Abstract

Background: Malocclusion is dentofacial growth deviation from its normal size and shape, thus causing abnormal occlusion. One of its etiology is heredity. Removable twin block, a functional appliance that can be used in interceptive orthodontic treatment, can be used to treat Class II malocclusion with large overjet in children during skeletal growth and development. Case: An 11-year-old boy came to pediatric dental specialist clinic with Class II division 1 Angle malocclusion (SNA 82°; SNB 74°; ANB 8°, CVM CS 4, overjet 14 mm, overbite 7 mm, and convex profile). Removable twin block for Class II malocclusion was chosen as the therapy. Case management: Removable twin block appliance with acrylic occlusal bite block was used on maxilla and mandible separately. An angle of 70° was made in the occlusal bite block, which was made based on the bite registration wax during centric occlusion. This device is used 24-hours a day. Monthly dental visit was done to reduce the maxillary and mandibular occlusal bite blocks. After 9 months, an overjet of 5 mm was obtained. Cephalometry calculations became SNA 82°; SNB 7 9 °; ANB 3 °. The patient was given further phase 2 treatment with fixed orthodontic appliance. Conclusion: With proper case selection and good patient cooperation, removable twin block appliance can correct skeletal malocclusion, improve facial profile, reduce overjet and overbite, and correct molar relations.

Abstract

Background: Pulpitis is an inflammation of dental pulp due to infection of microorganisms from dental caries. Pulpitis is clinically divided into reversible and irreversible pulpitis. Treatment of reversible pulpitis can be done by applying a pulp capping material. The use of calcium hydroxide as a pulp capping material is still not optimal, so an alternative material is needed, namely karamunting leaf (Melastoma malabathricum). The secondary metabolites in karamunting leaf extract have antibacterial activity. Antibacterial activity is needed in the regeneration of pulp tissue. Purpose: To analyze the effect of karamunting leaf extract (Melastoma malabathricum) on viability and proliferation of Baby Hamster Kidney-21 (BHK-21) fibroblast cells using the MTT Assay method. Methods: This study is a purely experimental study with post test only with control group design which were divided into 8 groups. The group was treated with karamunting leaf extract consisted of 5 groups, were 5%, 10%, 15%, 20%, and 25% concentrations also 3 control groups, were positive control (calcium hydroxide), cell control, and media control. Absorbance value was read using ELISA Reader and the percentage of cell viability was calculated. Results: All treatment groups had cell viability values higher than cell control and cause proliferation of BHK-21 fibroblast cells. The One Way Anova showed a value of sig=0,000 (p<0,05), which means that there was a minimum difference in one treatment group. Conclusion: Karamunting leaf extract and calcium hydroxide can maintain viability and cause proliferation of BHK-21 fibroblast cells. Karamunting leaf extract 25% concentration more effective in causing proliferation of BHK-21 fibroblast cells.

Abstract

Background: Classification of malocclusion, especially in the class II division, has various biological variations and dental abnormalities with the presence of distocclusion of the first molar relation and proclination of the maxillary fourth incisor and causing the patient's profile to become convex. Aesthetic correction in class II division 1 is determined by the choice of treatment and taking into account the rotational pattern of the mandible. Objectives: To determine the effect of orthodontic treatment in relation to changes in mandibular rotation with lower anterior facial height and to determine the parameters of the effect of mandibular rotation on lower anterior facial height (LAFH) in Angle Class II/1 dentoskeletal malocclusion. Methods: Cephalometric data were obtained from lateral cephalograms presented before and after treatment. Assessed changes in Y-axis FH angle, SN - mandibular plane angle (GoMe) and NPog-FH angle facial angle. Data analysis with Pearson was used to correlate the mandibular rotation parameter (angle) with the lower anterior facial height parameter, namely ANS-Me (linear) (p > 0.05). Results: Group 1 access maxilla, normal mandible showed a significant correlation on the Y-FH Axis with ANS-Me. Group 2 with normal maxillary, retrognathic mandibles, showed a significant correlation between NPog-FH with ANS-Me and NPog-FH with extrusion of the upper/lower first molars. Group 3, the combination group which is the maxillary retrognathic access, showed a significant correlation between Sn-GoMe and ANS-Me. Group 4 is with different teeth, but normal bone (grade 1). Conclusion: There is a relationship between changes in mandibular rotation and changes in lower anterior facial height (LAFH) in the orthodontic treatment of class II/1 Angle dentoskeletal malocclusion.

Abstract

Background: Glass ionomer cement (GICs) has become a common restorative material in dental practice. Therefore, there are so many GIC's brands can be found in the market with tremendous price disparities GIC brands can be found in the market with huge price disparity; Price of one brand can be up to tenfold than the others. For the cost-efficiency reason, many clinicians prefer the more economical. Therefore, questions arise from the clinician, does it influence the clinical performance. Method: Six cylindrical specimens (10mm x 2mm) from four GIC brands (Fuji IX, FXUltra, GlassyCem and Sangchi) were made based on the respective factory instructions and stored for 24 hours until the final setting reached. Micro Vickers hardness tester (Shimadzu) and Surface roughness tester were used as surface characterizations measurement. Collected data were analyzed statistically. Results: Kruskal-Wallis and Mann-Whitney analysis showed that Fuji IX and FXUltra significantly (p<0.05) higher surface hardness (68.47 and 63.29 VHN respectively) compared to GlassyCem and Sangchi (38.53 and 36.70 VHN). While for surface roughness measurement, Fuji IX has the highest surface roughness, 7.22 µm (p<0.05). GIC’s clinical performance is affected by composition and powder-liquid ratio. Strontium in Fuji IX could increase surface hardness. The high powder-liquid ratio in Fuji IX and FXUltra improve its clinical performance compared to the other group. Conclusion: The more expensive GIC (Fuji IX and FX ultra) had better surface hardness with a slight difference in surface roughness. Therefore, clinicians should choose GIC based on cost-efficiency without ignoring clinical performance.
