Abstract

Background: Acute coronary syndrome (ACS) is a form of life-threatening coronary heart disease. Interestingly, this entity has the possibility to recurrence with prevalence reaches 21-30% in a year. Purpose: This study aims to analyse risk factors associated with recurrent ACS incident. Methods: The analytic observation research with the case-control design was applied in this present study. Furthermore, this research was conducted at the Dr. Mohamad Soewandhie General Hospital, Surabaya. This study carried from February to July 2018. The samples used in this study cover 43 cases and 43 controls in the consecutive admission to the ACS patients who came to the cardiac clinic of the Dr. Mohamad Soewandhie General Hospital, Surabaya that meets the research criteria. On the other hand, bivariable analysis was performed using simple logistic regression and multivariable analysis was performed using multiple logistic regression. This study showed that the most influential risk factor for ACS recurrent. Results: incident were including control of Low-Density Lipoprotein Cholesterol (LDL-C) ≥ 100 mg/dL (p= 0.03; adjusted OR= 3.35; 95% CI= 1.16 < OR < 9.68), irregular exercise schedule (p < 0.01; adjusted OR= 9.15; 95% CI= 2.83 <OR <29.58), and smoking history (p= 0.02; adjusted OR= 4.07; 95% CI= 1.29 <OR <12.84). Conclusion: The control of LDL Cholesterol levels below 100 mg/dL, regular exercise, and avoid smoking are beneficial for people with ACS to reduce the risk of recurrent ACS incident.

Abstract

Background: The phlebitis incidence was found as the fourth highest infection in hospitalized patients. Indonesia has contributed in phlebitis incidence as the fifth highest in the world wide after India, Iran, Malaysia, and Philippines. Purpose: This study was aimed to analyze the influence of patients’ characteristic with intravena catheter to phlebitis incidence. Methods: The study design was case control study with 45 respondents in case group (diagnosed with phlebitis) and control (undiagnosed with phlebitis) group in public hospital Haji Surabaya from January to April 2017. The independent variables in this study were age, sex, nutrioninal status, hypertension history, and diabetes mellitus history. Meanwhile, the dependent variable was phlebitis incidence. Data were analyzed with multiple logistic regression. Results: Phlebitis was influenced by patients’ characteristics namely age (p = 0,01; OR = 9,63; 95% CI =3,67 < OR < 25,25), sex (p = 0,01; OR = 4,84; 95% CI =1,85 < OR < 12,66), nutritional status (p = 0,01; OR = 4,01; 95% CI = 1,69 < OR < 9,66), hypertension history (p = 0,01; OR = 6,18; 95% CI = 2,47 < OR < 15,51), and diabetes mellitus history (p = 0.01; OR = 17.88; 95% CI = 6,05 < OR < 52,85). Conclusion: Patients’ characteristics had influence on phlebitis incidence such as age, sex, nutritional status, hypertension history, and diabetes mellitus history.

Abstract

Background: Indonesia is one of countries with a quite high incidence of tuberculosis. One of the regions which has issue of tuberculosis incidence is Bali Province with a case notification rate that tends to increase in the last three years so that it has an impact on increasing the risk of disease transmission. Purpose: This research aims to identify the risk factor based on the environment/spatial incidence of the tuberculosis in Bali Province. Methods: This research used ecological study design through secondary data obtained from the Health Office of Bali Province, Indonesian Statistics of Bali Province, and Environmental Office of Bali Province. The dependent variable was tuberculosis incidence, while the independent variable was the level of PM10, population density, poverty percentage, healthy house percentage, percentage of Clean and Healthy Lifestyle (PHBS), ratio of healthcare facilities with the community. Results: The statistical model was obtained in the form of Spatial Error Model (SEM) with model ŷi=1612,57+ 0,96 * level of PM10 + 0,04 * population density - 2,56 * poverty - 0,58 * Healthy house - 3,099 * PHBS - 0,006 * health care facility, where μi  = 0,90  . Conclusion: The risk factor spatially affected the tuberculosis incidence in Bali Province, which were the factors of level of PM10, population density, poverty percentage, healthy house percentage, percentage of PHBS, and ratio of healthcare facilities with the community.

Abstract

Background: Scabies is found in crowded residential areas such as orphanages, dormitories, prisons, and Islamic boarding schools. In 2017, it was reported that scabies was among the 15 most common types of skin infections in Sumenep District, which reported 12,229 cases of scabies. Purpose: This study aims to analyze the relationship between the perception of individual susceptibility and barriers to treatment action in scabies patients at the Nasy’atul Muta’alimin Islamic Boarding School, Sumenep District, East Java. Methods: This research was an observational analytic study using a case-control design. The study population was patients with scabies in Nasy’atul Mutaalimin Islamic Boarding School, Sumenep District, East Java. The sample size was 70 people, consisting of 35 scabies patients who did not seek treatment as a case group and 35 scabies patients who did seek treatment as a control group. The sampling method used was simple random sampling. The two independent variables studied were the perception of individual susceptibility and barriers to treatment, while the dependent variable was treatment measures. The data analysis techniques employed were univariate and bivariate analysis. Results: This study shows that there are variables that have a relationship (p < 0.05) with the perception of individual susceptibility (OR = 99.00 95% CI 17.85 < OR < 548.86) and perceived barriers (OR = 29.00 95%, CI 7.968 < OR < 105.55). Conclusion: There is a relationship between the perception of individual susceptibility and barriers to treatment action in scabies patients.

Abstract

Background: In general, the entire population is vulnerable to COVID-19, but epidemiological research shows that the elderly and people with comorbid are more susceptible to COVID-19 and has a high risk of experiencing poor outcomes and death if infected with COVID-19. Purpose: This study aims to identify comorbidities with a high risk of death common among COVID-19 patients in China and the United States to provide learning for Indonesia in COVID-19 management. Methods: The literature review method was done by searching relevant articles through Google Scholar and ScienceDirect. Inclusion criteria are all original research and case series containing epidemiological information and comorbidities in COVID-19 patients aged >19 years with full text and open access. Exclusion criteria are study COVID-19 concerning children and adolescents (≤19 years) and study without comorbidities information. There is no language selection because all articles found are in English. Results: There are eight articles suitable for the criteria. Four articles were research conducted in China, and four articles were done in the United States. The articles use case series, cohort, and cross-sectional methods. Conclusion: Common comorbidities among COVID-19 patients are hypertension, diabetes, and cardiovascular disease in China and the United States. A more significant number of comorbidities also correlated with a higher risk of death.

Abstract

Background: Adolescent girls' anemia is a public health challenge. Anemia has an impact on cognitive abilities to reduce intelligence and results in adolescent achievement is down.Objective: The purpose of this study was to analyze the determinants of anemia in early adolescent girls in Kendari City.Methods: This study used a cross-sectional design with a sample of 97 students. Data on parental characteristics, breakfast habits, nutritional status, menstrual status, and history of infectious diseases were collected by interview using a questionnaire. Data on nutrient intake (iron, protein, vitamin C) obtained through the 2x24 hour recall method then compared to the Indonesian standard (AKG). Hemoglobin level was measured using the EasyTouch heamoglobin meter device. Data analysis was done by chi-square analysis and multiple logistic regression analysis.Results: The results showed 28.9% of adolescent girls had anemia. The results of chi-square analysis obtained that there was a relationship between the incidence of anemia on mother's education (p=0.010,), parental income (p=0.017), iron intake (p=0,000), protein intake (p=0,000), vitamin C intake (p=0.023), breakfast habits (p=0.006), menstrual status (p=0.007) and history of infection diseases  (p=0,000). While the results of multiple logistic regression analysis obtained that there was a relationship between iron intake (p=0.005,OR=3.009), protein intake (p=0.007,OR=2.012), vitamin C intake (p=0.038,OR=1.167), menstrual status (p=0.002,OR=3.181) with anemia.Conclusion: The dominant determinant of anemia in early adolescent girls in Kendari City is menstrual status. It is recommended to intervene in risk factors (mother's education, parental income, iron intake, protein, vitamin C, breakfast habits, menstrual status and history of infectious disease) to reduce the prevalence of anemia among adolescent girls.

Abstract

Diabetes melitus (DM) merupakan salah satu penyakit degeneratif, yaitu penyakit yang mengakibatkan jaringan atau organ tubuh dapat memburuk dari waktu ke waktu. Salah satu provinsi di Indonesia dengan prevalensi diabetes tertinggi diduduki oleh provinsi Jawa Timur. Upaya mencegah penyakit diabetes melitus harus disiapkan sejak dini. Pengetahuan terkait penyakit diabetes melitus hendaknya telah ditanamkan sejak bangku sekolah. Ketika seseorang memiliki pengetahuan yang cukup maka akan membentuk sikap dan tindakan yang baik pula. Penelitian ini bertujuan untuk mempelajari hubungan sikap dan pengetahuan mahasiswa kesehatan masyarakat dengan tindakan terkait pencegahan penyakit diabetes melitus. Jenis penelitian ini adalah penelitian deskriptif analitik dengan desain penelitian cross sectional. Populasi penelitian yaitu seluruh mahasiswa SI Ilmu Kesehatan Masyarakat yang berjumlah 870 mahasiswa. Teknik pengambilan sampel yaitu total sampling/sampel jenuh. Sehingga besar sampel penelitian yaitu 870 mahasiswa. Penelitian ini menggunakan analisis prevalence ratio (PR) untuk mengetahui kuat hubungan antar variabel dan besar risiko. Hasil penelitian responden dengan tingkat pengetahuan tinggi, memiliki peluang untuk melakukan tindakan yang baik sebesar 1,114 (95% CI = 0,888 – 1,399) kali lebih besar dibandingkan dengan responden yang memiliki pengetahuan rendah sedangkan responden yang sikpanya positif memiliki peluang untuk melakukan tindakan yang baik sebesar 0,597 (95% CI = 0,490 – 0,726) kali lebih besar dibandingkan dengan orang yang memiliki sikap negatif. Saran dari peneliti ini yaitu membuat poster yang berisi tentang pengetahuan mengenai tanda – tanda terkena penyakit DM dan, membuat iklan layanan masyarakat tentang kepatuhan dan kesadaran mahasiswa mengenai kebiasaan pola hidup sehat, terutama pembatasan makanan tinggi kadar gula, pembatasan mengonsumsi makanan siap saji, melakukan olahraga secara tepat dan teratur, serta waktu tidur yang cukup.

Abstract

Dukungan keluarga diperlukan untuk meningkatkan kualitas hidup pada pasien kusta. Penelitian ini bertujuan mengidentifikasi hubungan antara dukungan keluarga ditinjau dari empat dimensi (emosional, penilaian, instrumental dan informasional) terhadap kualitas hidup penderita kusta kecacatan tingkat 2 di UPTD Liponsos Kusta Babat Jerawat Surabaya. Desain dalam penelitian ini analitik cross sectional. Wawancara dilaksanakan pada 54 penderita kusta kecacatan tingkat 2. Subjek ditarik dari populasi dengan cara simple random sampling. Wawancara mendalam dilakukan untuk mendapatkan informasi yang lebih mendalam tentang variabel – variabel yang diteliti. Variabel bebas penelitian adalah dukungan keluarga. Uji analisa data deskriptif menggunakan tabulasi silang antara empat dimensi (emosional, penilaian, instrumental dan informasional) dukungan keluarga terhadap masing – masing domain kualitas hidup. Uji analisa data inferensi menggunakan uji regresi linier antara dukungan keluarga secara umum terhadap kualitas hidup. Hasil penelitian didapatkan ada hubungan dukungan keluarga terhadap kualitas hidup penderita kusta kecacatan tingkat 2 di UPTD Liponsos Kusta Babat Jerawat Surabaya (p value 0,001, alfa: 0,05). Hasil tabulasi silang dari empat dukungan keluarga didapatkan variabel dukungan emosional yang rendah memiliki persentase terbesar terhadap domain kualitas hidup. Kesimpulan yang dapat ditarik adalah ada hubungan dukungan keluarga terhadap kualitas hidup penderita kusta kecacatan tingkat 2 di UPTD Liponsos Kusta Babat Jerawat Surabaya. Penderita kusta kecacatan tingkat 2 di UPTD Liponsos Kusta Babat Jerawat Surabaya kurang mendapatkan dukungan keluarga, sehingga kualitas hidup yang didapatkan rendah. Anggota keluarga penderita kusta dan masyarakat sekitar hendaknya meningkatkan dukungannya terhadap penderita kusta.

Abstract

Healthcare Associated Infection (HAIs) saat ini menjadi isu global dalam bidang kesehatan. Di Indonesia, angka kejadian infeksi terkait pelayanan kesehatan cukup tinggi. Pasien hemodialisis berisiko tinggi terkena infeksi. Rantai utama penularan infeksi adalah dari tangan petugas kesehatan. Hand hygiene penting untuk mencegah infeksi. Namun, kepatuhan hand hygiene tenaga kesehatan masih rendah. Studi ini bertujuan guna mendapatkan gambaran kepatuhan hand hygiene perawat, mengidentifikasi akar penyebab masalah kepatuhan dan menentukan solusi untuk meningkatkan kepatuhan tersebut. Penelitian ini bersifat deskriptif dengan studi observasional. Populasi penelitiannya semua perawat di Unit Dialisis RSU Dr. Soetomo Surabaya. Besar sampel yaitu 200 kesempatan hand hygiene. Identifikasi akar masalah menggunakan diagram fishbone. Penentuan alternatif solusi menggunakan metode Capability, Accessibility, Readiness, Leverage. Hasil pengamatan memperlihatkan kepatuhan perawat dalam melakukan hand hygiene sebesar 28,5%. Angka kepatuhan perawat terendah adalah pada saat indikasi sebelum dan sesudah menyentuh pasien. Akar penyebab masalah kurangnya kepatuhan perawat terhadap hand hygiene yaitu minimnya pengetahuan dan kesadaran mengenai hand hygiene serta kinerja tim IPCLN yang kurang maksimal. Sosialisasi hand hygiene secara kreatif dan berkelanjutan serta peningkatan kinerja Tim Pencegahan dan Pengendalian Infeksi di unit Dialisis diharapkan dapat meningkatkan angka kepatuhan hand hygiene.

Abstract

This study aims to see the relationship between attitude, distance to health services, and parental support with the willingness to the HPV vaccine in adolescents. This research method is cross-sectional. The results of this study show that attitude (p=0.000) [PR= 2.234, 95% CI 1.512-3.300] and parental support (p=0.028) [PR=1.549, 95% CI 1.041-2.306] have a significant relationship with willingness to vaccinate HPV. Meanwhile, the distance from home to health services (p=0.151) [PR=0.729, 95% CI 0.464-1.145] had no relationship with willingness to vaccinate against HPV. In conclusion, adolescents with good parental attitudes and support are more likely to be willing to vaccinate against HPV.
