Abstract

Masih meningkatnya kasus positif dan kematian COVID-19 di Indonesia menuntut fasyankes agar bisa mempersingkat durasi hospitalisasi/rawat inap untuk efisiensi sumber daya kesehatan dan peningkatan kualitas pelayanan. Terapi ajuvan pasien rawat inap antara lain terapi plasma konvalesen (TPK). Review ini bertujuan untuk mengetahui pengaruh pemberian TPK dan terapi standar dibandingkan terapi standar saja terhadap durasi rawat inap pasien COVID-19. Penelusuran artikel secara manual pada empat database, yaitu PubMed, Cochrane, ScienceDirect, dan Scopus. Kata kunci yang digunakan adalah convalescent plasma, COVID-19 therapy, COVID-19 serotherapy, length of stay. Metode seleksi berdasarkan kriteria eligibilitas mendapatkan 1 artikel yang kemudian ditelaah kritis dari segi kesahihan, kepentingan, dan kemamputerapan. Satu artikel tinjauan sistematis merupakan artikel yang sahih dan penting, tetapi kurang memiliki kemamputerapan karena demografi dan kondisi klinis yang tidak sesuai. Review ini menyimpulkan bahwa pemberian TPK tidak memengaruhi durasi rawat inap pasien COVID-19. Diperlukan studi lebih lanjut dengan demografi dan kondisi klinis yang sesuai untuk mendapat keyakinan tertinggi.Indonesia is still in an ongoing trend of positive cases and deaths caused by COVID-19. This condition urges health care facilities to reduce length of stay for more efficient health resources management and higher quality of care. Adjuvant therapy to hospitalized patients includes convalesent plasma therapy (CPT). This review aims to determine the effect of CPT on length of stay of COVID-19 patients in comparison to standard therapy. Literature search is through hand searching and through four databases: PubMed, Cochrane, ScienceDirect, and Scopus. Keywords used are: convalescent plasma, COVID-19 therapy, COVID-19 serotherapy, length of stay. Selection through eligibility criteria resulted in one systematic review article, then critically appraised by its’ validity, importance, and applicability. The article selected is valid and important, yet lack of applicability due to discrepancy in demographics and clinical conditions. The review concluded that CPT does not affect the length of stay of COVID-19 patients. More research with similiar demographics and clinical conditions are needed to achieve highest level of certainty.

Abstract

Osteomielitis merupakan peradangan tulang dan sumsum tulang yang biasanya disebabkan oleh infeksi bakteri. Osteomielitis dapat menimbulkan gangguan fungsional irreversibel dan disabilitas. Namun, hingga saat ini, belum ada guideline khusus diagnosis dan manajemen tatalaksana osteomielitis. Diagnosis dan tatalaksana osteomielitis harus komprehensif agar memberikan hasil baik. Osteomyelitis is an inflammation of bone and bone marrow which is usually caused by bacterial infection. Osteomyelitis can cause irreversible functional problems and disabilities. There is still no specific guidelines for diagnosis and management of osteomyelitis. Diagnosis and management must be conducted comprehensively to result in good recovery.

Abstract

Thrombositosis, peningkatan jumlah trombosit dalam darah, telah dikaitkan dengan kanker pada keganasan ginekologi. Trombositosis dapat dipicu oleh pelepasan sitokin oleh tumor ganas, seperti IL-6. Trombositosis berperan dalam perkembangan kanker melalui perlindungan terhadap sel tumor, produksi faktor pertumbuhan, dan angiogenesis. Tinjauan literatur ini mengumpulkan dan menganalisis penelitian terkait trombositosis sebagai prediktor kanker pada keganasan ginekologi. Pemahaman yang komprehensif tentang hubungan ini dapat membantu pengembangan metode deteksi dini dan terapi yang lebih efektif untuk kanker pada keganasan ginekologi.

Abstract

Abstract Introduction: In 2020, cervical cancer ranked as the fourth most common cancer in women globally, with an incidence of 604,000 cases. In Indonesia, cervical cancer holds the second position among the most prevalent cancers in women. This study aims to evaluate the relationship between age and the occurrence of advanced-stage cervical cancer as a step towards effective prevention, early detection, and management. Methods: This research adopts a retrospective study design based on the Cancer Registration database of RSUPN Cipto Mangunkusumo, focusing on cervical cancer patients from 2019 to 2022. Multivariate analysis was conducted with age as the primary independent variable, considering parity and employment status in the analysis. The FIGO classification of cervical cancer stages was used to categorize patients into early and advanced stages. Results: Out of 512 cervical cancer cases, 492 were included in this study. The distribution of cervical cancer stages and the age distribution of patients is depicted in Figure 1 and Figure 2. The analysis of the relationship between cervical cancer stage, age, parity, and employment status is presented in Table 2. The association between age and cervical cancer stage is statistically significant (p<0.05) with an odds ratio of 2.13, particularly in individuals aged >54 years. Conclusion: Age over 54 years is a significant risk factor for advanced-stage cervical cancer. Although there is no significant association with parity and employment history, these findings support preventive and early detection efforts in the older population. Increased screening programs and education are expected to reduce cases of advanced-stage cervical cancer in the future.

Abstract

Introduction: Age-related macular degeneration (AMD) is a leading cause of blindness globally, with wet AMD being particularly debilitating. The prevalence of AMD is increasing, necessitating a deeper understanding of its clinical characteristics and management. This study aims to investigate the demographic and clinical features, management, and treatment outcomes of wet AMD at Cipto Mangunkusumo Hospital during January-December 2022. Methods: A retrospective descriptive study was conducted, analyzing medical records of wet AMD patients treated during the specified period. Data analysis included patient demographics, clinical characteristics, treatment modalities, and outcomes. Results: Of 129 eyes from 122 patients, 115 eyes met the inclusion criteria. The majority were female (55.7%) with an average age of 67 years. Most patients presented with blurry vision (93.9%) and received Patizra anti-VEGF injections (60.9%). Hypertension was the most common risk factor (28.7%). Anti-VEGF treatment significantly reduced central macular thickness (p < 0.05) but had no significant impact on visual acuity. Avastin and Patizra injections similarly influenced macular thickness but not visual acuity. There were no significant differences between loading and non-loading dose groups in terms of outcomes. Conclusion: Anti-VEGF treatment effectively reduces central macular thickness in wet AMD patients, regardless of the specific agent used. However, it does not significantly improve visual acuity. Further research is needed to explore differences between loading and non-loading dose protocols, compare treatment strategies, and investigate demographic risk factors. Consistency in visual acuity documentation using the ETDRS format is recommended for future studies.

Abstract

ABSTRACTIntroduction: Stress urinary incontinence (SUI) is a common consequence of childbirthand pelvic floor dysfunction. The risk of SUI differs between vaginal delivery and cesareansection, but this comparison in the Indonesian female population remains unclear.Methods: This study is a systematic review and meta-analysis comparing the risk of SUIbetween vaginal delivery and cesarean section among Indonesian women using reviewmanager 5.4. application. Literature searches were conducted in PubMed, Scopus, andCochrane databases using relevant keywords.Results: Analysis of two studies involving 438 women revealed a lower risk of SUI in thecesarean section group (16 out of 233 women) compared to vaginal delivery (38 out of 205women). The combined risk ratio was 0.41 (95% CI: 0.22–0.75), indicating that cesareansection significantly reduces the risk of SUI compared to vaginal delivery.Conclusion: Cesarean section is associated with a lower risk of SUI compared to vaginaldelivery in the Indonesian female population. These findings may assist clinicians andpatients in making informed decisions about childbirth methods.

Abstract

Background:  This review evaluates urethral reconstruction’s effectiveness in fertility restoration. Methods: A systematic review was conducted using PubMed, Scopus, and Cochrane databases. Studies on anastomotic urethroplasty, buccal mucosa graft (BMG) urethroplasty, and acellular dermal matrix (ADM) grafts were analyzed. Outcomes included patency rates, ejaculatory function, erectile function, and fertility restoration, along with postoperative complications and long-term reproductive outcomes. Results: Anastomotic urethroplasty achieved >90% patency for strictures <2 cm, with 100% antegrade ejaculation restoration in pelvic fracture cases. BMG urethroplasty showed an 85.7–100% success rate for strictures >3 cm, with 76% reporting improved ejaculation force. ADM grafts had a 94% success rate in complex cases. Erectile dysfunction occurred in 9.7% but resolved within 6–20 months. Discussion: Urethral reconstruction restores patency and ejaculation in >85% of cases, but fertility outcomes depend on factors like testicular atrophy and neurovascular integrity. Non-transecting techniques reduce vascular complications, and early intervention aids spermatogenesis preservation. A multidisciplinary approach integrating andrology and assisted reproductive techniques is crucial.

HIGHLIGHTS Potential benefits of vitamin D3 supplementation >20 ng/ml, along with myo-inositol, folic acid, and melatonin, on implantation rates. Subsequent studies revealed no substantial enhancement in pregnancy rates when specific supplements were used. Further research is needed to provide more definitive conclusions.   ABSTRACT Objective: In vitro fertilization is the most common form of assisted reproductive and fertility technology. However, it is important to note the success rates of IVF generally decrease as a person gets older. Identifying modifiable factors affecting human fertility, including diet, is significant for clinical and community wellness. This systematic review investigates the role of micronutrients supplementation in fertility and its potential impact on in vitro fertilization (IVF) outcomes. Materials and Methods: A systematic search in multiple databases using specific keywords related to in vitro fertilization, micronutrient supplementation, and outcome measures. Results: Out of 1462 retrieved studies, 8 were included in the systematic review. These studies covered a range of micronutrients and their effects on different IVF outcomes. Findings indicated potential benefits of vitamin D3 supplementation. Vitamin D3 supplementation was associated with a significant improvement in implantation rates (p < 0.05) in two studies, while no significant effect was observed in clinical pregnancy rates in other micronutrients. The systematic review highlights the diverse effects of various micronutrient supplements on IVF outcomes. While some supplements showed potential benefits, others did not significantly improve pregnancy rates. Further research with larger cohorts is necessary to establish conclusive evidence. Keywords: micronutrient supplements, IVF outcomes, pregnancy rates. Conclusion: In conclusion, this systematic review sheds light on the impact of micronutrients on IVF outcomes. Some studies suggested potential benefits of vitamin D3 supplementation >20 ng/ml, further research is needed to provide more definitive conclusions. Understanding the function of micronutrients in fertility could have significant implications for IVF treatment and female infertility management.

Abstract

Background: The incidence of prediabetes is predicted to continue to increase. Approximately 25% of prediabetes patients develop type 2 diabetes mellitus (T2DM) within 3–5 years. Zinc is known to have a role in the synthesis, storage, secretion, and action of the insulin hormone. Purpose: The aim of the study is to know about the impact of zinc supplementation in type 2 diabetes mellitus (DM) prevention. Methods: A systematic literature search was performed using the PubMed, Cochrane, and EBSCOHost search engines. The inclusion study includes systematic review and metaanalysis of randomized clinical trials study and randomized clinical study, and we only include the research of the last 10 years. Results: Three RCTs were selected from 175 articles to undergo a critical review with a total of 353 patients with a duration of 6-12 months (level of evidence II-III). The incidence of T2DM in the intervention group decreased significantly (OR: 0.37 [95% CI 0.7-0.8]; RR: 0.44; ARR: 14% [95% CI 3.5-24.5%]; NNT: 7.14 [4-29]) in one study. Zinc supplementation significantly reduced Fasting Blood Sugar (FBS) in two studies (mean difference -17.3 and -5.76) and oral glucose tolerance test (OGTT) in one study (MD -22.6). Conclusion: Administration of zinc in prediabetes patients to prevent the development of T2DM still requires further study with a larger sample, longer duration, and control variables by design and statistics.

Abstract

Introduction: Infertility is a reproductive health problem in various countries, including Indonesia, with a significant impact on population and community productivity. This study aims to assess the urgency of addressing infertility in the context of public health and population control in Indonesia. Methods: This research employs a literature review method, referencing relevant sources. Document searches were conducted through Google Scholar and manual searching using the keywords “(Infertility) AND (Indonesia).” Results: The review found 53 literatures consisting of scientific research, medical consensus, review papers, news, and legal products. Discussion: Infertility is a complex problem that requires a multidisciplinary approach. The discussion is organized into several multidisciplinary sections which include a discussion from the medical side in the form of etiology, risk factors, diagnosis flow, and management of infertility, a review of infertility as a public health problem, the impact of infertility on patients and society, sensitive public communication methods regarding infertility, and recommendations for management and policy regulation regarding infertility for government officials. Conclusion: Addressing infertility in Indonesia requires the support of various parties, including the government, health workers, and the community, to overcome its complex impact.
