Abstract

Tuberculosis (TB) is still a public health challenge in Serang Regency, with the coverage of Tuberculosis Prevention Therapy (TPT) that has not reached the target. This study aims to analyze the challenges and strategies for optimizing the role of TB program managers in Community Health Centers in increasing TPT coverage. This study used a qualitative method with in-depth interviews with nine informants, consisting of TB program managers, health workers, and policy makers at the Serang Regency Health Office. The results of the study showed that limited human resources (HR), low public awareness, lack of cross-sector support, limited number of active TB cadres, and drug distribution constraints were the main obstacles in implementing the TPT program. Efforts that have been made include training health workers, utilizing technology in patient monitoring, and cross-sector coordination, but still need to be strengthened to be more optimal. It is necessary to increase the capacity of health workers across programs and cadres who are trained in TB, increase public education, optimize drug distribution, and more supportive regional policies to increase TPT coverage and accelerate TB elimination.

Abstract

Ministry of Health Regulation No. 14 of 2021 regulates the standards of facilities, health workers, and clinic operations to ensure the quality of health services. However, the level of clinic compliance with this regulation still varies, especially in terms of the completeness of infrastructure facilities and the number of health workers who meet the standards. Therefore, this study aims to analyze clinic compliance with these regulations in Karawang Regency in 2024. Methods This study used a qualitative approach with interview and observation methods. Interviews were conducted with three main informants consisting of clinic owners/managers, medical personnel, and Health Office officers, as well as two supporting informants from patients or service users. Observations were made to assess the suitability of facilities and the number of health workers with the provisions in Permenkes No. 14 of 2021. The results showed that most clinics have tried to comply with the regulation, especially in providing examination rooms and basic facilities. However, there are still some clinics that have not met the standards, especially in the provision of appropriate action rooms and an adequate number of health workers. The main factors affecting compliance include budget constraints, lack of medical personnel, and lack of understanding of the applicable regulations by clinic managers. Conclusion Increased supervision, socialization of regulations, and support from local governments in the form of budgets and incentives for medical personnel in clinics are needed to improve compliance with Permenkes No. 14 of 2021.

Abstract

The advancement of information technology within the health sector in Indonesia, including the administration and management of health facilities, is undergoing significant changes. The Karawang District Health Office, which is responsible for health facility management, has implemented a registration system for independent practicing health facilities. The study aimed to identify and evaluate the input, process, and output aspects of the existing registration system. The research method used a qualitative approach with in-depth interviews with relevant parties and direct observation of the health facility registration flow. The results showed that the existing registration system has been quite effective in improving administrative transparency and efficiency. In the input aspect, the digital-based registration system makes it easier for applicants to upload important documents such as Practice License (SIP) and Registration Certificate (STR). Technical constraints such as internet network problems and large file sizes are still an obstacle. On the process aspect, the registration flow involving several related parties such as the verification team and licensing manager has been running quite well, although there is a need to improve access rights management to protect sensitive data. On the output side, the system provides results in the form of registration numbers and status notifications that support monitoring and supervision by the Health Office and the Ministry of Health, but there are still potential errors in data input that need to be addressed. Suggestions The Karawang District Health Office made improvements to the technical infrastructure, as well as strengthening data management and verification to improve the quality and accuracy of the registration system such as training and socialization of registration demonstrations.

Abstract
