Abstract

Background: Inflammatory response in the acute phase of ischemic stroke will trigger the process of neuroplasticity and determine the clinical outcomes. Angiogenesis and neurogenesis are induced by expression of vascular endothelial growth factor (VEGF) in the acute phase of stroke. The purpose of this study was to determine the association between VEGF serum level in acute phase of stroke with the clinical outcomes.Methods: This longitudinal cohort study was conducted on 64 patients suffering from first-attack of anterior circulation blockage as evidenced by cephalic diffusion-weighted magnetic resonance imaging (DWI). VEGF serum level was measured at 72 hours and 7 days after stroke and the clinical outcomes were assessed on day 30 post-stroke using the National Institutes of Health Stroke Scale (NIHSS).Results: VEGF level at hour-72 and on day-7 were 5.84 ± 0.736 ng/mL and 5.797 ± 0.96 ng/mL, respectively (p > 0.05). High VEGF levels at hour-72 can be used to predict poor clinical outcome 30 days after stroke (OR = 6.5; 95% CI = 1.15-36.61; p = 0.034). Subjects who have increasing levels of VEGF on day-7 compared to hour-72 tend to have better clinical outcomes on day-30. (NIHSS score = 1.33 ± 1.22 vs 3 ± 3.78; p = 0.232).Conclusion: VEGF levels in the acute phase of ischemic stroke reflect the degree of brain damage, the dynamic of the increase in VEGF levels after a stroke was associated with better clinical outcomes.

Abstract

Tujuan: Angka  kejadian   konstipasi  cukup  tinggi   pada   penderita  stroke iskemik yang mengalami  immobilisasi karena  pada  saat  pasien   mengalami   penurunan   aktivitas  akan  menyebabkan penurunan  fungsi otot  abdominal, penurunan peristaltik usus yang  dapat memperlama  pasase  feses sehingga pasien mengalami konstipasi. Tujuan penelitian ini  adalah  untuk mencegah dan mengatasi masalah konstipasi  pada  pasien stroke di RSUP DR Wahidin Sudirohusodo Makassar dengan  massage abdomen. Massage abdomen  dapat menurunkan konstipasi melalui beberapa mekanisme  yang berbeda - beda antara lain dengan menstimulasi sistem persyarafan  parasimpatis sehingga menurunkan tegangan pada otot abdomen, meningkatkan motilitas  pada sistem pencernaan , serta memberikan efek pada relaksasi sfingter. Metode: penelitian ini menggunakan desain Quasi Experiment, dengan tehnik pengambilan sampel yaitu Consecutive Sampling. Responden 30 orang dibagi ke dalam 2 kelompok yaitu: kelompok intervensi yang diberikan massage abdomen sebanyak sekali dalam sehari selama tiga hari berturut –turut, sedangkan  kelompok kontrol tidak diberikan perlakuan. Sebelum intervensi, dilakukan penilaian awal konstipasi dengan menggunakan kuesioner Constipation Assesment Scale (CAS) Hasil: Hasil uji statistik menunjukkan nilai p=0,000, p<0,005 sehingga disimpulkan ada perbedaaan yang signifikan pada skor konstipasi (CAS) antara kelompok intervensi dengan kelompok kontrol. Massage abdomen terbukti memiliki efek terhadap penurunan konstipasi. Rekomendasi: Massage Abdomen dapat menjadi intervensi mandiri yang dapat dilakukan oleh  perawat untuk menurunkan konstipasi  tanpa menimbulkan efek samping.

Abstract

Background: APOEε4 is a strong genetic risk factor for Alzheimer’s disease (AD). AD itself has been associated with reduced Aβ clearance from the brain and plasma. Understanding the potential pathogenic link between APOEε4 and plasma Aβ might allow for earlier identification of people at risk of developing AD. The aim of this study is to find out the correlation between APOEε4 and plasma Aβ in amnestic mild cognitive impairment (aMCI) and AD patients.Methods: This is a comparative cross-sectional study of patients attending a memory clinic in Siloam Hospital Lippo Karawaci, Tangerang, during the period of 2013-2014. Subjects were categorized into three categories: normal aging, aMCI, and AD. We performed blood test to examine APOEε4, plasma Aβ4o level, and plasma Aβ42 level. All data analyses were performed using correlation test and logistic regression.Results: Sixty subjects (normal aging = 23, aMCI = 17, AD = 20) were included. There were 19 (31.7%) subjects with APOEε4 positive. Subjects carrying ε4 allele were more likely to have AD by 3.9-fold than subjects with APOE ε4 allele negative. There is a significant difference between the mean of plasma Aβ40 in aMCI group and AD group. We also found correlation between APOEε4 (+) and higher plasma Aβ42 (p<0.05).Conclusion: There is a correlation between APOEε4 and plasma Aβ42 level, which supports the hypothesis that this genetic isoform accelerates the rate and progression of AD through Aβ-dependent pathways.

Abstract

Introduction : Tetanus is an acute, potentially fatal infectious disease that is characterized by seizure (general motor spasm), lockjaw, opisthotonus, defans muscular, rhisus sardonicus. A high mortality case remains tetanus need special attention. Pyridoxine 100mg orally introduced as adjunctive therapy for spasm, aimed to reduce mortality rate.Case report : A 38-years-old man with lockjaw (< 1cm) accompanied by severe seizures (general motor spasm) that arise many-times caused the patient body arched back (opisthotonus), rhisus sardonices and defans muscular categorized severe tetanus (5 points of Dakar score). Based neuroinfection study grup of Indonesian Neurologist, tetanus patient treated with Human Tetanus Immune Globulin (HTIG) to neutralized toxins, antibiotics penicillin and metronidazole to eradicated causative bacterial, and diazepam (benzodiazepine) to handle seizures (general motor spasm). Pyridoxine (vitamin B6) 100mg used as adjuntive therapy for treated seizure aimed to reduce mortality risk. Patient hospitalized for 18 days and discharged with good condition (free spasm).Conclusion : The role of pyridoxine in the management tetanus may sinergism with diazepam for reduced general motor spasm (seizures), but it should be re-examined in a blind randomised trial. Key word : tetanus, seizures (general motor spasm), lockjaw, pyridoxine, diazepam.

Abstract

Puskesmas is part of the government and is obliged to perform its functions in disaster preparedness. This study aimed to determine the description and the relationship of determinant factors of nurse preparedness to the eruption of Mountain Gamalama at the working area of the Health Center in ​​Ternate City Health Office. The research used the combined research method (Mixed Method), which was the combination between the quantitative method and the qualitative method. The research samples included all the nurses in the Health Center of the working area of the Health office of Ternate City comprising 41 people. The data analysis was conducted using the Chi-square test for quantitative research. The Knowledge variable was p=0,05, and the skill variable was p= 0,171. The related to the preparedness. Meanwhile, the qualitative research was in the form narration of the interviews with the respondents who had low skills because of the little training and stimulation about the calamity.

Abstract

The pattern of blood pressure hypertensive nondippers is one of the factors that increase the risk of suffering from cardiovascular and cerebrovascular disorders , but the effect of antihypertensive drugs work period ( OHT ) relating to the type of the pattern of blood pressure in patients with hypertension hypertensive nondippers still not widely studied . This study aims to analyze the working relationship with the incidence pattern of antihypertensive drug blood pressure hypertension nondippers in patients with post- acute stroke ischemic stroke with and without hypertension . The method used was a cross -sectional study with 60 samples ( 30 hypertensive patients with ischemic stroke and 30 patients with hypertension as a control ) of the Hospital Dr . WahidinSudirohusodo Makassar and its network in 2013 . The results showed chi-square test patterns obtained blood pressure in hypertensive nondippers larger user group types ACEI captopril group OAH 18 ( 94.7 % ) with grades ( p > 0.05 ) with OR = 4.6 . It can be concluded that there are patterns of blood pressure hypertension incidence nondippers against ischemic stroke incidence was greater in OHT group ACEI types captopril group , although not significantly.

Abstract

Penelitian ini bertujuan mengetahui hubungan serum lipid darah dengan degenerative disc disease berdasarkan klasifikasi pfirrmann menggunakan magnetic resonance imaging lumbosacral pada pasien nyeri punggung bawah. Penelitian ini dilaksanakan di Departemen Radiologi RS. Dr. Wahidin Sudirohusodo Makassar mulai bulan Oktober 2018 sampai Januari 2019. Subjek sebanyak 52 orang dengan rentang usia 30 - 60 tahun. Metode yang digunakan adalah uji Spearman. Hasil penelitian menunjukkan terdapat korelasi antara kadar low-density lipoprotein (LDL) dan trigliserida serum dengan degenerative disc disease dengan nilai p=0.02 (p0.05). Semakin tinggi kadar LDL dan trigliserida maka derajat degerative disc disease cendereng semakin berat. Tidak terdapat korelasi antara HDL dengan degenerative disc disease. Secara statistik tidak terdapat korelasi antara kolesterol total dengan degenerative disc disease. Namun, didapatkan pada grafik nilai kolesterol total pada setiap derajat degenerative disc disease meningkat.

Abstract

Penelitian ini bertujuan menganalisis hubungan hipertensi, dislipidemia, diabetes mellitus (faktor risiko stroke) dengan luaran klinis pasien stroke iskemik dengan hipersomnia berdasarkan perubahan skor The National Institutes Health StrokeScale (NIHSS). Desain penelitian ini adalah analitik observasional, 72 sampel pasien stroke iskemik yang dirawat di rumah sakit Dr. Wahidin Sudirohusodo sejak Januari sampai September 2017.Hasil penelitian menunjukkan luaran klinis pasien stroke iskemik dengan hipersomnia disertai hipertensi lebih buruk dibandingkan tanpa hipertensi (p=0.012; Mann Whitney Test), sama halnya jika disertai dislipidemia (p= 0.003; Mann Whitney Test). Berbeda dengan kondisi diabetes melltius (p=0.792; Mann Whitney Test) tidak berhubungan dengan luaran klinis pasien stroke iskemik dengan hipersomnia.

Abstract

Balut bidai training aims to minimize the impact that can occur so that every lay person can help if they find victims who have an accident.The research aimed to analyze to what extent the effect of the splint wrap training using the watching video method on the fracture management knowledge of the Youth Red Cross members. This was the qualitative research with the one group pretest posttest design method with the samples of 70 students of YRC members. The samples were selected using the non-probability sampling technique with the purposive sampling approach. The date were analyzed using T-Test. The research result indicates that there is no significant difference concerning the knowledge level between the simulation group and video group (? = 0.063), whereas on the skill level, there is the significant difference between the simulation group and video group (? = 0.000). Thus, it can be concluded that the training using the video method can improve the fracture management knowledge fester than the training using the simulation method, so that it is expected that the training using the video method can be utilized for almost all topics, and every cognitive, affective, psychomotoric and interpersonal domain.

Abstract

The Lumbar spinal canal stenosis is one of the most common problems found representing the spine degenerative disease in the elderly population. Its prevalence is 5 out of 1000 people over the age of 50 years. The magnetic resonance imaging (MRI) examination is the gold standard in confirming the diagnosis of the lumbar spinal stenosis. The Oswetry Disability Index (ODI) is a questionnaire to assess the disability in the Low Back Pain (LBP) patients. The research aims at investigating the correlation between the degree of lumbar spinal canal stenosis based on Lee and Schizas's classification of ODI in LBP patients. This was the prospective research. The data were obtained through the PACS computer of Dr Wahidin Sudirohusodo Hospital Makassar and ODI questionnaire interview. The research results indicates that of 73 patients suffering from lumbar spinal canal stenosis, most of them are females comprising 41 people (56.2%). The most frequent age groups are 46-55 and 56-65 years old with 24 people (32.9%) in each group. The highest Body Mass Index (BMI) was Obesity I, 25 people (34.2%). The most commonly affected disc level is L4-L5 52 people (71.2%). The conclusion of the research is that there is the significant correlation between the degree of the lumbar spinal canal stenosis of Lee’s classification (p<0.001, r = 0.960) and Schizas’s classification (p<0.001, r = 0.948) with ODI. The higher the degree of lumbar spinal canal stenosis, the higher the patient's disability will be.
