Abstract

The total number of TB cases in Indonesia is 969,000. This figure represents a 17% increase compared to 2020. SITB 2022 data shows there are 15,375 cases of co-infection of pulmonary TB with HIV. The treatment success rate for pulmonary TB co-infection with HIV in Indonesia in 2022 is 71%. This is still far from the national target of 90%.This case report discusses a 33-year-old man with pulmonary TB and TB lymphadenitis with HIV at clinical stage 4. The CD4 count was 10 cells/mm3. The degree of immunodeficiency suffered by this patient is severe immunodeficiency. The management was to administer OAT first, followed by ART within 2 weeks after OAT was administered and well tolerated. OAT was administered for 9 months due to the presence of TB lymphadenitis. Treatment evaluation showed clinical improvement and the patient did not develop IRIS. The incidence of IRIS in patients with TB is 8%-43%. Factors influencing the occurrence of IRIS include the CD4 count at the start of ART.

Abstract

Background: Smoking is the biggest health problem faced. This activity has at least had a negative impact  such as a decrease in sleep quality due the release of cathecholamines resulting in tachycardia and increase in blood preasure which increases alertness.Objective: This study aims to determine the characteristic, smoking status, and the relationship of smoking with the sleep quality of adolescent at the senior high school level. Methods: This type of research is a descriptive analytic study with a cross-sectional study design with convenience sampling technique. The population of this study was school-age students at the high school level in the districts of Bonjol and Lubuk Sikaping with a total sample of 96 people. Data obtained in the form of primary data from Pittsburgh Sleep Quality Index (PSQI) questionnaire Results: The highest group of school-aged adolescents were boys aged 17 and 18 years, with the most categories being smokers and poor sleep quality. Regarding the relationship between smoking status, there was a statistically significant relationship with sleep quality. This study also found that adolescents with smoking status were 5,402 times more likely than smokers (ex-smokers and non-smokers) to decrease sleep quality (p=0.001;OR=5.402) Conclusion: There is a significant relationship between smoking status and sleep quality.

Abstract

Hospital malnutrition (HM) is malnutrition happened during hospitalization and has an impact on patient complications and mortality. This study aims to determine the factors associated with the risk of HM in pediatric patients at RSUP Dr. M. Djamil Padang in 2020. This study was an analytical research with the cross-sectional approach. The research was conducted at RSUP Dr. M. Djamil Padang Medical Records Section between February 2020 and December 2022. The data used is secondary data from the medical records. Bivariate analysis was conducted using the Chi- square test and Fisher's exact test. Logistic regression test was used for multivariate analysis. Statistical significance is determined if the p value <0.05. This research result showed that most of the patients were found in the age group <5 years and the main disease are lung disease. The risk factors for HM are STRONGkids scores, nutritional status, and length of stay. The prevalence of HM is 21.2%. The results of bivariate analysis showed multiple diagnoses (p=0.328), STRONGkids score (p=0.001), nutritional status (p=0.012), type of nutritional therapy (p=0.123), length of stay (p=0.001), and history of critical illness (p=0.607). The results of the multivariate analysis test showed a STRONGkids score (OR=0.143) and length of stay (OR=0.222). According to the study’s findings, STRONGkids score and length of stay are the dominant factors associated with HM in children.

Abstract

COVID-19 telah menginfeksi banyak orang di seluruh dunia selama beberapa tahun terakhir. Virus ini terutama menargetkan sistem pernapasan dan membuat para penyintasnya berisiko mengalami efek jangka panjang setelah sembuh. Penelitian ini bertujuan untuk menentukan hubungan antara tingkat keparahan klinis COVID-19 dengan fungsi paru-paru pada penyintasnya. Spirometer berbasis PC digunakan untuk memeriksa fungsi paru pada penyintas COVID-19 ringan hingga sedang di Fakultas Kedokteran Universitas Andalas. Mayoritas penyintas COVID-19 di Fakultas Kedokteran Universitas Andalas memiliki tingkat keparahan klinis ringan (86,5%). Parameter spirometri yang mengalami penurunan paling banyak adalah VEP1 (16,2%), dan mayoritas penyintas COVID-19 memiliki fungsi paru yang normal (89,2%). Fungsi paru pada penyintas COVID-19 memiliki hubungan yang signifikan dengan keparahan klinis, yang dibuktikan dengan nilai p sebesar 0,005 (p < 0,05) pada uji Fisher exact. Terdapat hubungan antara tingkat keparahan klinis riwayat COVID-19 dengan fungsi paru pada penyintas, sehingga perlu dilakukan tindak lanjut pada pasien COVID-19 setelah dinyatakan sembuh dan diperlukan penatalaksanaan lebih lanjut pada pasien yang mengalami penurunan fungsi paru

Abstract

Tujuan : Penelitian bertujuan Untuk mengetahui hubungan komorbid dengan outcome pasien terkonfirmasi COVID-19 di RSUP. M. Djamil Padang tahun 2020. Metode : Penelitian bersifat analitik observasional dengan pendekatan cross sectional. Data dikumpulkan dan diolah dengan menggunakan teknik total sampling , yaitu semua pasien terkonfirmasi COVID-19 yang dirawat di RSUP M.Djamil Padang berdasarkan kriteria inklusi dan ekslusi . Jumlah sampel penelitian sebanyak 468 pasien terkonfimasi COVID-19 yang dirawat pada periode Maret – Desember 2020. Analisis yang digunakan adalah analisis bivariat berupa Chi-Square . Hasil: Hasil penelitian menunjukkan terdapat hubungan yang signifikan antara komorbid dengan outcome pada pasien terkonfirmasi COVID-19 (p=0,010) di RSUP. M.Djamil Padang. Kesimpulan : Berdasarkan hasil penelitian dapat diartikan bahwa terdapat hubungan antara komorbid dengan outcome pada pasien terkonfirmasi COVID-19.
