Abstract

Introduction: The Tuban Regency Health Profile in 2019 stated that the villages with ODF status had reached 64 villages (19.5%) out of 328 total villages, while the villages reported having implemented Community-Based Total Sanitation (CBTS) have reached 100%. The low ODF status achieved indicates that most people still defecate openly, which will increase health risks due to environmental pollution. This study aimed to determine the distribution map of diarrhea in toddlers in each district of Tuban Regency in 2019. Methods: This study was an observational research type supported by mapping through an application with a cross-sectional research design. This study was all districts in Tuban Regency. Data were obtained from secondary data, which were the Tuban Regency Health Profile in 2019. Results and Discussion: The percentage of latrine ownership was 71.15%, the open defecation was 70%, the percentage of children under five was not more than 14.69%, and cases of diarrhea among toddlers were relatively diverse in each district with the lowest number of cases was 0, and the highest number of cases was 841. Conclusion: Diarrhea in toddlers was commonly distributed in the Eastern and Southeastern Tuban Regency, directly adjacent to the Bengawan Solo River. There was a strong possibility that the occurrence of diarrhea was related to the water quality of Bengawan Solo River that was consumed for daily household activities.

Abstract

ABSTRACT Small intestine infection caused by Giardia lamblia (giardiasis) occurs mostly in children living in developing country with poor sanitation. This study aims to determine the risk of giardiasis and personal hygiene conditions in pre-school students at KB-TK Al Amin Paciran Lamongan aged 2-6 years. Number of samples were 61 students, the dependent variable was the incidence of giardiasis, and independent variable was personal hygiene including nail hygiene, hand washing habit, footwear habit, and defecating habits. The result showed that 5 out of 61 students (8,2%) were infected with Giardia lamblia. All children (100%) with giardiasis had poor nail hygiene, footwear habit, and defecating habit. As many as 80% of students with giardiasis have poor handwashing habits. Students with poor footwear habit (OR=43,71; 95% CI 3,98-2046,9); open defecation habits (OR=13,33; 95% CI 1,40-628,05); poor nail hygiene (OR=12,31; 95% CI 1,29-580,49); poor hand washing habits (OR=5,73; 95% CI 0,5-290,96) had a greater risk of developing giardiasis. Supervision and healthy behavior are highly recommended, including using footwear when playing on the ground, defecating in the latrine, maintaining nail hygiene, and washing hands with soap before eating or after defecating. Keywords: Prevalence, giardiasis, personal hygiene, pre-school students ABSTRAK Infeksi usus halus disebabkan oleh Giardia lamblia (giardiasis) banyak terjadi pada anak-anak yang tinggal di negara berkembang dengan tingkat sanitasi buruk. Penelitian ini bertujuan untuk mengetahui risiko terjadinya giardiasis dan kondisi hygiene perorangan pada murid PAUD di KB-TK Al Amin Paciran Lamongan usia 2-6 tahun. Jumlah sampel adalah 61 murid, variabel dependen adalah kejadian giardiasis, dan variabel independen adalah hygiene perorangan meliputi kebersihan kuku, kebiasaan mencuci tangan, kebiasaan menggunakan alas kaki, dan kebiasaan buang air besar (BAB). Hasil penelitian menunjukkan bahwa 5 dari 61 murid (8,2%) terinfeksi Giardia lamblia. Seluruh murid (100%) dengan giardiasis mempunyai kebersihan kuku, kebiasaan menggunakan alas kaki dan kebiasaan BAB yang kurang baik. Sebanyak 80% murid dengan giardiasis mempunyai kebiasaan mencuci tangan yang kurang baik. Murid dengan kebiasaan menggunakan alas kaki kurang baik (OR=43,71; 95% CI 3,98–2046,9); kebiasaan BAB sembarangan (OR=13,33; 95% CI 1,40–628,05); kebersihan kuku kurang baik (OR=12,31; 95% CI 1,29-580,49); kebiasaan mencuci tangan yang kurang baik (OR=5,73; 95% CI 0,5–290,96) mempunyai risiko lebih besar terkena giardiasis. Pengawasan dan berperilaku hidup sehat sangat dianjurkan antara lain menggunakan alas kaki ketika bermain di tanah, membiasakan BAB di jamban, menjaga kebersihan kuku, dan mencuci tangan dengan sabun sebelum makan atau setelah buang air besar. Abstract

Purpose – Elementary school students can become agents of change so that education on the concept of One Health. The purpose of this community service is to increase knowledge about the concept of One Health in students and the regeneration of Little One Health Ambassadors at SDN Kalisari II Surabaya City. Design/methodology/approach – The method was counseling on the concept of One Health, and the selection of Little One Health Ambassadors. Activities were carried out in August-September 2023. The educational material was from One Health Lessons and have been adapted for children aged 8 - 10 years. One Health Ambassadors were selected based on the results of the assessment of poster work about One Health, and mastery of One Health material. Counseling activities on One Health education were carried out in the fourth and fifth grades with a total of 80 students. Findings – The results showed that the education provided can improve the knowledge of the target group about the concept of One Health (p-value = 0.003). The first winner of One Health ambassadors was chosen who indicated that he mastered the material about One Health and his work was the most interesting and could present it well. Originality/value – Community service activities produce changes that are by external targets.

Abstract

Data Monitoring Pelaksanaan 8 Aksi Konvergensi Intervensi Penurunan Stunting Terintegrasi tahun 2024,  persentase  cakupan  pemberian  tablet  tambah darah (TTD) pada remaja di di  Jawa Timur  sebesar  68.8%.  Kabupaten Bangkalan masih memiliki cakupan  pemberian  TTD dibawah rata rata Jawa Timur. Persentase  cakupan  pemberian  TTD pada remaja di Kabupaten Bangkalan sebesar  41.5%. Rendahnya konsumsi TTD dikarenakan kurangnya pengetahuan, sikap dan dukungan orang tua. Peningkatan pengetahuan, sikap dan dukungan orangtua dapat dilakukan melalui aksi kolaboratif dalam peningkatan konsusi TTD. Tujuan pengabdian ini adalah untuk mendeskripsikan hasil pendidikan dan promosi kesehatan melalui Aksi Kolaboratif Orang Tua, Puskesmas, dan Sekolah dalam pencegahan Anemia pada remaja putri. Metode yang digunakan adalah dengan penyuluhan tentang anemia dan konsumsi TTD dan memperkenalkan edutaintment Unonemia. Hasil: Tahapan pemberian edukasi peningkatan konsusi TTD dilakukan dengan 5 tahapan yaitu : perencanaan dan manajemen program, pelaksanaan situational assessment melalui pertemuan dengan pihak Puskesmas, situational assessment melalui pertemuan dengan pihak Sekolah, pelaksanaan kegiatan penyuluhan tentang anemia dan tablet tambah darah, dan penilaian pengetahuan dan kesadaran remaja putri dan pengetahuan orang tua tentang anemia dan konsumsi TTD dan didapatkan terjadi peningkatan  pengetahuan dan sikap setelah dilakukan pemberian edukasi (pengetahuan orang tua 49,05 dan posttest 73,6. Nilai rata-rata pretest sikap remaja putri sebesar 62,07 posttest sebesar 70,27. Nilai rata-rata sikap orang tua pretest sebesar 73,35 dan nilai posttest 90,75).

Abstract

Purpose: Infections in children can affect weight gain and linear growth by influencing metabolism and nutrition. Chronic inflammation results in growth failure mediated by pro-inflammatory cytokines and poor nutritional intake, which affects the GH/IGF-1 axis. This study aimed to investigate the impact of the inflammatory response on children's anthropometry, particularly HAZ, and the role of IGF-1. Methods: An observational study with a cross-sectional design was conducted from September 2021 to July 2022. The study involved children diagnosed with infections. The subjects had undergone a physical and laboratory investigation, which included a thorax photo, urine culture, Mantoux test, and complete blood test in a hospital setting. A complete medical history was obtained from the pediatrician. Results: The prevalence of undernutrition was 37.33%; the prevalence of underweight/severely underweight, stunted/severely stunted, and wasted/severely wasted was 25.33%, 30.66%, and 14.67% respectively. Stunted/severely stunted was more prevalent in subjects under 2 years old, compared to subjects more than 2 years old (22/41 vs. 7/34, p=0.014). SEM analysis revealed that the inflammatory response affected IGF-1 levels (r=0.850, p=0.000), while IGF-1 affected body composition (r=0.245, p=0.025), and then affected HAZ (r=1.000, p=0.000). The effect of IGF-1 on HAZ appears to be indirect, acting through body composition. Parental height has a weak, albeit significant, effect on body composition (r=0.101, p=0.025) and HAZ (r=0.192, p=0.040). Univariate analysis revealed strong correlations between IL-6 and IL-10 and IGF-1 (r=0.870, p=0.000 and 0.876, p=0.000, respectively). In contrast, parental height showed a correlation with HAZ/LAZ (r=0.319, p=0.000). Maternal height was correlated positively with WAZ (r=0.320, p=0.044). Conclusion: The prevalence of stunting among children under two years of age indicates that early childhood constitutes a critical period for intervention in growth and development. Inflammatory response has been demonstrated to influence IGF-1 levels in children. The role of IGF-1 on HAZ was mediated by body composition.

Abstract

The prevalence of stunting in various regions of East Java, particularly in the Tuban Regency, necessitates targeted intervention. This study aimed to assess the incidence of stunting in Tuban Regency using diverse analytical methods. It utilizes geographic mapping to illustrate the spatial distribution of stunting rates, and histograms to represent the percentage of stunting across individual villages. Notably, Mander Village in Tambakboyo District exhibits the highest prevalence, with 43.59% of toddlers affected by stunting. In 2023, the average stunting rate across the Tuban Regency was 11.53% per village, with a median of 9.69%. Alarmingly, over half of the villages failed to meet the 2024 RPJMN target, and 112 of 328 communities (34.15%) reported stunting rates exceeding 14%. Furthermore, the analysis revealed a Moran's I value of 0.216 and a p-value of 0.001, indicating significant geographical clustering of stunting cases within the region. This suggests that the prevalence of stunted toddlers in each village is influenced by similar rates in neighboring communities, highlighting the importance of a collaborative approach in addressing this public health challenge.
