Abstract

Respiratory Tract Infections (RTIs) commonly occur as an infection in children. A considerable amount of antibiotics consumption is used to treat RTIs in many hospitals. Unwise antibiotic treatments in RTIs will increase bacterial resistance and treatment cost. This study aimed to investigate the quality or appropriateness of antibiotics use in RTI treatments in pediatric inpatients. A retrospective analysis of antibiotics consumption data was conducted through inpatient medical records for three months in 2017 at a primary hospital in Surabaya, Indonesia. The qualitative assessment was performed using a modified Gyssens algorithm based on the National Guideline and other relevant international standards of antibiotics applications in RTI treatment. The medical records of a total of 87 pediatric inpatients aged 0-14 y.o. were included in this study. The most common diagnosis was tonsillopharyngitis (32.18%), and the average length of hospital stay was 4.84 days. Antibiotics used in the treatments were from the class of -lactams and cephalosporins, all of which were applied in empirical therapy. According to the analysis, the antibiotic treatments were not appropriate. The most inappropriate use of antibiotics was in the form of wrong choice of drug choice (95.40%), followed by the sub-optimal duration of antibiotics (4.60%). The RTI treatment in pediatrics has to avoid any inappropriate uses of antibiotics, which can be achieved through the evaluation of institutional policy against the local guideline, antibiogram, or any relevant international guidelines.

Abstract

Salah satu faktor yang berkontribusi pada resistensi bakteri adalah penggunaan antibiotik yang tidak rasional seperti pada fenomena penggunaan antibiotik tanpa resep. Fenomena penggunaan antibiotik tanpa resep yang terjadi tidak lepas dari pola pikir masyarakat maupun petugas apotek. Kurangnya pengetahuan petugas apotek tentang antibiotik, resistensi bakteri, dan aspek legal dari dispensing antibiotik berkontribusi pada pemberian layanan antibiotik di komunitas di beberapa negara dengan pendapatan rendah. Penelitian ini bertujuan untuk menganalisis hubungan tingkat pendidikan dan pengetahuan petugas apotek tentang antibiotik terhadap pelayanan antibiotik oral yang mereka berikan di Kabupaten Sidoarjo. Studi observasional dengan pendekatan secara cross sectional dilakukan kepada petugas apotek dengan membagikan kuisioner secara daring. Sebanyak 233 petugas apotek dari 56 apotek di Kabupaten Sidoarjo telah dilibatkan dalam penelitian. Sebagian besar dari responden memiliki pengetahuan baik tentang antibiotik (48,93%) serta hanya melayani antibiotik oral hanya berdasarkan atas resep (68,00%) walaupun masih sekitar 30,00% yang melayani berdasar resep dan non resep. Terdapat hubungan yang signifikan antara tingkat pengetahuan petugas apotek tentang antibiotik terhadap bentuk pelayanan antibiotik oral yang diberikan (p-value 0,001). Peningkatan pengetahuan petugas apotek tentang antibiotik akan berkontribusi pada bagaimana mereka melayani anitibiotik berdasarkan aturan yang berlaku sehingga peningkatan pengetahuan tentang antibiotik ditengah petugas apotek yang bertugas di layanan sangat penting untuk dilakukan.

Abstract

Ketidakpatuhan dalam penggunaan antibiotik dapat meningkatkan resistensi bakteri. Faktor-faktor yang mempengaruhi kepatuhan pasien dalam menggunakan antibiotik adalah lupa, sibuk, dan kurangya pengetahuan. Beberapa penelitian pengembangan upaya peningkatan kepatuhan telah dilakukan seperti pemberian informasi, edukasi, dan konseling yang terbukti dapat meningkatkan kepatuhan penggunaan antibiotik. Namun, hal tersebut terkadang sulit dilakukan terlebih pada fasilitas kefarmasian dengan workload yang tinggi. Sebuah inovasi kalenderisasi kemasan obat telah diterapkan pada pasien hipertensi dan terbukti meningkatkan kepatuhan penggunaan obatnya. Penelitian ini bertujuan untuk menganalisis pengaruh kalenderisasi kemasan antibiotik pada kepatuhan pasien dalam menuntaskan terapi antibiotiknya. Studi dilakukan pada pasien usia 19-55 tahun yang menebus resep antibiotik oral tunggal jangka pendek (<7 hari), tidak mengkonsumsi lebih dari 5 obat per hari, dan bersedia bergabung dalam penelitian. Pasien dibagi kedalam dua kelompok yaitu kelompok kontrol dan kelompok dengan kalenderisasi pada kemasan antibiotiknya. Kepatuhan penggunaan antibiotik diukur dengan metode pill count pada hari seharusnya antibiotik habis. Jumlah pasien yang patuh akan dibandingkan dari kedua kelompok tersebut. Hasil menunjukkan sebanyak 23(92,00%) pasien patuh dalam menggunakan antibiotiknya dalam kelompok kalenderisasi sementara dalam kelompok kontrol hanya sebanyak 6 pasien (24,00%). Perbedaan jumlah tersebut menunjukkan hasil yang signifikan secara statistik sehingga dapat disimpulkan kalenderisasi kemasan obat dapat mempengaruhi kepatuhan penggunaan antibiotik jangka pendek. Namun, studi lebih lanjut dengan jumlah sampel lebih besar, randomisasi, dan perhitungan sisa obat secara langsung perlu dilakukan untuk mengkonfirmasi hasil penelitian ini.

Abstract

Vaccination program for Covid-19 in Indonesia has been conducted since 13th of January 2021. However, the vaccination process has been accompanied by an increase false information that can lead to the decrease of public acceptance. Therefore, public education strengthening is a key concerns to succed the vaccination program. This was a report of public education strengthening activity conducted in April 2021 aimed to increase public awareness about Covid-19 vaccination program. There were 34 highschool students joined in the event. The result showed after joining the events 14,7% of students who refuse to support vaccinated program claim thet they are ready for vaccination. In conclusion, continuity in public education strengthening is essential to increase public acceptance about Covid-19 vaccination.

Abstract

Pregnancy and breastfeeding are special conditions that need to be considered when taking drugs because not all drugs are safe for pregnant and breastfeeding mother. Mistakes in drug consumption can harm the fetus and baby. In the community, pregnant women and breastfeeding mothers often have difficulty in choosing symptomatic drugs available at pharmacies and drug stores due to the lack of knowledge about the safety of drugs for these conditions. Related to this, it is very important to increase public knowledge through the provision of information and tips so that people no longer have difficulty in choosing safe medicines for these conditions.

Abstract

Covid-19 pandemic has hit worldwide for three years. During the pandemic, a proper diet and optimum consumption of vitamins and suplements will help to maintain the immune system. However, there is still lack of public awareness in fulfillness the nutrition need during pandemic. Hence, it is essentials to increase public knowledge and awareness through the through the provision of information in order to make people easier in choosing the best diet, vitamins and suplement to combat the pandemic.

Abstract

Antibiotics resistance still remains the problem in treating infection disease both in developing and developed country. One of the factor contribute to the increase of antibiotics resistance is irrational antibiotics use without prescriptions. There is still lack of information about factors related to the antibiotics dispense by pharmacy staff. This study was aimed to observe pharmacy staff knowledge about the pharmacology and the regulation of antibiotics. A cross sectional online survey using questionnaire was conducted to pharmacy staff who work for service in Sidoarjo from February to April 2020. The knowledge of antibiotics was classified into lack, enough, and good knowledge. A total of 233 respondents from 56 pharmacy recruited in this study. Of them, 17,17% were pharmacist; 10,73% were pharmacy technician; 52,36% were pharmacy assistant and 19,74% non pharmacy worker. Overall the majority of respondents had a good knowledge about antibiotics (48,93%). However, most of them did not know that only pharmacist who had responsible to dispense antibiotics based on prescriptions (58,37%), fail to describe antibiotics indications (46,35%) and had poor knowledge about the minimum length of antibiotics course (58,37%). Educating and upskilling pharmacy personel about antibiotics will minimize the irrational use of antibiotics which lead to the increase of antibiotics resistance caused by the lack information received by patients.

Abstract

Inappropriate use of oral antibiotics will contribute to an increase in antibiotic resistance and treatment failure. Compliance with the use of oral antibiotics is one of the things that contributes to the inappropriate use of antibiotics. This study aims to analyze the effect of information booklet reminder media on adherence to short-term antibiotic use conducted in four puskesmas in the Surabaya area. Patients aged 19-50 years who were prescribed short-term antibiotics (<7 days) were included in this study. Patients were then grouped into two groups, namely the intervention group who was given an information booklet, and the control group. Compliance with the use of antibiotics was measured based on the calculation of the remaining antibiotics on the day the antibiotics ran out. A total of 192 people were recruited in this study, 96 people were in the intervention group and 96 people were in the control group. The results of measurement of adherence and analysis showed that 80.21% of patients in the intervention group were obedient in using their antibiotics, while patients in the control group were compliant with 60.42% (p-value 0.003). Based on these results, information booklets can significantly affect patient compliance in the use of short-term oral antibiotics so that information booklets can be used as one of the educational media in increasing adherence to antibiotic use.

Abstract

Tuberculosis (TB) is an infectious disease that still be the leading cause of mortality worldwide. In Indonesia, the success treatment rate still below the standard set by WHO. One of the factors contributed to the success treatment rate is patients medication adherence while the factors affected patients medication adherence are patients knowledge, social support and public stigma. Negative stigma on patients TB could be reduced by public education and campaign. This community service was aimed to educate public in order to eliminate negative stigma on TB patients. A total of 82 participants joined the event. 63 of them had filled the stigma questionnaire. The results showed that there was an increase in number of participants who had positive perpective on TB patients. A continuous campaign and public education will reduce the stigmatitation on TB patients.

Abstract

Irrational drug use usually happens in self medication practise without any supervision from health care providers. Self medication practise usually does not consider the potential of allergic reaction, pregnancy, breast feeding, diet, and medication history.The promotion of rational drug use through communication, information, and education still need to be conducted to the community. This event is purposed to improve perception, knowledge, and attitude of Desa Drenges residents in the term of drug usage. A total 23 person of Desa Drengeswere enrolled in this eventfacilitated by village administrators.The perception, knowledge, and attitude of the residents recruited were evaluate through questionnaire filled before and after the event. The result showed the increase of perception, knowledge, and attitude of the residents up to 2,69%. From the result, The community service event can improve the quality of health of residents in Desa Drenges through the improvement of perception, knowledge, and attitude in the term of rational drug use.
