Abstract

The increase in new leprosy cases in the year 2021 spread across 19 regencies, with the highest distribution in Padang Pariaman, and it is still a problem in West Sumatera Province. Based on this condition, "the pockets of leprosy" has been created, which play a role in the transmission of leprosy, so the elimination effort experiences difficult. Public Health Center of Pauh Kamba in Padang Pariaman Regency has 12 (19%) new multibacillary cases. In commemoration of World Leprosy Day in the year 2022, Indonesian Society of Dermatology And Venereology (INSDV/Perdoski), Indonesian Physical Medicine and Rehabilitation Association (Perdosri), and Padang Pariaman District Health Office have been struggling to break the chain of transmission of leprosy by increasing understanding of leprosy, self-care of leprosy, prevention of disability, and revitalize the activities of self-care groups by local health workers.

Abstract

Sifilis laten lanjut tidak menunjukkan gejala dan hanya menunjukkan hasil pemeriksaan serologis yang reaktif. Sifilis pada kehamilan dapat menyebabkan abortus berulang, morbiditas dan mortalitas neonatus. Satu kasus, pasien perempuan yang telah menikah usia 34 tahun dengan kehamilan ke 3 usia kehamilan 22-23 minggu, tidak ada mengeluhkan bercak kemerahan pada telapak tangan, kaki maupun bagian tubuh lainnya. Riwayat didiagnosis dan mendapatkan terapi sifilis juga tidak ada. Status dermatologikus dan venereologikus dalam batas normal. Hasil pemeriksaan laboratorium VDRL/RPR reaktif 1:2. TPHA reaktif 1:160. Perempuan hamil dengan tes serologik sifilis positif tanpa ada riwayat pengobatan sebelumnya harus dianggap terinfeksi sifilis laten lanjut dan harus mendapatkan terapi. Benzatin penisilin merupakan terapi utama dalam pengobatan sifilis pada wanita hamil. Reaksi alergi dan reaksi Jarisch-Herxheimer tidak menghalangi pemberian obat tersebut. Evaluasi kembali secara klinis serta serologis pada 1, 3, 6, 9 dan 12 bulan setelah pengobatan sangat diperlukan untuk menentukan keberhasilan pengobatan. Jika titer nontreponemal meningkat atau tidak menurun empat kali dalam 6-12 bulan setelah pengobatan, maka pengobatan dianggap gagal.

Abstract

Background: Longitudinal melanonychia (LM) describes a longitudinal brown-to-black band of discoloration affecting a finger, thumb, or toenail. Discoloration of the nails can be a sign of benign or malignant nail disorders. Although nail melanoma rarely occurs in children, health practitioners should be aware of melanonychia. Dermoscopy and histopathology are helpful in the diagnosis. Case Illustration: A case of LM in a thirteen-year-old girl was reported with a complaint of brownish-black discoloration, sometimes painful, on her right thumb since 1 year ago. She had no history of trauma, nail damage, or bleeding tendency. Dermatological examination found a homogeneous brownish-black band on the right thumbnail. Dermoscopy revealed regular, homogeneous black linear pigmentation and pigment through the translucent cuticle and proximal nail fold (pseudo-Hutchinson’s sign). Punch biopsy was done on proximal nail fold reaching nail matrix. Histopathology revealed increased melanocytes, some nests of melanocytes in rete Malpighi, and no atypical cells. She was diagnosed with nail matrix nevus. Five months after the punch biopsy, the nail grew well without onychodystrophy. Discussion: Nail matrix nevus is a benign LM. Diagnosis is based on anamnesis, physical examination, dermoscopy, and histopathology. Dermoscopy of the nails is a useful, quick, non-invasive, and highly effective tool that may help differentiate benign or malign melanonychia. Conclusion: Histopathology is important to determine whether the pigmented lesion is benign or malignant.

Abstract

Background: Herpes simplex virus type 2 (HSV-2) infection can be caused by genital herpes with a high recurrence rate. Recurrent genital herpes affected patients’ quality of life. Precise management is needed to prevent the progression of the disease. Case presentation: This study reported a 34 years old male with recurrent genital herpes. The patient developed painful vesicles in the genital area, which turned into an ulcer. History of genital herpes and gonorrhoea urethritis were reported and had been treated. The patient had a history of having unprotected sexual intercourse with multiple partners and reported having psychological stress within 1 month. DLQI score was 12, and the DASS score was 5-85, which confirmed that the patient had anxiety. Vesicles and erosion were found in the genital area. Tzanck test showed multinucleated giant cells with borderline anti-HSV-2 IgG serologies in 2 occasional evaluations. Acyclovir 3x400 mg for 5 days was used as therapy, and no lesion was found at follow-up. Conclusion: Psychological stress was found to be a risk factor for recurrent genital herpes

Abstract

Peningkatan kasus kusta baru pada tahun 2021 yang tersebar di 19 Kabupaten/Kota dengandistribusi terbanyak pada Kabupaten Padang Pariaman masih menjadi permasalahan di ProvinsiSumatera Barat. Hal ini menyebabkan masih terdapatnya kantong-kantong kusta yang berperandalam penularan kusta sehingga upaya eliminasi kasus kusta di tingkat Kabupaten/Kota sulit untukterwujud. Puskesmas Ulakan merupakan salah satu Puskesmas di Kabupaten Padang Pariamandengan 12 kasus baru (19%) multibasiler. Dalam upaya melakukan pemutusan rantai penularan danmeningkatkan pemahaman kepada masyarakat tentang penyakit kusta, maka DepartemenDermatologi dan Venereologi melakukan penyuluhan tentang pentingnya perawatan diri pasienkusta yang dapat dilakukan secara mandiri. Dari 14 pasien kusta didapatkan 70% pengetahuanpasien masih rendah dan setelah kegiatan, pengetahuan rendah turun menjadi 48% dan 52% pasiensudah memiliki pengetahuan yang tinggi. Tingkat perawatan diri pasien 72% masih buruk, stigmadidapatkan tinggi pada usia <45 tahun dengan latar belakang pendidikan tinggi. Dilakukan jugaadvokasi kepada pemegang program dan Kepala Dinas Kesehatan Kab. Padang Pariaman untukmengaktifkan kembali Kelompok Perawatan Diri (KPD).Kata kunci: perawatan diri, kusta, kelompok perawatan diri

Abstract

Background: Circumcision can reduce the risk of sexually transmitted diseases (STDs). It may reduce males acquiring HSV-2 by 30% and 68% lower prevalence of balanitis than uncircumcised males. There is no report of incomplete circumcision as a risk for STD infection. Case presentation: A 31-year-old male complained of multiple ulcers that covered a yellowish crust on the penile with a burning sensation since 7 days ago. Initially, it was vesicles that turned into ulcers. One month ago, he complained of moist scales on his preputium that felt itchy and smelly. The patient is married and sexually active. History of intercourse with female sex workers without condoms 2 weeks ago. The patient had incomplete circumcision when he was a child; the preputium is still persistent and seldom cleaned regularly. One year ago, there was a history of vesicles on the penis. Physical examination revealed obesity grade 2. Venereological findings showed a whitish pseudo-membrane on the preputium, glans penis, and ulcers in various sizes covered pseudo-membrane surrounded erythema oedema. Laboratory revealed positive IgG HSV1, IgM, and IgG HSV2. A fungal culture is positive candida. Diagnosis established as recurrent genital herpes, balanoposthitis candidiasis, non-specific genital infection, candidiasis intertrigo. The patient has persistent preputium as the entry of commensal pathogens through abrasions in the mucosa, which causes infection. The patient had complete resolution after being administered oral acyclovir, doxycycline, and topical miconazole. Conclusion: Persistent preputium in incomplete circumcision is a risk for developing candidiasis, balanoposthitis, and recurrent genital herpes.

Abstract

Background: According to WHO data, the number of new cases of leprosy has decreased in 2019. However, Indonesia continues to provide a significant number of cases. According to statistics, India, Brazil, and Indonesia account for 79 percent of all instances. Purpose: This study aims to describe the profile of leprosy patients, and involves all Dermatology and Venereology Academic Hospitals in Indonesia. Methods: This study was a retrospective study of 2461 patients from Dermatology and Venereology Outpatient Clinic at 13 Academic Hospitals in Indonesia between January 2018 and December 2020. Result: Subjects in this study were dominated by males (66.8%) and aged > 14 years (95.3%). The most common type of leprosy was multibacillary (MB) (86.2%), and erythema nodosum leprosum (ENL) was the most leprosy reaction (20.3%). Majority of the subjects experienced disability in the hands (26.6%), in grade 1. Conclusion: Leprosy cases in Indonesia are mostly experienced by adult males. The most common type of leprosy is MB, with ENL being the most common leprosy reaction. Grade 1 disability is the most prevalent, therefore proper education is necessary to keep patients from progressing to grade 2 disability.

Abstract

Leprosy is still a health problem in several countries, including Indonesia. Disability due to leprosy is a health threat for sufferers and causes quality-of-life problems that can disrupt the productivity of leprosy patients. Ocular involvement in leprosy is joint, but little data has been reported. Apart from multi-drug treatment (MDT), leprosy sufferers must also be able to carry out continuous self-care to prevent further disability. This is a collaborative activity between the Department of Ophthalmology and Dermatology Venereology of the Faculty of Medicine, the University of Andalas in Puskesmas Pauh Kamba district of Padang Pariaman. This is an advocacy activity on self-care (3M: Seeing, Examining, and Protecting) and eye examination. The activity was followed by interactive discussions and continued with a direct examination for skin abnormalities and disabilities, eye examination, and fundoscope. The activity results were obtained from 24 respondents, 11 men and 13 women. The age range is 16-59 years (67%), with the highest level of education being Elementry School graduates, which is 11 people (46%). In the self-care questionnaire of patients, 40% behaved well, and the results of eye examinations almost suffered from dry eye complaints without disabilities.
