Abstract

In the current era of global competition, it requires every hospital as a health service facility to be able to provide quality services in order to foster patient loyalty as service users. The medical record is one of the medical support services which is the basis for assessing the quality of medical services. Completeness of medical record files in RSUD Syekh Yusuf Kab. Gowa in a period of three years has fluctuated, namely 20% in 2017, 66% in 2018, and decreased in 2020 to 17%. This study aims to determine the description of medical record data management at RSUD Syekh Yusuf Kab. Gowa 2019.This study used a qualitative descriptive research method with the selection of informants using a purposive technique, and 6 informants were obtained, including 4 medical record officers, 1 head of the inpatient room, and 1 head of the medical records department. The results of the interview showed that the personnel in the medical records department were deemed insufficient, the flow and SOP were not implemented, the facilities and infrastructure were inadequate. In the implementation of medical records, there are still files that are filled in incompletely which results in delays in making reports. It is hoped that the hospital management will increase the number of personnel so that there is no double burden on officers, provide training for medical record officers, and pay attention to facilities and infrastructure to switch from a conventional system to an electronic-based system.

The method of writing literature review begins with selecting topics, determining keywords to find journal references. The databases used include Google Scholar, Elsevier, Portal Garuda, Shinta. Reference searches are limited from 2016 to 2020. The keywords used are "Marketing mix, 7p Marketing mix, Hospital, Patient Satisfaction". This study sought a reference list of studies, which included 85 studies taken from the search. A total of 25 articles in cleaning and there are 7 articles included in the discussion. Selection of journals selected based on inclusion criteria. The inclusion criteria in this research  is an analysis of the effect of 7P Marketing Mix on Patient Satisfaction  in the Hospital. Of the 7 studies reviewed, one study explained that the 7P marketing mix had a significant effect on patient satisfaction as evidenced by statistical tests with p value = 0.00 <? = 0.05. The questionnaire consists of 12 questions about what kind of health information they need and how to obtain it. Sampling techniques use is simple random sampling. Result: There are some KIA information needs sought by the KPM. But the greatest need is information about the KB. Information search is done by asking directly to the local midwife's conclusion: PKH escorts need to establish good cooperation with midwives and master the information about the KB because it is tasked to deliver it to the PKH beneficiaries through P2K2 activities.

Abstract

Chemical laboratories are environments where a wide variety of analytical activities are associated with hazardous chemicals that can have negative implications for occupational safety and health (OSH). The research objective is to analyze the level of discipline based on knowledge, attitudes and commitment to carry out OSH by PT Sucofindo employees in the chemical laboratory. The method used is crosstab analysis and multiple regression analysis. The results of the crosstab analysis showed that there was an influence between age and sex, but there was no effect of working time on the use of PPE. The multiple regression results show that there is a simultaneous influence on the four variables mentioned above with OSH behavior. As for partially, there is an influence between knowledge, PPE facilities and supervision on OSH behaviour there is no influence on supervision. It is recommended that a refresher and outreach should be done regarding the use of PPE, providing PPE equipment needed and supervising the use of PPE at least once time a month in a chemical laboratory.

Abstract

Abstract

ABSTRACTThe national and international scaled-accreditation in a hospital should implement Early Warning Score System (EWSS) as procedure to do early detection of any changing in condition which shows alarm, especially for patients. The data from RSUP H. Adam Malik Medan, revealed that of the 44 medical records, in the inpatiens wards, 46 % of them were are not accordance with the standardized. This research used cross sectional design which was aimed to analyze the influence of knowledge and work motivation on the implementation EWSS. The sample were 89 nurses who worked in the surgery department. The data were analyzed by using multiple linear regression analysis by applying the assumption of multicolliniarity, heteroscedasticity, and normality data. The result showed that the variables of knowledge and work motivation had positive influence on the implementation of EWSS (P-Value 0,00) which indicated that Y model = 0,236 X1+ 0,535 X2 in which Y was EWSS implementation, X1 was knowledge, and X2 was work motivation. The conclusion was that increase in knowledge and work motivation would be followed by the increase in EWSS implementation. It is recommended that the hospital management maximize the implementation of EWSS by increasing the level of knowledge and work motivation. Key Word: Knowledge, Work Motivation, EWSS

Abstract

Observations were made on the medical record documents of the Chronic Kidney Disease (CKD) case, found incomplete medical information, namely the results of X-rays of 70%, other tests of 20%, writing of disease history of 10% and some of the stages of disease were not written. The results of the interview with one of the coders were due to the lack of accuracy in filling in medical supporting data. The aim of the study was to describe the medical information in determining morbidity coding in CKD cases.Using a quantitative descriptive research approach, data from observations of 63 documents of CKD inpatient medical records were completed with information from one of the inpatient coder officers. The results showed that, there was still incomplete medical information such as the results of investigations and medical history. And several documents were found whose examination result sheets were not in DRM. The procedure for determining the code in the RS Panti Wilasa Dr Cipto in the CKD case did not differ in giving morbidity codes with other cases. The flow is in accordance with the implementation guidelines for coding in ICD 10 Revision 2010. However, in determining the code the main obstacle is the lack of clarity in writing medical diagnoses, and there is an incomplete history. The conformity of the morbidity code based on ICD-10 version 2010 in the case of CKD in Dr Cipto Panti Wilasa Hospital was 71.42% and the code mismatch was 28.58%. This is due to incomplete medical information and unclear stages of CKD disease. It is advisable for the coder to confirm to the DPJP if there is a diagnosis that is illegible or less specific so that the code generated is appropriate and precise, especially in the diagnosis of CKD. The need to complete medical information and double-check medical record documents before coding. Keywords: Medical Record Documents, Chronic Kidney Diseases, Morbidity Code, ICD-10
