Abstract

Specimen homogenization and the addition of anticoagulants are pre-analytical steps that influence the accuracy of the test results. Complete blood count (CBC) is a screening test that supports disease diagnosis and aids in determining the appropriate therapy for patients. Inadequate specimens are inappropriate for subsequent testing, as they can lead to inaccurate results in the CBC examination. This study aims to determine the optimization of complete blood count results with variations in specimen handling and secondary homogenization techniques for whole blood. The type of research used is quantitative analysis using laboratory experimental methods. The study was conducted in May 2024 in the Clinical Pathology Laboratory of the Diploma-4 Medical Laboratory Technology Study Program, Faculty of Health Sciences, Muhammadiyah University of Sidoarjo, using 48 blood samples. The results of the MANOVA test indicate that the type of anticoagulant has a significant effect on the erythrocyte count (p=0.041) but does not have a significant impact on the leukocyte count (p=0.844) and platelet count (p=0.920). Meanwhile, the homogenization technique does not significantly affect the erythrocyte count (p=0.959), leukocyte count (p=0.991), or platelet count (p=0.867). This study concludes that the secondary homogenization technique has no significant effect. In contrast, the type of anticoagulant significantly impacts the results of the Complete Blood Count (CBC) test. This research suggests collecting whole blood specimens using Ethylene Diamine Tetra Acetate (EDTA) vacutainers as the anticoagulant and applying a secondary homogenization technique before performing the CBC analysis.

Abstract

Type 2 Diabetes Mellitus (T2D) is a metabolic disorder caused by dysfunction of insulin secretion by pancreatic beta cells and the inability of insulin tissue to respond appropriately to insulin. Molecular-based examination can make it easier to determine appropriate diagnostic biomarkers and the biology of this disease appears long before clinical symptoms develop. Blood isolation samples from the buffy coat contained higher levels of DNA than whole blood samples. The aim of this research is to determine whether there are differences in the results of the centrifugation speed of blood samples for molecular-based examination of Type 2 Diabetes Mellitus (T2D). Optimal centrifugation speed can produce high-quality DNA required for molecular analysis, thereby improving the accuracy of diagnosis and effectiveness of T2D treatment. DNA contaminated by cellular debris can result in small amounts of DNA obtained for further analysis. The research was carried out qualitatively using descriptive experimental research using purposive sampling techniques. The total samples used were 25 samples, taken from 5 patients and then divided into 5 treatment groups (no centrifugation, centrifugation speed 500 rpm, 1500 rpm, 3000 rpm and 4500 rpm for 5 minutes). Test testing is carried out qualitatively (electrophoresis). The results showed that there was no significant difference in DNA visualization in molecular examination of T2D disease between blood samples that were not centrifuged and those that were centrifuged at speeds of 500 rpm, 1500 rpm, 3000 rpm and 4500 rpm for 5 minutes. However, centrifugation speeds between 1500 rpm and 4500 rpm showed thicker and clearer DNA bands.

Abstract

General background: Diabetes Mellitus is a chronic metabolic disorder characterized by persistent hyperglycemia and dyslipidemia, which often worsens with unhealthy lifestyle habits. Specific background: Smoking introduces toxic substances such as nicotine that can alter lipid metabolism, insulin sensitivity, and glucose regulation, thereby potentially aggravating complications in diabetic patients. Knowledge gap: While dyslipidemia and hyperglycemia in diabetes are well documented, evidence on the comparative differences between smokers and non-smokers with diabetes remains limited. Aim: This study investigated the differences in triglyceride and blood glucose levels among diabetic patients who smoke and those who do not. Results: An experimental design was conducted at Anna Medika Hospital Madura with 30 participants (15 smokers and 15 non-smokers with diabetes). The Mann Whitney test showed significant differences in triglyceride levels (p=0.000) and blood glucose levels (p=0.049) between the two groups, with smokers displaying higher values. Novelty: This study highlights nicotine’s role in exacerbating lipid and glucose dysregulation in diabetes within an Indonesian hospital setting. Implications: Findings emphasize the importance of targeted lifestyle interventions and smoking cessation programs in managing metabolic control among diabetic patients. Highlights: Smokers with diabetes had higher triglyceride and glucose levels. Nicotine worsens insulin resistance and lipid metabolism. Lifestyle modification is crucial for better diabetes management. Keywords: Diabetes Mellitus, Smoking, Triglycerides, Blood Glucose, Lifestyle

Abstract

Hematology analyzers are used in almost all clinical laboratories for CBC testing. CBC test results are crucial, as they play a significant role in establishing diagnoses and determining the appropriate therapy for patients. This study aims to analyze the effect of anticoagulant type, storage temperature, and delay time on the results of erythrocyte, leukocyte, and thrombocyte counts in whole blood samples. The study design employed was a laboratory experiment with a total of 64 samples, which were subjected to a combination of treatments involving anticoagulant type (Vacutainer and conventional), storage temperature (room temperature and refrigerator), and delay time for testing (0, 6, 12, and 24 hours). The examination was conducted using a hematology analyzer, and data analysis was performed using the Kruskal–Wallis test and the post hoc Mann–Whitney U test. The results showed that there was no significant effect of the three factors on the number of erythrocytes and leukocytes. However, the delay time significantly affected the platelet count (p=0.004), with significant differences observed between 6 hours and 24 hours, and between 12 hours and 24 hours. The delay in blood examination with K3EDTA and Na2EDTA affects the morphology of erythrocytes and leukocytes. Abnormal changes occur after 6, 12, and 24 hours of storage. The gold standard is a fresh whole blood sample collected in a K3EDTA vacutainer tube with the correct volume and analyzed within a maximum of 2 hours at room temperature.
