Abstract

Breast cancer cases in Indonesia have risen in alarming rate. Herbal medicines have been developed to complement conventional cancer therapy and to lessen the negative effects of the therapies. Phycocyanin, blue-green photosynthetic pigment from Spirulina platensis, becomes a potential candidate of herbal medicine for cancer therapy due to its capability to eliminate various cancer cell line on in vitro level. This research aimed to analyse the effectivity of phycocyanin to induce cytotoxic and apoptotic activity of T47D cells (breast cancer cell line). MTT assay was perform to measure IC50 value. Double-staining assay and flow cytometry were applied to visualize the appearance of apoptotic cells and to measure the percentage of apoptotic and necrotic cells. Result showed that the IC50 value of phycocyanin towards T47D cells is 1650 ?g/mL. Phycocyanin was also proven to induce apoptosis at IC50 with the apoptosis percentage of 45.93%. This research presented different results from previous researches that used phycocyanin from saltwater Spirulina platensis. Based on the results of this study, phycocyanin has the potential to be developed as anticancer agent by inducing apoptosis.

Abstract

Phycocyanin from Spirulina platensis extract has anticancer activity against various types of cancer cell cultures. However study about its effect on colon cancer cell lines, especially the WiDr, has not been reported before. This study aimed to reveal the anticancer activity of phycocyanin from Spirulina platensis extract on WiDr cells. The research was an in vitro experimental study, with the investigation on cytotoxicity also antiproliferation as the anticancer parameters. Both cytotoxicity and antiproliferation test was conducted through MTT assay to observe the visualization and inhibition of proliferation of different concentrations of phycocyanin in several incubation times on the WiDr colon cancer cell line. The obtained data were then processed statistically with the Two Way ANOVA test at a significance value of p <0.05 and followed with the Post Hoc test since there were significant differences. Based on the results, it could be postulated that phycocyanin extracted from freshwater Spirulina platensis was classified as non-toxic (IC50 of 855 µg/ml). Consequently, it is less potential to be used as the treatment for colon cancer. However, phycocyanin could inhibit the proliferation of the WiDr cell for approximately 47.4%, specifically at the concentration of 1710 µg/ml for 72 hours. It could be concluded that freshwater phycocyanin is less effective as an anticancer substance. The benefit of this study is to provide the new scientific evidence of the contrary results of freshwater phycocyanin activity from Spirulina platensis as an anticancer agent of colon cancer.

Abstract

Spirulina platensis is a spiral green-blue algae that is abundant in the subtropical and tropical water bodies. S. platensis has several potentials to be used in the medical fields due to its wound healing and antitumor potentials. This research aimed to evaluate the secondary metabolite compounds contained in the ethanolic extract of S. platensis from Karimun Jawa Sea, Indonesia. The dried S. platensis was macerated and extracted using ethanol, then was filtered using Whatman filter paper. A series of tests has been done to determine the secondary metabolites in the ozonated oil using several chemistry tests and reagents in the ethanol extract of S. platensis. The tests found that S. platensis microalgae harvested from Karimun Jawa sea contains alkaloid, saponin, flavonoid, and quinone. As S. platensis from different region contains different substances, this research shows the substances contained in S. platensis from Karimun Jawa sea. With this research, we knew that S. platensis from Karimun Jawa islands contains alkaloid, saponin, flavonoid, and quinone which can be used as a medicinal ingredient.

Abstract

Latar belakang: Psoriasis pustulosa generalisata (PPG) merupakan salah satu jenis psoriasis yang jarang terjadi. Angka kejadian PPG 0,6-0,7 kasus per satu juta jiwa. Gejala khas PPG yaitu terdapat pus steril dengan ukuran 2-3 mm yang tersebar generalisata diatas kulit yang eritem. Terapi yang biasa digunakan diantaranya siklosporin, acitretin, atau metrotekstat. Tujuan: Tujuan dari kasus ini untuk membahas tentang faktor pencetus PPG, pilihan obat, komplikasi, dan efek samping dari terapi PPG jangka panjang.Kasus: Seorang wanita usia 37 tahun dengan riwayat psoriasis datang dengan keluhan terdapat pustula generalisata, makula eritem dan skuama. Faktor predisposisi dan pencetus pada pasien ini adalah obesitas dan stres emosional. Pasien pernah mendapatkan terapi siklosporin selama 3 tahun kemudian diganti asitretin satu tahun terakhir. Hasil histopatologi didapatkan gambaran abses munro dan parakeratosis sesuai dengan PPG. Didapatkan leukositosis dan kelainan tes fungsi hati pada pemeriksaan laboratorium, sehingga terapi kembali diganti dengan siklosporin dosis rendah selama 10 hari dan memberikan hasil yang memuaskan.Pembahasan: PPG merupakan penyakit autoimun. Stres emosional merupakan faktor yang paling berpengaruh; pada kasus ini pasien telah lama menderita PPG dan terapi sistemik jangka panjang, emosi pasien menjadi tidak terkontrol sehingga sering terjadi kekambuhan. Acitretin dapat menyebabkan kerusakan hepar; siklosporin dapat menjadi pilihan yang lebih baik. Simpulan: PPG masih menjadi masalah besar. Stres emosional merupakan faktor yang mempengaruhi kejadian PPG. Terapi jangka panjang dan efek samping menyebabkan ketidakpatuhan pasien untuk konsumsi obat. Penting bagi pasien untuk melanjutkan terapi obat sistemik dan mengontrol emosiKata kunci: Psoriasis pustulosa generalisata, Acitretin, Siklosporin, Kerusakan Hepar.Key Words: Psoriasis, Hypocalcemia, Hypoparathyroidism, Psoriasis of von ZumbuschPsoriasis is an autoimmune disease triggered by different conditions in genetically susceptible people. It is characterized by variable cutaneous manifestations including localized or disseminated pustules. Generalized pustular psoriasis (GPP) has two main clinical forms: von Zumbusch psoriasis, characterized by severe erythrodermia and scaling skin after the resolution of pustules, and the annular form. GPP may also present severe extracutaneous manifestations including pneumonitis, heart failure and hepatitis. Old reports showed a relationship between hypoparathyroidism and hypocalcemia as triggers for GPP highlighting the importance of adequate workup of the patient and possible therapeutic changes in acute situations. Here, we present a case of severe von Zumbusch psoriasis with life-threatening complications triggered by severe hypocalcemia secondary to hypoparathyroidism successfully treated with aggressive calcium reposition.Key Words: Psoriasis, Hypocalcemia, Hypoparathyroidism, Psoriasis of von Zumbusch

Abstract

Sea cucumber, Holothuria scabra, can be found abundantly in Indonesian seas, which is also known to possess several medicinal properties. Spirulina platensis is another marine resources that has recently been extensively researched for its medicinal ability, such as anti-inflammatory effect. This study aims to evaluate the efficacy of H. scabra extract combined with S. platensis extract in reducing inflammation. This study uses male Wistar rats as the study animal. Inflammation was induced by injecting carrageenan solution into the mice paw. Combination of H. scabra and S. platensis extract with various combination ratio (1:1; 1:2; and 2:1) was applied to the mice paw. Diclofenac sodium was used as the standard control therapy. Edema inhibition rate and anti-inflammatory efficacy were measured by analyzing the edema size and calculating the edema difference. Combination of H. scabra and S. platensis with 1:1 ratio has the largest edema inhibition volume compared to the other treatments. H. scabra and S. platensis combination outperforms the positive diclofenac sodium control group in terms of edema inhibition. The highest anti-inflammatory effect is obtained in the combination of H. scabra and S. platensis with 1:2 ratio, however, the anti-inflammatory efficacy is not as potent as the positive control. The effectivity of Holothuria scabra and Spirulina platensis extract in reducing the edema might be caused by their ability to reduce the levels of several inflammatory markers, including IL-6, NO, MMP9, and COX-2. This result suggests that H. scabra and S. platensis combination has anti-inflammatory effect shown in mice paw edema model.

Abstract

Background: One of the most common difficulties found in leprosy management is how to manage leprosy reaction, even though MDT has proven to be effective for the patients. Reversal reaction is associated with cell mediated immunity (CMI), consists of T-helper (Th) 1 and Th2 activities. Moreover, subset of CD4+ Th17 and CD25+ FOXP3 Treg were known to have an important role in leprosy’s natural history. Prior study showed a decreasing number of CD25+ FOXP3 Treg and increasing number of CD4+ Th17 in type II reversal reaction (ENL). However, the distribution of Th17 and Treg in type I reversal reaction (RR) has not yet been identified.Objective: To compare the distribution of CD4+ Th17 and CD25+ FOXP3 Treg between RR and ENL patient groups.Method: A total of 50 samples, consisted of 27 samples of reversal reaction (RR) and 23 samples of ENL, were collected. Observation of CD4+ RORg-T Th17 and CD25+ FOXP3 Treg were conducted with immunohistochemistry staining technique using anti FOX-P3 and anti RORg-T. Expression of CD4+ ROR-g Th17 and CD25+ FOXP3 Treg in percentage were analyzed using T-test.Result: There is a significant difference in mean CD4+ ROR-g Th17 and IL17 cell distribution for RR patient group (14.96% and 10.72%) compared with ENL (9.12% and 4.28%). No significant difference were found between mean CD25+ FOXP3 Treg and TGF-β cell distribution in RR patient group (6.12% and 5.44%) compared with ENL group (6.16% and 5.96%).Conclusion: There is a significant increment od CD+RORg-T Th17 and IL17 in RR patients group compared with ENL patients group. however, the distribution of CD25+ FOXP3+ Treg and TGF beta in RR has no significant difference campared with ENL.

Abstract

The particulate matter in vehicle emission has been shown to cause an increase in free radical levels which can increase serum MDA levels and leukocyte count. Spirulina platensis is an algae that has antioxidant effects that can scavenge free radicals. In this research, twenty-four of wistar mice are divided into 4 groups. Groups X1 and X2 received vehicle emission smoke exposure treatment and were given oral Spirulina platensis extract at a dose of 500 and 750 mg / kgBW / day for 14 days, the negative control group (C2) was given 0.9% NaCl orally and the normal control group did not get vehicle emission smoke exposure and Spirulina platensis extract. Serum MDA levels were checked by ELISA method and leukocyte count by giemsa staining. Data analysis was performed by using the one-way ANOVA test followed by Post Hoc Bonferroni. The results showed the mean serum MDA levels there was a significant difference between group C2 and group X2. In the mean absolute value of neutrophils and lymphocytes, there were significant differences between the C2 group with the administration of X1 and X2. In the mean absolute value of eosinophils and monocytes, there were significant differences between groups C1 and C2 and group X2 and between groups X1 and C1. Furthermore, the given doses of Spirulina platensis extract at a dose of 750mg / kgBW had an effect on serum MDA levels as well as the number of neutrophils and lymphocytes and the administration of doses of 500 and 750mg / kgBW affected the number of eosinophils and monocytes.

Abstract

Latar belakang :Kusta ,merupakan penyakit infeksi kronis dengan kesulitan utama penatalaksanaan reaksi kusta. Reaksi reversal merupakan reaksi kusta tipe 1. Beberapa penelitian menunjukkan peningkatan IL-6, IL-8 pada reaksi reversal. Interleukin-8 merupakan faktor kemotaktik spesifik neutrofil. Efek utama IL-8 adalah aktivasi dan rekrutmen netrofil ke lokasi infeksi. Studi ini bertujuan mengetahui perbedaan perbedaan kadar IL-8 pada pasien kusta multibasiler dengan dan tanpa reaksi reversal.Metode: Desain penelitian ini adalah analitik observasional dengan pendekatan cross-sectional. Berdasarkan perhitungan sampel , kelompok reaksi berjumlah 28 sampel, kelompok non reaksi berjumlah 28 sampel sehingga sampel keseluruhan adalah 56 orang. Dilakukan pemeriksaan kadar IL-8 serum dengan metode ELISA kemudian dihitung dengan uji beda kadar IL-8 antara 2 kelompok dengan Uji T2 sampel bebas.Hasil: Hasil penelitian ini terdapat perbedaan bermakna kadar IL-8 diantara dua kelompok , didapatkan p>0.05 .Simpulan :Terdapat perbedaan kadar IL-8 pada pasien kusta dengan reaksi  reversalKata kunci: kusta tipe multibasiler, reaksi reversal, IL-8

Abstract

Background: According to WHO data, the number of new cases of leprosy has decreased in 2019. However, Indonesia continues to provide a significant number of cases. According to statistics, India, Brazil, and Indonesia account for 79 percent of all instances. Purpose: This study aims to describe the profile of leprosy patients, and involves all Dermatology and Venereology Academic Hospitals in Indonesia. Methods: This study was a retrospective study of 2461 patients from Dermatology and Venereology Outpatient Clinic at 13 Academic Hospitals in Indonesia between January 2018 and December 2020. Result: Subjects in this study were dominated by males (66.8%) and aged > 14 years (95.3%). The most common type of leprosy was multibacillary (MB) (86.2%), and erythema nodosum leprosum (ENL) was the most leprosy reaction (20.3%). Majority of the subjects experienced disability in the hands (26.6%), in grade 1. Conclusion: Leprosy cases in Indonesia are mostly experienced by adult males. The most common type of leprosy is MB, with ENL being the most common leprosy reaction. Grade 1 disability is the most prevalent, therefore proper education is necessary to keep patients from progressing to grade 2 disability.

Abstract

Abstract Background : Full thickness Skin graft is one technique to accelerate wound healing. EPC proliferation and new blood vessel formation have an important role in the success of Ful thickness Skin graft. Ozone content in Ozonated VCO has the ability to increase growth factors so that it can accelerate wound healing Aims : This study was aimed to prove the effects of ozonated vco in various doses in increasing EPC proliferation and angiogenesis in full thickness skin graft wounds. Methods : This research is an experimental study with randomized post test only with parallel group design. 40 Sprague Dawley rats performed autologous Skin grafts at the same time. Samples were randomly divided into 8 groups, two control groups (X1 and X5), and 6 other groups were given a multi-dose ozonated oil 50.6 mg / ml (X2, X6), 101.3 mg / ml (X3, X7) , 202.6 mg / ml (X4, X8). Conduct termination on day 6 and day 12. Examination of the number of EPC proliferation and the number of angiogenesis in each group. Data were analyzed by Saphiro Wijk, ANOVA, post hoc benferonni and pearsons Results : The number of EPC proliferation in groups X1, X2, X3, X4, X5, X6, X7, X8 has a mean of 6.2 + 0.837, 10 + 1.581, 13.80 + 2.049, 14.80+ 2,280, 9.40 + 2,074 , 13.20 + 1,483, 19,0 + 2,055 and 19.20 + 1,095.The number of Angiogenesis in groups X5, X6, X7, X8 has a mean of 9.20 + 1,095,14,60 + 2,881,17,40+ 3,286, 18,80 + 2,168,17,40 + 3,286,18.80+ 2,168. The administration of ozonated VCO dose of 202.6 mg / ml gave the best results in the amount of angiogenesis (p = 0.001) and the amount of angiogenesis (p=0,001).Correlation analysis between EPC proliferation and angiogenesis showed a significant correlation (p = 0.001 and r = 0.738). Conclusion : Ozonated oil can increase the number of EPC proliferation and angiogenesis in the wound healng process of Full thickness Skin graft. The ozone dose of 202,6 mg / ml showed the best effect on increase wound healing process. Keyword : Full thickness skin graft, Ozonated oil , EPC, Angiogenesis
