consumption and ARS (p = 1.000). In contrast, poor sleep quality was significantly associated with higher odds of severe ARS (p = 0.031; OR = 2.399; 95% CI: 1.068–5.387). Sleep quality was significantly associated with academic-related stress, whereas caffeine consumption was not. Medical students with poor sleep quality had more than twice the likelihood of experiencing severe academic-related stress, highlighting the importance of promoting healthy sleep habits to support students' psychological well-being.
