Abstract

ABSTRACTDiabetes Mellitus is a chronic disease whose prevalence continues to increase. WHO predicts that diabetes will be 7 major diseases that will cause the death in 2030. Many people with diabetes are not aware of  it so that they get complications. The increasing number of people with diabetes mellitus who develop complications because of the bad controlling of blood sugar levels  and the lack of support from family. The Complications were observed in this study is microvascular complications. This study aims to analyze between family support and control blood sugar levels with the incidence of microvascular complications in the patients with symptoms of diabetes mellitus in the Community Health Center Jagir Surabaya.This research uses analytic observational study sample of patients with diabetes mellitus who had diabetes mellitus ≥ 5 years as many as 34 samples. The Sampling was done by the simple random sampling, while the data analysis was done by the Chi Square test.The results showed that there was no relationship between family support with symptoms of microvascular complications (p=0.069), but there is a relationship between the control of blood sugar levels with symptoms of microvascular complications (p=0.002).The Microvascular symptoms that are often experienced by people with diabetes mellitus in the Community Health Center Jagir is symptomatic neuropathy complications . The proportion of patients with diabetes mellitus who received family support at 55.9%, while  the patients with diabetes mellitus who control the blood sugar levels well totaling 17 respondents. It is expected that people with diabetes mellitus to control eating diet and regular exercise. Keywords : Family support, blood sugar levels control, symptoms of microvascular complications.

Abstract

ABSTRACTTuberculosis in children can not be separated from the history of contact with smear-positive tuberculosis patients as a source of infection. The aims of this study was to identify and analyze the risks tuberculin test results among children who had contact with patients with smear-positive tuberculosis in health center Bangil and health center Raci Pasuruan. The design of this study was cross sectional using systematik random sampling technique. Sample were drown from children with history of contacts with smear positive tuberculosis patients during 2012. This studies was from January until Juni 2013. The independent variables included age, sex, BCG immunization status, nutritional status, the average duration of exposure per day and residential density, the dependent variable was the tuberculin test results. It was found that from the study the results that a positive tuberculin test result was 28.57%. There was a significant relationship between age, nutritional status, the average duration of exposure per day, residential density of the tuberculin test, and there was no significant relationship between sex and status of BCG immunization results and tuberculin test results. Male and a dense residential density increases the risk of a positive tuberculin test results. While a good nutritional status is a protective factor for tuberculin test result positive.Keywords: tuberculin test, children, history of contacts of smear positive

Abstract

ABSTRACTThe increasing of life expectancy led to a growing number of elderly. There are many thing that occurs caused by aging process, one of them is decreases of body function. Decreasing body function can cause variety of health problems. The problem that may occurs are malnutrition, chronic disease, and lack of independence in performing daily activities. All of them are risk factor of mental emotional disorder. This study was conducted to determined the relationship between mental emotional disorders in the Elderly Care Unit Jombang in Kediri. This research was analytical study and used cross sectional. Dependent variable was mental emotional disorders. Independent variables were BMI, physical independence and characteristic. Interview was conducted to obtain information about variables in this study. Data analyzed by chi square and pearson correlation. The prevalence of mental disorders found in this study was 23,4%. Respondents who didn’t have physical independence were 4,3%. Respondents who had body mass index that wasn’t normal is 26,2%. The variables showed correlation only variable physical independence and mental emotional disorders (p = 0,008). While the body mass index and characteristic were not showed correlation with mental emotional disorders. The conclusion of this study was the elderly at risk for emotional suffering mental disorders. Physical independence associated with mental disorders in elderly emotional. So the need for more attention especially from the psychological to the elderly who do not have physical independence.Keywords: risk factors, BMI, physical independence, mental emotional                           disorder, elderly

Abstract

ABSTRACTChildren are the ones who are at risk of contracting tuberculosis (TB), but so far it has not been a priority. The proportion of tuberculosis in children case detection remains low among all tuberculosis cases. Bojonegoro is one among regencies in East Java which was appointed as the operational trial tuberculin test. The case detection begins with screening of tuberculosis suspects. Screening of tuberculosis suspected children by health centers officer will determine how much the cases of tuberculosis in children are found in health centers. Health officers at the health centers became the most dominant factor in screening of tuberculosis suspected children. Training about tuberculosis in children was done in 16 health centers among 36 health centers in Bojonegoro. This study aimed to analyze the effect of training to the screening of tuberculosis suspected children by health center officers in Bojonegoro. This study is observational analytic with cross-sectional design. The sampling technique is simple random sampling with respondents consisting of 29 doctors and 29 nurses who are responsible for tuberculosis programs in 29 health centers in Bojonegoro. The variables studied were the training and screening of tuberculosis suspected children. The results showed that screening of tuberculosis suspected children was inappropriate from the target (75.9%) and officers who have not attended the training of tuberculosis in children were 58.6%. Logistic regression test showed that there was effect of training to the screening of tuberculosis suspected children (p = 0.019) and prevalence ratio showed that health officers who was attended the training of tuberculosis in children were able to do the screening of tuberculosis suspected children well 8,50 times greater than those who didn’t. The training for the officers who have not attended the training of tuberculosis in children needs to be done, so that screening of tuberculosis suspected children in Bojonegoro can be optimally implemented.Keywords: screening, suspected, training, tuberculosis, tuberculosis in children

Abstract

ABSTRACTDiphtheria is caused by Corynebacterium diphteriae. Children aged 2-10 years are a vulnerable group contracting. Diphteri crawl probable case is difficult, so there are still children who died from diphteri every year. Cadres holds a very important role in the field of public health services so that its presence should be maintained. Level of activity the role of cadres is very dependent on the LKMD role as a manager, and also the public community. This study aims to analyze the factors which associated with a cadres active role in detecting diphteri probable case in Sidoarjo health center. Type of this study is an observational analytic with cross sectional design. Population of this research was all cadres in Sidoarjo health center as much as 516 people. Samples are taken by simple random sampling of 103 respondents. The data was collected using a questionnaire research instruments (questionnaire). The variables of this study is the role of an active cadre to crawl probable case of diphtheria, internal factors (knowledge, attitudes of cadre), external factors (community support, support for community health centers, training of cadres). Techniques of data analysis was done by logistic regression.The results showed 75% of cadres in Sidoarjo health center is active in detecting diphteri probable case. Statistical analysis showed that the internal factors that affect the active role of cadres is the attitude of p = 0.024, external factors are affecting the support of community leaders with p = 0.022, p = 0.042 support community health centers, training of cadres p = 0.041. The advice that can be given is expected that the cooperation between the cadres, community leaders, health center personnel, in support of efforts to increase the capacity and activity of cadres in helping in the field of public health services.Keywords: diphtheria, crawl probable case, cadre, activeness, PHC in Sidoarjo

Abstract

The diabetics have risk of pulmonary tuberculosis It caused by lacking of immune system and malnutrition, eventhough the diabetics’ nutritional status commonly is normal or overweight. This research aimed to analyze risk of nutritional status and blood sugar to patients of diabetes mellitus with pulmonary tuberculosis. The study was observational analytic with case control design. There were 20 samples in each case and control located in Sawahan Sub-district, Surabaya. The data collection used questionnaire and medical record of participants from the community health care. The data was analysed using Pearson test correlation and odd ratio. The result showed no correlation between body mass index and blood sugar of both random (p=0.138) and fasting (p=0.166). The oberweight diabetics are much more likely to get pulmonary tuberculosis 1.3 times compared to normal diabetics (95% CI= 0.313<OR<5.393), while the odd ratio of pulmonary tuberculosis in underweight diabetics is estimated to be 1.2 times compared to diabetics who are normal (95% CI= 0,121<OR<11,865). The diabetics who have random blood sugar level of ?200 mg/dL are 0.46 times to have pulmonary tuberculosis compared to blood sugar level of <200 mg/dL (95% CI=0.111<OR<1.94). The diabetics who have fasting blood sugar level of ?126 mg/dL showed effect 2.43 times to be likely get pulmonary tuberculosis either they have <126 mg/dL 95% CI= 0.51<OR<11.51). There was no relationship between blood sugar level, nutritional status, and pulmonary tuberculosis on patients with type 2 diabetes mellitus.

Abstract

Introduction:Cervical cancer is cancer that affectthe cervix and mostly (99,7%) caused by oncogenic Human Papilloma Virus (HPV). This cancer affect women who is multipara and lack of maintaining of their reproductive health. Cervical cancer cases in East Java Province in 2017 was still high and there were about 3,013 cases. This cervical cancer requiredcontrol and prevention such as screening, finding and following-up the cervical cancer cases, increasing the life quality, and decreasing mortality rate. One of cheap and quick early detection methods for screening of cervical cancer was utilizing of the VIA test method (Visual Inspection of Acetic Acid). Objective:This research aimed to analyze and solve the problem on early detection program for cervical cancer through IVA test method in Health Office of East Java Province- Indonesia. Method:The data of cervical cancer program in 2017 was collected. The data of early detection program for cervical cancer through VIA test method was analyzed and interview was conducted on health program officer and manager of the cervical cancer program to acquire the health program issues. Afterwards, the priority of the issues found on cercival cancer program was determinedusingthe Capability Accessibility Readiness Leverage (CARL) method and the alternative problem solving was determined using NGT (Nominal Group Technique) method. Conclusion:In order to achieve the early detection of cervical cancer case through VIA test method targets/indicators, Health Office of East Java Province needed to conduct network and collaboration with all of related units on the existing cervical cancer program. Health Office of East Java Province also needed to take role actively in increasing the motivation of District Health Office so that they collected and reportedthe record of non-communicable disease surveillance datawell and on time. Abstract

Hypertension cases in Indonesia have been increasing. Hypertension can be caused by controllable factors such as consumption patterns and nutritional status. This study was aimed to analyze the risk of fast food and instant noodle consumption patterns, as well as the risk of nutritional status on the incidence of hypertension in Indonesian migrant communities. This study used a cross-sectional design with 6,928 respondents. This study involved secondary data from the 5th batch of the Indonesian Family Life Survey (IFLS). The results showed that the majority of respondents were female (51.24%), adult age category (58.63%), frequent fast food consumption patterns (89.42%), frequent instant noodle consumption patterns (56.63%), and normal nutritional status (72.42%). The subjects with frequent consumption of fast food (OR = 1.49; CI = 1.041–2.130) and instant noodle (OR = 1.25; CI = 1.030–1.520) as well as overweight (OR = 3.90; CI = 3.154–4,838) and obesity (OR = 6.75; CI = 5,110–8,935) have higher risk to experience hypertension. Based on these results, it is recommended to migrants to maintain their consumption patterns by limiting their fast food and instant noodles intake and also to maintain the nutritional status to prevent hypertension
