Abstract

In 2023, the government hopes to increase the achievement of zero HIV cases by 2030. However, the achievement of the target with the 95-95-95 indicator is still far from expected. Based on 2022 data, only 81% of People Living with HIV (PLHIV) have their HIV status known. Meanwhile, only 42% of PLHIV are treated from the first 95 findings, the third 95 percent indicator, only 20 percent have received VL tests from the second 95 findings. The Mojokerto Regency Health Office has made efforts to increase the achievement of viral load testing in HIV sufferers, but the results obtained are still not optimal. This activity aims to identify the causes of the still low viral load testing in PLHIV in the Mojokerto Regency Health Office Work Area in 2023 and increase awareness of people living with HIV (PLHIV) to undergo viral load testing. The activity consists of the stages of problem identification, problem prioritization, and finding solutions to problems. At the end of the activity, it was found that viral load testing on PLHIV in the Mojokerto District Health Office's work area was still lacking and that assistance was needed to increase PLHIV awareness to carry out regular viral load testing so that the risk of transmission could be reduced.

Abstract

Pulmonary tuberculosis or pulmonary TB is a disease that is easilytransmitted through the air from the source of transmission, namely BTA positiveTB patients when coughing or sneezing, patients spread germs into the air in theform of sputum splashes. For this reason, an examination method that has highsensitivity and specificity is needed as a tool for early detection of Pulmonary TB.Research Objective: To study the comparison of the sensitivity and specificity ofthe Ziehl-neelsen method and the Molecular Rapid Test method in examining BTAin the sputum of TB suspected patients at the Puskesmas against the gold standardusing BTA culture. This study is an analytic observational study with a cross sectional design. The population in this study were all patients suspected of having Pulmonary TBwho came to the Kupang Health Center, Jetis Health Center, and Gedeg HealthCenter who performed microscopic and culture examinations in the laboratoryduring 2023. Sample selection used simple random sampling method with a samplesize of 60 patients. The hypothesis was proven using the Kappa test. Results: the sensitivity and specificity of the Ziehl-Neelsen method BTAexamination were 60% and 100%, while the sensitivity and specificity of theMolecular Rapid Test method examination were 100% and 100%. The results ofthe Cohen's Kappa test showed that between the Ziehl-Neelsen and MolecularRapid Test methods both had high sensitivity and specificity (Kappa; 0.733).Conclusions and advice: Based on the results of the study, the molecularrapid test method has a higher sensitivity than the Ziehl-Neelsen method. Thismethod can be recommended for early detection of pulmonary TB

Abstract

Kata kunci: Faktor ibu, faktor individu, faktor lingkungan, stunting ABSTRACT Background: Toddler stunting requires special attention because it inhibits the physical and mental development of children which will indirectly be related to the risk of illness and death. Objective: The purpose of the study: to analyze the factors that influence the incidence of stunting in toddlers in the working area of the Klenang Health Center, Probolinggo Regency.Method: This study used a quantitative case-control design. The independent variables were individual factors (LBW, history of exclusive breastfeeding, toddler infectious diseases, immunization status), maternal factors (maternal nutritional status, maternal age during pregnancy, parity, birth spacing), and environmental factors (source of clean water, access to toilets). The dependent variable in this study was the incidence of stunting. The population was all 359 mothers and toddlers in the Cakar Community Health Center work area, Banyuanyar District, Probolinggo Regency. Proportional random sampling was used, with a questionnaire as the instrument. Analysis used a logistic regression test. Results: The majority of toddlers with LBW experienced stunting, 44 (52.4%). 71 (84.5%) were exclusively breastfed. 83 (98.8%) of the stunted toddlers did not have infectious diseases. 73 (86.9%) of the stunted toddlers had complete immunizations. Low birth weight (LBW) (P-value 0.000 < 0.05), exclusive breastfeeding (P-value 0.017 < 0.05), and immunization status (P-value 0.046 < 0.05) significantly affected stunting. Infectious disease status (P-value 0.556 < 0.05) did not influence stunting. Conclusion: Low birth weight (LBW) is the most dominant factor influencing stunting due to inadequate nutritional intake during pregnancy and a history of low birth weight.Suggestion: Health workers should increase education and counseling for pregnant women, mothers of toddlers, and expectant parents about stunting, nutrition, parenting, and infectious disease prevention, through integrated health posts (Posyandu), health cadres, and digital media. Keywords: Environmental factors, individual factors, maternal factors, stunting
