Abstract

Introduction: Hirschsprung's disease (HD) is characterized by the absence of ganglion cells in Meissner's plexus and Auerbach's plexus which used to be assessed by histopathological examination using Hematoxylin and Eosin (H&E).  In some cases is difficult to assess ganglion cells, thus requiring additional examination in the form of calretinin immunohistochemistry to detect the presence of ganglion. Methods: The 59 samples of suspected HD  were analyzed by H&E staining followed by calretinin immunohistochemical staining. This study aimed to assess the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of calretinin in HD diagnostics. Results:  The immunohistochemistry of calretinin showed a good diagnostic value in detecting aganglionosis in patients with suspected HD with a sensitivity value = 97.7%; Specificity = 100%; Positive Presumptive Value (PPV) = 100%; Negative Presumptive Value (NPV) = 93.7%; Accuracy= 98.3%. Thus, calretinin immunohistochemistry can be relied upon as an additional method in the diagnosis of HD

Abstract

Introduction:  Hepatitis B virus infection in fetal and newborn may result from the intrauterine vertical transmission. The purpose of our study is to determine the association between HBsAg expression in the placental and HBsAg serum in pregnant women. Methods: Total 67 placental tissues and serum were obtained from HBV-infected mothers. This study was conducted in various hospitals in Makassar, Indonesia from December 2017 until August 2018. Maternal serum and placental tissues were tested by ELISA and Immunohistochemistry staining. Results: 67 samples from placental tissues and serum were categorized into two different levels of serum titer and staining positivity. Twenty nine samples (43.3%) were categorized in low titer of HBsAg positive serum and 38 samples (56.7%) in high titer of  HBsAg positive serum. From the total of 67 placental samples, 43 (64.2%) samples had positive expression of HBsAg immunohistochemistry staining and 24 (34.8%) samples had negative expressions. Conclusions: There was a significant correlation between the expressions of HBsAg in the placental tissues and serum titers of HBV-infected mothers that is important in predicting the risk of transplacental transmission. The results of this study can be used as guidelines for the prevention and therapeutic approach of mother-to-child HBV infection.

Abstract

Background : Obesity in pregnancy due to carbohydrate over nutrition is still rarely reported and the incidence of obesity in pregnancy in the developing countries begin to increase. The aims of this study is to observe the detrimental effect of obesity in both mother and developing fetus of mice which fed by high carbohydrate diet. Methods : Female mice were divided to four groups. Group 1 (obese group) were fed with high carbohydrate (HC) diet and group 2 (control group) with standard diet for 12 weeks. All mice will be mated until pregnant and terminated for collecting blood with cardiac puncture every trimester. Another mice (7 mice from obese group and 7 mice from control group) will follow until 3rd trimester than terminated to collecting lipid profile of blood and examined the fetal’s anthrophometric. Lipid and blood glucose were examined by chemichal photometric and glucometer kit, while anthrophometric’s variables measured by weight scale and meter scale. Results : Blood glucose, total cholesterol, HDL and LDL are similar between obesity group and normal weight group every trimester, while only blood TG increased in obesity group particularly in first and second trimester of pregnancy. The detrimental effect also found in the developing fetus of obesity group which have significance higher of bodyweight, body lengths, head circumference, and waist circumference. Conclusions: Obesity in pregnancy due to carbohydrate over nutrition could have detrimental effect to both mother and developing fetus.

Abstract

Rabies is a neurological disease with fatal impacts because it has a high mortality rate. Animals, primarily dogs, mediate the spread of rabies. Clinical findings of rabies are divided into two categories: classic rabies (furious type) and paralytic type. Furious rabies has many cardinal features, such as fluctuating consciousness, aerophobia or hydrophobia, inspiratory spasm, and autonomic dysfunction. Ascending paralysis with a lower motor neuron lesion is the initial sign of paralytic rabies. The rabies virus invades and lives in neurons. It is virulent. Besides clinical findings, the diagnostic approach can involve radiology, microbiology, and histopathology. There are not only symptomatic treatments for rabies patients but also vaccines, immunoglobulin, and antivirals to decrease the progressivity of the disease. Vaccination programs and animal control have become essential in reducing rabies cases in Indonesia.

Abstract

Introduction: Online motorcycle taxi is a service that is currently widely used in goods delivery services and other forms of services. There are several things that are not considered by online motorcycle taxi drivers when driving, namely the driving position that is not good and correct. The more consumer demand for online motorcycle taxi applications, the higher the working hours of online motorcycle taxi drivers which leads to increased complaints of pain in the body. This study aims to determine the relationship between position and duration of work with complaints of suspected carpal tunnel syndrome in maxim motorcycle taxi drivers in Makassar City. Methods: The sampling technique in this study was purposive sampling through a cross sectional approach. Respondents in this study were riders aged 17 years and over as many as 373 riders who met the exclusion and inclusion criteria. Work position was measured using REBA, Work Duration was measured using a work duration questionnaire, while Suspect carpal tunnel syndrome complaints were measured using Phalen's Test. Results: The Kolmogorov Smirnov normality test shows a significance value (p) of 0.000, meaning that the data is not normally distributed (p <0.05). Then the correlation test between the two variables using the Spearman's rho test (p), namely the work position with complaints of suspected carpal tunnel syndrome obtained results (p=0.025; r=0.116 *) which means there is a significant relationship between the two variables and for the duration of work with complaints of suspected carpal tunnel syndrome obtained results (p=0.448; r=0.39) which means there is no relationship between the two variables. Conclusions: The distribution of maxim online motorcycle taxi drivers' work positions in the city of Makassar is dominated by respondents who experience the high risk category and category of work duration are more than 8 hours/day. There is a significant relationship between work position and complaints of suspected carpal tunnel syndrome on maxim online motorbike taxis in the city of Makassar. There is no significant relationship between work duration and complaints suspect carpal tunnel syndrome on maxim online motorcycle taxi in Makassar city.

Abstract

Introduction and importance: Dermatofibrosarcoma protuberans (DFSP) is a rare soft tissue sarcoma that primarily affects middle-aged adults but can also occur in children. Despite its low metastatic potential, DFSP displays aggressive local behavior and a high rate of local recurrence. Diagnosis in children can be delayed due to confusion with other skin conditions. This case report aims to discuss the case management of dermatofibrosarcoma protuberans in a 13 year old female. Presentation of case: A 13-year-old female patient presented with a progressively enlarging lump on the left eyelid, initially resembling small brown spots that coalesced over 7 months. Intermittent bleeding and pain were reported. Physical examination revealed a dark brown mass in the left frontotemporal region, with solid, non-fragile characteristics. Clinical diagnosis suggested a suspicious malignant skin tumor, confirmed as basal cell carcinoma. Treatment involved wide excision and flap reconstruction. Post-operative care included infusion, antibiotic injection, and pain management. The patient's prognosis was favorable, though functional impairment was possible. Follow-up revealed histopathological findings of pigmented dermatofibrosarcoma protuberans. Discussion: DFSP is a rare but significant malignancy that poses diagnostic challenges due to its varied clinical presentations and histopathological features. While primarily affecting young to middle-aged individuals, it can also manifest in pediatric populations. Diagnosis typically involves histopathological assessment, confirming characteristic spindle cell proliferation with CD34 expression. Surgical excision remains the cornerstone of treatment, complemented by adjuvant therapies like imatinib mesylate or radiation in select cases. Regular surveillance, comprising frequent evaluations in the initial post-excision period followed by yearly monitoring, is imperative for detecting any recurrence or metastasis promptly. Conclusions: The structured approach in DFSP ensures optimal patient management and outcomes. Surgical management with Wide local excision (WLE) or Mohs micrographic surgery (MMS) is the gold standard in the treatment of DFSP.
