Abstract

Extranodal non-Hodgkin lymphoma still seems to be a rare case and an issue to discuss a lot. Clinical evidence and guidelines on treatment have not yet been widely published and accessible. We present a case of a forty-five-year-old male with the chief complaint of a bulky and huge right mass in the testis with an ulcer on it. The patient underwent an orchiectomy, and the biopsy showed a malignant round cell tumor, suspected as non-Hodgkin lymphoma. The patient then received the following treatment: chemotherapy with R-CHOP regiment every three weeks, consisting of 6 cycles showing shrinkage size of the testis by day 7 and final complete response after 4th cycle with ADE grade 0 no sexual activity disorder after chemotherapy. This raises hope in developing treatment modalities that the right choice on chemotherapy regimen with complete control on the drug effects may improve clinical outcome and patient’s quality of life.

Abstract

ABSTRACT Extranodal Non-Hodgkin Lymphoma (NHL) remains an odd issue and something to discuss. Clinical evidence and guidelines on the treatment have not yet been broadly published and accessible. Here, we present a case of a thirty-eight years old woman complaining of mass on both lower lids of the eyes. The CT Scan examination showed enhancing solid masses on right-sided and left-sided palpebra inferior that press bulb oculi, suspected as bilateral malignant masses of bilateral palpebra inferior. Laboratory data revealed the surge of liver function marker. The patient was eventually diagnosed with Non-Hodgkin Lymphoma (NHL) of palpebra inferior B cell type high-grade stadium II E. Although there are still ongoing research and development of definitive treatments, R-CHOP a regiment given to this patient has shown complete favorable result with dosage adjustment due to the surge of basal liver function. Abstract

BACKGROUND: Carbohydrate antigens 15-3 (CA 15-3) is a conventional tumor marker in breast cancer, with low sensitivity and specificity. MicroRNA (miRNA)-21 showed its stability in circulation and could serve as powerful biomarker. The aim of this study was to evaluate miRNA-21 as breast cancer biomarker compared to CA 15-3 in Indonesian population.METHODS: Circulating plasma miRNA-21 expression was measured using qRT-PCR in 49 patients at various stages of breast cancer and 16 healthy controls. The relative expression value of miRNA-21 was calculated using 2-ΔΔCt. Meanwhile, CA 15-3 was quantified using electrochemiluminescence immunoassay (ECLIA) methods. The results of miRNA-21 and CA 15-3 plasma circulating expression were compared with controls at each stage and between stages of breast cancer.RESULTS: CA 15-3 median level in breast cancer group was 1.60 times higher compared to control group (p=0.019), 21.00 m/mL and 13.05 m/mL, respectively. Median miRNA-21 expression in breast cancer group was elevated 4.92 folds compared to control group (p=0.001), 4.43 and 0.90, respectively. There was no significant difference of CA 15-3 level between controls and all stages of breast cancer group. CA 15-3 cut-off value was 15.05 m/mL (p=0.016) with 59.2% sensitivity and 62.5% specificity. Meanwhile, there was a significant difference of miRNA-21 expression between controls and most stages of breast cancer group. Circulating miRNA-21 expression cut-off value was 2.07 (p=0.000) with 91.8% sensitivity and 87.5% specificity.CONCLUSION: Circulating miRNA-21 expression and CA 15-3 levels were significantly increased in breast cancer group compared to control group. The miRNA-21 expression increased consistently with breast cancer stage progression. miRNA-21 could serve as superior biomarker compared to CA 15-3.KEYWORDS: biomarker, breast cancer, circulating plasma, liquid biopsy, miRNA-21

Abstract

Breast cancer is still global burden especially for woman with 2.3 million cases every year dan 15% mortality among cancer diseases. In developing countries, most of the cases are diagnosed at terminal stage when metastasis already found. Bone metastasis is the highest among other metastasis sites such as: lung, liver and brain. Bone metastasis will cause hypercalcemia and bone pain as complications.  Both will gradually decrease patient’s quality of life. Comprehensive and holistic management for these complications will reduce deterioration and hopefully increase patient’s quality of life even they were at terminal stage. We describe a 40-year-old woman who got hypercalcemia crisis. Hypercalcemia usually manifest as a consequence of other diseases. Epidemiologically, majority come from metastasis, but can be other diseases, such as multiple myeloma. Interestingly, during medical investigation through her medical history, and physical examination, and laboratory examinations, we conclude that her hypercalcemia crisis was caused by bone metastasis from breast cancer.

Abstract

Bleeding is one of the most common complaints when coming to the hospital which can be mild to life-threatening. The balance of the impaired hemostasis system allows for abnormal bleeding such as hemorrhagic diathesis. Balance between blood clotting and bleeding is always maintained in the body under normal physiology. The coagulation system stops existing bleeding with vasoconstricts of blood vessels and the formation of early platelet plugs, this blockage is strengthened by the presence of a cascade of coagulations to form stable and sturdy blockages. Once bleeding has stopped, the fibrinolytic pathway is initiated to dissolve the blood clot to restore normal blood flow. balance the coagulants, fibrinolytic and inhibitor systems, creating a perfect physiological balance. Hemostatic imbalance is a global problem that can lead to thrombosis or hemorrhage.

Abstract

BACKGROUND: Despites its benefits, as one of COVID-19 principal treatments, anticoagulant raises a significant concern regarding the anticoagulant-related thrombocytopenia. However, up to date, there is lack of study examining anticoagulantinduced thrombocytopenia during COVID-19, hence this study was conducted to determine the factors inducing anticoagulant-induced thrombocytopenia in COVID-19 patients.METHODS: An observational cross-sectional study of 106 anticoagulant-treated COVID-19 subjects was conducted. Blood serum was drawn from subjects, then platelets, prothrombin time (PT), activated partial thromboplastin (aPPT), international ratio (INR), D-dimer, ferritin, fibrinogen, and high-sensitivity C-reactive protein (hs-CRP), were measured. For thrombocytopenia risk assessment, the 4T score was calculated. To assess the risk of thrombocytopenia. Statistical analysis using Chi-square and Mann-Whitney U test were performed and followed by multivariate analysis to examine the correlation among the thrombocytopenia risk factors.RESULTS: Significant differences were identified in the length of stay (LOS) (p=0.04), disease severity (p=0.021), sepsis (p=0.006), hs-CRP (p=0.003), and mortality rate (p=0.028) between thrombocytopenia and nonthrombocytopenia groups. A multivariate analysis through linear and logistic regression disclosed an increase in hs-CRP (OR=-0.29; p=0.045) and sepsis (OR=4.32; p=0.03) that precipitate the thrombocytopenia events.CONCLUSION: In severe and critically ill COVID-19 patients, the occurrence of thrombocytopenia was followed by an increase in inflammatory parameters such as D-dimer, fibrinogen, ferritin, hs-CRP and prolonged coagulation. The increase in hs-CRP and sepsis may raise the risk of thrombocytopenia, especially in severe and crtically ill cases of COVID-19.KEYWORDS: COVID-19, anticoagulant, thrombocytopenia, inflammation, infectious disease

Abstract

Hepatitis B is a disease caused by Hepatitis B virus (HBV) and the most common type of Hepatitis in Indonesia at 21.8%. High-risk behaviors such as unsafe sexual intercourse and indiscriminate use of syringes can cause Hepatitis B. This study aimed to analyze the relationship between knowledge of Hepatitis B and high-risk behaviors with the incidents of Hepatitis B among adolescents. The study was conducted by using a cross-sectional design and 50 adolescents as the respondents. The data collection was carried out by using a questionnaire. Correlation analysis was performed by Fisher’s exact test. 62% of adolescents had a low level of knowledge about Hepatitis B, 54% of adolescents never engaged in high-risk behavior, and 70% of adolescents claimed that they were never exposed to Hepatitis B. This study did not show any relationship between knowledge of Hepatitis B and the incidents of Hepatitis B (p 0.086 > α 0.05) and there was a relationship between high-risk behaviors in adolescents and the incidents of Hepatitis B (p 0.002 < α 0.05). Adolescents' knowledge of Hepatitis B and high-risk behaviors in adolescents are important factors that need to be controlled to decrease Hepatitis B rates. Socialization of Hepatitis B knowledge is needed to understand the virus, causes, transmission channel, how to stop the spread, and treatments should be taken when being infected by Hepatitis B virus.

Abstract

The global population living with Parkinson’s disease is estimated to reach 9.4 million people, which has increased significantly since 2016, with a total of 6 million people. Parkinson's is a neurodegenerative disease of the substantia nigra that causes a decrease in dopamine production and is characterized by the appearance of cytoplasmic misfold proteins called Lewy bodies. The study found that abnormalities or mutations in the SNCA and LRRK2 genes correlated with the overproduction of the ɑ-synuclein protein, which forms Lewy bodies that cause Parkinson's. Current Parkinson's medications only temporarily replace lost dopamine but do not treat the direct cause of Parkinson's; this research used qualitative literature study with content analysis, observation, and development; the use of CRISPR-Cas9 through AAV genetic engineering in repairing SNCA and LRRK2 mutant gene. This genetic therapy works by cutting the mutant DNA base sequences in the SNCA and LRRK2 genes and then replacing them with normal sequences through a homology-direct repair mechanism. As a result, the abnormalities or mutations that cause Parkinson's in these two genes can be corrected, so that dopaminergic levels in the brain can return to normal and excessive accumulation of α-synuclein protein can be suppressed.

Abstract

Alzheimer's attacks 24 million global population and dominates 60-80% of existing cases of dementia. It causes the accumulation of beta-amyloid (Aβ) plaques in the hippocampus and entorhinal cortex, resulting in decreased mass from the brain. Recent studies have shown that the manifestation of this disease is due to an overaccumulation of abnormal Aβ protein due to abnormalities in the APP and APOE4 genes. Point mutations in the APP gene will create the toxic form of Aβ protein, namely Aβ42, and the toxic APOE4 gene will accelerate the onset of Aβ42 deposition and pro-inflammatory activity that exacerbates the degenerative process of the brain. Gene editing as a potential definitive therapy was recently a concern by researchers. CRISPR-Cas9 repairs the APP gene and substitutes the APOE4 gene with APOE3 by modifying the gene's DNA sequence. Nano complex CRISPR-Cas9 tandem peptide lipid is a model for clinicians to target brain nerve cells. In vivo research on an Alzheimer's mouse model proved the potential of nano-complex-based peptides as carriers of CRISPR-Cas9 in brain nerve cells. This engineering technology offers satisfactory results with high precision, minimal side effects, and a relatively low price for long-term therapeutic effects and even a lifetime.

Abstract

Coronavirus Disease-19 (COVID-19) adalah penyakit yang disebabkan oleh Severe Acute Acute Respiratory Coronavirus-2 (SARS-CoV2) yang berasal dari China, menyebar dengan cepat ke seluruh bagian negara lain yang menyebabkan pandemi dunia. Dengan derajat gejala yang bervariasi yang disebabkan oleh COVID-19, virus ini menyebabkan kerusakan pada beberapa organ, baik karena efek infl amasi tidak langsung maupun efek sitopatik. Data terkait keterlibatan pankreas dalam kasus COVID-19 masih belum jelas. Seorang laki-laki usia 83 tahun dirawat karena gejala COVID-19 berat. Dalam perawatan, pasien memberikan gejala dan tanda pankreatitis akut tanpa diketahui faktor resiko yang terkait. Pada pemeriksaan didapatkan RT-PCR SARS-CoV2 positif dari swab nasofaring, amilase lipase yang meningkat serta gambaran ultrasound khas untuk pankreatitis akut. Tatalaksana pasien tetap berdasar pada kasus SARS-CoV2 dengan isolasi, oksigenasi, pemberian anti virus dan suportif. Pemberian antibiotik juga didasarkan pada terapi empiris yang kemudian disesuaikan hasil sensitifi tas kultur. Skor prognosis pankreatitis menunjukkan risiko kematian pada kasus moderate. Pada perjalanan, pasien meninggal karena shock sepsis. Prevalensi pankreatitis akut dan tingkat keparahannya perlu diamati. Dalam artikel ini, kami menyajikan kasus pankreatitis akut yang terjadi pada COVID-19 parah dengan faktor risiko yang tidak diketahui.Diagnosis penyebab kasus pankreatitis masih belum jelas tetapi beberapa bukti autopsi kasus infeksi SARS-CoV2 dengan pankreatitis menyebutkan bahwa infeksi virus ini dapat menyebabkan injuri pada pankreas.. Kondisi sepsis dapat diakibatkan infeksi virus SARS-CoV2 (viral sepsis) atau ko-infeksi bakteri. Oleh karena itu, rasionalisasi penggunaan antibiotik juga diperlukan. Kasus ini merupakan kasus yang membutuhkan managemen holisitik dan intensif karena kedua kondisi berpotensi dapat memperberat satu sama lain. Pengenalan awal kegawatan serta terapi tepat merupakan hal yang penting dapat menunjang kesintasan pasien.
