Abstract

ABSTRAKPrevalensi pressure ulcer yang tinggi menimbulkan kerugian bagi banyak pihak dalam pelayanan perawatan akut dan jangka panjang. Tujuan penelitian: Penelitian ini bertujuan mengidentifikasi hubungan sikap dan pengetahuan dengan perilaku pencegahan pressure ulcer pada mahasiswa profesi keperawatan. Metode: Penelitian ini berdesain cross sectional, melibatkan 227 sampel dengan teknik consecutive sampling.  Penelitian menggunakan The Pressure Ulcer Knowledge Assessment Tool (PUKAT) 2.0, Attitude towards Pressure Ulcer Prevention (APuP), dan kuesioner perilaku yang valid dan reliabel. Analisis univariat menggunakan distribusi frekuensi menggambarkan karakteristik responden, pengetahuan, sikap, dan perilaku. Analisis bivariat dengan uji Fisher exact dan chi square. Hasil: Hasil penelitian menunjukkan bahwa sebagian besar mahasiswa profesi memiliki pengetahuan masih kurang (53,7%), memiliki sikap yang baik (59%), dan sebagian besar memiliki perilaku melakukan tindakan pencegahan pressure ulcer (55,9%). Tidak ada hubungan signifikan antara pengetahuan dengan perilaku (p value = 0,071; α = 0,05), tetapi ada hubungan signifikan antara sikap dengan perilaku (p value= 0,010; α = 0,05). Diskusi: Sikap yang baik dipandang dapat menghindari terjadinya pressure ulcer. Pengetahuan tidak berpengaruh terhadap perilaku yang menandakan pengetahuan masih bersifat recall memory tentang materi yang dipelajari ketika tahap akademik.  Kesimpulan: Penelitian ini merekomendasikan agar institusi pendidikan merekonstruksi kurikulum, ujian teori preklinik, dan evaluasi berkala yang dilakukan pada tahap profesi.Kata Kunci: pengetahuan, perilaku pencegahan, pressure ulcer, sikap  Effects of Attitude and Knowledge on Pressure Ulcer Prevention Behavior ABSTRACTThe significant prevalence of pressure ulcers imposes detrimental consequences on various stakeholders within acute and long-term care services. Objective: This study aims to identify the correlation between attitude and knowledge with regard to pressure ulcer prevention behavior among nursing profession students. Methods: The research design employed in this study was cross-sectional, involving a sample size of 227 participants selected through the consecutive sampling technique. The data collection instruments included The Pressure Ulcer Knowledge Assessment Tool (PUKAT) 2.0, Attitude towards Pressure Ulcer Prevention (APuP) questionnaire, and validated and reliable behavior questionnaires. The univariate analysis entailed the use of frequency distribution to delineate the respondents' characteristics, knowledge levels, attitudes, and behaviors. The bivariate analysis consisted of Fisher's Exact test and Chi-square test. Results: The findings indicated that a majority of nursing profession students possess inadequate knowledge (53.7%), demonstrate positive attitudes (59%), and engage in pressure ulcer prevention behaviors to a considerable extent (55.9%). Notably, no statistically significant correlation was observed between knowledge and behavior (p-value=0.071; α=0.05); however, a significant correlation was found between attitude and behavior (p-value=0.010; α=0.05).  Discussion: This study highlights the pivotal role of positive attitudes in preventing pressure ulcers. Moreover, it is suggested that knowledge has limited influence on behavior, as it primarily relies on the recollection of information acquired during the academic phase. Conclusion: This study recommends educational institutions to reconstruct their curriculum, refine pre-clinical theory examinations, and implement regular evaluations during the professional stage.Keywords: knowledge, prevention behavior, pressure ulcer, attitude

Abstract

This study aims to identify the factors that influence interest in becoming an intensive nurse. The design of this study used a cross-sectional approach. The results showed self-efficacy, appreciation, work unit, age, family support and supervisor support (p=<0.001), years of service (p=0.005), gender (p=0.002), career level (p=0.004), and experience. Training (p=0.003) affected the interest of intensive nurses. On the other hand, knowledge and level of education were not influenced by the pull of intensive nurses. The results of multiple linear regression analysis found that the most influential interest factors in becoming an intensive nurse were self-efficacy, followed by rewards, work units, and gender. These four factors contributed 57.4% in influencing the interest in becoming an intensive nurse. In conclusion, what most determines interest in becoming an intensive nurse is self-efficacy. Abstract

This study aims to explore nurses' experiences in improving formal education at X Lampung Selatan Hospital. The method used is a phenomenological qualitative research design using a descriptive approach. The research results show that there are five themes for nurses' experience, namely: 1) Understanding of the importance of formal education for nurses; 2) Support for nurses in continuing their studies; 3) Obstacles in increasing formal education; 4) Satisfaction with the manager's efforts to support education improvement; 5) Impact of formal education improvement program. In conclusion, the nursing staff development program must be a priority in efforts to improve service quality. The role and function of the nursing manager are significant so that formal education improvement programs can run well and in line with expectations. The need for managers to provide policies and planning related to improving formal education in the form of SK, Guidelines, Guidelines, SPO, and Programs. Abstract

This study aims to identify and analyze problems in the implementation of freezing documentation using the role and function of freezing management approach, as well as problem-solving options. The research method uses case studies through interviews, observations, problem analysis with fishbone diagrams and then compiling innovations. The results of the study showed that documentation of accommodation in the inpatient room of Hospital X before implementation was 45% and after implementation it became 55%, there was an increase especially in the preparation of the closing diagnosis and protection action plan. In conclusion, the documentation process increased by 10% through the Flipbook SAK (Nursing Care Standards) innovation, this shows that the implementation carried out is quite effective in improving the quality of protection documentation. Abstract

Kualitas tidur yang buruk dapat memengaruhi kejadian excessive daytime sleepiness (EDS). Kejadian EDS merupakan kantuk yang berlebihan pada siang hari. Tujuan: Penelitian ini bertujuan untuk mengidentifikasi hubungan antara kualitas tidur dan kejadian EDS pada perawat. Metode: Penelitian kuantitatif dengan desain analitik korelasional cross sectional. Sampel yang digunakan sebanyak 174 perawat yang bekerja di salah satu rumah sakit di Kota Depok dengan teknik purposive sampling dan kriteria inklusinya perawat yang melaksanakan dinas kerja shift. Kualitas tidur diukur dengan instrumen Pittburgh Sleep Quality Index (PSQI) dan kejadian EDS menggunakan instrumen Epworth Sleepiness Scale (ESS). Uji hipotesis menggunakan uji chi square. Hasil: Sebanyak 98 perawat (56,3%) memiliki kualitas tidur yang buruk dan 24 perawat (13,8%) mengalami EDS. Sebanyak 13,4% perawat yang mengendarai kendaraan sendiri mengalami kejadian EDS. Terdapat hubungan yang signifikan antara kualitas tidur dan kejadian EDS (p = 0,015; OR = 3,4). Perawat yang kualitas tidurnya buruk sebanyak 3,4 kali berisiko untuk mengalami EDS. Diskusi: Kualitas tidur yang buruk pada perawat dapat menyebabkan terjadinya EDS yang berisiko terhadap kecelakaan terutama saat berkendara. Kesimpulan: Perlunya manajemen untuk mengatasi kualitas tidur yang buruk dan meningkatkan kualitas tidur perawat seperti menghentikan penggunaan barang elektronik (misalnya, handphone) 30 menit sebelum tidur, mengurangi kebiasaan konsumsi kopi setiap malam hari terutama saat shift malam, dan memperbaiki gaya hidup dengan tidak merokok, tetapi memakan permen sebagai pengganti rokok. Waktu libur sebaiknya digunakan untuk beristirahat atau melakukan kegiatan yang disukai sebagai koping adaptif sehingga terhindar dari stres. Waktu istirahat di rumah sakit juga perlu dimanfaatkan dengan sebaik-baiknya. Kata Kunci: excessive daytime sleepiness, kualitas tidur, perawat  Sleep Quality and the Occurrence of Excessive Daytime Sleepiness among Hospital Nurses in Depok ABSTRACT Poor sleep quality can affect the occurrence of Excessive Daytime Sleepiness (EDS). EDS refers to excessive daytime drowsiness. Objective: This study aims to identify the correlation between sleep quality and the occurrence of EDS among nurses. Methods: This quantitative study employed a correlational analytic cross-sectional design. A total of 174 nurses working at a hospital in Depok City were selected using purposive sampling, with the inclusion criterion being nurses who work in shifts. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), and the occurrence of EDS was assessed using the Epworth Sleepiness Scale (ESS). The hypothesis was tested using the Chi-Square test. Results: A total of 98 nurses (56.3%) were found to have poor sleep quality, and 24 nurses (13.8%) experienced EDS. Among nurses who commuted by driving themselves, 13.4% experienced EDS. There was a significant correlation between sleep quality and EDS occurrence (p = 0.015; OR = 3.4). Nurses with poor sleep quality were 3.4 times more likely to experience EDS. Discussion: Poor sleep quality among nurses may lead to EDS, which increases the risk of accidents, especially while driving. Conclusion: There is a need for effective management strategies to address poor sleep quality and improve the sleep quality of nurses. Suggested interventions include avoiding the use of electronic devices (e.g., mobile phones) 30 minutes before bedtime, reducing coffee consumption at night, particularly during night shifts, and adopting healthier lifestyles by replacing smoking with alternatives like candy. Rest days should be utilized for relaxation or engaging in enjoyable activities as adaptive coping mechanisms to prevent stress. Abstract

Background: The Covid-19 pandemic has occurred all over the world and has had a significant impact on patient care. The headroom is the first-line nursing manager, who has an essential role in providing care. The headroom has vital role in directing staff to work according to standards during a pandemic. Purpose: To analyze nurses' perceptions of the management function of the headroom during the Covid-19 pandemic. Methods: This study uses a descriptive correlation research design with a cross-sectional approach. The sample consisted of 250 participants. Collecting data using questionnaires on respondents' demographic data and nurse perception of the headroom management function using the Managerial Competencies of Head Nurse Tool instrument Result: The analysis results showed that most respondents were female nurses, had a vocational nursing education, were married, had attended patient safety training, and worked in inpatient rooms. The results of the analysis management function of the headroom in the hospitals are 89.59 (69.59%), staffing and actuating function have the highest presentation of 69.50% from the three hospitals. At the same time, the lowest in the planning function of 43.31%. Conclusion: Based on the results, the function with the lowest implementation is the planning function. Meanwhile, staffing and actuating have the best performance. Non-optimal management function can be because of a lack of management support.

Abstract

Latar belakang supervisi klinis oleh kepala ruangan memiliki dampak signifikan terhadap kualitas kinerja perawat pelaksana, sementara keberhasilan supervisi manajer keperawatan bergantung pada keterampilannya dalam melakukan supervisi. Riset ini bertujuan untuk mengidentifikasi faktor-faktor yang mempengaruhi supervisi klinis kepala ruangan. Metode yang digunakan adalah pendekatan tinjauan sistematis dengan daftar periksa JBI serta pedoman PRISMA (Preferred Reporting Items for Systematic and Meta-Analysis). Hasil penelitian menunjukkan bahwa faktor yang mendukung pelaksanaan supervisi oleh perawat senior meliputi ketersediaan waktu, kesediaan kepala ruangan untuk mendiskusikan hasil supervisi, dukungan dari perawat, keterampilan supervisi, kesiapan, serta dukungan terhadap peran dan kemampuan kepemimpinan yang memadai. Kesimpulan yang didapat ialah keterampilan supervisi klinik kepala ruangan efektif memerlukan pelatihan yang kontinu. Rekomendasi pelatihan keterampilan supervisi klinik dapat dillakukan secara kontinu terutama kepala ruangan baru untuk menjaga dan meningkatkan keterampilan supervisi kepala ruanagan.

Abstract

Background: Nurse burnout can reduce performance and quality of service. Nurses who experience burnout syndrome will lose motivation for work due to prolonged responses to emotional, physical, and mental exhaustion. Purpose: To analyze the effectiveness of mindfulness in reducing burnout in implementing nurses. Method: Systematic review using protocols and rules in accordance with the Preferred Reporting Items for Systematic Review (PRISMA), using databases from Scopus, Proquest, Pubmed, SAGE, and Science Direct in 2019-2024, found 220 articles and 8 selected articles. Results: Mindfulness training reduces fatigue levels, produces lower scores for emotional exhaustion and depersonalization, and is higher for personal achievement. Conclusion: Mindfulness in implementing nurses is an effective effort to choose what nurses will think and maintain the attention needed to maintain awareness of current experiences or in providing nursing care, thereby helping implementing nurses reduce stress due to burnout in the long term. Suggestions: This review has the potential to be biased due to the lack of studies and high heterogeneity. Practical mindfulness techniques for nurses still need further research and should be guided by professionals to be effective.   Keywords: Burnout Syndrome; Mindfulness; Implementing Nurses.   Pendahuluan: Burnout perawat dapat menurunkan kinerja dan mutu layanan. Perawat yang mengalami sindrom burnout akan kehilangan motivasi terhadap pekerjaan karena respon yang berkepanjangan terhadap kelelahan emosional, fisik, dan mental. Tujuan: Untuk menganalisis efektifitas mindfullness dalam menurunkan burnout perawat pelaksana. Metode: Sistematik review menggunakan protokol dan kaidah yang sesuai dengan Preferred Reporting Items for Systematic Review (PRISMA). Pencarian literatur berasal dari database, Scopus, Proquest, Pubmed, SAGE, dan Sciencedirect tahun 2019-2024, ditemukan 227 artikel dan sebanyak 8 artikel terpilih. Hasil: Pelatihan mindfulness mengurangi tingkat kelelahan, menghasilkan skor yang lebih rendah untuk kelelahan emosional dan depersonalisasi, serta lebih tinggi untuk pencapaian pribadi. Simpulan: Mindfulness pada perawat pelaksana merupakan upaya yang efektif untuk memilih apa yang akan perawat pikirkan dan mempertahankan atensi yang dibutuhkan untuk menjaga kesadaran terhadap pengalaman saat ini atau dalam melakukan asuhan keperawatan, sehingga membantu perawat pelaksana menurunkan stress akibat burnout dalam jangka waktu yang lama. Saran: Tinjauan ini berpotensi bias karena minimnya studi dan tingginya heterogenitas. Teknik mindfulness yang praktis bagi perawat masih perlu diteliti lebih lanjut dan sebaiknya dipandu profesional agar efektif.   Kata kunci: Burnout Syndrome; Mindfulness; Perawat Pelaksana.

Abstract

Background: The use of information technology is increasingly widespread in the health sector, especially for nurses. The intensity of the use of this technology has created a new phenomenon called technostress. The European Risk Observatory categorizes technostress as an increasing occupational health risk caused by the negative psychosocial effects of the use of information technology. These problems are correlated with decreased job satisfaction, productivity, and work-life balance that have the potential to threaten patient safety. Purpose: To analyze the dimensions, measurement instruments, and research results of technostress in nurses. Method: A systematic review was conducted by searching the literature related to technostress in nurses using PubMed, ScienceDirect, EBSCO, Proquest, and Scopus databases. Studies that met the inclusion criteria were assessed using the JBI Critical Appraisal Checklist for Cross-sectional Studies. Results: A total of seven relevant studies were identified with five high-quality studies and two moderate-quality studies. Five main dimensions of technostress were identified: techno-overload, techno-invasion, techno-complexity, techno-insecurity, and techno-uncertainty. Six measurement instruments were identified with moderate levels of technostress in nurses. Conclusion: Based on the results of the study review, six main instruments were obtained that were used to measure technostress in nurses and five main dimensions of technostress in nurses, including techno-overload, techno-invasion, techno-complexity, techno-insecurity, and techno-uncertainty, as well as moderate levels of technostress to impacts that include emotional exhaustion, work-privacy conflict, burnout, decreased job satisfaction, desire to move, and physical and mental health problems.   Keywords: Measurement Instruments; Nurses; Technostress.   Pendahuluan: Penggunaan teknologi informasi semakin meluas di sektor kesehatan, terutama pada perawat. Intensitas penggunaan teknologi ini telah menciptakan fenomena baru yang disebut technostress. European Risk Observatory mengategorikan technostress sebagai risiko kesehatan kerja yang terus meningkat yang diakibatkan efek psikososial negatif dari penggunaan teknologi informasi. Permasalahan tersebutlah yang berkorelasi dengan penurunan kepuasan kerja, produktivitas, dan keseimbangan kehidupan kerja yang berpotensi mengancam keselamatan pasien. Tujuan: Untuk menganalisis technostress  pada perawat dan instrumen pengukurannya. Metode: Penelitian tinjauan sistematis terkait technostress pada perawat, pencarian literatur berasal dari database antara lain PubMed, ScienceDirect, EBSCO, Proquest, dan Scopus. Studi yang memenuhi kriteria inklusi dinilai menggunakan JBI Critical Appraisal Checklist for Cross-sectional Studies. Hasil: Sebanyak tujuh studi relevan diidentifikasi dengan lima studi berkualitas tinggi dan dua studi berkualitas sedang. Lima dimensi utama technostress teridentifikasi: techno-overload, techno-invasion, techno-complexity, techno-insecurity, dan techno-uncertainty. Enam instrumen pengukuran teridentifikasi dengan tingkat technostress  moderat pada perawat. Simpulan: Berdasarkan hasil tinjauan studi, diperoleh sebanyak enam instrumen utama yang digunakan untuk mengukur technostress pada perawat dan lima dimensi utama technostress pada perawat yang meliputi, techno-overload, techno-invasion, techno-complexity, techno-insecurity, dan techno-uncertainty, serta tingkat technostress  moderat hingga dampak yang meliputi kelelahan emosional, konflik kerja-privasi, burnout, penurunan kepuasan kerja, keinginan berpindah, serta masalah kesehatan fisik dan mental.   Kata Kunci: Instrumen Pengukuran; Perawat; Technostress.

Abstract

The aging process affects the immune system, making the elderly more vulnerable to bacterial and viral infections. Urinary tract infection (UTI) is the second most common infection among geriatric patients. UTIs caused by urinary catheter use, known as Catheter Acquired Urinary Tract Infections (CAUTI), are the leading cause of nosocomial infections, with 80% linked to catheter use. Preventing catheter use beyond 72 hours can reduce CAUTI incidence. The use of Nurse Driven Protocols (NDP) is one of the interventions recommended by the CDC to reduce catheter-related risk factors. This articles aiming to discuss the impact of NDP implementation on the incidence of CAUTI in geriatric patients. This study was conducted using the PRISMA checklist. Literature searches were conducted through several databases, including ProQuest. Science Direct, Scopus, and ClinicalKey Nursing, within the last five years (2020–2024). From 110 identified articles, five met the inclusion criteria, and two were analyzed in depth. The implementation of Nurse Driven Protocols (NDP) significantly reduced the incidence of CAUTI by ensuring catheters are only used when absolutely necessary and removed as soon as no longer indicated. Routine 24-hour assessments enable nurse to independently evaluate catheter use based on clear clinical criteri , such as the HOUDINI system.
