Abstract

Erythrodermic psoriasis (EP) is a rare form of psoriasis but has a high mortality and morbidity rate. The management of erythrodermic psoriasis includes prevention of complications and administration of immunosuppressant drugs such as methotrexate. This article reports the case of a 37-year-old man with complaints of red, scaly spots almost all over his body since the past month. Initially, the red spots only appeared on the right and left arms and legs. On dermatological examination found lesions on >90% of the patient's total body area in the form of erythematous patches, well-defined, irregular shape and covered with thick scales, there was onycholysis of the toenails, and the Karvslek phenomenon in the patient. Based on the history and physical examination, the patient was diagnosed with erythrodermic psoriasis with a PASI score of 60.2. The patient received oral methotrexate therapy 7.5 mg/week, folic acid once daily, cetirizine 10 mg twice daily, and topical therapy with desoximetasone cream 0.25%. Methotrexate is taken alternately with a single dose of 2.5 mg. Evaluation of patients at 1 month after therapy showed improvement in clinical symptoms with reduced subjective symptoms followed by a change in the PASI score to 45.8. Proper diagnosis and management as well as monitoring for side effects of medication can reduce morbidity and prevent complications from EP. Abstract

Fatigue is part of the main problem in workers that can reduce health status resulting in accidents to death. Work fatigue is still the main cause of work accidents, especially for drivers. Online motorcycle taxi drivers are a group at high risk of fatigue because most of their working time is on the road. This study aims to analyze the effect of risk factors on work fatigue on online motorcycle taxi drivers in the Jember Region. The type of this research is observational analytic with cross-sectional design. The sample is 100 respondents. The sampling technique used consecutive sampling method. The research was conducted from February 2021 to April 2021 using a questionnaire. Data analysis using Spearman Correlation test and Logistic Regression test. The results of Spearman's analysis were age (p = 0.000), smoking habits (p = 0.000), work motivation (p = 0.000), years of service (p = 0.000), and length of work (p = 0.002). The results of the Logistics Regression analysis are the length of service (p = 0.000). The conclusion of this study is that age, smoking habits, work motivation, years of service, and length of work have an influence on work fatigue and tenure is the most influential risk factor for online motorcycle taxi drivers in Jember Region. The suggestion of this research is that it is necessary to research other risk factors and other fatigue measurement methods and the driver is expected to reduce the level of risk of work fatigue. Abstract

Diabetic ketoacidosis (DKA) is an acute complication of diabetes mellitus classified as mild, moderate, and severe based on the severity of the metabolic acidosis and level of consciousness. As one of hyperglycemic crisis, DKA causes kidney dysfunction which then trigger acute kidney injury (AKI). High mortality rates, recurrent DKA (repeated episodes), increased health care costs, and the risk of further complications can occur due to AKI. Early detection of AKI risk by renal function tests such as blood urea nitrogen (BUN) and serum creatinine can be done because its easy method hence it is widely used for routine evaluation. This study aims to determine the differences of BUN and serum creatinine values ​​in DKA patients classified by severity as a prognostic markers associated with the risk of AKI in RSD dr. Soebandi Jember. The design of this study was analytic observational using a cross-sectional design obtained from medical records of 34 patients. The results showed a non-significant difference in BUN values ​​(p=0.085) and a significant difference in serum creatinine values ​​(p=0.033) between mild, moderate, and severe DKA. Serum creatinine evinced to be more reliable indicator than BUN as prognostic marker of DKA in the setting of AKI risk. Keywords: DKA, prognosis, acute kidney injury, AKI

Abstract

Pulmonary tuberculosis (PTB) is an uncommon respiratory infection that rarely leads to the development of hyponatremia, a condition characterized by low levels of sodium in the blood. In most cases symptoms andiated hyponatremia are mild to moderate, often lacks noticeable symptoms, and can be effectively reversed through treatment with anti-TB medications. Hyponatremia is diagnosed when the serum sodium level falls below 135 mmol/L, and it is considered severe when the level drops below 125 mmol/L. Among electrolyte imbalances, hyponatremia is one of the most frequently observed, with a prevalence ranging from 1 to 4% in its severe manifestation. This journal reports the case of a male patient who presented with decreased consciousness, suspected to be associated with moderate hyponatremia, tuberculous meningoencephalitis, pulmonary tuberculosis, and sinus bradycardia. The patient's symptoms associated with hyponatremia were relatively mild, prompting the need for further evaluation to ascertain the root cause of the electrolyte imbalance. This is particularly important for patients with suspected tuberculosis who are also experiencing complications related to hyponatremia.

Abstract

Chromolaena odorata leaves have been used by the people of Aceh Besar as herbal medicine. The chemical compounds in the leaves of this plant act as antioxidants, anti-inflammatories, antibacterials and wound healers. The research objective was to determine the phytochemical content of C. odorata leaves through qualitative methods. The extraction method used is maceration and testing of bioactive compounds using a color test. The results showed that the yield percentage was 6.05%. The results of the analysis of phytochemical compounds obtained seven phytochemical compounds contained in C. odorata, namely compounds belonging to the class of flavonoids, phenolics, alkaloids, terpenoids, saponins, steroids and tannins. It is recommended to identify chemical compounds using the GC-MS test to obtain specific compounds that play an active role in anti-inflammatory, antioxidant and antibacterial activities. Abstract

Background: Aging occurs in all organs, including the skin. As the largest organ of the body, wrinkles on the skin show signs of aging that are visible and affect appearance. Thread embedding acupuncture (akupunktur tanam benang/ATB) is one of acupuncture methods used in various cases, including aesthetics. This case report describes the role of ATB in the management of nasolabial folds (NLF). Case Illustration: A 45-year-old woman came complaints of deep and long smile lines. On physical examination, the NLF length (measured with digital vernier caliper) right/left: 34.15/30.3 mm; Wrinkle severity rating scale (WSRS) right/left: 3/3. ATB was given once and the results were observed until week 12. The best results were obtained at week 12 of follow-up (NLF: 22.00/21.46 mm; WSRS: 2/2) Conclusion: NLF improvement in ATB is getting better over time. Further clinical trials are needed to determine the effectiveness of ATB in improving NLF. Keywords: Thread embedding acupuncture; Wrinkle; Aesthetic acupuncture

Abstract

Preeclampsia is hypertension that occurs after 20 weeks of gestation with proteinuria. As one of the second highest causes of maternal death in Indonesia, preeclampsia must be detected and managed quickly and appropriately before complications that threaten the mother and fetus arise. One of the conditions that can be associated with preeclampsia is the outcome of low birth weight babies (LBW). This study aimed to determine the relationship between the degree of proteinuria in preeclampsia mothers and incidence of LBW at RSU Karsa Husada Batu. This study used a cross-sectional study design. Data was collected from medical record. The independent variable in this study was the degree of proteinuria and the dependent variable was diagnosis of LBW. The population in this study were all preeclampsia mothers and their babies from preeclampsia patients at RSU Karsa Husada Batu in 2021 with total of 38 patient. Sampling was done by total population sampling. Data analysis used in the study is chi square test. The results of this study showed that there is a relationship between the degree of proteinuria in preeclampsia mothers and incidence of LBW or the higher the degree of proteinuria, the higher the risk of babies from preeclampsia patients experiencing LBW ( p=0,005 ; CI = 95%). Abstract

Thalassemia β major is a hereditary disorder caused by mutations in the β-globin gene, which regulates the formation of one of the components that make up hemoglobin. This disorder results in the production of β-globin chains being reduced or not formed. The prevalence of thalassemia carriers in Indonesia reached 3.8% of the entire population based on data from the Ministry of Health in 2019. Therapy for thalassemia β major patients is lifelong transfusion, which results in iron overload and triggers hypocalcemia and osteoporosis. The aim of this study was to determine the relationship between ferritin levels and calcium levels in thalassemia β major patients. This study uses an observational analytic research type and a cross-sectional research design. This research was conducted at the RSD dr. Soebandi Jember, Jember Clinic Plantation Hospital, and in December 2019-January 2020. The total sample was 13 patients suffering from thalassemia β major aged 1-18 years. The results of the Pearson analysis showed that p=0.858 showed that there was no ignificant correlation between ferritin levels and calcium levels in Thalassemia β major in Jember. Abstract

Hypertension has a high prevalence of patients in the world, as well as in Mumbulsari Jember District. This is due to many risk factors for the incidence of hypertension and doesn’t cause significant symptoms. Psychological stress is one of the factors causing hypertension. Work, economic, and social problems are triggers for stress. This is experienced by farm laborers with complex problems that cause psychological stress conditions. The purpose of this study was to determine whether there is a correlation between stress and increased blood pressure in hypertensive farm laborers in the Mumbulsari Jember Health Center Working Area. This research is an analytic observational research with cross sectional study design. The sampling method was convenience sampling. The data used were primary data (Self Reporting Questionnaire 20 (SRQ-20)) and secondary data (medical records of patients in Puskesmas Mumbulsari Jember). The results of this study showed normal psychology, namely in 11 respondents (42.3%) experiencing stage 1 hypertension conditions and 15 respondents (57.7%) experiencing stage 2 hypertension. SRQ-20 results identified mental distress, namely 23 respondents (60.5%) experienced stage 1 hypertension and 15 respondents (39.5%) experienced stage 2 hypertension. The results of the analysis showed there was no significant correlation between stress and increased blood pressure in hypertensive farm laborers in the working area of the Mumbulsari Jember Health Center with a ρ-value of 0.151 (ρ>0.05). This study concluded that there is no correlation between stress and increased blood pressure in hypertensive farm laborers in the Mumbulsari Jember Health Center Working Area.
