Abstract

Background: Hydatidiform mole, a type of gestational trophoblastic disease, arises from abnormal placental tissue and has the potential for malignant transformation. The condition’s incidence varies globally, with notable prevalence in Indonesia. Advanced maternal age, dietary factors, and a history of previous moles contribute to recurrence risks. Understanding the molecular and cellular mechanisms is critical for effective management.Methods: This narrative review synthesizes findings from primary studies and reviews exploring the molecular and cellular characteristics of hydatidiform mole. Data were sourced from Google Scholar, PubMed, and ScienceDirect using keywords like “hydatidiform mole,” “molecular mechanisms,” and “cell signaling pathways.”Results: The pathogenesis involves chromosomal abnormalities, particularly in complete moles characterized by a diploid genome lacking maternal DNA. P57KIP2 expression serves as a diagnostic marker, distinguishing complete from partial moles. Molecular genotyping, including short tandem repeat (STR) analysis, aids in accurate diagnosis and risk stratification for malignancy. Genetic factors, including mutations in NLRP7 and KHDC3L, are associated with recurrence in familial cases.Conclusion: The cellular and molecular insights into hydatidiform moles enhance understanding of their pathophysiology and inform diagnostic strategies. Recognizing genetic predispositions enables better patient management, emphasizing the need for continued research into the condition’s underlying mechanisms to improve clinical outcomes and therapeutic approaches.

Abstract

Background: This literature review aims to present the latest scientific findings that explain the relationship between chronic prostatitis and male reproductive disorders. Method: This study was a narrative literature review that identified articles from PubMed, Scopus, and Google Scholar using the keywords "chronic prostatitis", "CP/CPPS", "male infertility", "erectile dysfunction", and "ejaculatory dysfunction". Inclusion criteria include publications in the last 10 years that discuss the association between chronic prostatitis and male reproductive disorders. The data were analyzed descriptively to identify patterns of interdependence. Results: Data synthesis showed that chronic prostatitis was associated with a significant decrease in sperm concentration, motility, morphology, and viability. Patients also had a threefold risk of erectile dysfunction, as well as a high prevalence of painful ejaculation (30–65%) and premature ejaculation (±40%). Inflammatory mechanisms and oxidative stress have been shown to play an important role in pathogenesis, while multimodal therapies, including antibiotics, antioxidants, PDE5 inhibitors, phytotherapy, and pelvic floor physiotherapy, show better effectiveness than monotherapy. Implications: These results confirm the need to evaluate the reproductive aspect in patients with chronic prostatitis, as well as a multidisciplinary approach to improve quality of life and fertility. Novelty: This study presents an up-to-date comprehensive review that integrates clinical evidence, pathophysiology, and therapeutic strategies, thereby enriching understanding and supporting the development of more effective management.

Abstract

Cervical cancer is the third most common cancer in women in the world and the second most common in women in Indonesia. Cervical cancer accounts for 7.5% of cancer deaths in the world and most of them come from developing countries including Indonesia. The cause of cervical cancer is high-risk HPV and can actually be prevented through high-risk HPV vaccines that are available throughout the world, including Indonesia. Discussion: The HPV vaccine has been shown to be effective in preventing cervical cancer, which is known to reduce the incidence by up to 90%. Various preparations of the HPV vaccine have been shown to be safe to give to patients. Conditions in Indonesia currently make the HPV vaccine a mandatory vaccine given as part of the school children's immunization month program as an effort to prevent cervical cancer for the community in the future. Conclusion: The HPV vaccine has effectiveness and side effects that are safe to be given to the public at large. The policy in Indonesia to require the provision of the HPV vaccine in the schoolchildren's immunization month program is the right step to prevent cervical cancer.

Abstract

Introduction: Retinal vein occlusion (RVO) is a significant retinal vascular disease, often resulting in macular edema and vision impairment. This study aims to investigate the clinical characteristics, management, and outcomes of RVO patients with macular edema at Cipto Mangunkusumo Hospital Kirana from January 2020 to December 2021. Methods: This retrospective descriptive study analyzed medical records of 85 RVO patients with macular edema. Demographic data, clinical characteristics, management approaches, and treatment outcomes were examined. Data were analyzed using SPSS. Results: Most patients were over 50 years old, predominantly male, and affected in one eye. Hypertension and diabetes mellitus were common comorbidities. Central RVO cases had worse initial visual acuity and macular thickness than branch RVO cases. Anti-VEGF injections were the primary therapy, and patients received an average of two injections in the first year. Macular thickness reduced after anti-VEGF injections, but visual acuity improvement was minimal. Conclusion: Patients with RVO and macular edema are often older males with systemic risk factors. Anti-VEGF injections are the primary treatment, with improvements in macular thickness but limited visual acuity gain. Patient education, comprehensive management, and public awareness are recommended to enhance RVO care. Further research to analyze parameter relationships is needed.
