Abstract

A medical record is a file that contains records and documents regarding patient identity, examination, treatment, actions, and services that have been provided to patients. Medical records are one of the assessments in the accreditation of PHC which are contained in Chapter VIII of standard 8.4 medical record information. The method used in this research is qualitative with in-depth interviews with 6 informants and each PHC namely puskesmas Narmada with middle accreditation and Puskesmas Karang Pule with basic accreditation and consists of 3 people, the person in charge of medical records and doctors. Checking the validity of the data using the triangulation method. The results of the study based on interviews, obtained standardization of diagnostic classification codes and standardization of abbreviations in both PHC using ICD-10 but in practice, Puskesmas Karang Pule was not in accordance with the SOP. Access to medical record information in both PHC is in accordance with the SOP. which was 5 years, while the medical records of Puskesmas Karang Pule that were still active and inactive were still stored on the same shelf. The conclusion of this study is that both PHC is good in the medical record management system but must be adjusted to the existing SOPs. Suggestions should be an evaluation in each health center related to the medical record management system.

Abstract

Chronic kidney disease (CKD) is a disease with various causes that result in a progressive decline in kidney function and ends with kidney failure. The cause of kidney damage is an increase of body urea levels which can damage all cells, including neuron cells. Continuous brain cell damage can impair cognitive function. Commonly, cognitive impairment in patient with CKD is caused by 4 factors, include azotemic syndrome, hemodialysis, vascular risk factors and the presence of comorbidities. Azotemic syndrome occurs due to increased levels of urea and creatinine in the blood which can cause Uremic encephalopathy, a complex process associated with decreased cognitive function. Patients undergoing prolonged hemodialysis cause a decrease in cerebral perfusion and a decrease in blood flow velocity, resulting in a decrease of oxygen metabolism in the brain which causes a decrease in cognitive function. Cardiovascular system disorders in patient with CKD show cerebral atrophy and white matter lesions reflecting vascular damage and cerebral ischemic areas which are major contributors to cognitive impairment in patient with CKD. The presence of comorbidities such as low levels of hemoglobin and albumin is a significant risk factor for cognitive impairment.

Abstract

Tekelan (C. odorata) contains phenolic compounds that play an important role in wound healing. The potential of C. odorata leaves as a wound healer needs to be developed through the formulation of ointment to facilitate its use. The selection of ointment base will affect the effectiveness. The aim of this study was to determine the physical stability of ointment from C. odorata leaf extract on varios types of bases. The ointment of C. odorata leaf extract with four types of bases, i.e hydrocarbon base (F1), adsorption base (F2), washable base (F3), and water soluble base (F4) were tested for stability using mechanichal method and freeze-thaw cycle. Mechanichal stability test was carried out by centrifuging the samples at a speed 3750 rpm for 30 menutes, while the freeze-thaw cycle test was carried out for 12 days (6 cycles). The physical properties of ointment i.e organoleptics, homogeneity, pH, spreadability, and sticky power before and after the test were compared by ANOVA. The result of mechanichal test of ointment showed that there is no separation in all types of bases. Stability test by freeze-thaw cycle method indicated a change of homogeneity and organoleptic of F1 and F2. Spreadability of all formulas was not significantly different before and after the freeze-thaw cycle, while the stable sticky power only occurred at F4 (p-value=0.523). Formula F4 which is an ointment of C. odorata leaf extract with a water soluble base has the best stability among other bases.

Abstract

Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) is an emerging, highly contagious and pathogenic coronavirus. To date, there is no definitive therapy for SARS-Cov-2 infection known as COVID-19. Supportive therapy that can be done is the provision of vitamin and mineral supplementation by consuming vitamin C, vitamin D, and zinc. Several studies have shown that oral and intravenous administration of vitamin C can reduce the increased risk of complications, reduce severity, improve symptoms and improve prognosis with COVID-19. Zinc might reduce the risk of SARS-CoV-2 infection and shorten the duration and severity of illness.

Abstract

Abstract Background: Electroneurography (ENG) is also known as nerve conduction examination, which includes examination of motor, sensory and slow response nerve conduction velocity (KHS). Abnormalities of KHS and evoked potentials may reveal the underlying pathophysiology of peripheral nerve disorders. With a good clinical examination, the ENMG examination will very helpfull main diagnosis. This examination helps determine the topical diagnosis, pathology and prognosis of peripheral nervous system disorders. Methods: This type of research is a descriptive observational study from the patient's medical record to determine the profile and results of the examination of patients undergoing electroneurography examination at the outpatient polyclinic, Mataram University Hospital. Sampling in the form of medical record data from January 1, 2020 - December 31, 2020 with a sample of 40 people. The research data will be entered and presented in tables and figures. Then the tables and figures will be analyzed. Results: Patients the most research subjects male as many as 27 people (67.5%). Patients who have the most body weight with a weight range of 51-100 kg 30 people (75%). For the highest height of the sample is in the range of 150-200 cm with a total of 34 people (85%). Most of the patients with non-traumatic complaints were 34 people (85%). The highest number of patients was polyneuropathy with a total of 22 people (55%). Conclusion: From this study, it was found that patients who take electroneurographic examinations at the Neurology Polyclinic at the University of Mataram had very varied characteristics and examination results.

Abstract

Hepatocellular carcinoma (HCC) is a primary liver cancer that occurs due to the abnormal growth of hepatocytes. HCC is one of the cancers with the highest prevalence and incidence in the world. The main risk factors for HCC in Indonesia are chronic infection with hepatitis B virus, hepatitis C virus, and liver cirrhosis. The selection of the appropriate treatment modality for each patient is based on several patient-specific characteristics, such as tumor size, location, portal vein thrombosis, and liver function. Treatment options for unresectable hepatocellular carcinoma include intra-arterial therapy, multikinase inhibitors, and immunotherapy. Determining the stage is an important part of managing HCC because it can determine the treatment. One of the staging systems is the Barcelona Clinic Liver Cancer (BCLC) which categorizes HCC into 5 stages. Clinical severity criteria with BCLC stage are often used because they have good validity in predicting the prognosis of HCC patients.

Abstract

Abstract Background: Vascular cell adhesion molecule-1 (VCAM-1) is an important protein in the progression of mitral valve damage in rheumatic heart disease. To measure the severity of the damage, there is a scoring system known as Wilkins Score, consisting of valve mobility disorder, valve thickening, valve calcification and sub-valvular thickening. Methods: This analytic-observational study was using cross-sectional design. There were 45 patients with mitral stenosis of rheumatic heart disease who were collected by consecutive sampling. The peripheral VCAM-1 level was obtained from peripheral venous access, and echocardiogram was obtained to assess Wilkin Score as well as other related parameters. There were also 22 normal subjects who got involved as comparison. Results: The mean Wilkins Score of patients are 9,09 ± 1,81. The mean peripheral VCAM-1 level of patients and normal subjects are 1155,3 ± 328,8 ng/mL and 605,5 ± 100,4 ng/mL. There is significant difference in peripheral VCAM-1 level between 2 groups (p<0,001). There are also significant positive correlations between peripheral VCAM-1 level and valve mobility disorder (p<0,001, r = +0,522), valve thickening (p<0,001, r = +0,583), valve calcification (p<0,001, r = +0,538), sub-valvular thickening (p <0,001, r = +0,647), and also Wilkin Score (p<0,001, r = +0,841). Conclusion: There is a significant positive correlation between peripheral VCAM-1 level and Wilkins Score in mitral stenosis of rheumatic heart disease. Abstract

Astrocytoma is a primary brain tumor originating from astrocyte cells. The study was conducted using scientific literature review methods from various sources such as google search and google scholar with relevant keywords such as "prevalence of astrocytoma in Indonesia", "incidence of astrocytoma in Indonesia", "prevalence of astrocytoma in Indonesia", and "epidemiology primary brain tumor in Indonesia". Indonesia". After reading the titles and abstracts of the articles found, there were five research articles that met the criteria. From this literature study, there were 269 cases of astrocytoma in several hospitals in Indonesia in 2007-2018. At RSUP Dr. Hasan Sadikin Bandung in 2007–2009 astrocytomas were most commonly found in patients aged 20–34 years. In 2010–2012 astrocytoma was most commonly found in patients aged 31–40 years. And in 2013-2016 astrocytomas were most commonly found in patients aged 46 to 55 years. While at RSUP Dr. Wahidin Sudirohusodo in 2016 most astrocytomas were found in patients aged 20-29 years. Astrocytoma at RSUP Dr. Hasan Sadikin Bandung in 2007-2009 was mostly located in the frontal area, in 2010-2012 astrocytoma was mostly located in the cerebral hemispheres, and in 2013-2016 astrocytoma was mostly located in the cerebellum. At RSUP Dr. Wahidin Sudirohusodo astrocytoma mostly located in frontotemporal. At RSUP Dr. Hasan Sadikin Bandung astrocytoma is more often found in men, while in RSUP. Dr. Wahidin Sudirohusodo astrocytoma is more common in female patients

Abstract

Background: Currently there is a rise in the study of saliva as alternative clinical samples for the detection of SARS-CoV2 as it is considered less invasive, able to be obtained independently by patients and does not cause discomfort compared to nasopharyngeal swab (NPS). Nevertheless, there has been an inconsistency of methods applied in sample collection, storage and laboratory assay from previous research. As the pre analytical phase may influence diagnostic result, therefore this scoping review would like to identify the varation approach of saliva collection methods conducted by previous studies. Study Design: We conducted a scoping review of the literature in 3 databases (PubMed, Science Direct and Google Scholar) and included articles published bewteen January 2020 until March 2021. Results: This review included 22 publications that met the inclusion criteria. All articles compared saliva with nasopharyngeal swabs. There were variations of sample collection approach which include type of saliva collected, type of container, the use of presrvative or stabilizing agent, time and volume of saliva collected and requirements for patients prior to sample collection. Moreover, not all study included a detailed information regarding salivary collection methods. Conclusion: Information regarding saliva sample collection and prepartion is required to not only be reproduced by following research but also to determine the best approach for optimal outcome.

Abstract

Background: Obesity is a fat metabolism disorder arised from the accumulation of excess fat in adipose tissue. The over accumulated fat in adipose tissue, results in an increases in the secretion of inflammatory cytokines that might hazardous to the health of maternal and infant. Increased inflammatory cytokines can leads to impaired hypothalamic-pituitary-axis, myometrial contractility and cervical ripening associated with gestational age at birth. Futhermore, the increase in inflammatory cytokines also causes an increase in the placental amino acid transport system and decreased placenta growth factor which is correlated with infant birth weight. Methods: This is a retrospective cohort study collecting cohort data of pregnant women that had met the inclusion and exclusion criteria at the Puskesmas Cakranegara from 2019 to 2021. The sampling method used in this study is total sampling. Results: There were 243 data maternal pregnancy that met the inclusion. There was a statistically significant correlation between BMI of pregnant women at first trimester and gestational age at delivery (p=0.032). There was also a statistically significant relationship between obesity in pregnancy and the incidence of preterm (p=0.029) and postmaturity (p=0.047). Otherwise, researcher didn’t find any statistical correlation between BMI of pregnant women measured in the first trimester and birth weight (p=0.066).
