Abstract

Implementasi manajemen risiko sudah sangat jelas tertuang menjadi salah satu elemen penilaian dalam standar akreditasi rumah sakit, yaitu pada standar peningkatan mutu dan keselamatan pasien.  Salah satu risiko yang menjadi isu penting yang diangkat di RSUD dr. Soeratno Gemolong adalah Ketepatan Identifikasi Pasien pada saat Pelayanan. Ketepatan identifikasi pasien merupakan risiko yang terbilang sangat riskan apabila sampai terjadi, karena hal tersebut tentu akan berdampak langsung pada Keselamatan Pasien. Oleh karena itu kegiatan Pengabdian kepada Masyarakat ini bertujuan untuk meningkatkan kepatuhan petugas terhadap ketepatan identifikasi pasien. Metode yang digunakan sebagai upaya peningkatan kepatuhan identifikasi pasien yaitu dengan metode ceramah dengan lembar observasi sebagai instrument untuk mengukur keberhasilan kegiatan. Bedasarkan kegiatan pengabdian Masyarakat yang telah terlaksana diperoleh bahwa petugas pelayanan yang diobservasi telah melaksanakan prosedur identifikasi pasien sesuai dengan SOP. Pelayanan dengan memperhatikan prosedur ketepatan identifikasi pasien sangat penting untuk dimonitor setiap periodenya karena berdampak langsung pada efektivitas pelayanan pengobatan pada pasien.

Abstract

Urinary tract infection (UTI) is the second highest infectious disease among children in Indonesia. Management of UTI requires antibiotic therapy, but the irrational use of antibiotics makes this disease one of the contributors to the high rate of antibiotic resistance. Therefore, there is a need for a study to measure the rationality of antibiotic use in children with UTI. This study is a descriptive observational study with a cross sectional design. Data were collected retrospectively using a purposive sampling technique based on medical record data of pediatric patients at Dr. Moewardi Surakarta Hospital in 2021-2023. The results showed that out of 100 medical records of pediatric patients with UTI, 68.00% were women and 32.00% were men. Based on age, the highest incidence of UTI occurred in the age range of 10-18 years as much as 52.00%. The most common antibiotic use was ampicillin 68.00%. The results of the rationality study showed 69.00% in category 0 (rational use of antibiotics). Irrationality occurred in category IIA (antibiotic use is not the right dose) 17.00%, IIIA (antibiotic use with too long duration) 1.00%, IIIB (antibiotic use with too short duration) 4.00%, IIA & IIIB (antibiotic use is not the right dose & duration is too short) 2.00%, IVA (there are other more effective antibiotic options) 4.00%, IIIB & IVA (antibiotic use is too short duration & there are other more effective antibiotic options) 3.00%. It can be concluded that the rational use of antibiotics needs to be improved.

Abstract

Geriatric patients with heart failure generally receive more than 1 type of drug therapy, called polypharmacy, which typically involves at least four types of treatment: ACEI (Angiotensin Converting Enzyme Inhibitors), diuretics, beta blockers, and digoxin. This causes an increase in the occurrence of drug interactions. This study aims to determine the potential drug interactions that occur in geriatric patients with heart failure who undergo hospitalization at Dr. Moewardi Surakarta Hospital from January to December 2023. This study is a descriptive observational study with a cross-sectional design, and the data were collected retrospectively using a purposive sampling technique based on the medical records of geriatric patients at Dr. Moewardi Surakarta Hospital in 2023. The inclusion criteria were inpatients aged ≥55 years who were diagnosed with heart failure. The exclusion criteria were patients with incomplete or illegible medical record data or who died during treatment. Drug interaction research was conducted based on Drugs.com and Medscape. The results showed that out of the 100 patients, 98 experienced drug interactions. The incidence of drug interactions was 252 minor cases (21.65%), 818 moderate cases (69.50%), and 106 major cases (9.00%). The mechanism of drug interaction was pharmacodynamic in 919 cases (65.50%), pharmacokinetic in 213 cases (30.40%), and unknown in 44 cases (4.10%).

Abstract

During the birth period, sometimes support from the family is needed. This kind of situation can be a trigger that causes depression in postpartum mothers. Depression felt by the individual can cause an attempt to react to the depression experienced. This reaction is an activity to make adjustments to certain stimulus situations, which if not done properly will cause physical and psychological disorders. Symptoms arising from postpartum depression are filled with feelings and depression accompanied by crying for no reason, having no energy or only a little energy they have, unable to concentrate, feeling guilty and worthless in themselves, becoming uninterested in babies. For mothers who suffer from postpartum depression, they cannot establish a good relationship with their newborn baby. Group therapy to reduce depression includes psychoeducation groups, supportive groups, and self-help groups. The purpose of this study is to analyze the provision of self-help group therapy to mothers with postpartum depression. The research design uses an experimental quasy with one group pretest-posttest. Abstract

Diabetes melitus adalah suatu gangguan metabolik yang ditandai dengan peningkatan kadar glukosa darah (hiperglikemia) akibat kerusakan pada sekresi insulin dan kerja insulin. Hipertensi adalah peningkatan tekanan darah yang tidak normal ditandai dengan tekanan darah diatas 140/90 mmHg. Diabetes melitus seringkali disertai dengan hipertensi atau sebaliknya. Hal ini menyebabkan penggunaan banyak obat yang berbeda dan cenderung mendorong interaksi obat. Interaksi obat adalah interaksi yang terjadi ketika efek suatu obat diubah oleh efek obat lain. Penelitian ini bertujuan untuk mengetahui potensi terjadinya interaksi pada pasien dewasa diabetes melitus tipe 2 dengan hipertensi di instalasi rawat inap RSUD Dr. Moewardi Surakarta periode Januari sampai Desember 2023. Jenis penelitian ini adalah deskriptif observasional dengan rancangan cross sectional, memperoleh data secara retrospektif dan analisis interaksi obat menggunakan software drugs.com versi 2.14.11. Pengambilan sampel secara purposive sampling. Kriteria inklusi yaitu pasien berumur 18-59 tahun, pasien yang menjalani rawat inap, menerima sedikitnya satu jenis obat antihipertensi dan satu jenis obat antidiabetes. Kriteria eksklusi yaitu pasien dengan data rekam medis tidak lengkap atau tidak terbaca, pasien yang meninggal selama perawatan. Hasil penelitian potensi terjadinya interaksi pada pasien dewasa diabetes melitus tipe 2 dengan hipertensi di RSUD Dr. Moewardi Surakarta dari 100 pasien terdapat 90 pasien yang ada interaksi obat. Interaksi obat berdasarkan tingkat keparahan minor sebanyak 20 (5,10%), moderate sebanyak 340 (86,30%) dan major sebanyak 34 (8,60%). Interaksi obat berdasarkan pola mekanisme farmakokinetik sebanyak 12 (3,00%), farmakodinamik sebanyak 318 (80,70%) dan unknown sebanyak 64 (16,20%).

Abstract

Service quality is the main basis for knowing the level of consumer satisfaction, in which case a company can be said to be good if it is able to provide goods or services in accordance with customer desires. The aim of the research is to determine performance evaluation and work motivation on service quality at RSUD dr. Soeratno Gemolong Sragen. This type of research is descriptive correlational, namely research that aims to find relationships between variables. Cross-sectional research approach. How to take simple random sampling with a sample of employees at RSUD dr. Soeratno Gemolong Sragen. The method of collecting data is using a questionnaire, where hypothesis testing uses descriptive statistical analysis with F test analysis, t test. The results of this research show that variables, performance evaluation, and work motivation have a positive and significant influence partially or simultaneously on service quality variables. The results of this research also show that the dominant variable that has an influence on service quality is the work motivation variable.

Abstract

Gastroesophageal Reflux Disease merupakan kondisi di mana cairan lambung/isi perut kembali ke kerongkongan. Rasionalitas penggunaan obat merupakan proses penjaminan mutu resmi dan terstruktur, dilakukan secara konsisten untuk menjamin kelayakan, keamanan, dan kemanjuran obat. Tujuan penelitian ini untuk mengevaluasi rasionalitas penggunaan obat GERD pada pasien dewasa di instalasi rawat inap RSUD Dr. Moewardi Surakarta tahun 2020-2023. Penelitian ini adalah penelitian yang bersifat observasional deskriptif dengan pendekatan secara kualitatif yang menggunakan desain penelitian cross sectional. Data dikumpulkan menggunakan teknik purposive sampling dan diperoleh secara retrospektif. Sampel yang digunakan rekam medik pasien dewasa yang terdiagnosis GERD tahun 2020-2023 berdasarkan kriteria inklusi. Hasil penelitian menunjukkan bahwa dari 100 sampel pasien dewasa yang menderita GERD, mayoritas perempuan sebanyak 61%, dengan usia rata-rata 56-65 tahun sebanyak 33%. Profil penggunaan obat pada pasien GERD paling banyak menggunakan variasi terapi obat monoterapi mencapai 83%, dengan golongan obat PPI (Proton Pump Inhibitor)sebanyak 80% dan jenis obat yang paling sering digunakan adalah omeprazole 40 mg iv sebanyak 55%. Penggunaan terapi obat GERD berdasarkan tepat diagnosis diperoleh persentase sebesar 100%, tepat indikasi 100%, tepat pemilihan obat 97%, tepat dosis 100%, tepat cara pemberian 100%, tepat interval waktu pemberian 100%, tepat lama pemberian 100%, dan tepat penilaian kondisi pasien 100%.

Abstract

Type 2 diabetes mellitus represents a persistent metabolic dysfunction marked by elevated blood glucose levels attributable to insulin resistance. Geriatrics pertains specifically to individuals aged 60 years and older. Managing diabetes mellitus necessitates intricate therapeutic interventions, often comprising single or combined pharmacological agents aimed at achieving euglycemia. The concurrent administration of multiple medications, termed polypharmacy, amplifies the likelihood of pharmacological interactions. This investigation endeavors to delineate potential drug interactions observed in geriatric inpatients afflicted with type 2 diabetes mellitus at Dr. Moewardi Regional General Hospital in Surakarta throughout the calendar year 2023. The study adopted a descriptive observational design employing a cross-sectional approach. Data collection was conducted retrospectively through purposive sampling based on the medical records of geriatric patients at Dr. Moewardi Regional General Hospital in Surakarta. Inclusion criteria encompassed patients aged 60 years and older diagnosed with type 2 diabetes mellitus who had been hospitalized. Exclusion criteria included incomplete or illegible medical records, patients diagnosed with anemia and hemoglobinopathies, and those who expired during treatment. The findings revealed that out of 100 patients, 96 experienced drug interactions. Most patients (47.00%) were aged between 66 and 74 years. Females constituted a larger proportion (51.00%) of the patient cohort. Among hospitalized patients, 83 (83.00%) were admitted for 2-6 days. A total of 70 patients (70.00%) were prescribed between 4 and 13 medications. The predominant severity level of drug interactions was moderate, comprising 1,017 cases (74.60%). Pharmacodynamic mechanisms accounted for most drug interactions observed, totaling 1,054 cases (77.30%).

Abstract

Health is a pivotal factor in enhancing human quality of life by improving healthcare service quality. Pharmacies serve as crucial healthcare facilities widely frequented by the populace. High-quality pharmaceutical services in pharmacies can gratify patients, foster patient loyalty, and augment patient quality of life. This study aims to ascertain patient satisfaction levels regarding pharmaceutical services at several pharmacies in Karanganyar Regency. This descriptive observational study employed a quantitative approach. Sampling was conducted using non-probability sampling with an accidental sampling technique, resulting in a sample of 105 respondents. Patient satisfaction levels were computed using SPSS Statistics 25. Research findings revealed satisfaction rates for pharmacy service indicators: pharmacy appearance achieved 85.10% (very satisfied), staff friendliness achieved 80.23% (very satisfied), medication information provision achieved 82.66% (very satisfied), medication availability achieved 83.27% (very satisfied), and service speed achieved 87.93% (very satisfied). In conclusion, patients expressed overall satisfaction with pharmaceutical services at several pharmacies in Karanganyar Regency.
