Abstract

COVID-19 cases in Indonesia in the period of June-July 2021 showed a catastrophic spike. During this period, a recently discovered variant, the delta variant, appeared to be one of the sources of COVID-19 infection. Treatment modalities are limited due to reduced stock of drugs. A case of a 63-year-old man has been reported, with a history of having been vaccinated with two doses of Sinovac, experiencing moderate-to-severe symptoms of COVID-19 infection then given convalescent plasma therapy since his initial admission to the hospital. Three days after being given convalescent plasma therapy, the improvement was noticeable. Shortness of breath, cough, fever, and weakness were less complained. On the seventh day the patient fully recovered and got discharged. Convalescent plasma therapy was e ective in early stage and was able to improve outcomes. Indonesia needs sucient stocks of convalescent plasma as a therapy to overcome the limitations of medicines.

Abstract

Introduction: The aging process can lead to knee pain in the elderly, which negatively impacts their quality of life by decreasing their physical functioning and potentially affecting their psychological well-being over time. This study aimed to determine the association between knee pain and the quality of life of the elderly. Methods: This analytical study involved elderly patients aged 60 years and over who presented with complaints of knee pain at the Geriatric Outpatient Clinic of Universitas Airlangga Hospital, Surabaya, Indonesia, from September 2022 to March 2023. The participants were selected by consecutive sampling until a total of 50 samples were collected using the interview method. The independent variable was knee pain, while the dependent variable was quality of life. The instruments used were the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) to measure knee pain and the Barthel Index for Activities of Daily Living (ADL) to assess the quality of life. The data were analyzed using Spearman's rho, with p<0.01 indicating a significant correlation. Results: The majority of the elderly were female (54%) and fell within the age group of 66–70 years. The most prevalent complaint was mild knee pain (52%). A total of 50% of the elderly had an independent quality of life. A strong correlation was found between the degree of knee pain and quality of life in the elderly (p=0.000). The correlation coefficient indicated an inverse relationship between the degree of knee pain and quality of life in the elderly. Conclusion: The findings suggest that the higher the pain degree suffered by the elderly, the lower their quality of life.   Highlights: 1. This study evaluated the factors that influence the quality of life of elderly individuals with knee pain, an issue that has been relatively overlooked in Indonesia. 2. The results of this study will hopefully assist scholars and stakeholders, enabling them to optimize the quality of life for elderly individuals experiencing knee discomfort.

Abstract

Highlights:1. This study was the first to analyze the therapeutic response in children with ITP at Dr. Soetomo General Academic Hospital, Surabaya.2. This study analyzed the response of platelet count and indices, including MPV, PDW, P-LCR, and PCT to therapy, which provides a more comprehensive perspective on therapy response in children with ITP.3. This study explored the effectiveness of various therapeutic approaches based on ITP categories, offering new insights into the most effective treatment options for children with ITP. AbstractImmune Thrombocytopenia Purpura (ITP) is an autoimmune disorder triggered by antiplatelet autoantibodies. Clinically, ITP is classified into three phases including Newly-Diagnosed ITP, Persistent ITP, and Chronic ITP, each with distinct durations and therapy implications. Patients with ITP who do not receive appropriate or optimal treatment are at a heightened risk of morbidity and mortality related to bleeding complications, the condition could worsen, potentially resulting in fatal consequences. In ITP patients, platelet counts decrease, accompanied by abnormal shifts in platelet indices, including Mean Platelet Volume (MPV), Platelet Distribution Width (PDW), Platelet Large Cell Ratio (P-LCR), and Plateletcrit (PCT). Thus, therapeutic response in ITP patients can be evaluated through increased platelet counts and normalization of platelet indices. This study aimed to assess the therapeutic response of platelet counts and platelet indices in pediatric ITP patients by comparing pre- and post- therapy levels overall by category, across all medication types administered, and according to the specific medication used within each category. The result of this study are expected to provide an overview of the most effective treatments across all ITP categories in order to prevent severe complications and reduce the risk of mortality. This retrospective study included ITP patients under 18 years old at Dr. Soetomo General Academic Hospital, Surabaya, conducted from September 2023 to March 2024. Platelet count and platelet indices pre- and post-therapy levels were analyzed using the Paired T-test for normally distributed data and the Wilcoxon test for non-normally distributed data, with significance set at p < 0.05. In summary, there were notable changes in the pre- and post-therapy levels of platelet, MPV, PDW, P-LCR, and PCT in each ITP category and for all therapies. Platelet count and PCT increased, while MPV, PDW, and P-LCR decreased. Patients treated with prednisone exhibited the best therapeutic response. Among the categories, Newly Diagnosed ITP demonstrated the most optimal therapeutic response. Overall, ITP therapy led to significant differences between pre- and post-therapy levels, marked by an increase in platelet counts and normalization of platelet indices.

Abstract

Thalassemia is a disease classified in the group of hereditary hemolytic anemia caused by the failure of the formation of one of the four amino acid chains forming Hb, causing incomplete Hb formation. Thalassemia carriers account for 7% of the world's population. At present, the cornerstone of treatment for β-TM in Indonesia remains supportive, including blood transfusions and iron chelation therapy. With a comprehensive discussion about thalassemia in Indonesia, it is hoped that the diagnosis and treatment of thalassemia will be more optimal and effective in future research.

Abstract

Pencapaian sasaran Sustainable Development Goals SDGs salah satunya tujuannya merupakan tentang air bersih serta sanitasi, pada prilaku hidup sehat salah satunya bisa diwujudkan dengan membuang kotoran manusia( feses)/ buang air besar dengan metode yang sehat ataupun pada tempatnya, semacam jamban. World Health Organization tahun 2018, memperkirakan sebanyak 2, 4 miliyar ataupun 1 dari 3 penduduk dunia tidak mempunyai jamban, serta tidak menyesuikan hidup bersih serta sehat. Permasalahan minimnya pemakaian jamban dipengaruhi oleh minimnya sikap kesehatan area warga. Riset ini bertujuan buat mendeskripsikan prilaku warga dalam memakai jamban bersih serta sehat di Desa Mambulu Barat, Kecamatan Tambelengan, Kabupaten Sampang. Riset ini memakai tata cara riset deskriptif. Populasi dalam riset ini merupakan segala kepala keluarga di Desa Mambulu barat yang bersedia. Terdapat 50 responden dalam riset ini. Pengumpulan informasi memakai kuesioner. Hasil riset menampilkan kalau prilaku warga Desa Mambulu Barat, Kecamatan Tambelengan, Kabupaten Sampang dalam memakai jamban bersih serta sehat dari 50 responden( 54%) ataupun 27 responden berperilaku negatif, sebaliknya( 46%) ataupun 23 responden berperilaku positif. Buat tingkatkan kepemilikan, pemanfaatan serta pemakaian jamban bersih serta sehat di Desa Mambulu Barat, Kecamatan Tambelengan, Kabupaten Sampang dibutuhkan motivasi serta pembinaan.

Abstract

Background: Cancer is one of the most burdensome diseases in Indonesia, ranking as the second-highest cost contributor in the National Health Insurance (Jaminan Kesehatan Nasional) scheme, with expenditures reaching 3.2 trillion rupiahs (approximately 221 million USD). Palliative care is an essential component of cancer management, aimed at alleviating physical, emotional, and psychosocial suffering. However, access to palliative care in Indonesia is extremely limited, with services available in only seven hospitals across three major islands, leaving most regions underserved. Objective: This policy brief aimed to analyze existing challenges and propose strategic policy recommendations for strengthening palliative care services to address healthcare system pressures in Indonesia, particularly related to cancer. Methods: A narrative policy analysis was conducted using secondary data from national healthcare reports, cost analyses, and peer-reviewed literature on palliative care effectiveness. The review assessed service coverage, accessibility, financial burden, and policy frameworks to identify key gaps and opportunities. Results: The analysis highlighted critical disparities in access to palliative care across Indonesia’s geographically dispersed regions. Despite evidence supporting the cost-saving and quality-of-life benefits of palliative services, implementation remains centralized and fragmented. Lack of national integration, workforce capacity, and public awareness were identified as major barriers. Conclusion: Developing an integrated, nationwide palliative care strategy is urgently needed to mitigate healthcare costs and improve quality of life for patients with cancer and other life-limiting illnesses. Policy efforts should focus on decentralizing services, expanding training for health professionals, and incorporating palliative care into primary healthcare systems.

Abstract

The high morbidity and mortality rates due to pneumonia in developing countries indicate that a more affordable marker is needed to determine the degree of inflammation in cases of pneumonia infection. The neutrophil-to-lymphocyte ratio (NLR) is a simple indicator used extensively in evaluating the systemic inflammatory response. An observational cross-sectional study of 297 pneumonia patients was conducted using medical records as the source document. The patients’ blood was taken during the hospital admission to obtain neutrophil and lymphocyte values. Meanwhile, the PSI score was used during the hospital admission to assess the severity of the pneumonia. Bivariate analysis using Spearman correlation was used to determine the correlation between NLR and pneumonia severity. The ROC curve was used to determine the cut-off value of NLR to distinguish mild from severe patients. The results of the Spearman correlation test analysis showed that the correlation between the NLR and the pneumonia severity was significantly positive (r = 0.130; p = 0.025). Furthermore, the median NLR value was higher in patients with severe pneumonia than in those with milder pneumonia severity (13.01 vs. 11.79 vs. 6.75). The NLR cut-off value of 9.07 was obtained to differentiate patients with mild severity and those with more severe disease. In conclusion, there is a correlation between the neutrophil-to-lymphocyte ratio and the severity of patients’ pneumonia.

Abstract

Patients suffering from beta thalassemia major experienced iron overload due to hemolysis, increased iron absorption in the gastrointestinal tract, and primarily due to repeated blood transfusions. This iron accumulation was exacerbated by the lack of a physiological mechanism for the body to naturally eliminate excess iron. One of the main complications of this condition was iron toxicity, which could damage various organs, including causing renal dysfunction. The persistence of this condition was associated with an increased risk of kidney injury, which could potentially lead to acute kidney injury and renal complications. The objective of the present study was to ascertain the correlation between serum ferritin levels and eGFR in adult patients diagnosed with beta thalassemia major in order to achieve the third focus of Sustainable Development Goals (SGD’s) in good health and well-being. The present study was a quantitative research study that utilized an observational analytic design with a cross-sectional approach. It was based on secondary data from the medical records of adult patients with beta thalassemia major at the Department of Internal Medicine, Universitas Airlangga Hospital. Subjects were selected using consecutive sampling. The statistical analysis of the correlation between serum ferritin levels and eGFR was conducted using Spearman tests. A total of 62 adult patients with beta thalassemia major were include this study, with mean age of 26 years. The majority of participants (42 patients or 67,7%) had serum ferritin levels below 1000 ?g/L. All of Subjects (62 patients) had eGFR within the normal range, all were classified as G1 (> 90 mL/min/1.73 m²). The Spearman analysis results showed a weak positive correlation between serum ferritin levels and eGFR with a correlation coefficient (r = 0.277) and a p-value of 0.029. Conclusion: There is a weak positive correlation between serum ferritin levels and eGFR in adult patients with beta thalassemia major.

Abstract

Alzheimer's attacks 24 million global population and dominates 60-80% of existing cases of dementia. It causes the accumulation of beta-amyloid (Aβ) plaques in the hippocampus and entorhinal cortex, resulting in decreased mass from the brain. Recent studies have shown that the manifestation of this disease is due to an overaccumulation of abnormal Aβ protein due to abnormalities in the APP and APOE4 genes. Point mutations in the APP gene will create the toxic form of Aβ protein, namely Aβ42, and the toxic APOE4 gene will accelerate the onset of Aβ42 deposition and pro-inflammatory activity that exacerbates the degenerative process of the brain. Gene editing as a potential definitive therapy was recently a concern by researchers. CRISPR-Cas9 repairs the APP gene and substitutes the APOE4 gene with APOE3 by modifying the gene's DNA sequence. Nano complex CRISPR-Cas9 tandem peptide lipid is a model for clinicians to target brain nerve cells. In vivo research on an Alzheimer's mouse model proved the potential of nano-complex-based peptides as carriers of CRISPR-Cas9 in brain nerve cells. This engineering technology offers satisfactory results with high precision, minimal side effects, and a relatively low price for long-term therapeutic effects and even a lifetime.
