Abstract

Kidneys are the primary organs responsible for the excretion of metabolic waste products. Glomerular Filtration Rate (GFR) serves as a critical parameter for assessing renal function, with serum creatinine traditionally utilized as the primary biomarker. However, the accuracy of creatinine-based assessments is limited by its susceptibility to external variables, including age, sex, muscle mass, and dietary intake. These factors can lead to variability in results and reduce diagnostic precision. To overcome these limitations, cystatin C (CysC) has gained prominence as a superior alternative biomarker. Unlike creatinine, serum CysC levels are minimally influenced by external factors, offering greater stability and sensitivity in detecting acute kidney injury (AKI), chronic kidney disease (CKD), and other renal impairments such as diabetic nephropathy. Cystatin C provides notable advantages in the reclassification of CKD stages, particularly in cases where GFR measurements are borderline. CysC has demonstrated superior reliability over creatinine in predicting complications such as delayed graft function (DGF), a condition often necessitating hemodialysis. Preoperative measurement of CysC levels is instrumental in predicting long-term renal function and identifying patients at elevated risk for adverse outcomes. Elevated serum CysC concentrations in patients with renal cell carcinoma (RCC) have been correlated with poorer prognoses and an increased likelihood of postoperative renal injury. Despite its clinical advantages, the routine adoption of CysC is constrained by certain limitations. Chief among these is the significantly higher cost of CysC assays compared to traditional creatinine tests, which restricts its widespread application to confirmatory diagnostics or complex clinical scenarios.

Abstract

Dyspepsia is a syndrome characterized by a collection of symptoms such as epigastric discomfort, burning sensation, nausea, vomiting, bloating and satiety. These symptoms can be chronic and recur frequently, requiring long-term therapy. If left untreated, dyspepsia can interfere with daily activities and increase medical costs. In Indonesia, the prevalence of dyspepsia is increasing significantly, with major risk factors including irregular diet, alcohol consumption, stress, and the use of non-steroidal anti-inflammatory drugs (NSAIDs). Long-term use of NSAIDs can damage the gastric mucosa, increase gastric acid production, and cause gastrointestinal complications such as peptic ulcers and gastrointestinal bleeding. The main mechanism of NSAID side effects involves inhibition of the COX-1 enzyme which reduces the production of protective prostaglandins in the gastric mucosa. In addition, some patients with a history of gastric disease are more susceptible to side effects due to NSAIDs. Therefore, preventive strategies such as the use of selective COX-2 inhibitors and the combination of NSAIDs with proton pump inhibitors (PPIs) are effective approaches in reducing the risk of dyspepsia and related complications. The impact of dyspepsia is not only physical but also affects mental, social and emotional aspects. Sleep disturbances and anxiety are common in people with chronic dyspepsia, which can reduce productivity and quality of life.

Abstract

Tuberculosis (TB) in children with Human Immunodeficiency Virus (HIV) is a complex clinical challenge due to the synergistic interaction between these two diseases, which mutually worsen the patient's clinical condition. HIV decreases immunity, thus increasing the risk of TB infection, while TB accelerates HIV progressivity. TB-HIV co-infection is a major cause of morbidity and mortality, especially in children with undetected M. Tuberculosis infection. By 2022, an estimated 710,000 HIV patients will develop TB, of which 187,000 will die. This article aims to provide an update on the management of pulmonary TB in children with HIV who are routinely on antiretroviral (ARV) treatment. The discussion covers the definition, epidemiology, etiology, disease progression, pathogenesis, diagnosis, supporting investigations, and management of TB-HIV. The diagnosis of TB in children with HIV is often difficult due to non-specific clinical manifestations and limitations of diagnostic tests. Therefore, early detection through comprehensive testing is essential. TB-HIV management requires a multidisciplinary approach, including appropriate TB treatment and integrated ARV administration. Intensive monitoring is also required to prevent complications, such as drug resistance or adverse effects of therapy. In conclusion, this article emphasizes the importance of a holistic approach in evaluating and managing pulmonary TB in children with HIV, including interprofessional collaboration, family education, and preventive efforts to improve patients' quality of life and reduce mortality.

Abstract

As the world's largest palm oil producer, Indonesia bears a significant responsibility for the well-being of its farmers, including mental health aspects, which remain a pressing issue. Farmers face various occupational pressures that have the potential to trigger psychological disorders, particularly anxiety. Poorly managed anxiety can lead to decreased concentration and productivity. This study aims to describe the anxiety levels experienced by palm oil farmers in Simpang Pematang District, Mesuji Regency, Lampung. A cross-sectional design was employed in this study, utilizing purposive sampling for sample selection. A total of 109 palm oil farmers participated as respondents, and the collected data were analyzed using frequency distribution. The results indicate that 14.7% of farmers exhibited symptoms of anxiety, with 11.9% experiencing mild anxiety and 2.8% experiencing moderate anxiety. No cases of severe anxiety were found among the respondents. Although the overall anxiety level identified in this study was lower than in previous research, it remains a matter of concern. These findings highlight the necessity of continuous efforts to enhance the mental well-being of farmers. Therefore, a more intensive mental health promotion program is required to improve farmers' understanding of stress management strategies. By doing so, farmers are expected to maintain their psychological well-being, ultimately contributing to increased productivity in Indonesia's palm oil agricultural sector.

Abstract

Low Back Pain (LBP) is one of the most prevalent musculoskeletal disorders globally and a leading cause of disability. One of the occupational groups at high risk of LBP is online motorcycle taxi drivers, who spend long hours in a static sitting position while riding. The primary factors contributing to LBP among online motorcycle taxi drivers include long working hours and extended work tenure. This study aims to analyze the relationship between working duration and work tenure with the occurrence of LBP among online motorcycle taxi drivers. Based on various studies, it has been found that working more than 8 hours per day can increase the risk of LBP by up to 3.6 times due to excessive muscle strain and spinal load. Additionally, work tenure exceeding 5 years is associated with a higher risk of LBP due to accumulated physical stress, which can reduce muscle performance and deteriorate overall physical condition. Studies conducted in various cities, such as Makassar, Surabaya, and Padang, have demonstrated a significant relationship between working duration, work tenure, and the incidence of LBP among online motorcycle taxi drivers. Implementing proper work time management and ensuring sufficient rest breaks during riding are crucial steps in reducing the risk of LBP among online motorcycle taxi drivers. Additionally, attention to ergonomic factors in riding posture is essential to minimize the negative impact on musculoskeletal health.

Abstract

A student is an individual who studies at a higher education institution. A medical student is a student who requires dedication and undergoes significant pressure to complete their studies. Medical students are highly vulnerable to academic stress. Stress is a threatening state experienced by an individual, while a stressor is the source of stress. Academic stress occurs when students face academic pressure, such as intensive learning activities. Stressors that can contribute to academic stress include exams, practical sessions, assignments, and time constraints that must be managed within a short period. Academic stress impacts mental health, which in turn affects students' academic performance. Stress induced by psychological pressure causes changes in the body, resulting in two types of responses: slow and fast responses. These responses are linked to two physiological systems: the sympathetic adrenomedullary system and the hypothalamic-pituitary-adrenal  HPA   axis. Several factors influence academic stress in medical students, including self-efficacy, hardiness, optimism, achievement motivation, procrastination, and social support. High self-efficacy helps students manage stress, while hardiness strengthens their resilience in facing pressure. Optimism and achievement motivation contribute to students' ability to recover from failure and stay focused on their goals. Conversely, procrastination worsens stress due to delays in completing tasks. Social support from family, friends, and institutions plays a crucial role in reducing stress by providing a sense of security and emotional balance for students.

Abstract

Migraine is a neurovascular disorder characterized by recurrent headaches with symptoms such as nausea, phonophobia and photophobia. Factors related to migraine include food, psychosocial and environmental factors. The purpose of this article is to identify migraine trigger factors. The type of research is a literature review. Migraines can occur due to trigger factors including foods with certain ingredients (such as tyramine, chocolate), menstrual cycle, weather, sleep patterns, bright light, and screen time. The prevalence of migraine in the world reaches 10-14% with the highest incidence in North America, Central America and South America. In Indonesia alone, the prevalence of migraine has reached 3.5 million people with the highest coming from individuals aged 15-24 years. Until now, the exact cause of migraines is not known. However, there are many risk factors that can influence the occurrence of migraines. The diagnosis of migraine can be made through the results of the history and physical examination, and if necessary, supporting examinations can also be carried out to rule out other secondary causes that may influence migraine. Efforts to prevent migraines from occurring by controlling and avoiding trigger factors and living a healthy lifestyle. Migraine prevention can be done by avoiding trigger factors and adopting a healthy lifestyle, such as maintaining a regular sleep pattern, managing stress, and consuming a balanced nutritious diet.
