Abstract

Vulva tumors are generally a rare disease encountered in gynecological clinical practice, especially at a young age. Vulvar tumors, more common in the labia major and rarely in the labia minor, clitoris, vestibulum, and posterior comicura. The most common vulvar benign tumors are fibroma, papilloma, lipoma, angioma, and others. A 39-year-old woman, at RSCM Urogynecology Jakarta clinic with chief complain mass or swelling in right labia minor. Size of mass increases in period of one year. Mass began to grow at 8 months of pregnancy. By 2014, patients have same mass in the right and left labia minor, and tumor excision has been performed. Conclusions of the specimens sent for pathological assessment are fibroepithelial papillomas. Gynecological and palpation examinations show 2 solid papilloma masses, in right labia minor, 8x4x2 cm and 3x2x2cm, painless, and firmly defined. In mons pubis area, skin looks coarse, thickened, hyperkeratotic. Speculum examination showed no abnormality. Patient diagnosed as a recurrent minor armored labia tumor. On December 5, 2017 an excision and reconstruction operation was conducted. Obtained pathological exam on December 20, 2017 with results stromal fibroepithelial polyps. Among all tumors in the vulva, tumors in labia minor appear less frequently. Biopsy should be performed for histologic examination to exclude malignant allegations. Furthermore, because of the location and size of the tumor, it can cause severe interference symptoms in the patient and therefore early diagnosis and treatment is essential.

Abstract

Background: The development of screening methods for infertility patients during the Coronavirus-2019 (COVID-19) pandemic provides new challenges in the adaptation of children's want programs in the era of the COVID-19pandemic. Several screening methods have been developed but various factors affect the awareness of couples to want to do COVID-19 screening. This study aimed to analyze biopsychosocial factors associated with awareness of COVID-19 screening in infertile couples. Subjects and Method: A cross-sectional study conducted at The Sekar Polyclinic of Dr. Moewardi Regional General Hospital during the COVID-19 pandemic. A number of 60 infertile couple of reproductive age who underwent the ART program was selected by random sampling. The dependent variable was awareness for COVID-19 screening. The independent variables were anxiety, knowledge, age, and length of infertility. The data were obtained from medical record and questionnaire and analyzed by a multiple logistic regression. Results: Anxiety (OR= 26; 95% CI; p<0.001) and above-average knowledge levels (OR=16; 95% CI; p<0.001) affected awareness for COVID-19 screening. While the age and length of infertility do not affect awareness for COVID-19 screening (p >0.05). Conclusion: Anxiety and above-average knowledge levels affect awareness for COVID-19 screening.

Abstract

Obesity poses a substantial health challenge globally, affecting various aspects of reproductive health, including adenomyosis. Adenomyosis, characterized by the presence of endometrial tissue within the myometrium, commonly affects women during their reproductive years, leading to symptoms such as dysmenorrhea and lower urinary tract symptoms (LUTS). This study aimed to investigate the influence of obesity on dysmenorrhea and LUTS severity in adenomyosis patients, considering demographic factors such as age and educational levels as potential moderators. Forty-four adenomyosis patients participated in a cross-sectional study at Dr. Moewardi General Hospital in Surakarta City of Indonesia, between April and September 2023. Diagnosis was confirmed using 2D Transvaginal Ultrasound (2D-TVUS). Obesity was defined using Body Mass Index (BMI) categories, and dysmenorrhea and LUTS were assessed using the Numerical Rating Scale (NRS) and modified International Prostate Symptom Score (IPSS) questionnaire, respectively. Statistical analyses included Pearson correlation. Results indicated a significant positive correlation between obesity and dysmenorrhea (rho = 0.256, P = 0.049) and LUTS (rho = 0.292, P = 0.029). Age and educational level did not significantly moderate these relationships, emphasizing the independent influence of obesity on symptom severity. In conclusion, obesity significantly impacts the severity of dysmenorrhea and LUTS in adenomyosis patients, highlighting the need for tailored management strategies that address both hormonal and inflammatory pathways.

Abstract

Informasi mengenai infertilitas serta tatalaksana dan sistem rujukan merupakan komponen penting dalam pengobatan infertilitas. Ketidakadekuatannya dapat mempengaruhi pasangan dalam menjalankan perawatan. Tujuan dari seminar ini adalah untuk meningkatkan wawasan masyarakat umum mengenai berbagai prosedur tatalaksana infertilitas seperti bedah laparoskopi, histeroskopi, prosedur inseminasi buatan, dan sistem rujukan infertilitas. Seminar dilakukan secara luring bekerja sama dengan organisasi IIDI sebagai jembatan informasi. Tingkat pengetahuan peserta dinilai berdasarkan nilai pre-test dan post-test. Peserta diberikan pre-test sebelum seminar dan post-test setelahnya. Fokus seminar mencakup bedah laparoskopi dan histeroskopi, prosedur inseminasi buatan intrauterin, hingga sistem rujukan infertilitas secara umum. Hasil pre-test dan post-test dianalisis menggunakan uji normalitas Kolmogorov-Smirnov dan uji Wilcoxon. Hasil dari Uji Wilcoxon menunjukkan peningkatan signifikan pada nilai post-test dibanding pre-test (p<0.001). Sebanyak 63 (56.7%) partisipan mengalami peningkatan nilai post-test, 47 (42.3%) partisipan memiliki nilai yang tetap, dan hanya 1 (1%) partisipan mengalami penurunan nilai. Kesimpulan: terdapat perbedaan signifikan antara nilai pre-test dan post-test, menunjukkan bahwa partisipan dapat memahami penjelasan yang diberikan oleh pembicara. Seminar ini terbukti sebagai sarana yang efektif untuk meningkatkan pengetahuan mengenai tatalaksana infertilitas.

Abstract

In the era of the coronavirus disease 2019 (COVID-19) pandemic, COVID-19 screening education is essential for the general public, especially for those who undergo assisted reproductive technology (ART) programs. Understanding the preventive measures and precautions of COVID-19 during the treatment process is necessary. Health care staff must also understand the importance of proper COVID-19 screening and control measures. This educational activity is in the form of a YouTube live streaming program that can be followed by the general public, including infertile couples who want to know about infertility, examination procedures, COVID-19 screening guidelines, and the TRB program. Education about COVID-19 screening can minimize stigma and enhance public confidence in the screening procedures. The results of this educational delivery make the general public, especially couples who will undergo the ART program, ready and have the ability to undergo COVID-19 testing, services, and screening. In addition, by providing education and implementing effective screening, we can work together to combat the COVID-19 pandemic and ensure that people receive quality and safe health services.

Abstract

Kolaborasi antara tenaga medis dan masyarakat memiliki peran penting dalam mendukung penanganan infertilitas, sebuah kondisi yang sering menyebabkan kecemasan, stres, dan dampak negatif lainnya pada kesehatan mental. Tenaga medis berperan dalam diagnosis, perencanaan pengobatan, dan mendukung pasien mencapai tujuan mereka untuk memiliki anak, sementara masyarakat dapat memberikan dukungan emosional yang membantu mengurangi isolasi dan depresi pasien. Kegiatan pengabdian ini berfokus pada edukasi dan pemberdayaan tenaga medis serta masyarakat untuk meningkatkan pemahaman dan kapasitas mereka dalam mendukung individu dengan infertilitas. Evaluasi menggunakan pre-test dan post-test menunjukkan bahwa 25% peserta mengalami peningkatan pengetahuan setelah kegiatan, sementara 72% telah memiliki pemahaman yang memadai sebelumnya. Hasil ini menggambarkan efektivitas program dalam meningkatkan pemahaman terkait infertilitas. Selain itu, kegiatan ini berhasil menyebarkan informasi akurat dan memerangi mitos yang beredar di komunitas, sehingga meningkatkan kesadaran dan mengurangi stigma terhadap individu dengan infertilitas. Kesimpulannya, program ini efektif dalam meningkatkan dukungan sosial dan kapasitas tenaga medis untuk memberikan perawatan holistik yang lebih baik bagi pasien infertilitas, serta berkontribusi pada pengurangan dampak psikologis dan sosial yang dialami pasien.

Abstract

Objective : This study aims to explore the relationship between micturition disorders and adenomyosis and dysmenorrhea and provide insight into the factors associated with the severity of LUTS symptoms in these conditions. Methods : This study was a descriptive qualitative study. We retrospectively collected medical record data from the Obstetrics Gynecology Outpatient Ward of Dr. Moewardi Surakarta General Hospital in the period January 2021 to May 2023. The results of this study analyzed using Pearson correlation analysis if normally distributed and using Mann-Whitney if not normally distributed, thus, statistically analyzed. Results: There is a significant relationship between adenomyosis and dysmenorrhea on urinary disorders (p value of 0.016) where urinary disorders are found to be more severe in adenomyosis patients with dysmenorrhea. Conclusion : Lower urinary tract symptoms are common in symptomatic adenomyosis patients and greatly affect the patient's quality of life. Dysmenorrhea can be a potential risk factor that increases the occurrence of moderate to severe LUTS. Keywords: adenomyosis; dysmenorrhea; urinary disturbance.

Abstract

HIGHLIGHTS Placenta Accreta Spectrum cases rise in proportion to the high frequency of uterine wall damage. A significant association was found between curettage history and Placenta Accreta Spectrum.   ABSTRACT Objective: Placenta Accreta Spectrum (PAS) involves abnormal placental adherence to the myometrium, causing severe obstetric hemorrhage and increased maternal morbidity and mortality (3,000–5,000 mL blood loss). Its global incidence has risen from 0.12% to 0.31%, linked to uterine trauma from cesarean sections or curettage. This study evaluates the association between curettage history and PAS. Materials and Methods: This study employed an analytical observational design with a case-control approach. Purposive sampling was utilized, resulting in the inclusion of 134 participants who met the predefined criteria. The study population consisted of women who delivered and were referred to Dr. Moewardi General Hospital, Surakarta, Indonesia, between May 2022 and May 2024. Data were analyzed using IBM SPSS version 25. The Chi-square test was applied to assess the association between variables at a significance level of p < 0.05, while logistic regression analysis was conducted to identify the most influential variables. Results: The study cohort comprised 67 patients diagnosed with PAS and 67 without PAS. A statistically significant association was observed between a history of curettage and PAS, as determined by the Chi-square test, with a p-value of 0.000. Logistic regression analysis further confirmed this association, yielding a p-value of 0.001 and an odds ratio (OR) of 5.769 (95% CI: 2.090–15.928) for a history of curettage. Conclusion: A history of curettage is significantly associated with the development of PAS. Patients with a prior curettage procedure are 5.769 times more likely to develop PAS compared to those without such a history.

Abstract

Menopause is a natural phase that every woman must experience, but it is often accompanied by various physical and emotional complaints that can interfere with daily life and reduce the quality of life. The lack of understanding and access to information makes most women unprepared to face this phase. The purpose of community service activities is to increase the knowledge and awareness of women aged 40 years or older about the concept of healthy menopause. The activity was carried out in a hybrid manner at the Auditorium of the Faculty of Medicine, Sebelas Maret University, involving obstetrics and gynecology specialists, health workers, and members of the IIDI Solo Branch. The implementation method includes counseling, interactive discussions, and evaluation through a pretest and a posttest. The results of the activity showed an increase in the average knowledge score of participants from 82.43 to 85.59, accompanied by an increase in awareness of the importance of early detection and handling of menopausal complaints. These findings confirm that sustainable education is an effective strategy to improve the quality of life of women in the lead-up to old age, in line with the goals of SDG 3, which emphasizes the importance of ensuring healthy lives and well-being for all ages.
