Abstract

Introduction:: Patient-centered care requires medical personnel to address not only physical illnesses but also the psychosocial well-being of patients. older adultsOlder adults often have medically complex conditions, including one or more chronic diseases, requiring the use of multiple medications (polypharmacy). Polypharmacy is a common problem among older adults, with a reported prevalence of 30% to 80%. Many prescribed medications can lead to xerostomia and hyposalivation, which negatively impact the quality of life , including Oral Health-Related Quality of Life (OHRQoL). This systematic review aims to determine the impact of polypharmacy on OHRQoL in older adults and explore strategies to optimize OHRQoL in this population. Method: The dataset of articles concerning polypharmacy and OHRQoL in older adults was compiled from Google Scholar, PubMed, Semantic, and OpenAlex. The search encompassed publication from 2019 to 2024. Five articles were selected for in-depth analyses, and variations in methodologies used by the researchers in these selected studies were identified and considered. Results: Patients with polypharmacy hyposalivation exhibited significantly higher scores of the Summated Xerostomia Inventory questionnaire (SXI-PL) (8.60 ± 2.56) and the Oral Health Impact Profile-14 (OHIP-14sp) (16.0 ± 15.8). A statistically significant association was found between hyposalivation and both SXI-PL and OHIP-14sp scores (p < 0.05). Elderly individuals on continuous medication demonstrated increased odds of self-reported xerostomia (OR: 2.3; 95% CI: 1.19-4.67; P = 0.009). Conclusion: This study demonstrates an association between polypharmacy and decreased oral health-related quality of life (OHRQoL) in older adults.

Abstract

Dalam rangka menjaga suistanibilitas JKN utamanya kecukupan dana JKN terhadap pembiayaan kesehatan bagi peserta JKN, maka kenaikan pembiayaan pelayanan kesehatan dengan sistem INA CBG yang terus meningkat dari tahun ke tahun tersebut harus dilakukan evaluasi dan monitoring secara berkala oleh seluruh ekosistem yang terlibat dalam JKN salah satunya adalah Rumah Sakit dan BPJS Kesehatan. Evaluasi tersebut salah satunya dapat dilakukan dengan Case Mix Index. Hasil analisa Case Mix Indeks tersebut sebagai evaluasi yang penting artinya bukan hanya bagi rumah sakit namun juga BPJS Kesehatan. Adapun hasil analisa Case Mix Indeks di Provinsi Banten Tahun 2019 – 2022 didapatkan hasil yaitu terdapat perbedaan signifikan antara kelas RS dimana RS dengan tingkatan lebih tinggi memiliki nilai CMI yang lebih tinggi dari pada kelas dibawahnya. Nilai CMI rawat jalan di provinsi Banten tahun 2019 - 2022 dipengaruhi signifikan oleh proporsi kunjungan pasien lanjut usia, segmen kepesertaan PBI , segmen kepesertaan Non PBI hak perawatan kelas tiga dan proporsi kunjungan pasien hak perawatan kelas satu. Sedangkan Nilai CMI rawat inap di provinsi Banten tahun 2019 - 2022 dipengaruhi signifikan oleh proporsi kunjungan pasien lanjut usia

Abstract

Purpose: The study aims to calculate the rational outpatient tariff based on unit cost analysis, ability and willingness to pay. Method: The research is an observational study with a qualitative approach. Secondary data are obtained retrospectively to calculate unit costs and tariffs. Primary data is collected through questionnaires to see the ability and willingness to pay (WTP). Result: The calculation using the step-down method obtained the unit cost for general practice IDR 79,337, dental care IDR 151,635, psychologist consultation IDR 115,283, and fitness center IDR 236,555. The respondent's ATP value is IDR 138,808, with an average examination fee of IDR 56,093. When coupled with an improvement in service and facility quality, 58.1% of respondents agreed to a 10% rate increase. With a 20% rate increase, the willingness to pay decreases to 40.6%. In the bivariate analysis using the chi-square test, the variables significantly affecting WTP are patient perceptions of the suitability of service costs (p = 0.000). In contrast, age, gender, occupation, education, income, number of family members, and insurance ownership do not significantly influence WTP. Conclusion: The service rate is lower than the unit cost calculation. By looking at the ability and willingness to pay, it is possible to evaluate the tariff.

Abstract

ABSTRACTBackground: The utilization of dental prosthesis services in Indonesia is rare, compared to other dental and oral health services. The use of prosthesis services in Bali is still relatively low compared to other parts of Western Indonesia. Based on data from BPJS Kesehatan Denpasar Branch Office, the demand for dental prostheses services by participants in the three working areas of BPJS Kesehatan Denpasar Branch Office is still low. Method: This research was conducted using quantitative research. The population were all participants providers of FKTP BPJS Kesehatan Badung Regency, and samples were taken through purposive sampling. Quantitative data were analyzed using binary logistic regression.Result: The results that the predisposing factors for JKN program participants (income, knowledge of dental prostheses, knowledge of JKN dental prosthesis services, perceptions of service quality, and perceptions of cost); enabling factor (access), and the need factor (the perception of pain when losing a tooth) contributions have a significant positive effect on the utilization rate of JKN dental prostheses. The predisposing factors (age and education) and enabling factor (social support) do not have a significant effect on the utilization rate of JKN dental prostheses. Conclusion: The results that the predisposing factors for JKN program participants contributions have a significant positive effect on the utilization rate of JKN dental prostheses.
