Abstract

Introduction: Dental caries is one of the most common health problems among school-aged children and can have long-term impacts on their quality of life. Most oral health promotion interventions to date remain conventional and are often unappealing to children. On the other hand, advancements in digital technology offer significant opportunities for developing more interactive and engaging educational approaches. Review: This literature review aims to analyze the relevance and effectiveness of integrating the Health Belief Model (HBM) with gamification in the development of digital interventions for school-aged children in the field of oral health. The literature findings indicate that HBM-based interventions are effective in enhancing children’s perceptions of susceptibility, benefits, and self-efficacy in maintaining oral health. Gamification has been shown to improve engagement and motivation through game elements such as points, challenges, and rewards. Several studies conclude that, although each approach has its own strengths, the strategic integration of HBM and gamification can address their respective limitations—particularly in achieving long-term behavior change. Conclusion: However, most digital interventions have not yet explicitly incorporated all components of the HBM, nor have they adequately addressed local contexts such as culture, technological access, and the role of teachers and parents. Therefore, there is a need for the design of digital intervention applications based on HBM and gamification through a co-design approach. This concept holds potential as a direction for further research in developing sustainable and evidence-based oral health promotion solutions in Indonesia.

Abstract

Oral health promotion among school age children requires innovative and engaging strategies to address persistent challenges such as dental caries and poor hygiene practices. Co-design methodology emphasizes the active involvement of end user chil-dren, parents, teachers, and health professionals in developing interventions tailored to their needs and context. This study re-views the use of a co-design approach in developing a school-based digital health application to improve oral health behaviors in children. The review examined stakeholder involvement, including children, parents, teachers, and health professionals in sha-ping content, usability, and engagement features of oral health applications. Evidence indicates that co-design enhances the re-levance, usability, and acceptability of digital oral health interventions by integrating the lived experiences and preferences of tar-get users. Applications developed through co-design showed higher levels of engagement, improved oral health knowledge, and better adherence to preventive behaviors among school children. It is concluded that co-design enhances the relevance, usabi-bility, and acceptability of digital oral health interventions by integrating the lived experiences and preferences of target users.

Abstract

ABSTRACTIntroduction: The issue of oral health within the prison's environment remains a conspicuously neglected aspect of prisoner welfare. The underutilization of access to oral care constitutes a significant barrier. The necessity for effective oral health promotion programs tailored to this vulnerable population is further underscored by the high rates of oral disease and the lack of preventive measures. The objective of this study was to evaluate the extant literature on oral health programs among prisoners. Tujuan penelitian ini bertujuan untuk melakukan kajian pustaka mengenai program promosi kesehatan di lembaga pemasyarakatan. Methods: A comprehensive search was conducted in various databases, including PubMed, Scopus, Cochrane, ScienceDirect, ProQuest, EbscoHost, Nature, JSTOR, and Sage Journal, to identify oral health promotion programs in correctional institutions. The data were meticulously mapped and thematically analyzed to distill the key findings by the PRISMA-ScR guidelines. Results: the study is as follows: A review of the extant literature reveals that while many programs focus on improving oral health knowledge and behaviors, few report long-term outcomes or address systemic barriers such as inadequate access to dental care. Interventions that are tailored to the unique needs of correctional populations, such as culturally appropriate education and peer-led programs, have shown promising results. Nevertheless, the extant evidence regarding the effectiveness of these measures remains limited. Conclusion: The prisoner training program through peer coaching can be applied to improve prisoners' knowledge and literacy of dental and oral health. Prisoners who act as peer coaches for other prisoners facilitate the effective development of the program in accordance with the conditions of the correctional institution. It is hoped that improving inmates' oral health literacy will increase their motivation to access dental services and their commitment to maintaining oral health.KEY WORDS: oral health promotion, prisons, correctional populations, preventive dentistry, public health.

Abstract

Introduction: Untreated caries is one of the oral diseases that poses a significant burden in Indonesia. The perceived need for people to receive dental treatment is influenced by sociodemographic, economic, environmental, and healthcare service factors. The purpose of this study was to determine sociodemographic factors towards untreated caries and dental fillings in Indonesia, and then visualize them in geospatial mapping. Material and Methods: A cross-sectional study used secondary data from Basic Health Research 2018. A sample was 14.031 subjects, then classified based on WHO age group and sociodemographic factors. As gender, residence, educational level, employment status, and frequency of dental visits as independent variables. Then, the number of untreated caries and fillings is the dependent variable. The bivariate analysis was used to determine different factors. QGIS was used to visualize dependent variables in 26 provinces in Indonesia. Results and Discussions: There was a statistically significant difference (p<0.05) in the 35-44 years age group between females, rural residents, those with no schooling, and informal workers concerning untreated caries. Meanwhile, females, urban residents, college diploma holders, formal workers, and those with dental visits showed a tendency toward dental fillings. This result closely aligns with overlay mapping geospatial analysis. Conclusion: Sociodemographic characteristics had an impact on untreated caries and dental fillings, and should be considered to find out comprehensive strategies to improve oral health care.

Abstract

Background: Root caries (RC) remains a global burden and is often found in older adults. Risk factors for RC can vary in different countries. Purpose: This study aimed to determine the prevalence and risk factors of RC in Indonesian adults. Methods: This study analyzed data from Indonesia’s 2018 National Health Survey, which included 37,057 respondents aged ≥15 years. The survey collected data on sociodemography, oral health practices, general health behaviors, and oral conditions. An oral examination was conducted to measure the number of teeth with RC, decayed, missing, or filled teeth, as well as bleeding on probing (BOP), attachment loss, pocket depth, and number of eroded teeth. Logistic regression was generated to estimate odds ratios and confidence intervals. Results: The prevalence of RC was 59.4%, mainly occurring in participants aged >45 years. Those aged >60 years had 2.52 times (adjusted odds ratio [AOR] = 2.52; 95% confidence interval [95%CI], 2.26–279) the number of RC compared with those who were younger. The likelihood of RC was increased by having a lower education (1.47 times; AOR = 1.47; 95%CI, 1.28–1.70) and never visiting a dentist (1.64 times; AOR = 1.64; 95%CI, 1.22–2.19). Decayed, missing, or filled teeth were shown to increase the risk of RC, where more coronal caries indicated a higher risk. Those with BOP (+) and attachment loss of >3 mm had a greater risk of developing RC. Conclusion: The prevalence of RC was relatively high, occurring mainly in middle-aged and older adults. Root caries in the Indonesian adult population reflects a complex interplay of multiple contributing factors, including sociodemography, oral health practices, general health behaviors, and oral conditions.

Abstract

Introduction: The high burden of childhood dental caries highlights the need for innovative approaches. Artificial Intelligence (AI) offers a breakthrough in enabling more efficient, measurable efforts to prevent and detect dental caries. Therefore, this scoping review aims to map the existing evidence on the use of AI interventions in the prevention of childhood dental caries based on Leavell and Clark’s prevention model. Review: This scoping review analyzed 49 studies from various countries that examined the application of AI in childhood caries prevention, using the Leavell and Clark framework. At the health promotion level, behavior-based chatbot applications were shown to significantly improve caregivers’ knowledge and toothbrushing habits. In the specific protection setting, predictive models based on tabular data and the salivary microbiome identified high-risk children before lesions formed, with AUC values ranging from 0.77 to 0.94. For early detection, deep learning algorithms trained on radiographic images and intraoral photographs demonstrated high accuracy in detecting caries lesions, including incipient lesions, with a sensitivity of up to 94.7%. For disability prevention, AI models successfully predicted the success of caries arrest therapy with silver diamine fluoride. The main limitations identified were a small sample size, a lack of cross-population external validation, and gaps in model interpretability at the clinical level. Conclusion: The use of AI interventions for childhood dental caries is unevenly distributed across Leavell and Clark's five preventive levels, especially for specific protection.

Abstract

Introduction: Gingivitis, a reversible inflammatory condition, is the second most prevalent oral health issue in children, after dental caries. A national health survey in 2018 revealed a high prevalence of gingivitis among Indonesian children, with 5.8% among 5-year-old and 14.3% among 12-year-old children. Childhood is a critical period for the development of oral health behaviors, which are influenced by genetic, familial, and environmental factors. Research highlights the intergenerational transmission of gingivitis, linking parental oral health to child’s outcomes. This study aimed to analyze the relationship between the occurrence of gingivitis among parents and their 3--14-year-old children in Indonesia. Materials and Methods: Cross-sectional study using secondary data from the 2018 Riskesdas. The data were collected by the Republic of Indonesia Ministry of Health's Research and Development in March 2018. The subject criteria were 3--14-year-old children and their biological parents who had clinical data on gingivitis status and interviews for behavioral variables. Data analysis was performed using SPSS ver.27 for univariate and bivariate analyses. Results and Discussion: The relationship between the occurrence of gingivitis in parents and children was significant (p< .001). The parental risk factors included parents’ perceptions of oral health problems. Parents’ habits were also significantly related to their children’s habits. Conclusion: The significant relationship between the occurrence of gingivitis among parents and children, and the significant role of parents in shaping their children’s oral health behaviors were in accordance with the concept of the intergenerational life course.

Abstract

Introduction: Oral health literacy is an important determinant of oral health, particularly among older adults who are vulnerable to oral diseases, tooth loss, and functional decline. This study aims to evaluate the association among oral health literacy, oral health status, and oral health behaviors among independent older people. Materials and Methods: This cross-sectional study involved 195 independent older adults aged 60 years and above in Depok. The subjects completed a self-administered questionnaire collecting information on socio-demographics. Oral health literacy was assessed using the Health Literacy in Dentistry scale with 29 items (HeLD-29), and oral health behavior was assessed using an interviewer-administered questionnaire. Oral health status was recorded by clinical oral examination using the DMF-T index, CPI-modified, denture status, OHI-S, and masticatory performance was assessed subjectively. Results and Discussion: Several HeLD-29 domains were significantly correlated with clinical oral health indicators. Significant differences were found between low and high oral health literacy groups in the number of missing teeth, filled teeth, and deep periodontal pockets (p<0.05). Oral health literacy scores also differed significantly according to age group and educational level (p<0.05). No significant association was found between oral health literacy and oral health behavior. However, DMF-T score was significantly associated with dental visit pattern, and gingival bleeding was significantly associated with smoking status. Conclusion: Oral health literacy was associated with several indicators of oral health status and with sociodemographic factors, particularly age and educational level. An association was also found between oral health status and oral health behavior of independent older adults in Depok.
