Abstract

ABSTRAK Henti jantung adalah hilangnya fungsi jantung secara mendadak dan bisa terjadi pada seseorang yang memiliki penyakit jantung atau tidak. Penyebab kejadian henti jantung terbanyak adalah penyakit pada sistem kardiovaskular seperti bradikardia, takikardia, kardiomiopati. Resusitasi Jantung Paru merupakan tindakan pertolongan untuk mengembalikan fungsi pernapasan dan sirkulasi pada orang henti jantung. Peneliti tertarik memperluas informasi mengenai Angka Keberhasilan Resusitasi Jantung Paru Pada Pasien Yang Mengalami Henti Jantung di Rumah Sakit Umum Pusat Sanglah. Penelitian ini dilakukan dengan metode deskriptif potong lintang. Subjek penelitian dipilih menggunakan metode consecutive sampling berdasarkan kriteria inklusi terhitung sejak 1 Januari 2021 sampai 30 Juni 2021 di IGD dan ruang rawat inap RSUP Sanglah Denpasar. Variabel dalam penelitian ini meliputi: data demografi pasien berupa usia, jenis kelamin, luaran. Data dianalisis secara deskriptif. Sebanyak 294 pasien yang di RJP pada tahun 2019, dan 121 pasien yang di RJP pada tahun 2020. Subjek penelitian pada tahun 2019 terdiri dari 42,5% perempuan, dan 57,5% laki-laki. Pada tahun 2020 terdiri dari 41,3% perempuan dan 58,7% laki-laki. Usia subjek penelitian dibagi menjadi delapan kelompok, namun usia dengan persentase terbanyak pada tahun 2019 adalah >65 tahun sebanyak 29,9%. Pada tabel menunjukkan bahwa populasi laki-laki yaitu 57,8% dengan usia terbanyak >65 tahun sebanyak 29,4% mengalami henti jantung. Terdapat 68,0% yang meninggal selama dua tahun. Kejadian henti jantung diperoleh sebanyak 415 kasus di RSUP Sanglah. Diperoleh angka keberhasilan RJP pasien henti jantung sebesar 32%. Keberhasilan RJP ini dapat dipengaruhi oleh beberapa faktor seperti usia, jenis kelamin. Kata kunci : angka keberhasilan, henti jantung, resusitasi jantung paru.

Abstract

The Covid-19 pandemic has left healthcare workers working in uncertain and stressful situations to care for all patients with disabilities. The second wave of the Covid-19 pandemic that swept across the world caused many nurses in health facilities to experience fatigue in carrying out their duties. This study aims to determine the cause of burnout symptoms in isolation room nurses in the Covid-19 pandemic. The study was conducted at X Hospital in September-October 2021. The design of this study was descriptive analysis. Observations were made on the service system, data, and environment of the Covid-19 isolation room at X Hospital. Interviews and discussions were conducted with the head nurse, the head of the occupational safety and health committee, and the head of nursing medical services related to nurse burnout in the isolation room. 67.74% experienced emotional exhaustion in the medium and high categories in isolation room nurses in the second wave of the Covid-19 pandemic caused by a lack of staffing. The solution to detect and overcome burnout is to conduct mental health checks and regular medical check-up programs for further treatment by considering the comfort factor and high functional value.

Abstract

Non-Covid-19 service closure has formed a stigma that hospitals only serve Covid-19 patients. This condition affects brand awareness and decreases the number of hospital visits. An interactive talk show using Instagram can be used to reintroduce hospital services to the public. This study aimed to measure the success of the talk show program in building brand awareness of Udayana University Hospital's Instagram account. One group pretest-posttest research was carried out at Udayana University Hospital on October 7, 14, and 21, 2021, by intervening with the Instagram content of Udayana University Hospital. After the live broadcast ended, the talk show content was published on the Instagram feed. The success of the talk show program was seen by comparing the audience growth, engagement rate, and brand awareness measurements before and after the talk show took place. Brand awareness can be seen in profiles, post-impressions, and profile visits to the Instagram account. The highest post-engagement (36.46%) was obtained in the first episode of the talk show and was in the very high category. Instagram followers and engagement rates increased and categorized very high. Profile impressions and visit profiles increased, indicating a wide range of content on the Udayana University Hospital's Instagram account. This study shows that talk show program can be implemented as a routine program that builds brand awareness on the Instagram account of Udayana University Hospital. This program can be used as a solution to introduce Udayana University Hospital to Instagram users by inviting quality guest stars and using easy-to-understand material.

Abstract

This article aims to analyze legal arrangements related to the protection of non-civil servant radiation workers in the Province of Bali, identify legal protection gaps between civil servant and non-civil servant radiation workers, and provide recommendations to ensure proper and fair protection for all radiation workers. This study uses normative legal methods to analyze regulations on the protection of non-civil servant radiation workers. Qualitative-descriptive analysis is used to identify legal gaps and formulate recommendations based on the principles of justice. The results of the study show that the analysis of legal protection for non-civil servant radiation workers in Bali Province shows a significant gap compared to civil servants, especially in benefits, access to health facilities, and legal support. Despite the regulations, their implementation still faces challenges in monitoring and workers' awareness of their rights. Regulatory revisions, increased oversight, and equality of benefits are needed to ensure equitable protection for all radiation workers. This strategic step will create a safe work environment, respect human rights, and uphold the principle of justice without discrimination.

Abstract

Introduction: Anxiety is a prevalent emotional response in patients undergoing surgical procedures, including cesarean sections. Perioperative anxiety can negatively impact patient outcomes, affecting physiological parameters and postoperative recovery. This study aimed to identify the prevalence and risk factors associated with perioperative anxiety levels in patients undergoing cesarean section at Prof. Dr. I.G.N.G. Ngoerah Hospital, Bali, Indonesia. Methods: This cross-sectional study involved 37 patients scheduled for elective cesarean section at Prof. Dr. I.G.N.G. Ngoerah Hospital. Anxiety levels were assessed using the Perioperative Anxiety Scale (PASS), a validated instrument for measuring anxiety related to surgery. Data on sociodemographic characteristics, obstetric history, and medical history were collected through a questionnaire. Descriptive statistics and logistic regression analysis were used to analyze the data. Results: The majority of participants (59.5%) experienced mild to moderate anxiety levels. Age, occupation, education level, history of emergency surgery, previous surgical history, and gravidity were identified as potential risk factors associated with higher anxiety levels. Patients aged 25-29 years, housewives, those with a high school education, a history of emergency surgery, previous surgical experience, and primiparity were more likely to report mild to moderate anxiety. Conclusion: Mild to moderate anxiety is prevalent among cesarean section patients. Identifying risk factors associated with perioperative anxiety can aid healthcare providers in implementing targeted interventions to reduce anxiety and improve patient well-being.

Abstract

Introduction: Cardiac arrest in end-stage renal disease (ESRD) patients presents unique challenges due to their complex medical conditions. Post-resuscitation care for these patients requires careful management of various factors, including hemodynamic instability, electrolyte imbalances, and fluid overload. This case report describes the successful post-resuscitation care of an ESRD patient who experienced cardiac arrest and achieved a return of spontaneous circulation (ROSC). Case presentation: A 50-year-old female with ESRD on hemodialysis (HD) presented with acute dyspnea and cardiac arrest. After 8 minutes of cardiopulmonary resuscitation (CPR), ROSC was achieved. The patient was managed with a comprehensive post-resuscitation care protocol, including brain resuscitation (targeted temperature management, ventilation optimization, and hemodynamic control) and sustained low-efficiency dialysis for fluid balance. Despite the severity of her condition, the patient showed significant neurological recovery and was successfully extubated after 7 days of mechanical ventilation. She was discharged after 12 days with follow-up for her chronic conditions. Conclusion: This case highlights the importance of individualized management strategies for ESRD patients post-cardiac arrest, including the need for prolonged resuscitation and careful monitoring to improve patient outcomes.

Abstract

Electronic medical records must adhere to the principles of data and information security, which include availability, integrity, and confidentiality. Medical records serve as legitimate legal evidence, thereby satisfying these principles both individually and institutionally. Consequently, the aim of this research is to explore the roles of law on medical records for data and information security. To achieve this aim, a systematic literature review (SLR) was employed. The Scopus and PubMed databases were chosen for their global recognition. Full-text articles from these databases were subjected to PRISMA analysis using Mendeley Reference Manager. The researchers collected 32 research articles by the categories of data protection law, medical records' law enforcement, and data protection law recommendation. The research findings indicate that the roles of law deal with the role of law in compliance and data protection, law enforcement and sanctions, a recommendation for improving compliance and data protection. In addition, the law is crucial for ensuring compliance and data protection in electronic medical records implementation in hospitals. However, compliance levels have not reached optimal levels. To improve security, awareness, and enforcement, collaboration with legal bodies, and educational initiatives are needed. Legislation like HIPAA and GDPR influence data protection measures, but penalties are insufficient. To enhance compliance, medical personnel should undergo rigorous training and improve oversight of health data management procedures

Abstract

Manajemen perioperatif pasien hamil dengan penyakit jantung bawaan dan preeklampsia memerlukan pendekatan multidisiplin yang cermat. Atrial Septal Defect (ASD) dengan shunt bidirectional dan hipertensi pulmonal meningkatkan risiko gagal jantung dan komplikasi obstetrik. Anestesi regional sering dipilih untuk mengurangi dampak hemodinamik, dengan pengawasan ketat terhadap risiko tromboemboli dan stabilitas volume cairan. Seorang perempuan dua puluh dua tahun, hamil tiga puluh lima minggu tiga hari, dengan ASD, hipertensi pulmonal, dan preeklampsia berat, mengalami sesak napas progresif dan sianosis intermiten. Pemeriksaan echocardiography menunjukkan ASD moderat dengan shunt bidirectional, hipertrofi ventrikel kiri, dan fungsi ventrikel kanan yang menurun. Pasien menjalani Sectio Caesarea Transperitoneal (SCTP) dengan anestesi spinal menggunakan Bupivakain hiperbarik tujuh koma lima miligram dan Morfin nol koma lima miligram intratekal. Hemodinamik pasien tetap stabil sepanjang prosedur dengan pemantauan ketat dan manajemen cairan yang cermat dan teliti. ASD dengan shunt bidirectional meningkatkan risiko emboli paradoks, gagal jantung kanan, dan hipoksemia, terutama saat kehamilan. Preeklampsia memperburuk kondisi dengan meningkatkan resistensi vaskular dan disfungsi endotel, yang dapat menyebabkan hipertensi pulmonal progresif dan dekompensasi jantung. Manajemen anestesi bertujuan menjaga stabilitas hemodinamik dengan anestesi spinal dosis rendah untuk menghindari hipotensi mendadak. Morfin intratekal memberikan analgesia pascaoperasi yang efektif tanpa mengganggu fungsi pernapasan dan hemodinamik. Keberhasilan manajemen perioperatif pada pasien dengan ASD, hipertensi pulmonal, dan preeklampsia berat bergantung pada pendekatan multidisiplin yang komprehensif. Penggunaan anestesi spinal dengan Bupivakain dan Morfin intratekal terbukti efektif dalam menjaga stabilitas hemodinamik serta mengurangi risiko komplikasi kardiovaskular dan neonatal.

Abstract

The advancement of information and communication technology, particularly telemedicine, has transformed the healthcare sector, including its application in traditional medicine in Indonesia. Traditional medicine is recognized under Law No. 17 of 2023 concerning Health; however, the implementation of telemedicine in this field has not yet been specifically regulated. This study aims to analyze the legal aspects of using telemedicine in traditional medicine based on the aforementioned Health Law. The research method employed is normative legal research using statutory and conceptual approaches. The findings reveal several challenges, including regulatory gaps, standardization issues, patient data protection, and practitioner competence. Although the Health Law provides a general legal framework, specific regulations regarding the use of telemedicine in traditional medicine are still lacking. The study concludes that additional regulations are needed to govern the use of telemedicine, ensure quality control, protect patient rights, and certify practitioners. The recommendations include drafting specific regulations, enhancing practitioner competence, and increasing public education to ensure the safe and effective implementation of telemedicine in traditional medicine.

Abstract

ABSTRAK Penelitian ini bertujuan untuk membandingkan efektivitas blok ESP dan TAP terhadap durasi efek analgesik pasca operasi, nyeri pasca operasi, total konsumsi opioid pasca operasi, dan perubahan dalam nilai Neutrophil-Lymphocyte Ratio (NLR) sebelum dan sesudah operasi laparatomi ginekologi. Jenis penelitian ini adalah eksperimental murni (eksperimental sejati). Desain penelitian yang digunakan adalah uji acak tunggal tersamar. Subjek penelitian adalah pasien berusia 18-65 tahun yang menjalani operasi laparatomi ginekologi di ruang operasi Instalasi Bedah Pusat. Analisis data dilakukan dengan bantuan SPSS versi 26 termasuk analisis deskriptif dan uji perbandingan rata-rata menggunakan uji t independen. Blok ESP memiliki durasi analgesik yang lebih lama dengan rata-rata 6.13 ± 3.30 jam sedangkan TAP memiliki durasi analgesia sebesar 3.93 ± 1.98 jam, dengan perbedaan rata-rata sebesar 2.18 jam (IK 95% 0.22-4.15 jam; p = 0.030). Terdapat perbedaan pada skala VAS pada 6, 12, 24, dan 48 jam dengan hasil ESP lebih rendah dari TAP. Rata-rata kebutuhan morfin pada blok ESP adalah 1.62 ± 0.71 mg dan kelompok TAP dengan total kebutuhan rata-rata ± SB 3.31 ± 1.74 mg, perbedaan yang diperoleh adalah 1.68 mg (IK95% 0.72-2.64 mg; p = 0.001). Hasil perbedaan nilai perubahan NLR antara sebelum dan sesudah operasi antara blok ESP bilateral dan TAP ditemukan memiliki perbedaan yang signifikan dengan perbedaan rata-rata sebesar 0.36 (IK 95% 0.04-0.69; P=0.029). Kesimpulan dari penelitian ini adalah blok ESP memiliki efektivitas yang lebih baik dibandingkan dengan blok TAP sebagai analgesia setelah operasi laparatomi ginekologi.
