Abstract

At RSUD Jayapura, from 2021 to 2023, 62 babies were born to HIV-positive mothers. However, no prior study has evaluated whether all these babies were tested for HIV. Babies born directly into the delivery room without passing through the VCT clinic often face challenges in follow-up, especially with high dropout rates in the Prevention of Mother-To-Child HIV Transmission (PMTCT) program.  This study aimed to evaluate HIV transmission in babies enrolled in the PMTCT program at RSUD Jayapura and assess the involvement of healthcare providers (doctors, midwives, nurses, counselors, and lab technicians) in the program. A mixed-methods approach was used, combining quantitative (case-control design) and qualitative (content analysis of in-depth interviews). The quantitative sample included 64 babies born to HIV-positive mothers who participated in the PMTCT program from 2021 to 2023. The qualitative sample comprised 5 healthcare providers involved in the program. Data were analyzed using Chi-Square tests for quantitative analysis and content analysis for qualitative data. Chi-Square analysis showed a significant association with a p-value of 0.00 (0.05) and an Odds Ratio (OR) of 217.000, indicating a strong impact of the PMTCT program in preventing HIV transmission. In-depth interviews with the VCT head revealed many cases were lost to follow-up, especially those whose families moved out of the city. Babies who remained in the area were mostly HIV-negative by 18 months. Interviews with midwives highlighted that HIV-positive mothers with low viral loads typically delivered via caesarean section, though they faced delayed wound healing and higher infection risks. No discrimination was observed in room usage for HIV-positive mothers.

Abstract

Cervical cancer is one of the leading causes of death among women worldwide, including in Indonesia. Although it can be prevented through cervical cancer screening such as Pap smears and HPV tests, many women, including students, are unaware of the importance of routine screening. Universities, as educational institutions with a significant female student population in a vulnerable age group, play a crucial role in raising awareness and encouraging participation in cervical cancer screenings. However, several challenges exist, including low awareness among students about cervical cancer, social stigma, limited access to medical services, and the lack of integration of health education into the Curriculum. This policy brief aims to provide policy recommendations that universities can implement to address these issues. Key recommendations include integrating health education on cervical cancer into the Curriculum, collaborating with health institutions to provide screening services on campus, and developing sensitive and inclusive awareness campaigns. By increasing knowledge, reducing stigma, and improving access to services, this policy is expected to enhance student participation in cervical cancer screening, reduce mortality rates from cervical cancer, and foster a healthier reproductive health culture among students. These policies promise positive impacts on student health and have the potential to enhance the University's reputation as an institution that cares about its students' well-being. Successful implementation of this policy will reduce the prevalence of cervical cancer among young women and improve the overall quality of life for students.

Abstract

One of the biggest causes of death for women in Indonesia is cervical cancer. The incidence of cervical cancer in Indonesia was 0.8% or 23.4/100,000 population. Delay in early detection is the main cause of the high incidence of cervical cancer in Indonesia. Patients with cervical cancer are generally detected at an advanced stage. This is caused by the awareness of WRA to carry out early detection of cervical cancer through IVA examination which is still very low. To find out the factors that influence interest of of women of reproductive age (WRA) in the Acetic Acid Visual Inspection (IVA) examination during the Covid-19 pandemic in the working area of the Kebun Tebu Health Center, West Lampung Regency. This was a cross sectional study. The population in this study was 459 women who were taken from the target number of IVA examinations at the working area of Health Center of Kebun Tebu  in 2022. The number of samples in this study were 78 respondents. Measuring tool used in this study was a questionnaire. This research was conducted from October - November 2022 at the Kebun Tebu Health Center, West Lampung Regency, Indonesia until the sample and research data limits were met. The results of this study showed that the education level did not influence interest with a p-value of 0.423, knowledge level influenced WRA interest with a p-value of 0.024, attitude level influenced WRA interest with a p-value of 0.037, husband's level of support did not affect WRA interest with a p-value of 0.313 , and the level of information access influences WRA interest with a p-value of 0.030. Knowledge factors, attitude factors, and information access factors influenced WRA interest in conducting an Acetic Acid Visual Inspection (IVA) examination. Meanwhile, the Education Factor and the husband's support factor did not affect WRA interest in conducting an IVA during the Covid-19 pandemic in the working area of the Kebun Tebu Health Center.

Abstract

Pregnant women with Chronic Energy Deficiency (CED) can cause health problems for both mother and baby. Among them are babies with low birth weight (LBW), who are at risk of stunting. The Pekalongan City Government issued Mayor Regulation Number 11 of 2023 concerning the Acceleration of Integrated Stunting Reduction, where the group of CED pregnant women is one of the targets of Stunting-prone Families who need specific and sensitive interventions. This study aimed to analyze implementation of the policy, especially sensitive interventions of CED pregnant women in Pekalongan City. This study used a descriptive qualitative method. The research was conducted from August to September 2024. The object of the study was the implementation of sensitive interventions of CED pregnant women which was analyzed with Van Metter-Van Horn policy implementation model. The research was conducted through in-depth interviews, observation and documentation. The determination of informants used a non-probability technique with a purposive method, and 11 informants were obtained. The stages in data analysis were data collection, reduction, presentation, data verification and conclusion drawn. The results of the study found that sensitive interventions in form of access to clean drinking water and sanitation, housing improvement programs, social assistance recipient programs and training facilitation for families of CED pregnant women are still not optimal. Pregnant women in CED had not been considered as a priority in obtaining the benefits of assistance programs. It needed to be improved again in policy socialization, quality and quantity of resources, coordination and communication, and improvement of the environmental situation.

Abstract

Postpartum maternal pain increases with injuries to the birth canal. Some cases of postpartum infections are caused by improper perineal wound care, which causes bacterial growth. If the perineal wound is not treated with proper care, it can lead to infection and even death in some cases. Traditional plant as nonpharmacological therapy such as Jatropha multifida L. could be one of alternative therapy for wound healing. The purpose of this study was to examine the potential of secondary metabolites found in the Jatropha multifida L. plant as antibacterial and anti-inflammatory as an effort to heal perineal wounds in postpartum mothers. This study used the PRISMA (Preferred Reporting Items for Systematic Review and Meta Analysis) method. Five literatures was obtained from several literature search engines, namely Science Direct, Google Scholar, PubMed, ResearchGate, and ProQuest. The keywords used in the search were "Jatropha multifida" and "Wound healing". The results of a literature review related to Jatropha multifida L. showed that this plant has the potential to be used as a therapy for healing perineal wounds in postpartum mothers because it contains several secondary metabolites such as alkaloids, flavonoids, tannins, and saponins which act as antibacterial and anti-inflammatories in the wound healing process.

Abstract

Anxiety in pregnant women is a common emotional condition that occurs during pregnancy, characterized by excessive feelings of anxiety, worry, or fear. Pregnancy anxiety itself is experienced by many pregnant women due to lack of knowledge, readiness and support from family, community and environment. Excessive anxiety during pregnancy can lead to an increased incidence of uterine atony, infection, maternal fatigue, and shock, while in infants it can increase the risk of preterm birth and low birth weight (LBW). This study aims to examine the relationship between family support and maternal readiness with the anxiety level of pregnant women before delivery. This study used a quantitative research design with a cross-sectional approach, conducted in the working area of Andalas Padang Community Health Center. The sample consisted of 59 third-trimester pregnant women, selected through Simple Random sampling. Data collection was carried out using a questionnaire. The results of this study showed that the percentage of pregnant women who received family support and experienced less severe anxiety was 52.2% (p=0.048), including analysis of this data using the Chi-Square Test. Furthermore, it was found that pregnant women who were less prepared for childbirth experienced severe anxiety at a rate of 50.0% (p=0.002). In conclusion, there is a relationship between family support and maternal readiness with anxiety levels before delivery in the working area of Andalas Padang Community Health Center.

Abstract

Premenstrual Syndrome is a condition commonly experienced by young women with physical and emotional symptoms that interfere with daily activities. The prevalence of PMS in Indonesia reaches 85% of the entire female population of reproductive age, consisting of 60-75% experiencing moderate and severe PMS. This study aimed to prove the effectiveness of pumpkin seed extract (Curcubita moschata) in reducing the level of Premenstrual Syndrome pain in young women. This quantitative research, using a True Experimental design, employed a pre-test and post-test with a control group design. The study population consisted of all young women experiencing premenstrual syndrome in the Semarang Ministry of Health Polytechnic area. The sampling technique used was a randomized clinical trial with 42 samples. Ethical approval from the Health Research Ethics Commission of Poltekkes Kemenkes Semarang with Number 1185/EA/F.XXIII.38/2024. The research was conducted on three treatment groups: intervention 1 with a dose of 500 mg 2x2 a day, intervention 2 with a dose of 500 mg 2x1 a day, and the control group which received starch tablets 500 mg 2x1 a day for 10 days. The results analyzed using the Paired T-Test showed that giving pumpkin seed extract at a dose of 500 mg 2x2 a day for 10 days could most significantly reduced the intensity of premenstrual syndrome pain (p-value 0.000; mean 3.86) compared to intervention 2 and control. These findings suggest that pumpkin seed extract has the potential to be an effective natural alternative for reducing PMS symptoms in adolescent girls, particularly in pain reduction.
