Abstract

The high number of medical errors, especially in medication administration errors (MAE) in the last few decades that have occurred in hospitals in developed and developing countries makes patient safety an important issue. This requires the hospital to take steps that prioritize patient safety by focusing on preventive measures, so as to reduce the risk of an MAE. The writing of this article of literature review aims to explain the determinants of MAE in hospitals and their prevention efforts through preventive measures, so that patient safety standards in hospitals (zero defects) can be achieved. The design of the article search in this literature study was carried out through Google scholar, JAKI, and PSNet with the keywords of patient safety incident, medication administration error, contributing factor of MAEs, and determinant factor of MAEs. Based on 13 articles that have been obtained in accordance with the criteria, there are 38 determinant factors in MAE which are grouped into three categories, namely ineffective communication factors, work environment factors, and human factors. Various preventive efforts that can be done to prevent MAEs include: implementing crew resource management, clarifying the chain of command, using the communication form of SBAR, designing an ergonomic workplace, implementing Patient Advocacy Reporting System (PARS), providing training and education for health workers, and setting work schedules that do not exceed workload. Implementation of effective MAE preventive measures can reduce the number of MAEs in the hospital directly.

Abstract

The selection of input and output variables usually pose a problem when carrying out efficiency assessment in hospitals. Data Envelopment Analysis (DEA) is an instrument that is used to calculate the efficiency of a hospital using some inputs and outputs. Therefore, this study aims to identify the most frequently used hospital inputs and outputs from an existing paper,, in order to assist the hospital management staffs in choosing the relevant variables that can represent available inputs, are easily accessible, and need improvement. It was conducted using keywords such as “hospital efficiency” and “DEA for hospital” to search for peer-reviewed journals in the PubMed and Open Knowledge Maps from the year 2014-2020. From, the 586 articles, 54 samples were obtained from the about 5-3504 hospitals which were analyzed from 23 countries. The results showed that, the five most used inputs were the number of beds, medical personnel, non-medical staff, medical technician staff and operational costs, while the most used outputs were number of inpatients, surgeries, emergency visits, outpatient service, and days of inpatients. These variables are often used for accessing the efficiency of hospitals in the DEA application.

Abstract

The implementation of safe and quality care with attention to patient safety, requires organization’s effort to create and cultivating patient safety culture. The purpose of this article was to map the instruments used in measuring patient safety culture in healthcare organizations. The method used integrated literature review from various sources of research articles published from 2015 to 2020. The article included if it was available in full text and open access as well as articles described the instruments of patient safety culture or measurement of patient safety culture using one of the instruments of measurement of patient safety culture. The results of the literature review unravel the findings of three instruments such as HSOPSC (Hospital Survey on Patient Safety Culture), MaPSaF (Manchester Patient Safety Assessment Framework) and SAQ (Safety Attitudes Questionnaire). We concluded all three instruments contained four dimensions of patient safety culture, namely open culture, just culture, reporting culture and learning culture and were widely used to measure patient safety culture in hospitals, primary health facilities and other health facilities.

Abstract

Diberlakukannya kebijakan lockdown di beberapa negara karena Pandemi COVID-19 saat ini menyebabkan sistem pelayanan kesehatan ibu dan anak terganggu. Salah satunya dalam hal pembatasan kunjungan ibu hamil untuk memeriksakan kehamilan di fasilitas kesehatan secara langsung. Hal tersebut mengakibatkan terbatasnya akses ibu untuk melakukan kunjungan pelayanan kesehatan ibu dan anak di fasilitas kesehatan. Tujuan dari penelitian ini untuk mengetahui bagaimana akses pelayanan Kesehatan ibu dan anak di puskesmas selama pandemic COVID-19. Penelitian ini menggunakan metode scoping review dengan database yang digunakan adalah PubMed dan Google Scholar. Pencarian artikel diperoleh hasil 5 artikel yang memenuhi kriteria inklusi dari 86 artikel yang tersedia. Hasil dari penelitian menunjukkan bahwa kebijakan pembatasan pergerakan selama pandemi COVID-19, ketakutan tertular virus COVID-19 di fasilitas kesehatan serta tantangan ekonomi yang dihadapi ibu selama pandemi COVID-19 menjadi hambatan yang dialami ibu dalam mengakses pelayanan KIA selama pandemi COVID-19. Kebijakan lockdown menjadikan ibu tidak dapat melakukan kunjungan ANC secara lengkap. Kesulitan ekonomi mempengaruhi ibu dalam mengakses fasilitas kesehatan karena ibu lebih memilih untuk memenuhi kebutuhan sehari-hari dibanding untuk mengakses pelayanan KIA. Ketakutan tertular COVID-19 juga menjadikan ibu memilih untuk tidak mengunjungi pelayanan KIA di fasilitas kesehatan. Sehingga, sulitnya akses ibu ke pelayanan KIA di masa pandemi mengakibatkan ibu hamil berisiko tinggi mengalami komplikasi karena tidak teridentifikasinya kemungkinan tanda-tanda bahaya saat kehamilan di fasilitas kesehatan.

Abstract

Pelaporan insiden keselamatan pasien merupakan hal yang sangat penting dalam sistem perawatan kesehatan, karena bermanfaat untuk mengidentifikasi risiko dasar dan mencegah kesalahan yang sama terulang kembali. Rendahnya tingkat pelaporan insiden keselamatan pasien di rumah sakit menyebabkan sulitnya identifikasi kesalahan dan melakukan investigasi lebih lanjut. Tujuan: Menentukan faktor yang menghambat atau mempengaruhi pelaporan insiden keselamatan pasien di rumah sakit. Metode: Penelusuran artikel dilakukan melalui database PubMed, Sciencedirect, dan Google Scholar menggunakan kata kunci "patient safety incident" AND "incident reporting" OR "medical error reporting" AND "barriers incident reporting" OR "under reporting" AND "hospital". Total temuan artikel sebanyak 385, tetapi hanya 12 artikel yang sesuai dengan kriteria inklusi. Hasil: Terdapat total studi pada 23 rumah sakit di sembilan negara yang menunjukkan bahwa masing-masing rumah sakit memiliki beberapa faktor yang menghambat atau mempengaruhi pelaporan insiden keselamatan pasien. Paling banyak ditemukan yaitu ketakutan staf terhadap hukuman dan intimidasi, kurangnya pengetahuan terhadap prosedur melapor, rendahnya umpan balik yang positif dari manajemen, serta undang-undang yang tidak melindungi pelapor. Simpulan: Hambatan pelaporan insiden keselamatan pasien di rumah sakit dipengaruhi oleh 3 faktor penting yaitu faktor individu, faktor organisasi, dan faktor pemerintah.Kata kunci: hambatan pelaporan insiden, insiden keselamatan pasien, pelaporan insiden, rumah sakit

Abstract

The Covid-19 pandemic has an impact on health services, where there is a change in providing services to the community in order to avoid Covid-19 transmission in health service facilities. Therefore, this study aims to find out the changes carried out by public health center in providing individual health services, including outpatient, inpatient and delivery, emergency, pharmacy service, laboratory service, referral system, and funeral services. The method used in this study was a literature review. The type of review was narrative review, where the researcher looked for reference sources through policies, laws and regulations, guide book, or technical instructions regarding individual health services in Puskesmas before and during the Covid-19 pandemic. The study found some changes carried out including the application of triage/screening for every visitor, the application of physical distancing, changes in service flow, patient seating arrangement, use of transparent screen. Moreover, there was a modification of service schedule, the limitation of dental polyclinic service, and the utilization of information and communication systems in providing health services during the Covid-19 pandemic. Changes made by public health centers in the provision of individual health services are part of an effort to prevent and control the spread of Covid-19 infection on health services.

Abstract

The job satisfaction that employees get at the organization depends on whether or not the employee's needs are met. Employee needs that can be met will provide job satisfaction. The greater the employee's needs that can be met, the higher the employee's job satisfaction. Meanwhile, if the employee's needs are not met, the employee will have dissatisfaction. Objective: To identify the influence of internal service quality (intqual) on employee satisfaction at RSIAS. Methods: This study analyses an employee satisfaction survey conducted in 2020 using a cross-sectional study design. Population of this research is all employees in RSIAS. The sample of this study is also all employees in RSIAS, which is 421 people. The satisfaction survey is filled in using a score of 1 to 5, then averaged so that a score is obtained to determine the category of each aspect. The data obtained were analyzed based on the theory of internal service quality and employee satisfaction, then tested using a multiple linear regression statistical test model to determine the effect of internal service quality (intqual) on employee satisfaction at RSIAS. Results: The results of statistical tests showed that the value of p = 0.000, meaning that internal service quality has an effect on employee job satisfaction because the value of p <0.05. Several aspects of internal service quality affect job satisfaction, namely the aspects of tools, policies and procedures, management support, goal alignment, effective training and reward and recognition.

Abstract

AbstractProviding safe services is important in the implementation of health services. Medication errors are a failure of the treatment process that has the potential to cause harm to patients. The incidence of treatment errors can be prevented through increased understanding of health workers and the community regarding the safety of treatment. The active role of patients and their families during treatment process can minimize harm that may happened. This program aims to increase the knowledge of health workers and community as an important element of health services in order to prevent the occurrence of medication error. Efforts to increase knowledge are carried out through the holding of webinars for the community related to 5 moments for medication safety and treatment safety training activities for health workers. Knowledge change was measured using a pre and posttest questionnaires. The target of activities amounted to 37 people and 15 health workers. The results of the analysis showed positive changes in average knowledge with an increase in community knowledge (97.3%) and health workers (100%). The results of this activity become the provision of the community or health workers to increase their role in the safety of treatment with knowledge that has been obtained during community service activities.Keywords: Medication safety, health workers, community, medication error AbstrakPenyediaan layanan yang aman penting dalam penyelenggaraan pelayanan kesehatan. Kesalahan pengobatan merupakan kegagalan proses pengobatan yang berpotensi menimbulkan bahaya bagi pasien. Insiden kesalahan pengobatan dapat dicegah melalui peningkatan pemahaman tenaga kesehatan dan masyarakat terkait keamanan pengobatan. Peran aktif pasien dan keluarganya dalam pelayanan kesehatan dapat meminimalisir bahaya yang mungkin timbul pada proses pengobatan. Tujuan pengabdian masyarakat ini adalah meningkatkan pengetahuan tenaga kesehatan dan masyarakat sebagai unsur penting pelayanan kesehatan agar dapat mencegah terjadinya kejadian medication error. Upaya peningkatan pengetahuan dilakukan melalui penyelenggaraan webinar bagi masyarakat umum terkait 5 moments for medication safety dan kegiatan pelatihan keamanan pengobatan bagi tenaga kesehatan. Perubahan pengetahuan diukur menggunakan kuesioner pre dan post test. Sasaran kegiatan berjumlah 37 masyarakat dan 15 tenaga kesehatan. Hasil analisis menunjukkan adanya perubahan positif rata-rata pengetahuan dengan peningkatan pengetahuan masyarakat (97,3%) dan tenaga kesehatan (100%). Hasil kegiatan ini menjadi bekal masyarakat ataupun tenaga kesehatan untuk meningkatkan perannya dalam keamanan pengobatan dengan pengetahuan yang telah diperoleh selama kegiatan pengabdian masyarakat dilakukan.Kata kunci: Keamanan pengobatan, tenaga kesehatan, masyarakat, kesalahan pengobatan

Abstract

The rapid development of the corona virus has caused many positive cases to be confirmed both globally and nationally. The development of COVID-19 cases that continues to grow requires every hospital to always strive to improve hospital preparedness. One of the tools that can be used to improve hospital preparedness is the Rapid Hospital Readiness Checklist developed by WHO. Based on the background of this community service activity, it aims to assist hospitals in conducting self-assessment through the development of applications related to hospital preparedness in the face of the COVID-19 pandemic. This community service method is divided into two, namely, Focus Group Discussion (FGD) which aims to obtain information needed for application development and socialization which aims to practice application use. The presence of this application was welcomed by hospitals and related service units because it can help make it easier to fill out a checklist related to hospital preparedness. So with this application, it is hoped that it can help hospitals and stakeholders in efforts to improve hospital preparedness.

Abstract

AbstrakDeteksi terjadinya kejadian yang tidak diharapkan (KTD) telah menjadi salah satu tantangan dalam keselamatan pasien oleh karena itu metode untuk mendeteksi terjadinya KTD sangatlah penting untuk meningkatkan keselamatan pasien. Tujuan dari artikel ini adalah untuk membandingkan kelebihan dan kekurangan dari beberapa metode untuk mendeteksi terjadinya KTD di rumah sakit, meliputi review rekam medis, pelaporan insiden secara mandiri, teknologi informasi, dan pelaporan oleh pasien. Studi ini merupakan kajian literatur untuk membandingkan dan menganalisa metode terbaik untuk mendeteksi KTD yang dapat diimplementasikan oleh rumah sakit. Semua dari empat metode telah terbukti mampu untuk mendeteksi terjadinya KTD di rumah sakit, tetapi masing-masing metode mempunyai kelebihan dan kekurangan yang perlu diatasi. Tidak ada satu metode terbaik yang akan memberikan hasil terbaik untuk mendeteksi KTD di rumah sakit. Sehingga untuk mendeteksi lebih banyak KTD yang seharusnya dapat dicegah, atau KTD yang telah terjadi, rumah sakit seharusnya mengkombinasikan lebih dari satu metode untuk mendeteksi, karena masing-masing metode mempunyai sensitivitas berbeda-beda.AbstractDetecting adverse events has become one of the challenges in patient safety thus methods to detect adverse events become critical for improving patient safety. The purpose of this paper is to compare the strengths and weaknesses of several methods of identifying adverse events in hospital, including medical records reviews, self-reported incidents, information technology, and patient self-reports. This study is a literature review to compared and analyzed to determine the best method implemented by the hospital. All of four methods have been proved in their ability in detecting adverse events in hospitals, but each method had strengths and limitations to be overcome. There is no ‘best’ single method that will give the best results for adverse events detection in hospital. Thus to detect more preventable adverse events, or adverse events that have already occurred, hospitals should combine more than one method of detection, since each method has a different sensitivity.
