Abstract

Intra-aortic balloon pump (IABP) is often used in patients who undergo high risk percutaneous coronary intervention (PCI) procedures, particularly if associated with hemodynamic impairment. However, this device is still underused in this center. In this case report, a 65-year old male patient with triple vessel disease (chronic total occluded left anterior descending artery-LAD, and significant stenoses of left circumflex and right coronary arteries), low ejection fraction and preprocedure blood pressure, was reported to have IABP support applied before PCI. The intervention to penetrate the occluded LAD was not success. After procedure, the patient was observed in intensive care unit. Weaning of IABP was performed after about 30 hours usage, without complication.

Abstract

The objective of this study was to analyze correlation between hearing impairment and communication disorder among geriatrics in Geriatric Clinic, Dr. Soetomo Hospital. A randomized cross-sectional study design has been used in this study. Analysis was done by filling in some questionnaire and interview to some patients who were randomly selected according to predetermined condition. The data were collected and reviewed for the factors, which were assumed as risk factors of hearing disorder and life's quality deterioration. Statistical analysis was performed using Fisher's Exact Test using SPSS program version 17. Results showed correlation between hearing impairment and the quality of life and social communication (p=0.033, OR=6.11). However, there was no correlation between hearing impairment with diabetes mellitus (p=0.255), with hypertension (p=0.565), and noisy environment (p=0.689). In conclusion, the study showed that there is correlation between hearing impairment and communication disorder.

Abstract

Currently, therapy with Platelet Rich Plasma (PRP) has been widely used and continues to grow for various clinical applications. Along with its development, there are various options in the method of obtaining PRP, either automatic or manual, while one of the most reliable according to the literature is a double centrifugation method. The purpose of this research is to produce an optimization of the double centrifugation method. This study uses experimental data obtained by researchers at the Clinical Pathology Laboratory of Dr. Soetomo Hospital, Surabaya. Experiments generally were conducted on stored blood obtained from the blood bag from PMI with CPD anticoagulant. Optimization of double centrifugation method to produce PRP with optimum platelet count could be obtained by using an "I"-shaped tube as well as other specialized equipment which are currently not available in standard clinical pathology laboratory, and also fresh blood. The resulting increase amounted to 4.11 times with the platelet count of 1.152 million.

Abstract

Cyclosporine and methylprednisolone combination are second line therapy for moderate to severe systemic lupus erythemathosus. Some study suggest that the combination were effective to decrease of systemic lupus erythematosus disease activity. But record from the study, cyclosporine cause nephrotoxicity side effect. Therefore, this study should be considered to monitore therapy effect on disease activity and renal side effect. The aim of this study is to analyze the effect of cyclosporine and methylprednisolone combination therapy on disease activity in systemic lupus erythematosus (SLE) assessed by MEX-SLEDAI and renal side effect assessed by creatinine, ureum and proteinuria. A cohort, observational prospective study was conducted to determine the effect of cyclosporine and methylprednisolone combination therapy on disease activity of SLE and renal side effect of this combination. Patients who met criteria were given cyclosporine and methylprednisolone combination that normally renal function tests. MEX-SLEDAI score, creatinine, ureum and proteinuria were measured for fourth times (one time in one mounth), before study, 1st mounth, 2nd mounth, and 3rd mounth. The study comprised 9 patients SLE were given cyclosporine and methylprednisolone combination that normally renal function tests. All patients were female and had productive age. At 3rd mounth, there was increase patients who had MEX-SLEDAI score <2 (55,6%) and one patient (11,1%) had increase of creatinine, ureum and proteinuria. In conclusion, cyclosporine and methylprednisolone combination therapy showed the effectiveness and safety in 88,9% patients and renal dysfunction in 11,1% patients.

Abstract

Treatment strategy of ischemic stroke is to reduce the extent of the damage and rescue neurons from death in the early days of ischemic events. Recombinant Tissue-Plasminogen Activator (r-TPA) is the only recommended therapy, but their use is very limited. Citicoline is a neuroprotectant with a therapeutic effect on several stages of the ischemic cascade. However, its use is still being debated. The purpose of this study was to analyze the use of supplementation citicoline injection in patients with acute ischemic stroke in relations to differences in changes in the level of interference (impairment), rate limitation (disability) and the level of obstruction (handicap) between the group receiving supplementation of citicoline injection 2x500 mg iv and the group without supplementation during acute phase treatment. This study was a prospective cohort study using experimental design in patients with acute ischemic stroke who met the inclusion and exclusion criteria with or without supplementation citicoline between January - April 2015 in the National Stroke Hospital, Bukittinggi. Rate of interference was assessed with NIHSS, level of limitations with Barthel Index, and level of obstruction with modified Rankin Scale. Assessment was done 2 times, before and after the treatment. Statistical methods used in this study were Wilcoxon signed rank test, paired T-test and Mann-Whitney test. This study was conducted on 50 subjects divided into 2 groups, a control group without supplementation and group treated with injected citicoline of 2x500 mg iv. Demographic and baseline characteristics did not differ between groups. There were differences in level of interference changes. Mean decrease in control group was 0.96 ± 1.74 NIHSS, while that in treatment group was 2.84 ± 1.46 NIHSS (p <0.05). There were differences in changes in the level of limitations. Mean increase of Barthel Index in control group 9.60 ± 11.17 and in treatment group 20.40 ± 13.99 (p <0.05). However, changes in the level obstacle showed no difference. In conclusion, citicoline injection supplementation in patients with ischemic stroke during acute phase treatment showed improvement differences in changes in the level of distraction (impairment) and the rate limitations (disability), but showed no difference in changes in the level of obstruction (handycaps).

Abstract

Idiopathic thrombocytopenic purpura is a autoimmune disease characterized thrombocytopenia casued by excessive platelet destruction. However, both platelet destruction and reduced thrombopoietin level are occurred in some cases. Therefore, new management of ITP is emerged which target is to increase platelet production rate via eltrombopag or romiplostim as the thrombopoietin receptor agonist (TPO-RA). Eltrombopag is given orally while romiplostim is given subcutaneously or intravenously and dose adjustment is depend on platelet count. Both eltrombopag and romiplostim is indicated in minimal response glucocorticoid or intravenous immunoglobulin or splenetomy treatment.

Abstract

Stroke is the leading cause of death among Indonesian people over the age of five years, comprising 15.4% of all deaths, with a mortality rate of 99/100 000, and the number of disability is 685/100 000. Medical rehabilitation has an effect in improving functional status of patients with stroke, especially if it is done intensively in the first 6 months after stroke attack. Measurement of functional status is commonly used in clinic, including in Dr. Soetomo Hospital. However, no study had been conducted in measuring functional status using Barthel Index in Surabaya. Therefore, this study identified the difference of the functional status of stroke patients before and after undergoing medical rehabilitation. The study used an experimental method with pre and post design with total sampling. The samples were 47 stroke patients in Dr. Soetomo Hospital, Surabaya. Data were obtained from March - April 2014 using Barthel Index. Data analysis used Wilcoxon Sign Rank test, including the distribution of each variable, which showed signification value of <0.05. This study concluded that Barthel Index score increases after undergoing medical rehabilitation.

Abstract

Hyperbilirubinemia is a condition of high levels bilirubin accumulating in blood and known as jaundice, a yellow coloration of the skin, sclera and nails. Hyperbilirubinemia is a problem that often occurs in newborns. Patients with neonatal hyperbilirubinemia are treated with phototerapy and exchange transfusion. The purpose of this study was to identify the hyperbilirubinemia treatments on neonatal in Dr. Soetomo Hospital Surabaya. Medical records were used to collect information on hyperbilirubinemia status and type of treatments in 2010. Inclusion criteria were used to select samples in this study. All hyperbilirubinemia patients (100 neonates) were treated with phototherapy, in which 32% of them got the treatment for less than 24 hours; and 68% got the treatment more than 24 hours. After treatment, 100% patients cured.  Hyperbilirubinemia neonates in RSUD Dr Sutomo were treated with phototherapy and the treatment was successful.

Abstract

PCOS is an ovarian dysfunction syndrome which characterized by the present of unovulation, hyperandrogenism, and polycystic ovarian morphology. The main cause of PCOS is still unclear, but many studies report that genetic factor have a great role. There are also two big concept in PCOS, hyperandrogenism and insulin resistance. Previous research by Ayaz et al (2013) has shown that metformin was given if there has been a resistance of clomiphene citrate. This study was designed to provide metformin and clomiphene citrate combination from the early administration to determine the effectiveness of the combination therapy. The objective of this study is to analyze HOMA-IR value, follicle size, and estradiol concentration after metformin and clomiphene citrate treatment in PCOS patient. Design of this study was an observational longitudinal prospective method. There was only a group with a total sample size 19 women of productive age (18-40 years) diagnosed with PCOS. The pre and post combination therapy was observed for ten weeks. Metformin 500 mg three times daily for ten weeks and clomiphene citrate 100 mg in 3rd day until 7th day of menstruation for two cycles were given to the subjects. The result showed that HOMA-IR value decrease after 10 weeks in which value before therapy was 4.9 ± 2.4 and after therapy 3.7 ± 2.2 with the achievement 21.1% HOMA-IR under 2.5. Estradiol concentration after therapy also decreased from 64.8 ± 24.7 pg/ml to 37.9 ± 17.9 pg/ml in ten weeks this concentration remained the range of the follicular phase estradiol levels. On the other hand, follicle size increased from 4.7 ± 0.6 mm to 10.7 ± 2.2 mm after combination therapy of metformin and clomiphene citrate with the occurrence of ovulation 31.6% but none of the samples were successfully pregnant. It is concluded that women with PCOS, declined HOMA-IR, estradiol, and increased follicle size was associated with a good response to combination therapy of metformin and clomiphene citrate.

Abstract

Indonesia is a developing country with a dense population, so the government carry on KB (Family Planning) program in which the targets are mostly women. Contraceptives are intended for women are birth control pills and IUD (Intra Uterine Device), whereas until now the contraceptive for men who are considered well-established are condom and vasectomy. Thus, the development of herbal medicine, which is cheap and affordable, makes it possible to be consumed in a long term with less meaningful side effects. Papaya seeds (Carica papaya) is one of the many plants used by communities to eradicate intestinal worms, menstrual laxative, and abortivum. Empirically papaya fruit, leaves, and seeds of papaya also contains carpaine, an alkaloid which is used as anthelmintic. Papaya seeds, which are edible and spicy in India are used for contraception. Alkaloid content in seeds of papaya can be antifertility herbal plants for males that can be measured from the quality of spermatozoa. Alkaloid compounds contained in papaya seeds can disrupt the hormone testosterone, which would interfere the sperm quality, making papaya seed potential to become a candidate for contraceptive.
